The study was conducted with the aim of investigating the effects of perception of the hospital workplace factors on the job satisfaction and psychological well-being of a sample of paramedical health care employees (N =200) from a medical college (teaching) hospital and public (non-teaching) government-run hospitals. Statistical analysis of the data revealed that despite significant differences in perceived work-place factors, the reported levels of psychological well-being and job satisfaction of the participants were nearly similar in the two types of public hospitals. Stepwise regression analysis of the data revealed that the organization’s structure-related factors, co-ordination and work autonomy were significantly predictive of job satisfaction and psychological well-being of ...

Morbidity, Health Expenditure and Willingness to Pay for Health Insurance amongst the Urban Poor: A Case Study

The study aimed to assess the demand for health insurance, gather evidence on willingness to pay (WTP) for health insurance and its determining factors amongst the urban poor in Mumbai. This was as-certained through dichotomous bidding process on 300 households (HHs). The findings reveal characteristics of a low-income group with a burden of disease and treatment financed through out-of-pocket (OOP) expenditure, which is at times catastrophic. These factors justify the demand for health care insurance in the study population expressed by one-third of the population. Results of the multivariate regression model suggest that HHs with presence of morbid conditions and prior experience of inpatient admissions were more likely to join and pay for health insurance. Within the ‘urban poor&r...

Private Health Insurance in India: Evaluating Emerging Business Models

Health care in India is severely underfunded and neither the Government nor the common citizen has the financial ability to pay for quality medical care. Given this situation, private health insurance should have a wide acceptance in the market and play a significant role in meeting the health care funding shortfalls. But in reality, the sector has been mired in restrictive regulations and outdated business models and continues to operate under losses, in an unsustainable mode. Consequently, the health insurance industry needs to relook their business models and evaluate emerging concepts in health care that can help correct the imbalance and help the industry operate in an effective manner. Some of the models which the health insurers need to evaluate are the managed care model, integrate...

The SAMU, ‘Service d’ Aide Medicale Urgente’, was set up in Mauritius in December 1997 in collaboration with University of Bordeaux II, with the Ministry of Health and Quality of Life and the Mauritian Institute of Health in a bid to offer pre-hospital emergency treatment to people in distress. Recorded data concerning the public use of the service were analyzed: 1,967 run sheets were investigated and trends were identi-fied. The study showed that a large proportion of the service is being used by the aged, and the commonest reasons for calling SAMU were road traffic accidents, cardiac problems and bronchial asthmas. The rate of use of the service is almost the same for every day of the week, although it is more in demand between 0900 hours to 1500 hours. The emergency te...

This study used hospitals from the Korean Hospital Association (data of the 181 hospitals enrolled with the Korean Hospital Association) published in 2004, and divided the hospitals into five groups according to the number of hospital beds.
This study was to analyze the relation between the number of medi-cal specialists per 100 beds and managerial performance per medical specialist on the each group. Thus, this research was tested using regression analysis, setting up the number of medical specialists per 100 beds as independent variable and the index of hospital managerial per-formance as dependent variable. For the part used as index of hospital managerial performance, medical revenue index is used for total income and growth index is used for the number of outpatients and inpatients. T...

User satisfaction is an important factor which should guide the running of any establishment. In the field of healthcare delivery, taking feedback is rare at least in the state of Kerala. Due to paucity of reports in scientific journals, a survey was conducted at a Government Medical College to assess the level of user satisfaction.
Using a pre-designed proforma, trained social workers gathered information from the discharged patients during a period of two weeks. The coverage was 31.6 per cent of all discharged patients. About two-thirds of the respondents gave positive results on registration for-malities, accommodation facilities, water supply and lighting facilities, general cleanliness, bathrooms and toilets, availability of medicines, service of doctors, services of different wings o...

We describe here the process of quality control and observations of the ASS rounds held from 2005 to 2007. The performance of a majority of sites was satisfactory. The testing laboratories usually adhered to standard operating procedures (SOPs). Lacunae observed during the supervisory visits have been enumerated along with the recommendations for the future surveillance rounds. (Source: Journal of Health Management)

The objective of the present article, besides documenting the different economic implications of HIV/AIDS on medical expenditures of individuals/HHs, is to find through a comparative analysis if the expenses of HIV-positive respondents were significantly higher compared to those of all members from the control group of non-HIV/AIDS HHs taken together. On account of the peculiar nature and constraints involved, the study attempts the above through use of non-parametric tools of Chisquare and Mann–Whitney U. Findings reveal significant hardships faced by HIV/AIDS HHs, despite respondents opting more for free treatment; considering details of only one person per HH; often getting assistance from others like NGOs; and often not seeking treatment due to financial impediments—all unl...

The present research work aims to study the effect of young maternal age on the prevalence of sexually transmitted diseases (STDs) and anaemia using National Family Health Survey-3 (NFHS-3) data for India. Analysis has been done for currently married women in the reproductive age group and who had their first birth in the age group 29 or below. Data have been analysed for urban and rural areas separately. The bivariate analysis showed that per cent of women who had their first birth during adolescence is higher in rural areas than urban areas. Further, women who had their first birth during adolescence reported a higher prevalence of STDs and anaemia than women who had their first birth at later ages in both settings. Logistic regression analysis has been used to examine the net influence ...

