From May 2011, NICE NHS Evidence will launch a redesigned and improved specialist evidence service to replace the existing model of 30 Specialist Collections. In view of the change to the new service the learning disabilities RSS feeds will cease from March 31st. Specialist evidence on learning disabilities may be accessed by setting up search alerts on NHS Evidence, setting up RSS feeds directly from NHS Evidence available from May 2011 or subscribing to the Eyes on Evidence bulletin which will provide access to all new significant new evidence.
People with learning disabilities have poorer health than their non-disabled peers, differences in health status that are, to an extent, avoidable.
The health inequalities faced by people with learning disabilities in the UK start early in life, and result, to an extent, from barriers they face in accessing timely, appropriate and effective health care. (Source: Learning Disabilities Specialist Library)

It is not a comprehensive or systematic review of the literature, but should provide a broad based starting point for people wanting to put the current personalisation agenda in context and find tools and documents to help explain the key concepts and terms.
Literature review to identify problems faced by direct supporters in observing and determining alertness in individuals with profound intellectual and multiple disabilities (PIMD). 42 publications found. Two types of descriptions of alertness can be distinguished, one with focus on individual only and second with focus on interaction of individual and environment. Reliability of observations was core problem in most of the studies. Modifications of environment, interaction strategies, stimulation strategies and staff training found to have positive impact on ,alertness whereas studies about treatment activities led to conflicting results.
Differential reinforcement of alternative behavior (DRA) is a common behaviour analytic intervention used to decrease unwanted behaviour. DRA literature from past 30 years reviewed to identify aspects that are thoroughly researched and those that would benefit from further emphasis. 116 empirical studies found. DRA has been successful at reducing behaviours on continuum from prelinguistic communication to life-threatening failure to thrive. DRA with and without extinction is well established for treating destructive behaviour of those with developmental disabilities, and to combat food refusal.
Meta-analysis of placebo-controlled randomized trials of melatonin in individuals with intellectual disabilities and sleep problems between January 1990 and July 2008. Influence of melatonin on sleep latency, total sleep time, and number of wakes per night examined. Quality of trials assessed using Downs and Black checklist. Nine studies (total of 183 individuals with ID) showed melatonin treatment decreased sleep latency by mean of 34 minutes, increased total sleep time by mean of 50 minutes significantly decreased the number of wakes per night. Authors conclude that melatonin decreases sleep latency and number of wakes per night, and increases total sleep time in individuals with intellectual disabilities.
Research survey, 1997 to 2007 on deinstitutionalisation and community living. 68 articles found. In 7 of 10 domains, majority of studies show community-based services to be superior to congregate arrangements. Variability found in results, suggesting factors other than basic model of care are important in determining outcomes. (Source: Learning Disabilities Specialist Library)

This review identified one randomised controlled trial of donepezil in people with Down syndrome. This shows, at best, a modest, non statistically significant trend in favour of people with Down syndrome and Alzheimer's dementia who are able to tolerate donepezil. The trial was of good quality, but small. It is important to note that people with Down syndrome may often have other conditions which mean that the drug is not suitable for all. Further research is needed. (Source: Learning Disabilities Specialist Library)

The drug galantamine has been reported to have benefits for people with mild to moderate Alzheimer's disease who do not have Down syndrome. However, people with DS tend to present with AD at a much younger age than the general population as well as being physically different in terms of size, metabolism and heart rate, and may therefore have different requirements. This review identified no randomised controlled trials of galantamine in people with Down syndrome. (Source: Learning Disabilities Specialist Library)

Memantine is thought to improve cognitive function and slow the decline of AD over time. The effects of memantine on AD are reported to be beneficial for people with moderate to severe AD in the general population, However, people with DS tend to present with AD at a much younger age than the general population as well as being physically different in terms of size, metabolism and heart rate, and may therefore have different requirements. Results from the one randomised controlled trial for the treatment of dementia in DS are not yet available (expected 2009). (Source: Learning Disabilities Specialist Library)

The objective of this review was to assess whether amfetamine is effective in treating ADHD in people who also have an intellectual disability. We found only one study, which examined this question in a small group of participants. This did not find an effect, but we do not think this is enough evidence to draw conclusions about whether or not amfetamine is effective. More research in this area is urgently needed. (Source: Learning Disabilities Specialist Library)

The authors conducted the review process with transparency, thus minimising the potential for error and bias. The narrative synthesis was appropriate given the differences between the studies. Although a formal validity assessment was carried out, the criteria were not relevant to all studies, which means that the true quality of this limited evidence base cannot be verified. Together with the fact that the review included only a small number of small-sized, heterogeneous and largely uncontrolled studies, this suggests that the authors' cautious interpretation of the results is justified.
“First-born children of older parents are more likely to be autistic,” warned The Daily Telegraph. It reported on a study that examined the medical records of 240,000 children born in the US in 1994, and found that both maternal and paternal age were independently associated with autism. It said mothers aged 35 or older had a 30% higher chance of having an autistic child compared to mothers aged 25 to 29, while Fathers over 40 years had a 40% higher risk than those aged 25 to 29.
“Fresh research rules out MMR-autism link”, is the headline in The Daily Telegraph. The newspaper goes on to describe a new study that replicates one conducted in 1998 by Dr Andrew Wakefield. This earlier study is described by the Daily Mail as being the one that “caused a furore” by suggesting a link between autism and the MMR vaccine.
The Healthcare Commission has published a report that gives supplementary details on the findings of the national audit of specialist inpatient healthcare services for people with learning difficulties in England. It relates specifically to specialist services for young people and adolescents. The report states that young people in these services were generally safe and protected from abuse and that their health needs were met overall. The audit also found that they were well supported by dedicated staff. However, the Healthcare Commission found that young people in these services should have more independence and control over their lives and the environment in which they live.
