Due to historical and technical r...

Adenomyoepitheliomas (AMEs) are rare breast lesions, which are characterized by the proliferation of epithelial and myoepithelial cells. They are circumscribed lesions, which can be clinically and mammographically detected. However, when they are indistinctly limited, differential diagnosis with malignant lesions becomes difficult. In this paper, we report a case of adenomyoepithelioma with malignant transformation of the myoepithelial component, a rare histological type that arises from the malignant transformation of epithelial, myoepithelial, or both elements. Its course and prognosis are uncertain. Mastectomy with sentinel lymph node biopsy followed by radiotherapy are the most effective treatment approachesAdenomioepiteliomas (AMEs) são lesões raras na mama, caracterizadas pela prol...

CONCLUSION: Three hundred twenty-six samples of non-fermenting Gram-negative bacilli were isolated. The most prevalent species were Pseudomonas aeruginosa (65.03%), Acinetobacter baumannii (16.56%), and Stenotrophomonas maltophilia (9.5%). Other species showed rates lower than 5%. The microorganisms were isolated from several infectious sites and the biological materials that showed higher positivity were the following: tracheal aspirate (38.34%), spittle (18.71%) and urine (15.95%). Bacterial resistance was higher with tetracyclines (89.57%) and sulfamethoxazole/trimethoprim (79.75%). The most active antimicrobials were polymyxin B and piperacillin/tazobactam with 100% and 75.2% sensibility, respectively (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

CONCLUSION: Although TMP-SMX is the standard treatment for S. maltophilia infections, there may be resistance to this antibiotic, which hinders the therapeutic approach, hence the significance of susceptibility tests. The disk diffusion technique showed a good correlation with microdilution. Among the new therapeutic options, both tigecycline and moxifloxacin presented significant activity in vitro (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

The measurement of nitric oxide metabolites may be useful for better understanding of different physiopathological processes....

CONCLUSION: The Etest® method overestimates the MIC values, however it is easily and quickly accomplished. In the other hand, broth macrodilution is very laborious and time consuming. MicroScan Walk Away 96 (Siemens®) results are accurate, but does not provide the exact MIC value requiring an additional test to estimate the effectiveness of treatment. In conclusion, the Etest® can be used as an alternative for routine monitoring. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

Epidermoid cysts of the central nervous system are uncommon conditions, which are frequently located in the cerebellopontine angle and around the pons. They are covered with keratinized squamous epithelium and keratin lamella, which give its contents a soft, white-pearly appearance. Epidermoid cysts are mostly originated from malformations, presumably associated with surface elements of the nervous system ectoderm during the closure of the neural groove or formation of secondary cerebral vesicles. The authors describe a case of epidermoid cyst in the posterior fossa causing hydrocephalus and review morphologic and diagnostic criteria of this lesion.

CONCLUSION: In placentas from preeclampsia pregnancies the fibrinoid material area is larger (1.8x) compared to normal pregnancies. Nuclei and cytoplasm areas were larger in the control group. There was no statistical difference regarding the stromal area. There was also a reduction in villous space in preeclampsia, which is consistent with villous hypotrophy. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

We report a patient with cutaneous Rosai-Dorfman disease that presented on the right buttock, a red brown plaque with yellowish areas and red-brown satellite papulas. The histopathologic exam revealed inflammatory infiltrate predominantly of lymphocytes and histiocytes with wide and clear cytoplasm containing intact inflammatory cells (emperipolesis). Any other alteration in the physical exam and laboratorial was not found. The Rosai-Dorfman disease is a rare form of histiocytic proliferation and the strictly cutaneous form is still rarer. The aetiology is unknown and the clinical and histologic correlation enables the correct diagnosis. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

CONCLUSION: As far as mutational status is concerned, it was not possible to confirm any predictive value for histopathological patterns such as circumscription, presence of isolated melanocytes in the lesion and cytological features. Difficulties in the interpretation of some histological criteria were demonstrated by the variation in the observers' conclusions. It is difficult to state if genetic alterations such as BRAF mutations may serve as biomarkers for melanoma classification.INTRODUÇÃO: Mutações do gene BRAF localizado no cromossomo 7q são as mais frequentemente encontradas em melanomas cutâneos (60%-80%).

Sudden death of 23 year-old asymptomatic male patient led to furt...