This study examines the Indian health insurance market by empirically observing the provider’s perceptions and its relationship with the insured, the insurer and the third party administrators (TPAs). The study tries to find out the awareness level among the insured population and their attitude towards treatment cost. It then examines the role of TPAs and the impact of cashless services on the cost of treatment by studying a few cost drivers. Apart from studying the provider’s per-ceptions it also tries to look at some of the evidence of moral hazards and that of fraudulent activity. The findings suggest that the awareness level regarding policy terms and condition is low among the insured population and most of them do not care for the cost of treatment. The providers increas...

Psychiatric doctors and nurses are two groups of professionals whose responsibility is to offer mental health services: advice on issues related to mental health, treatment of cases of mental disorders and rehabilitation of individuals with mental health problems in the community. The demand and supply of these services are influenced extensively by the prevailing business environment, specially, the economic factors. The current global economic meltdown is an uncontrollable variable facing all economies of the world. Nigeria is no exception. What differentiates one economy from the other is the degree of impacts on the well-being of the citizenry. In this article, the authors X-ray the challenges of psychiatric nursing services in Nigeria and the impacts of the current global economic mel...

Satisfaction has been shown to predict important health-related behaviour, such as adhering to treatment recommendations and maintaining continuity of care. The study resulted in two self-designed standardised questionnaires which were used to evaluate service quality and patient satisfaction. The study has yielded three factors of service quality namely promptness, medical aid and patient interest for service quality and amenities, clinical services and physical services. The study explains that service quality and patient satisfaction are more strongly associated with adherence and continuity of visit; when service quality and patient satisfaction were placed in the regression model, they are positively associated. The findings suggest that service quality and satisfaction level of the p...

Conclusion The rising prevalence of chronic diseases in the ageing population is a substantial burden not only for health-care systems but also for social and family structures due to dependency. Since a majority of the health problems among the elderly are diet related and nutritionally dependent, appropriate and adequate nutrition is essential for the health and well-being of the elderly.
In today’s customer-driven market, it is being felt increasingly that it is quality that will ultimately decide the value of the services. With in-creasing awareness among consumers about their rights, the patient, as a consumer of health services, expect and demand quality health care. The health care customer is changing qualitatively. The rising literacy rate, higher levels of income and increasing awareness through deeper penetration of the media, has brought the Indian consumer closer to demand quality health care. All these factors have not only contributed to the growth of health care sector in India but also for quality health care services. In the light of these developments, health care providers need to have a closer look at the perception of their patients and try to prov...

Data Envelopment Analysis by using the Malmquist Productivity Index and its decompositions has been applied in order to measure the technical efficiency and productivity. Clinics were considered to transform inputs labour (medical and nursing staff) and capital (hospital beds) into health services, approximated by the number of in-patient discharges and in-patient days, used as outputs. The model is output-oriented and assumes variable return to scale. Data were collected from hospitals’ records. Overall productivity progressed in all clinics.
A study sponsored by Central TB Division, MOHFW, Government of India, with the overall objective of enhancing of PPM under RNTCP in the state of Rajasthan was conducted by National Institute of Health and Family Welfare (NIHFW). After completing the diagnostic part of the study, a new approach of horizontal integration of PP and NGO schemes was suggested to overcome implementation constraints at government functionaries level, private providers level and community level. During the study it emerged that under RNTCP, PPs and NGOs are being involved but demand for services and supply of services is not properly matched. It was found that 59.2 per cent of eligible PPs were willing to be involved in various schemes under RNTCP. 51 per cent PPs informed that detection rate and cure rate can be ...

The article investigates the link between health infrastructural inputs and outputs and their impact on economic growth in India during 1980–2005. The empirical investigation has been carried out by simple regression technique and Granger causality test. The findings confirmed that health infrastructural inputs have substantial impact on health infrastructural outputs such as crude birth rate (CBR), crude death rate (CDR), infant mortality rate (IMR), life expectancy at birth (LER) and couple protection rate (CPR). The Granger causality further confirms that there is presence of unidirectional causality from economic growth to CBR, CDR, IMR and LER at birth. But it does not find any causality between economic growth and CPR. The study also finds no causality from CDR to IMR, CDR to C...

This study identifies the problems related with target setting for immunisation service in Ethiopia. We adopted qualitative case study and drew concepts from institutional theory to collect data and analyse our findings. The findings showed that the plan of district health offices depends on the population data projected from census. Peripheral level health workers, on the other hand, enumerate the number of population in their respective localities. Subsequently, the ambiguity occurs from mismatch between the target from census and number of eligible children counted by health workers. Health facilities are expected to follow the target given from district health offices, which is higher than the local count in most cases. Thus, we found most peripheral level health workers considered tha...

Despite the interest in social capital, the value of this concept for health promotion and inequality reduction has yet to be firmly established. Although it has been suggested that the concept of social capital may be useful for exploring factors within individual and social health and for identifying different approaches to community-based healthcare, the interlinkages between primary healthcare, social capital and community development have yet to be considered in much detail.
The purpose of this article is to analyse, through the examination of a case study from southern India (Tamil Nadu), how far social capital can trigger community participation and an effective process of enhancement of basic health system in rural areas, while conserving local medicinal knowledge. The case study i...

