            <description>CONCLUSIONS: The incidence of extubation failure in our population was similar to rate reported in the literature. The main risk factor for extubation failure was prematurity (&amp;lt;/= 28 weeks). In this population of extreme preterm infants, implementation of strategies for early extubation, use of methylxanthines, prevention of patent ductus arteriosus, and use of different modes of assisted ventilation after extubation may improve the outcomes.

            <description>CONCLUSIONS: The prevalence of Down syndrome and congenital heart defect in our region is similar to the rates found by other authors; therefore, we highlight the importance of diagnostic suspicion and early referral by pediatricians to cardiac evaluation. Another relevant aspect is the small number of patients who underwent karyotype testing. In addition, the number of associated malformations was lower than that found by other authors.

Balanced nutrition, with intake of essential energy and nutrients is usually enough to achieve good athletic performance. The use of dietary supplements must be allowed only for selected cases in which specific nutritional deficiencies are identified.

The absence of good quality evidence to treat sickle cell priapism suggests the necessity to conduct good prospective multicenter protocols to investigate the condition.

Laboratory examination identified anemia (hemoglobin = 9.14 g/dL), total leukocytes of 11,920 mm(3), platelets of 250,000 mm(3), elevated sedimentation velocity of red cells and elevated C-reactive protein. Critically ill children may require a reduction by as much as 40-50% of the currently recommended maintenance volumes. Our results revealed that the dietary factors which were most responsible for risk of anemia were a greater proportion of calories from cow's milk and lower density of nonheme iron, in addition to age below 24 months and low maternal educational level.

Patients need appropriate treatment by eliminating foods that cause symptoms, while avoiding the nutritional side effects and the cost of inappropriate diets.
