AbstractThe skin‐blanching assay is used for the determination and bioequivalence of dermatologic glucocorticoids (GCs). The exact mechanism of the production of blanching is not fully understood, but it is considered that local vasoconstriction of the skin microvasculature and the consequent blood‐flow reduction cause this phenomenon. Several factors influence skin blanching, including drug concentration, duration of application, nature of vehicle, occlusion, posture and location. The intensity of vasoconstriction can be measured in several ways: visual or quantitative methods, such as reflectance spectroscopy, thermography, laser Doppler velocimetry and chromametry. In literature, contradicting results in the correlation of the skin‐blanching assay with different tests to determine...

Unique features of PTCH1 mutation spectrum in Chinese sporadic basal cell carcinoma

Conclusion  Mutations and LOH in PTCH1 were also highly prevalent in Chinese sporadic BCCs. However, UV light plays a less role in causing these mutations, suggesting other potential mechanisms in the development of sporadic BCC in Chinese patients.
Conclusions  Our DE unit satisfies a genuine need. Its activity increased over 10 years, most likely because of the decrease in the number of dermatologists in France. Although our results are close to those reported in the literature, comparison with previously published studies is difficult because of the heterogeneity of the definition of DE.
Conclusion  Our data do not support prevalence of GST isoenzymes or polymorphisms as a protective effect against PLE. Especially a higher carrier frequency of GSTP1 Val105 as a protective factor against PLE which has been published before could not be proved. The GST genotypes GSTM1, GSTT1 and GSTP1 (including SNPs) seem to have no relevant association with PLE. (Source: Journal of the European Academy of Dermatology and Venereology)

Conclusion  The present study confirms that topical nitrogen mustard is a safe and effective therapy. The treatment response in patients with parapsoriasis was not statistically different from the response in patients with MF. This supports, that parapsoriasis is not a distinct entity, but an early stage of MF. Nitrogen mustard should therefore still be considered as an important treatment modality in patients with early stages (parapsoriasis) and later stages of MF either as monotherapy or in combination with other topical or systemic therapies.
AbstractField cancerization is a term that describes the presence of genetic abnormalities in a tissue chronically exposed to a carcinogen. These abnormalities are responsible for the presence of multilocular clinical and sub‐clinical cancerous lesions that explains the increased risks of multiple cancers in this area. With respect to the skin, this term is used to define the presence of multiple non‐melanoma skin cancer, its precursors, actinic keratoses and dysplastic keratinocytes in sun exposed areas. The multiplicity of the lesions and the extent of the area influence the treatment decision. Providing at least equivalent efficacy and tolerability, field directed therapies are therefore often more worthwhile than lesion targeted approaches. Photodynamic therapy (PDT) with its selec...

Conclusions  Our study increases the total number of FLG mutations. We clearly demonstrated that FLG loss‐of‐function mutations were significantly associated with AD in northern China. The FLG null mutations in the Chinese population differed not only from that in the European population but also from that in sub‐populations of Asians outside of the Chinese mainland.
AbstractIn the past 5 years since their US introduction, there has been a rapid proliferation of light‐based hair removal devices intended for home‐use. In the last 2 years in Europe, sales already run into many tens of thousands of units with well‐known multi‐national companies entering the market. These guidelines provide a definition of light‐based home‐use technology, to inform healthcare professionals about home‐use light‐based technology and encourage manufacturers wishing to sell in Europe to adopt ‘best practice’. The review presents the current status on standards and regulation issues and considers home‐use safety issues, encompassing human, device and electrical safety, given risks to the eyes and skin from optical radiation both to the consumer and per...

AbstractBacterial skin and soft tissues infections (SSTI) often determine acute disease and frequent emergency recovering, and they are one of the most common causes of infection among groups of different ages. Given the variable presentation of SSTI, a thorough assessment of their incidence and prevalence is difficult. The presence of patient‐related (local or systemic) or environmental risk factors, along with the emergence of multi‐drug resistant pathogens, can promote SSTI. These infections may present with a wide spectrum of clinical features and different severity, and can be classified according to various criteria. Many bacterial species can cause SSTI, but Gram‐positive bacteria are the most frequently isolated, with a predominance of Staphylococcus aureus and Streptococcus ...