This article presents a conceptual structure for enabling knowledge creation and knowledge transference in the knowledge-intensive health services. Identifying the proposed critical elements in health management, it analyses the required organisational characteristics for the change from a traditional context to a complex context. The conceptual framework applied to the hospital management highlights the differences between ’structure based on information’ and ’structure based on knowledge’. Hospitals which are focused on solving complex problems demand new service strategies for the creation and transference of knowledge. A qualitative research was applied in a in-depth case study and conducted as an exploratory investigation in the Vascular Medical Center, that in...

In recent years, many healthcare organisations in the world have been changing in the direction of a professional management leaving behind a model of physician dominance. As one of the most essential functions in hospitals, professional management is vital in order to achieve performance efficiently. The main point which must not be ignored is that the education, the knowledge and the skills are key elements for professional management in the healthcare sector. This issue is also currently one of the major topical issues in the Turkish healthcare system to overcome existing management problems. The study assesses the roles of physicians for the future environment by analysing the management roles of Turkish physicians based on available data.
This article presents the findings of the study. 72.6 per cent of the respondents reported one or more symptoms of RTI. Only 31 per cent of the respondents were aware about RTI and 21 per cent about HIV/AIDs. Bivariate analysis indicated statistically significant association between educational level, age at marriage, place of delivery and awareness about RTI with presence of self-reported symptoms of RTI among the study population. Improving literacy and increasing awareness level among women about reproductive health is needed to reduce incidence of RTI in the study area. (Source: Journal of Health Management)

The study examines whether widow/widower status, educational level and family income influence post-tsunami trauma of survivors. The data were collected 14 months after the tsunami from 416 adult survivors in nine semi-urban habitations of Nagapattinam district in the state of Tamilnadu (India). Post-tsunami trauma was assessed using inventories of post-traumatic stress, depression, negative and positive affectivity, physical health symptoms and anxiety. Post-tsunami trauma of survivors has increased with increasing age, female gender and more exposure. Controlling the confounding effects of age, gender, and exposure; widow/widower status, illiteracy and lower family income are found to intensify trauma and are the risk factors for post-tsunami morbidity. Only illiterates do not report mor...

This article examines whether variation in media exposure and gender would yield diverse stigma perceptions on HIV AIDS, and finds the mechanism behind these phenomena. It employs data from 2005–06 NFHS (National Family Health Survey) for Gujarat, a major western state of India, supplemented with some qualitative information. Logistic regressions were undertaken modelling five different stigma perceptions for women and men in varying contexts such as maintaining secrecy of HIV infection, care and support to and service from HIV infected persons. A chi-square test on response from men and women reveals that significant gender differential exists in terms of nature of HIV-related stigma perceptions. While women prefer to keep family member’s HIV infection secret, men rebuff provi...

Socially constructed gender has significant influence on the health and Household Health Expenditures (HHE). From the gender and health literatures, three things are clear: most of the studies are macro in nature and based on secondary data; there is lack of research on the issues of gender and health, and out of whatever researches exist, most focus on the female sex; and as health conditions are localised, there is a need for research on the micro aspects of gender and health. In this connection, the present article is a modest attempt to study the gender bias (or unbais) in the HHE based on primary data collected from four districts of Odisha, India, by adopting multi-stage random sampling method. To substantiate the gender bias (or unbais) in health expenditure, multiple regression ana...

The major objective of this article is to take stock of the overall healthcare infrastructure for children in the district hospitals in the state of Karnataka. Governmental healthcare initiative in India can be traced back to the year 1951. Interestingly, India was the only country in the world to launch a Family Planning Programme intended to reduce population growth. This was renamed as Family Welfare Programme in 1977. Thereafter, in 1983, the National Health Policy was institutionalised with the objective of reducing Infant Mortality Rate (IMR), Neo Mortality Rate (NMR) and Child Mortality Rate (CMR) by the year 2000. Concurrently, programmers like Prevention of Acute Respiratory Infection (ARI) and Oral Dehydration Therapy (ORT) came into existence in the country. As follow-up of thes...

In the labour-intensive health sector industry the quality of services is ultimately linked to the skills, motivation and satisfaction of the workers providing the healthcare services. Thus in order to study the job satisfaction level of the healthcare providers, so as to suggest measures to enhance it, the present study was carried out at CGHS which is one of the biggest public health sector organisations in the country and caters to a huge section of the organised sector.
Understanding the healthcare provider’s work satisfaction is crucial to the understanding of the dynamics of the healthcare provider’s work force. Job satisfaction has been shown to be important in the retention of the healthcare providers in a given community as low job satisfaction has been associated with...

The need for control of population growth is quite pressing for India, as it hampers the country’s growth prospects and leads to sustenance of poverty and unemployment, etc. Hence, India was the first nation to have officially launched the family planning programme. However, the health care system created for delivery of various health and family welfare services has not been effective enough to curtail the enormous population growth as well as to better the general health standards of the common masses. The article analyses in detail the various factors responsible for it. It brings out the lacuna in the health system as well as the unmet needs and deficiencies at different levels. The article not only throws light on the qualitative inadequacies in the health manpower but also sugges...

