            <title>Pyogenic granuloma: a rare side complication from an orthodonticappliance [Clinical Section]</title>

            <description>This case report discusses a rare side effect associated with the use ofa fixed quad helix orthodontic appliance. A 14-year-old healthy girl presentedwith a painful enlarging mass on her tongue, which was causing distress toboth her and her parents. Investigations confirmed that the mass was a pyogenicgranuloma and management involved surgical excision of the mass and removalof the quad helix appliance.             <description>We present two cases where unfavourable splits occurred during a bilateral sagittal split osteotomy (BSSO) of the mandible. Both cases were investigated post-operatively with CBCT with the aim of identifying the potential cause of the unfavourable split and to establish accurate localization of the condyle in the glenoid fossa following intra-operative attempts at proximal segment control. CBCT should be considered for evaluation of the mandible following an unfavourable split where it is deemed that plain radiographs would not provide adequate information.             <description>This case report describes the combined orthodontic and orthognathic management of a 14-year-old girl affected with Apert syndrome. She presented with a severe Class III skeletal relationship, midfacial hypoplasia and an large anterior open bite. Intraorally, she had severe crowding, a narrow maxilla and lateral posterior crossbites. The patient was treated with a combination of removable and fixed appliances, a transpalatal skeletal distractor and Le Fort I surgery.             <description>Conclusion: Angular cheilitis is a multifactorial condition that can occur in a small percentage of patients during orthodontic treatment. Good oral hygiene may be associated with a reduced risk. A new clinical grade of angular cheilitis is suggested that may help future research. Further studies are required to investigate the microbiological features associated with angular cheilitis occurring in orthodontic patients, as well as associations with medical conditions, such as asthma.             <description>This case report illustrates the treatment of a 12-year-old boy with maxillary deficiency using miniscrew implants. The patient rejected the use of extraoral appliances and future surgical correction; therefore the patient was treated using Class III elastics connected from two mandibular miniscrew implants to an upper removable appliance. The miniscrews were inserted between the permanent canines and first premolars under local anaesthesia. The treatment lasted for 8&amp;nbsp;months after which favourable correction of the malocclusion was observed. The SNA and ANB angles increased by 3&amp;deg; and the IMPA increased by 4&amp;deg;.             <description>Conclusions: Travel commitments for trainees have reduced as a result of introducing the web-based resource, but not as expected. Demands on academic staff have not reduced but have changed. The resource has had positive effects on postgraduate orthodontic teaching and learning. Important themes of interest emerging from the data are improvements in the flexibility and efficiency of learning and the value of the resource as a repository of information and in the organization of teaching and learning. Despite the popularity of this web based learning resource, trainees continue to value the opportunity to interact face to face with their teachers and peers and are prepared to travel for organized teaching sessions.