Conclusion  Our study points to the possible role played by OPN in the pathogenesis of psoriasis and in reflecting disease severity. These standard therapeutic modalities used in the current study were associated with a significant decrease in PASI and OPN levels. They constitute highly effective therapeutic modalities for psoriasis, which might exert their anti‐psoriatic activity partially through altering the expression of OPN. Dermatology with the benefit of being able to visualize phenotypes was one of the first specialities reporting on the fascinating concordance in identical (MZ) twins in the 1920’s. Over the last 20 years, the heritability of skin diseases using twins has been clearly demonstrated, across a wide variety of traits including melanoma, polymorphic light eruption, psoriasis, eczema and acne. Other rarer diseases have also been shown to have a significant genetic basis such as lupus, sarcoidosis and lichen sclerosus. Following evidence of heritability for many skin disease the next step was Genome‐Wide Association Studi...

Conclusions  Systemic isotretinoin and antibiotic treatments in acne patients precisely caused variations in the microbial floras of several sites of the body, while isotretinoin was commonly more responsible than antibiotics. Knowing that alterations in the microbial colonization of the flora regions may preceede infectious disease and bacterial resistance, treatment options and follow‐up procedures in acne vulgaris should be carefully determined to reduce the risk of destruction of the microbial flora.
AbstractAs the most common form of skin cancer, basal cell carcinoma (BCC) is typified by locally infiltrative growth and a very low risk of metastasis. On occasion, however, this otherwise indolent neoplasm may behave aggressively, demonstrating deep tissue invasion and a high rate of postsurgical recurrence. The pathogenesis and determinants of such tenacious growth are not completely understood. Only 1% of all BCC’s achieve the status of ‘giant’, as defined in 1988 by the American Joint Committee on Cancer. In this article, the authors provide a comprehensive review of the scientific literature on giant basal cell carcinoma (GBCC) of the skin and report their experience with this rare tumour subtype.
AbstractIn wound healing, a variety of mediators have been identified throughout the years. The mediators discussed here comprise growth factors, cytokines and chemokines. These mediators act via multiple (specific) receptors to facilitate wound closure. As research in the last years has led to many new findings, there is a need to give an overview on what is known, and on what might possibly play a role as a molecular target for future wound therapy. This review aims to keep the reader up to date with selected important and novel findings regarding growth factors, cytokines and chemokines in wound healing.
AbstractPhotodynamic therapy (PDT) is an attractive therapy for non‐melanoma skin cancers including actinic keratoses (AKs) because it allows treatment of large areas; it has a high response rate and results in an excellent cosmesis. However, conventional PDT for AKs is associated with inconveniently long clinic visits and discomfort during therapy. In this article, we critically review daylight‐mediated PDT, which is a simpler and more tolerable treatment procedure for PDT. We review the effective light dose, efficacy and safety, the need for prior application of sunscreen, and potential clinical scope of daylight‐PDT. Three randomized controlled studies have shown that daylight‐mediated PDT is an effective treatment of thin AKs. Daylight‐mediated PDT is nearly pain‐free and m...

Conclusions  Serum leptin displays an association with obesity and insulin resistance. Assessment of HOMA‐IR in patients with skin tags may serve as a useful approach for diagnosis of insulin resistance. Waist circumference is the only criteria of metabolic syndrome that correlates with number of skin tags. (Source: Journal of the European Academy of Dermatology and Venereology)

AbstractBackground  Atopic dermatitis (AD; OMIM#603165) and psoriasis (OMIM#177900) are two common inflammatory skin disorders. Both are genetically complex, multifactorial and do not follow a Mendelian pattern of inheritance. Both diseases share several genetic susceptibility loci such as the epidermal differentiation complex (EDC) on chromosome 1q21. Within the EDC, mutations in the filaggrin (FLG) gene are strongly associated with AD whereas no association has been replicated with psoriasis. However, reduced levels of filaggrin have been reported in psoriatic skin. Further, filaggrin deficiency was shown to be a modifying factor for the phenotype in another epidermal skin disorder, X‐linked recessive ichthyosis. Altogether, this raises the question if FLG mutations may modify the di...