The attempts of developing Health Accounts are on the learning curve in most of the developing countries. Contrary to this most of the European countries and few countries in the American continent have produced health accounts. Though India did produce its Health Accounts very recently, more distance needs to be covered at the state level. Health Accounts are considered to be more useful as a policy tool for bringing in corrective measures as regards the financing of disease specific interventions. The conceptual clarity of developing such accounts are also in infancy in most of the developing countries due to the fact that the OECD model of System of Health Accounts (SHA) as well as the Producers’ Manual of Health Accounts (WHO, USAID & WORLD BANK) cannot be easily adopted and ...

Health Sector Reforms in Orissa: The Disconnecting Paths

Any health sector reform aims at improving the health status of the people by affecting the organisational structure, governance system and financing variables. The health system has to perform a number of functions in order to enable more people to avail health benefits. In this context this analyses health sector reforms, approaches and challenges faced in Orissa during the reform process. The reform effort has not made substantial improvement in rural health indicators as indicated by NFHS-III. Most of the organisational and institutional changes were introduced as a part of a donor support programme and this has not been integrated into the system. Thus the health system did not improve on a sustained basis in order to achieve health sector goals. As a part of public financing, the sta...

Family care experiences of caregivers and care receivers in the context of HIV/AIDS were studied in Mumbai, India, following van Manen’s hermeneutic phenomenological approach. Data were gathered through conversational interviews and were subjected to holistic and highlighting thematic analyses. The findings underscore that interactions with the formal health care system contributed to the essential meaning of participants’ experiences even though the context of care was primarily familial. These interactions included instances of violations of testing and confidentiality guidelines and refusals to provide treatment in private, and some public health centres and the provision of instrumental and affective support by the voluntary sector. The poor quality of care in public sector...

This article makes an attempt to review the relationship between the TB control experience and external aid for TB control over a period in India. Expenditures by the Revised National Tuberculosis Control Programme (RNTCP) had increased considerably from US$ 36 million in 2002 to US$ 67 million in 2008 and according to the WHO, in Global TB Control Report for 2008 there were no funding gaps reported for TB control since year 2002. More funding came from grants and a World Bank loan. By March 2006, 100 per cent of the population had been covered under RNTCP. Since the inception of RNTCP more than 8 million patients have been initiated on treatment with the treatment success rate of 86 per cent. This suggests that having a committed programme with good funding has helped in improving the per...

This article brings out the importance of the reproductive health problems of the older women in the context of the recent reproductive and child health (RCH) programmes and examines the need to integrate services to the older women with the existing RCH programme, which is otherwise neglected. The study also identifies major challenges in the provision of RCH services to older women as women beyond 40 years of age need both maternity and menopausal related services simultaneously as many women in their 40s reach menopause and many others continue to bear children. Further, this article also identifies the need to address other reproductive health problems like cancers to breast, uterus and ovary, hysterectomy and other gynaecological disorders as well in the programme.
The objectives of this study are: (i) to estimate the relative technical efficiency (TE) and scale efficiency (SE) of a sample of public hospitals in Tamil Nadu; and (ii) to demonstrate policy implications for health sector policy makers. The Data Envelopment Analysis (DEA) approach, a well-known operations research (OR) technique for evaluating the relative efficiency of a set of similar decision making units (DMU), was used to estimate the efficiency of these hospitals. To do so we made use of the data collected from the Directorate of Medical and Rural Health Services (DMRHS) for 29 districts of Tamil Nadu in 2004–05. The output data included are outpatient visits, number of inpatients, number of surgeries undertaken, number of deliveries and number of emergency cases. The numbers...

The objective of this study was to assess the need for health care management training in Pakistan using semi-structured interviews of key informants in different cities. We asked experts about the existing HCO management training opportunities, demand for such skills and its impact on the future of health services delivery. Unanimous support for HCO management training emerged in the wake of dissatisfaction with limited opportunities for such training in the country. The participants agreed that trained HCO managers can help introduce modern management practices in health care service delivery, thereby making the HCOs more efficient and effective. We conclude that well-reputed management schools would be performing an important service to the improvement of the health sector in Pakistan i...

The first is to examine the extent of stress, negative mental health outcomes and variations in mood states experienced by shift workers (N = 100) and a control group of day workers (N = 100) employed in Feeder Balancing Dairy, Ramnagar Varanasi and Kanpur, Dugdth Utpadak Sahakari Sangh (brand name Parag, Ltd. for both organisations). Second, to find out the relative contribution of stress variables and mood states in the prediction of mental health outcomes separately for both shift workers and a control group of day workers. Our overall findings concur with other studies and provide evidence that shift workers significantly experienced higher levels of job and life stress, higher indices of negative mental health outcomes (as measured by GHQ – ...

The present article discusses the nature of quality of life (QOL) in general and critically evaluates its application in the area of health and illness. As regards health related quality of life (HRQoL), it is now widely recognised that treating a disease in a purely biomedical framework, where emphasis is only on curing the disease and on providing prolonged life to the patient, is not sufficient. Now the focus is also on how to enhance the QOL of patients. In this context this article attempts to identify and critically evaluate some of the contemporary issues relevant in the area of HRQoL. The issues covered in this article include relatively simple issues related to the meaning, importance, usefulness and prevalent approaches in the area of HRQoL research and complex issues such as (i)...

It is observed that 54.4 per cent of the younger mothers (< 25 years) experienced neonatal mortality, while about 46 per cent mothers of age 25 years and above experienced neonatal mortality. Male children have a higher neonatal mortality rate as compared to female children. The analysis reveals that mothers’ age at birth is inversely related to neonatal mortality. Children born to mothers aged below 20 years are 1.5 times more expected to die in their neonatal period as compared to children born to mothers between 20–29 years. Children born less than two years after a previous birth are about 2 times more likely to die in thei...

