            <description>MB Murthy, B MurthyJournal of Postgraduate Medicine 2011 57(4):341-342A patient of essential hypertension stabilized on 10 mg amlodipine once daily developed brownish black petechial non-blanching macular rash bilaterally covering the limbs below the knee and dorsum of the feet. History, general and clinical examinations and lab investigations revealed no abnormalities. Temporal association of the onset of rash with amlodipine use, inability to explain rash by natural history of hypertension, possibility of rash with amlodipine and rash resolution on dechallenge placed this reaction in Naranjo score of 6, a probable adverse reaction to amlodipine. Hence the drug was replaced by enalapril. The rash resolved completely over a period of eight weeks. (Source: Journal of Postgraduate Medicine)</description>

            <description>We report a case of a 59-year-old man who presented with diverticular bleeding and developed prolonged ileus postoperatively. After observing arthritic joints on examination, further questioning revealed long-term, high-dose steroid use for analgesic effect. Failure to produce an effective cortisol response was due to adrenal suppression from continuous steroid use. Immediate improvement of his ileus was seen after steroid replacement. Unreported self-medication is a frequent problem encountered in developing countries. RAI can be easily missed and requires a high index of suspicion in any patient who fails to respond to conventional treatment or with long-term steroid use. She developed a cardio-respiratory arrest and was resuscitated with aspiration of cerebrospinal fluid from the ommaya reservoir kept during the primary surgery. Keeping all the patients under strict surveillance for stoma patency is mandatory and in addition, ommaya reservoir in certain high-risk patients may be a useful option for achieving quick ventricular access by medical and nonmedical personnel in case of deterioration. This case is the first reported case of acute deterioration after ETV from India. Previously, 14 such cases have been reported worldwide and only 2 of them have survived. Both parasites produce essentially similar clinical presentations in man, related mainly to the pathology of the lymphatic system. Filariasis is endemic in 17 States and six Union Territories, with about 553 million people at risk of infection. The Government of India has accorded a high priority for elimination of this infection through mass chemotherapy programme (annual, single dose of Diethylcarbamazine citrate, i.e. DEC - 6 mg/kg of bodyweight, plus Albendazole repeated four to six times).             <title>Esomeprazole-induced photoallergic dermatitis</title>

            <description>We report here a 58-year-old lady with prior history of propylthiouracil and carbimazole-induced photoallergy, who presented with heartburn and dysphagia. She was diagnosed to have erosive esophagitis and was treated with esomeprazole, following which she developed photoallergic dermatitis. It improved on cessation of the drug and did not recur on subsequent treatment with ranitidine. Naranjo score for this adverse drug event was 8, thereby making it a probable adverse drug reaction. This reaction may be due to sulphur moiety, which is common to all these drugs. Physicians must be aware of this possible side-effect, especially in patients with prior history of photoallergy to other drugs. (Source: Journal of Postgraduate Medicine)</description>