Antifibrotic effect after low‐dose imatinib mesylate treatment in patients with nephrogenic systemic fibrosis: an open‐label non‐randomized, uncontrolled clinical trial

Conclusion  Imatinib mesylate may be an effective drug in the treatment of skin fibrosis in moderate to severe NSF cases, even at reduced doses. We found a positive clinical effect on the skin, but no convincing improvement of the joint mobility. Only few patients could be recruited limiting the interpretation and conclusions of the results.
Conclusion  In this study, skin pain and skin discomfort were significantly related to HRQoL when controlling for demographic and clinical characteristics. In addition, sleep disturbance mediated the association between skin pain and HRQoL. An understanding of the complex association among physiological and psychological factors, and HRQoL is clinically important in order to provide proper treatment and care of patients with psoriasis.
Conclusion  The study’s findings demonstrate that the studied hypopigmented (vitiligo, hypopigmented MF, hypopigmented TV) disorders show similar changes in their cutaneous microenvironment with increased TNF‐α and decreased bFGF mRNA expression. This cytokine microenvironment change may be implicated in the pigment loss and hence these cytokines may have future therapeutic implications.
Conclusion  Our study illustrates the poor prognosis of pemphigus by a long duration to disease control, a high initial dose of oral steroid, a high rate of relapse and a short remission period. Only mucosal involvement at presentation was identified as a poor prognostic factor. (Source: Journal of the European Academy of Dermatology and Venereology)

Conclusion  Mastocytosis in children usually has a benign course. Nevertheless, severe mediator‐related symptoms and systemic involvement may appear. Therefore, a multidisciplinary approach involving careful monitoring of the serum tryptase level, SCORMA Index and the organ function is recommended. Both tryptase levels and the SCORMA Index are of a great value as disease severity parameters and they should be assessed simultaneously in all mastocytosis patients.
Conclusion  Our study provides novel information relative to the clinical details, demographic features and serological parameters of a population of vitiligo patients from Mumbai, India. Important distinctions from similar surveys conducted in European patients were evident such as an infrequency of family history, a low prevalence of clinical autoimmune disease, and an absence of particular antibody specificities. These differences may have a bearing on the pathogenesis and course of the disease in Indian patients. (Source: Journal of the European Academy of Dermatology and Venereology)

AbstractThe patterns of acute‐phase response (APR) biomarkers differ upon various inflammatory conditions. Little information is available on the systemic inflammatory response in urticaria/angio‐oedema. It has been shown that concentrations of circulating APR biomarkers, IL‐6 and C‐reactive protein (CRP), are elevated more in severe chronic urticaria (CU) than in patients showing milder urticarial symptoms. It is not clear whether the increase of IL‐6 and CRP is merely an epiphenomenon or may contribute to the pathogenesis of CU. It is tempting to speculate that mediators of APR may enhance urticarial inflammation. In addition, there is some association between APR and activation of coagulation/fibrinolysis in CU. It is well known that even slight elevation in CRP baseline conce...

An alternative approach for Mohs surgery using a combination of a flexible blade and the traditional scalpel

