            <description>AbstractPatients taking antiplatelet agent for the prevention of cardiovascular diseases who develop gastrointestinal bleeding represent a serious challenge in clinical practice. The initial step in reducing gastrointestinal risk of antiplatelet therapy is to assess whether the patient has a continued need for antiplatelet therapy. The next step is to eliminate the risk factors that may place the patient at increased gastrointestinal risk. In the management of bleeding ulcer patients with high‐risk stigmata of recent hemorrhage, resuming antiplatelet agents at 3‐5 days after the last dosing is a reasonable strategy. Patient's ethnicity and associated language and cultural barriers may affect screening uptake, which may negatively affect the health of immigrants.             <description>ABSTRACTGallbladder cancer (GBC) is the leading cause of cancer related mortality in certain geographic areas. Most of the patients with GBC have advanced disease at presentation, precluding curative resection resulting in a dismal prognosis. However, recent advances in the understanding of its epidemiology and pathogenesis coupled with development of newer diagnostic tools and therapeutic options, has resulted in enhanced optimism towards the management of the disease.The leading risk factors are gallstones, advancing age, female gender, anomalous pancreaticobiliary ductal junction, certain ethnic groups and geographic populations. The pathophysiology of ACLF in CHB is still poorly understood. Despite the identification of important predisposing factor and prognostic markers, ACLF in CHB remains a disease associated with high mortality. The majority of studies using nucleoside analogue therapy did not show any significant improvement in survival, although larger prospective studies are needed. Liver transplantation is the definitive treatment for ACLF in CHB. The challenge ahead would be prognosticating cases with favorable or unfavorable outcomes in order to streamline patients for early transplantation or for medical therapy.