CONCLUSION: The identification of both BRAF and c-Kit mutation is not a suitable prognostic indicator in the Brazilian population. Moreover, the relatively low frequency of BRAF mutations brings into question if it actually plays a key role in melanoma pathogenesis.INTRODUÇÃO: Técnicas de biologia molecular permitem a identificação de marcadores moleculares como mutações dos genes BRAF e c-Kit em melanomas.
The validation of the analytical technique for the determination of polyamines in cerebral tissue using HPLC based on o-phthalaldehyde post-column derivatization is described. The polyamines were separated in a LiChrospher100 RP18 column. Elution gradient was formed with two mobile phases: A (sodium acetate 0.1 M + sodium octanesulphonate 0.01 M, pH = 4.5) and B (sodium acetate 0.2 M + sodium octanesulphonate 0.01 M)/acetonitrile (10:3), pH = 4.5) in a 1.2 ml/min flow rate. The derivative eluent was monitored by fluorescence (excitation, 345 nm; emission, 455 nm). Besides excellent linearity (putrescine, r = 0.9816; spermidine, r = 0.9920; spermine, r = 0.9901), the technique demonstrated intra and inter-day precision (< 20%) as well as recovery (spermidine = 92.56%; spermine = 84.47%).
CONCLUSION: Our study emphasizes that even when CG is normal, PSC should be exclusively diagnosed by liver biopsy, hence cholangiography being unnecessary. Chronic portal inflammation, neutrophilic pericholangitis, periductal sclerosis and "onion skinning" are characteristic histopathological findings. Neutrophilic pericholangitis may be subtle and easily overlooked in early disease, leading to strong suspicion of PSC.INTRODUÇÃO: Colangite esclerosante primária (CEP) é crescentemente diagnosticada em crianças e adolescentes devido ao melhor reconhecimento das apresentações clínicas por imagem e manifestações patológicas. OBJETIVO: As primei...

Myelodysplastic syndromes (MDS) are a group of clonal stem cell diseases characterized by ineffective hematopoiesis, bone marrow hyperproliferation, cytopenias in peripheral blood and risk of transformation into acute leukemia. We decided to investigate the effects of a soy concentrate on MDS patients based on the follow-up results of a 61 year-old Japanese female patient who was diagnosed with MDS and refractory cytopenia with multilineage dysplasia in 2003 (hemoglobin = 11g/dL; white blood cells count = 2,500/uL and platelets = 25,000/uL; marrow with mild dysplasia and normal karyotype; paroxysmal nocturnal hemoglobinuria was excluded). She started using soy as a dietary supplementation in May 2004 and presented a gradual increment in blood counts, achieving normalization approximately e...

CONCLUSION: Ghrelin and preproghrelin immunoreactive cells may be found in variable number in Type I neuroendocrine gastric tumors and in hyperplastic nodules associated with these tumors. However, it remains unclear what role these peptides play on the development of these tumors.INTRODUÇÃO: Grelina é um peptídeo de 28 aminoácidos, reconhecido como liberador endógeno do hormônio do crescimento, sendo secretado principalmente por células endócrinas da mucosa gástrica.

A potential diagnostic pitfall in the histological assessment of melanomas is the difficulty in recognizing unusual melanoma variants. Among them, the most challenging examples comprise desmoplastic melanomas, nevoid melanomas, the so-called minimal-deviation melanoma, melanomas with ...

This study aimed at providing a review of candidemia, showing some important aspects of etio-epidemiology, fungus/host relation, and laboratorial diagnosis of this emerging systemic infection. During the onset of candidemia, yeasts play a major role on several virulence factors, which ultimately promote its pathogenesis. Among them, it may be highlighted the extracellular enzyme production, dimorphism, hemolysin synthesis, adhesion ability and biofilm development. In infected patients, medicines must be administered in the early stages to minimize the risks of complications and antimicrobial resistance. Therefore, an efficient laboratory diagnosis is required. The study of these factors associated with genotype analysis may contribute decisively to the discovery of new substances that may ...

The objective of this work was to optimize KIR genotyping method described by Martin (2004). It was used PCR-SSP (polymerase chain reaction-sequence-specific primers) with primers synthesized by Invitrogen® and visualization of the amplified products on 2% agarose gel electrophoresis, containing ethidium bromide. Some adaptations were made and the reagents had their concentrations increased: the internal control from 100 nM to 150 nM, forward and reverse specific primers KIR12.5/12.3, KIR13.5/13.3, KIR14.5/14.3, KIR22.5/22.3 and KIR36.5/36.3 from 500 nM to 750 nM, and MgCl2 solution from 1.5 mM to 2 mM. Other reagent concentrations and amplification temperatures were maintained. Satisfactory results were obtained with Taq DNA Polymerase Recombinant (Invitrogen®). The results of seventy s...