Discussion  The use of the flexible blade alone in Mohs may yield jagged tissue peripheral edges after cutting on the cryostat. The classic technique with a scalpel excision often results in a deeper cut than desired, resulting in a larger defect. We modified both the traditional Mohs technique and the flexible blade by combining both. We have performed this method in approximately over 100 cases on the scalp, face and ears, preserving tissue, minimizing bleeding and avoiding alopecia.
Conclusions  Diffuse cutaneous mastocytosis, DCM is a heterogeneous, severe, cutaneous disease, associated with mediator‐related symptoms and risk of anaphylactic shock. Although our results suggest generally favourable prognosis, the review of the literature indicate that SM may occur. Therefore, more guarded prognosis should be given in DCM patients.
AbstractHair colour is one of the most conspicuous phenotypes in humans, ranging from black, brown, blond to red. This diversity arises mostly from the quantity and ratio of the black‐dark brown eumelanin and the reddish‐brown pheomelanin. To study the chemical basis underlying the diversity of hair colour, we have developed several chemical methods to quantify those two pigments. Alkaline H2O2 oxidation affords pyrrole‐2,3,5‐tricarboxylic acid (PTCA) as a eumelanin marker and thiazole‐2,4,5‐tricarboxylic acid (TTCA) as a pheomelanin marker. Pheomelanin can also be analysed as 4‐amino‐3‐hydroxyphenylalanine (4‐AHP) after hydroiodic acid hydrolysis. Using those methods, we evaluated the contents of eumelanin and pheomelanin (the ‘chemical’ phenotype) in human hairs o...

Conclusion  The clinical and histopathological features of the CRP patients in our study are generally comparable to those published in the literature, with minor differences. Clinically, one case had an atypical clinical presentation, and microscopically follicular plugging was seen in the majority of cases. Yeast‐like spores were seen in six cases further supporting a role of Malassezia furfur in the pathogenesis of CRP.
Conclusion  Skin type appears to be a significant factor in psychological morbidity in acne. The FSPC is a reliable method for assessing skin phenotype, even when elicited via a concise questionnaire item suitable for assessing skin type as a potential confounder in studies of other outcome factors.
Conclusion  Being a systemic vasculitis, BD may cause cardiovascular involvement. In this study, dyslipidemia, insulin resistance and low adiponectin levels were not detected among our patients with Behçet’s disease. Our results suggest that there exists no increased risk for atherosclerotic cardiovascular disease associated with adiponectin levels and insulin resistance in patients with Behçet’s disease.
Conclusion  The presence of white shiny lines of any length accompanied by white shiny areas is most suggestive of a diagnosis of BCC (P < 0.001). Melanomas are more likely to display short white shiny lines in an orthogonal distribution (P < 0.001) and without white shiny areas. Actinic tumours are most likely to exhibit rosettes (P < 0.001). (Source: Journal of the European Academy of Dermatology and Venereology)

Conclusion  Increased expression of COX‐2 and decreased expression of miR‐26b in OLP suggests both to play a role in OLP. COX‐2 has been connected to both malignant development and autoimmunity but as malignant development of OLP is quite rare we suggest that the increased levels of COX‐2 seen here support an autoimmune cause of the disease.
AbstractPapulopustular rosacea (PPR) is a common facial skin disease, characterized by erythema, telangiectasia, papules and pustules. Its physiopathology is still being discussed, but recently several molecular features of its inflammatory process have been identified: an overproduction of Toll‐Like receptors 2, of a serine protease, and of abnormal forms of cathelicidin. The two factors which stimulate the Toll‐like receptors to induce cathelicidin expression are skin infection and cutaneous barrier disruption: these two conditions are, at least theoretically, fulfilled by Demodex, which is present in high density in PPR and creates epithelial breaches by eating cells. So, the major pathogenic mechanisms of Demodex and its role in PPR are reviewed here in the context of these recent d...

AbstractBackground  FDG PET‐CT is the superior imaging modality for the detection of visceral metastases (M+) in patients with melanoma. Conflicting evidence exists regarding its role for the initial staging of patients with high risk localized melanoma (large Breslow Thickness (BT) and/or ulceration). Objective  To assess the role of routine staging with FDG PET‐CT in melanoma patients with localized high risk melanoma. Methods  Forty‐eight consecutive patients with 1 < BT < 4 mm with ulceration and with BT ≥ 4 mm were staged with PET‐CT. PET‐CT procedures were performed on a GE Discovery ST® scanner. PET‐CT findings for regional nodal status and presence of distant metastatic disease were collected.
