            <description>CONCLUSIONS: Governments wanted a national strategy, national framework and agreement on a measuring agency. Good general agreement existed about reporting criteria. A partnership model may lead to quicker results given the difficulties within governments. NGOs and others need to continue collaborative advocacy. Monitoring is one of two steps that could help turn collections of services into self-regulating systems.

            <description>We report the case of a 17-year-old adolescent girl who presented with de novo self-mutilation and depressive symptoms three and a half weeks after the administration of 150 mg of Depot-Medroxyprogesterone Acetate (DMPA).
    METHOD: Clinical case report and literature review. Possible confounding factors are reviewed.
    RESULTS: The patient had no personal psychiatric history and no significant family psychiatric history. A DSM-IV diagnosis of &quot;mood disorder due to DMPA with depressive features&quot; was formulated. There was no evidence of abnormal personality functioning. The mental status exam and collateral information validated the severity of her condition.

            <description>CONCLUSIONS: Some patients with 22qDS who present with psychotic symptoms do not develop a psychotic disorder; therefore, the use of antipsychotics for every child or adolescent with 22qDS who experience psychotic symptoms is debatable. Long-term follow-up, phenomenological and treatment efficacy studies in larger samples are needed to determine optimal treatment of psychotic symptoms in children and adolescents with 22qDS.

            <description>This article will review the case of a young patient with mental retardation, autistic disorder, and Tourette Syndrome who exhibited a favourable treatment response preferentially to risperidone. His presentation, however, was complicated by an exquisite sensitivity to risperidone displayed in the form of recurrent oculogyric crises. In this article, we will outline a review of the case, a survey of the incidence and risk factors of oculogyric crises, as well as a review of the literature on risperidone sensitivity, followed by a review of alternate options for the prevention of oculogyric crises.

            <description>CONCLUSION: Multiple randomized controlled trials (RCTs) have established the efficacy of many of the SGAs in pediatric mental health disorders. These benefits however do not come without risk; both metabolic and neurological side effects occur in children treated with these SGAs. The risk of weight gain, increased BMI and abnormal lipids appears greatest with olanzapine, followed by clozapine and quetiapine. The risk of neurological side effects of treatment appears greatest with risperidone, olanzapine and aripiprazole. Appropriate monitoring procedures for adverse effects will improve the quality of care of children treated with these medications.

    Uncommonly, older children and adolescents can present with a history of enuresis. Resolution of enuresis followed the diagnosis and treatment of Attention Deficit Hyperactivity Disorder (ADHD) in each child in this case series. Subjects were three children with DSM-IV-TR ADHD, who presented with a history of primary nocturnal enuresis (PNE). Our results reveal that a subgroup of children with ADHD plus enuresis, when treated with stimulant medication, demonstrated resolution of enuresis as well as ADHD. These results suggest clinical implications for providers treating children and adolescents with enuresis and ADHD.

The majority of patients did not require ongoing psychiatric care. Further investigation into the differences between PEM physician and child psychiatrist diagnoses is needed to ensure patients and families receive accurate and consistent mental health information and recommendations from all members of their health care team.

This may be helpful in mediation.

A well-designed prospective longitudinal study utilizing structured diagnostic interviews to assign DSM-IV TR diagnosis from child hood to adulthood would improve the reliability of diagnoses and perhaps time for crystallization of psychopathology and clarification into more discrete diagnostic entities.

            <description>CONCLUSION: Most of the 34 patients traced seemed to have satisfactory personal and professional lives, in contrast to the high level of repeated suicide attempts. Repeated suicide attempts during adolescence appear to be one of the major risk factors in attempting suicide again within 10 years. This confirms the importance of preventive measures after a first suicide attempt.