Choice of poor people towards the type of health care services (public or private) has not been much discussed. This is mainly because of the underlying assumption that their choice has always been restricted to public health care services. The inefficiency of the public health care system in India has created a vacuum among the poor people regarding their preference for health care. Their tendency to transform from accessing public to private health care services is discussed in this context.
The study was conducted among the low-income urban households of Kerala. It was noted that there is a strong willingness among these categories to access private health care services. Efficiency of the health care services was pointed out as a major reason for this. However, issues such as the high c...

This article reviews literature on the characteristics and role of TBAs in South Asia with an emphasis on India. The aim was to assess the contribution of TBAs in providing maternal and infant health care service at different stages of pregnancy and after-delivery and birthing practices adopted in home births. The review of role revealed that apart from TBAs, there are various other people in the community also involved in making decisions about the welfare and health of the birthing mother and new born baby. However, TBAs have changing, localised but nonetheless significant roles in delivery, postnatal and infant care in India. Certain traditional birthing practices such as bathing babies immediately after birth, not weighing babies after birth and not feeding with colostrum are adopted i...

Many low-income households in developing countries cannot afford the high costs of medical treatment. They often resort to cost-saving, but risky options for their treatment. The article uses data published in earlier studies in India to analyse the treatment decisions of low-income households. We compare parents’ decisions about their children’s treatment with those about their own treatment. The analysis shows that parents often adopt a cost-saving (and risk-taking) attitude about their own treatment, relative to that for their children. This difference is found even when the parents and children have the same sickness profiles and also, when the parents have a higher incidence of serious sicknesses. There is a need for policies to increase affordable access to medicines. Thes...

This study was designed to obtain information related to work schedules and environment, professional training, financial status, accommodation facilities and health of resident doctors working in a large teaching hospital in the state of Gujarat in India. A pre-designed and pre-tested questionnaire was used to obtain necessary information. These questionnaires were used later on to derive data and were analysed. The study was done during a period of three months, from 1 October 2004 to 31 December 2004. Out of the 200 residents contacted, 160 responded and out of these only 154 qualified for the final analysis. It was observed that more than 50 per cent did not receive any stipend. Teaching, training and library facilities have not been considered satisfactory by majority of them. Their h...

The second objective is to determine the reasons given by the individuals who did not use the referrals given. The methods used during this article are: A retrospective study looking at who uses the referrals given to them in the ED during a six month ED-based pilot programme. A second prospective study using a phone survey was then done; those who did not use the referrals were contacted, which helped in determining the reasons for the non-usage. The study was approved by the Institutional Review Board (IRB).The results of this article indicate that there was a significant differ...

Patients' Satisfaction with Quality Attributes of Primary Health Care Services in Nigeria

This study, based on administered questionnaires, examines patients/users’ satisfaction with quality attributes of health care services at the primary level facilities in order to provide feedback to health personnel and management for change and learning. The study employs correlation and multivariate regression analysis to determine the quality attributes that determine overall satisfaction with care. Our findings suggest the need to emphasis ‘empathy’ for care providers; and while a reasonable level of physical facilities should be provided, care providers have the task to communicate their technical competence to care seekers to ensure capacity utilisation at the primary level.
Health care workers play a vital role in patients’ well-being. Their satisfaction is an important issue for all health care managers. Health care providers can be categorised into two groups on the basis of their financial goals and sources of funding. The two categories are: not-for-profit (NFP) publicly funded organisations; and for-profit (FP) privately funded organisations. In the United Arab Emirates (UAE), FP and NFP health care organisations operate under similar government regulations and workforce diversity. However, the objectives and source of funds for FP and NFP health care organisations always affect the management style and decisions. These differences—in addition to differences related to job satisfaction and personal development—such as medical equipment,...

A lack of management capacity has been identified as the key stumbling block to attaining the goals of health for all in South Africa. As part of the overall management development process, this research aimed to identify the skills important for health services management and to evaluate managers’ self-assessed proficiency in each of these skills. We also examined the impact of past training on perceived competency levels. A cross-sectional survey using a self-administered questionnaire was conducted among 404 hospital managers in the South African public and private sectors. Respondents were asked to rate the level of importance that each proposed competency had in their job and to indicate their proficiency in each skill. The results of this study show that both the public and priv...

Public health care system is a crucial element of social welfare, particularly in underdeveloped areas where a majority of the populace lingers in abject poverty and deprivation. The system is typically assumed to be cheap and based on a public welfare perspective. However, it is argued that in the name of reform process, it is usually targeted as a soft option. Using a panel data of all the (reported) hospitals in the Indian state of West Bengal over the seven-year period, 1997–2003, we try to answer this question. The framework of input–output data is used in order to assess the performance of these hospitals. The non-parametric Data Envelopment Analysis (DEA) is used to assess the efficiency of these hospitals over these periods. The results do suggest initial adverse impact...