CONCLUSION: Patients undergoing hemodialysis have high endothelial lesion, nevertheless, the presence of HCV chronic infection did not prove to be an aggravating factor. This result may be due to the small number of patients, hence further analyses with a larger sample are required for definitive conclusions.
CONCLUSION: The analysis of the cases herein and the other 119 cases reported in the literature reveals that oral lymphoepithelial cysts present a predilection for males, with peak of incidence in the third decade of life. Characteristically, these lesions consist of small asymptomatic nodules located in the buccal floor. Histologically, most cases show a parakeratinized epithelial lining and germinal centers in the fibrous capsule.

The differential diagnosis between small cell variant of squamous carcinoma and sma...

Epithelioid trophoblastic tumors are rare neoplasias that develop from the neoplastic transformation of chorionic-type intermediate trophoblasts. They show a wide spectrum of differential diagnoses. The authors describe a case of epithelioid trophoblastic tumor with atypical presentation in a 26-year-old patient that had recently undergone conization for in situ squamous cell carcinoma of the uterine cervix. The histopathology, immunohistochemistry and differential diagnoses of the lesion are also discussed. No similar case had been reported in the medical literature and it is an example of the hindrances to the diagnosis of epithelioid trophoblastic tumors.

Malignant endometrioid ovarian tumors are rare epithelial neoplasias among young patients.
Discussion and conclusion: The results show that the presence of Hb H, which is indicative of alpha thalassemia, may not be confirmed in a subsequent analysis. Iron deficiency in Hb H carriers is among the factors that may influence on the absence of Hb H in alpha thalassemia proven patients. Alpha thalassemia is associated with genetic defects involving alpha chain encoding genes, but may be also associated with a temporary imbalance of globin gene expression, alpha chain reduction or beta increase, which could explain the presence of beta chain tetramer (Hb H) leading to the diagnosis of alpha thalassemia minor.
We report a case of DSRCT affecting an 11-year-old male patient, with intra-abdominal involvement characterized by a large retroperitoneal mass in left hypochondrium. Histological examination of the mass showed the presence of clusters of small blue round tumor cells surrounded by a dense desmoplastic stroma. Immunohistochemical analysis disclosed a positive reaction to desmin, WT-1 and cytokeratins. After diagnosis, the patient underwent chemo radiotherapy treatment, but died at the 24th month of follow-up. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

We report a case of IVLBCL in a young female patient with bilateral involvement of the adrenal gland. Immunohistochemical examination confirmed IVLBCL. The relationship between endocrine organs and lymphoid cells is a hypothesis to explain the correlation observed between IVLBCL and the involvement of the adrenal gland. Moreover, the association between IVLBCL and the described symptomatology seems to reflect a pattern that may assist in a more effective diagnosis.
This study reports the case of a patient who allegedly had paracoccidioidomycosis (PCM) for fifteen years. Despite the fact that his baciloscopy result had always been negative, he was empirically treated for tuberculosis twice without clinical improvement. PCM diagnosis through simple methodology draws attention to the importance of parallel investigation into tuberculosis and paracoccidioidomycosis as differential diagnosis in chronic symptomatic patients.
This report describes the case of a 45 year-old patient with breast carcinoma characterized by choriocarcinomatous features, presence of bizarre anaplastic cells with areas of necrosis and multinucleated giant cells. The immuno-histochemical analysis showed positivity for chromogranin and β-HCG. The tumor size was 5.7 cm at the largest axis and it was refractory to initial neo-adjuvant treatment. Mastectomy was performed and after microscopic and immuno-histochemical examination it was diagnosed as carcinoma with choriocarcinomatous features. These lesions are rare and liable to synthesize hormonal substances not specific to breast tissue. Its diagnosis is highly significant to the therapeutic and prognostic approach insofar as these lesions are potentially aggressive. (Source: Jornal Bra...

CONCLUSIONS: The study revealed a statistically significant difference for p27 expression when comparing the control and oral squamous papilloma groups with the oral squamous cell carcinoma group. p21WAF/Cip1 did not prove to be useful to differentiate the groups. p16INK4a showed diffuse immunopositivity in a minority of the oral squamous cell carcinoma cases. The oral squamous papilloma group behaved similarly to the control group as to the three markers. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)

CONCLUSIONS: HC is widely accepted for the detection of HPV, however, it may lack sensitivity in comparison with PCR techniques. RT-PCR has further advantages over the conventional PCR in terms of speed as well as it is slightly more sensitive. Due to their high sensitivity and fast response, PCR methods could be used as a screening method for HPV detection in ectocervical samples.
CONCLUSION: The data reported herein show that the etiology of urinary infections is very similar to those found worldwide. Nonetheless, the resistance pattern of uropathogens may have different characteristics according to the history of antimicrobial consumption in each community. Thus, it is important that epidemiological data are regularly published to aid the medical community. (Source: Jornal Brasileiro de Patologia e Medicina Laboratorial)