This article, therefore, focused on reading, primarily to investigate its effectiveness as a receptive communication skill, through which the public can be better educated about HIV/AIDS and thus, dispel their misconceptions about the disease and its patients. An experimental method was employed, using 220 respondents. Two separate instruments—attitude scale/questionnaire and an HIV/AIDS-related story—were used. The results showed that absolute majority of respondents (78.2 percent) from the experimental group, which actually read the select HIV/AIDS-related story demonstrate better knowledge about HIV/AIDS than do the respondents from the control group. Thus, the study has established reading as an effective communication strategy for educating people and dispelling their misc...

Adolescence in girls is an unexplored research area. The reproductive and sexual stage of adolescent girls, complemented with lack of forceful advocacy strategies, puts them on the verge of a myriad of psychological and social pressures and increases their vulnerability of falling prey to risky behaviours. The focus of this experimental research is the impact of Information, Education and Communication (IEC) amongst adolescent girls in the age group of 13–19 years in the Mandi district of Himachal Pradesh (HP). An inferential analysis of the impact of achieved and ascriptive variables on the knowledge and awareness of reproductive health, gender stereotypes and sources of health information of adolescent respondents has been analysed. The research reveals that adolescents lack complete...

The United Nations’ Millenium Development Goals (MDGs) work in pervasive and powerful ways in the international imagination, by naturalising a strategic set of goals, indicators and targets, and in doing so, either opening up, or precluding and prescribing particular possibilities for understanding the state of the world and people's experiences. Our departure point for exploring the need for alternative research routes is therefore an engagement with the conceptualisation of ‘development’, the hopeful product of the 2015 target. This conceptual critique is three-fold: 1. There is a slippage in the discourse of the MDGs between ‘poverty’, ‘health’ and ‘development’ and consequently, the relations between different goals are poorly theor...

The United Nations Development Programme's (UNDP) Millennium project argues for the importance of sexual and reproductive health in the achievement of all Millennium Development Goals (MDGs). Sex education programmes, aimed principally at the youth, are thus emphasised and are in line with the specific MDGs of reducing the incidence of HIV and improving maternal health. In this article, I analyse recent South African sex education and Life Orientation (a learning area containing sex education) manuals. Danger and disease feature as guiding metaphors for these manuals, with early reproduction and abortion being depicted as wholly deleterious and non-normative relationships leading to disease. I argue, first, that these renditions ignore well-designed comparative research that calls into que...

The relevance and appropriateness of western oriented psychology in practice and research is a concern in developing and non-western contexts. It is difficult to address this problem from any alternative position other than the western academic frame if one is situated in a tertiary educational institution in South Africa. In acknowledgement, this article explores the academic context including some local voices from the field in a search for possible congruent research methodologies, which may echo knowledge systems of the traditions of the local context in South Africa and its broader context in the continent. Constraining factors to the development of an appropriate praxis have been suggested to include epistemological issues, western academic hegemony and the perceived elitism of psych...

We present a longitudinal case study of lay women's participation in a project seeking to facilitate home-based care of people dying of AIDS in a rural community in South Africa, drawing on four sets of interviews conducted with volunteers over a five-year period. We link participation in the project to three dimensions of women's agency: their knowledge and skills, their confidence; and their personal experiences of efficacy. We show that whilst the experience of participation enhanced each of these dimensions of volunteers’ agency at various stages of the project, the empowerment that did take place appeared to be limited to women's project-related roles, rather than generalising to other areas of their lives beyond the project. The project had limited impact on women's ability to ...

The focus is on a specific aspect : how the aid emphasised by Goal 6 is channelled towards community groups responding to the HIV/AIDS crisis. It does so by exploring the institutional mechanisms used by one of the key international organisations involved in the realisation of the MDGs—the World Bank and its Multi-Country HIV/AIDS Programme (MAP) in Kenya, Tanzania and Uganda. The article explores the role of local government, rivalry between different forms of civil society organisations, the problems associated with effective delivery and community feedback, d...

The objective of this article is to locate disability issues within the discourse of the Millennium Development Goals (MDGs). The idea is to question the failure of the goals addressing disability. MDGs cannot work as a universal remedy. It is critical to foreground the meaning of disability and underscore the reasons for the disabled people's absence from the agenda of the MDGs. Further, I discuss the ways in which state policy has addressed ‘disability’ in a globalising context. Finally, I outline the paradox of identity politics and its nuances based upon an understanding of the issues and related questions from my own experiences as a disabled Indian woman, having to contend with the existential realities of a visible physical disability. The plea is to expand the democrati...

We welcome readers to the second special issue (11.2) of the Journal of Health Management in 2009. We hope the readers find the articles and various reviews enriching and provocative, both in terms of the range of ideas and critical approaches addressed. The key theme of this double issue concerns the political limits of mega-development projects such as the Millennium Development Goals (MDGs). The primary focus of the articles collected here is to provide an insightful, constructive and in-depth critique of the United Nations (UN) MDGs along with critical deliberations on their short- and long-term implications not only for health management but also for a wide range of issues around development and social change.
This article examines the policy of using target-setting and measurement to deliver on the United Nations’ (UN) development goals. Using evidence from similar monitoring and management strategies in the United Kingdom (UK), we question the purpose of using process and outcome targets and suggest that this approach can be counter-productive. It can also lead to a situation where maintaining public relations and image is prioritised at the expense of making real impacts on key development issues. While the UN's aims are praiseworthy, we suggest that the somewhat simplistic methodology adopted is damaging to the very people that the UN is seeking to help.
This article aims to explore the psychosocial practices in one of the sites that respond to vulnerable women who suffer abuse in their couple relationships: the criminal justice system. A discursive theoretical–methodological perspective is used (Foucault 1969; Ibáñez and Iñiguez 1997; Wetherell and Potter 1993), based on the discourse analysis and their positions that can be found in the practices of professionals in the criminal justice system (judges, prosecutors, lawyers, police), as well as in some of the narratives of vulnerable women. These accounts have been gathered through participant observations in courtrooms and police stations where complaints are filed, and through in-depth interviews. The results show the influence and the effects of: (a) an &lsqu...

There are several issues to be faced in developing mental health services in South Asia if they are to be culturally and socially appropriate to the needs of the communities in the region. The meanings of mental health relevant to culturally appropriate service development can be obtained by exploring local notions of well-being, systems of care available to people and current practices among those seeking help for mental health problems. Participatory research carried out in communities in Sri Lanka affected by prolonged armed conflict and by the 2004 tsunami clarified the nature of well-being as perceived by communities themselves. Subsequent development of mental health services for Sri Lanka can be based on community consultation, using methodologies and interventions that involve the ...

In the context of the targets for primary health identified by the MDGs this article evaluates the link between decentralisation and positive outcomes for women and children. Using India as a case study, the article traces the changes in health attainments as a result of decentralisation reforms. The evidence presented, drawn from the experiences of two states, speaks to the relevance of such a link. Despite the heterogeneity of contexts and in implementation, in general democratic decentralisation has enhanced health outcomes for women in the selected village Panchayats. However, the article unearths significant differences in the impacts of decentralisation between the two states. The variations in outcomes between the two states are found to be linked to the architecture of decentralisa...

Water is essential to human life, health and development. Prepaid water metres are a popular yet controversial approach to extending water services to the poor. They operate on an upfront payment system which automatically disconnects water if households do not have enough money to replenish supplies. While prepaid water metres have been criticised on a number of levels, this article focuses on the equally problematic discourses used to promote them amongst the poor. By drawing on a case study of a poor South African community's struggle against prepaid water metres, this article highlights how prepaid metres were promoted using psycho-education campaigns under the guise of ‘participation’. It also documents community resistance to the campaigns ending in a court battle that ru...

The present article analyses the association between the way in which public policy conceptualises poverty (in discourse and practice), the conception of poverty of people who live in extreme poverty conditions and the strategies they develop to affront it, in the light of their participation in the Puente Programme in Chile. The data emanate from research work carried out from a qualitative perspective using in-depth interviews with people considered to be living in extreme poverty and including a gendered analytic. The ‘Puente Programme’ is part of Chilean public policy aimed to reduce levels of poverty and extreme poverty, in consonance with the Millennium Development Goal (MDG) One as stated by the UNDP.Indeed, such policies seem to have had the effect of generating a self-p...

In this article I critically reflect on the hegemonic modernist framework of development by focusing on its reflection and application in the United Nations’ (UN) Millennium Development Goals (MDGs) and practices. Not only do I find it falling short of explaining, understanding or bringing about global development, but quite the opposite, since it feeds into and reproduces the status quo. While I plead for serious (re)definitions, and genuine global commitment towards the (re)solution, of the current micro/macro social problems, which are interdependent and need to be treated in connection and at once, I pick four urgent areas of transformation as sub-topics for the present discussion. They include the (meta)theoretical framework and discourse of development, historical and contextua...

Health Inequality in Resource Poor Environments and the Pursuit of the MDGs: Traditional versus Modern Healthcare in Rural Indonesia

The article examines health inequalities and the impact of changing healthcare provision in rural Indonesia. Traditional medicine is often the only source of medical care for a majority of the population in rural Indonesia. However, the pursuit of the Millennium Development Goals (MDGs) requires the provision and implementation of modern healthcare systems. Using case studies from four rural districts in Kaledupa, a remote island in southeast Sulawesi in Indonesia, the study shows that although modern healthcare facilities are present in the sampled island, they seem to be remote with limited access in comparison with the number of traditional practitioners. High costs, cultural beliefs, distrust and distance to modern healthcare facilities appear to be the most common reasons for people o...

This article critically analyses the mechanisms through which international expertise has come to influence the practice of rapid testing within the South African context. Through this analysis the article aims to illustrate the unintended consequences of the process of HIV/AID globalisation and internationalisation. While the article argues that knowledge claims about the pandemic are always produced through an interaction of local and international processes, it contends that these interactions are characterised by inequalities when the experiences of local people and contextual factors are not adequately fed into the process of policy development and implementation. The article concludes by arguing that more intercultural inquiry that prioritises cognitive justice is required if we are ...

This article reflects on a gender mainstreaming intervention in the East African region to explore the role that gender mainstreaming can play in achieving the Millennium Development Goals. In this article we discuss how gender mainstreaming has, at times, functioned as a retreat from women's equality and is used to render feminist perspectives more palatable to those who resist them. Far from being a simple critique of gender mainstreaming this reflects the broader tensions and debates that are shaping what gender has come to mean in different contexts. This brings difficult tensions over who develops a gender mainstreaming agenda and who claims to have expertise on gender. We explore how much is at stake in claims to represent the ‘beneficiary’ groups and the ways that donor ...

In this article we reflect upon the relationship between the Millennium Development Goals (MDGs) formulated by the United Nations (2000) and the migration phenomena that is characteristic of the present globalised society. First, we argue that the Millennium Goals have not sufficiently considered migration processes as a structural phenomenon. Development goals must take in account this emergent phenomenon in post-industrial societies in order to achieve human rights and social equity and to promote the well-being of all people in their origin and host societies.Second, by interpreting the results of an ethnographic study carried out in social services aimed at migrant women in the city of Barcelona, while focusing on the 3rd Millennium Goal: gender equity and the empowerment of women, we ...

This article discusses how Erasure came to be and its consequences. The plight of those Erased who are without legal status even today is discussed with regard to issues concerning health and access to health services from which they are excluded through a combination of nationalist and neo-liberal policies. As such, they not only form a section of society that can be completely overlooked by the implementation strategies of the Millennium Development Goals (MDGs), but can also be seen as indicative of some of the conditions that may also effect other vulnerable individuals and groups such as those emerging from various conflicts, precarious workers, migrants and ethnic minorities.
The Millennium Development Goals are framed within the post-war discourses of development that also gave us Basic Human Needs and Human Security. The Goals set out a consideration of the failures of earlier strategies along with an agenda for the accelerated reduction of poverty and its accompanying human insecurities. Though the more critical aspects of the MDG discourse were sorely needed, they also left space for the repetition of earlier top–down development strategies, and, more generally, for a (re)vision and wider implementation of globalised intervention by developed countries into the less-developed. In this discourse developed countries identify needs on the part of the less-developed and then supply these needs. The ‘need’ discourse focussed on here represents ...

We welcome readers to the first special issue (11.1) of the Journal of Health Management.
This article studies the situation of health awareness among the inhabitants of Chars (river islands) of Nalbari, Borpeta and Jorhat districts of Assam. The health awareness is measured by health expenditure. The findings reveal that all the variables—caste-residence compound status, educational attainment, economic consideration of the family, family structure and primary occupation of the families have desirable impact on health awareness. In particular, the inhabitants of Char areas of Nalbari and Borpeta districts are seen to be least health aware. Thus on the basis of the findings of this study, it can be stated that in spite of launching several schemes for the upliftment of Chars (like forming Char area development authority, forming a separate state ministry for Char developm...

This article explains the procedure adopted in developing a scale to determine the barriers to paediatric eye care. A 22-item barrier to paediatric eye care scale was specifically developed. The scale, along with the full-length questionnaire, was pre-tested and later administered to 207 parent-respondents at Sadguru Netra Chikitshalaya (SNC), Chitrakoot, in the state of Madhya Pradesh (India). A fiveround factor analysis variable deletion process resulted in a three factor structure. Although this procedure reduced the number of items in the scale from 22 to 9, the variance explained by the factors increased from 61 to 78 per cent. The three factors were labelled as economic, logistic and perception of service. Regression analyses of the three factors/barriers, economic, logistic and beli...

Making Drugs Affordable for the Poor in a Tertiary Care Hospital: A Case Study

The public sector health care facilities are expected to provide health care services including medicines at low costs. The costs are escalating and the poor population is unable to afford them. The most important factor for high costs is prescribing of expensive newer drugs irrationally due to the pressure of pharmaceutical companies. An attempt to promote rational prescribing through interventions, such as essential drugs lists, clinical protocols and training has not been very effective. The margin of profit manufacturer to retailers is high. Two attempts to supply medicines at low cost, one through a not-for-profit association, Rajasthan Medicare Relief Society (RMRS) and the other through a public sector undertaking, Rajasthan State Consumer Cooperative Federation (COOPS) are describe...

Is Lifestyle Influencing Morbidity among Elderly?

The main focus of the article was to examine the type of lifestyle adopted by the elderly and its effects on their health conditions and this was based on the data available from the National Family Health Survey–2 (NFHS–2). The study found that lifestyle adversely affected health and increased morbidity conditions among the elderly. Lifestyles such as alcohol consumption, regular smoking and tobacco chewing had adverse effects on one's ability to control diseases. It was observed that asthma was higher among the elderly who smoked regularly, consumed alcohol and chewed tobacco. About 144 persons per 1,000 population who consumed alcohol were suffering from asthma. Similarly, 143 persons who smoked regularly and 97 persons who chewed tobacco were suffering from asthma. Though T...

This article applies the concepts of organisational learning in general and absorptive capacity in particular to the efforts by rural hospitals to continuously improve patient safety performance. Strategies are discussed through which rural hospitals might better identify patient safety related information, techniques and technologies and convert them to organisational use.
When Staff Create the Organisational Culture: A Case Study in the Spanish Emergency Health Care System

Emergency units suffer from continuous overload because all types of users demand the service. The literature shows that in general, the percentage of non-urgent users varies from 20 per cent to 80 per cent, depending on the type of centre analysed, the research approach or the methodology. While some studies have analysed this phenomenon focusing on the users, the current research adopts a different perspective. In this article, we try to explain how the break-down in the emergency services affects the work that the staff do. Drawing on evidence obtained from a six-month ethnographic study in two Spanish public hospitals, we conclude that in this overloaded context, official definitions of emergencies and formal classification protocols are completely useless. Exploring the staff's percep...
