Conclusions: The proposed classification procedure is based on the model parameters. These have proved to be sensitive indicators of lung microangiopathy. Close to 80% of microangiopathy cases have been classified as such. Less than 20% were false alarms. The oxygen pathway model allows for simulations. Blood saturation and oxygen partial pressure have been simulated for the organism's various needs for oxygen, both for the normal and the impaired alveoli-capillary barrier.

Conclusions: This paper contains the reflections of a number of the invited experts on the character of biomedical informatics. Most of the authors agree that biomedical informatics is an interdisciplinary field of study where researchers with different scientific backgrounds alone or in combination carry out research. Biomedical informatics is a very broad scientific field and still expanding, yet comprised of a constructive aspect (designing and building systems). One author expressed that the essence of biomedical informatics, as opposed to related disciplines, lies in the modelling of the biomedical content. Interdisciplinarity also has consequences for education. Maintaining rigid disciplinary structures does not allow for sufficient adaptability to capitalize on important trends nor ...

Conclusions: The challenges posed by the complexity and diversity of the domain knowledge, system infrastructure, and usage pattern are highlighted. New requirements and computational paradigms for representing, using, and acquiring biomedical knowledge and healthcare protocols are proposed. The underlying common themes identified for developing next-generation decision support include incorporating lessons from history, uniform vocabularies, integrative interfaces, contextualized decisions, personalized recommendations, and adaptive solutions.

Conclusions: Significant challenges, as well as opportunities, appear when cumulating the intrinsic multidisciplinary nature of health informatics interventions with the diversity of contexts at the global level, in particular when considered in the perspective of a confluence, i.e., the mixing of different waters and their merging into a new, stronger entity. Health informatics experts reflect on key issues such as collaborative software development and distributed knowledge sourcing, social media and mobile technologies, the evolutions of the discipline from an historical perspective, as well as examples of challenges for implementing ubiquitous healthcare or for supporting disaster situations when infrastructures get disrupted.

    Abstract
This issue of Methods of Information in Medicine celebrates the journal's first 50 years. As the oldest journal in biomedical and health informatics and, being broader in its scope, as the journal dealing with the methodology and scientific fundamentals of organizing, representing and analysing data, information and knowledge in biomedicine and health care, the journal publications during the last five decades also reflect the formation of a scientific field that deals with information in biomedicine and health care. Five papers that arose from a scientific symposium on "biomedical informatics: confluence of multiple disciplines" held in Heidelberg, Germany, in June 2011 are included in this volume. The papers reflect not only the broad interdisciplinary sc...

Conclusions: A broad range of information is available on German PRWs. While structural information can give a detailed overview of the financial, technical and human resources of a practice, other outcome measures have to be interpreted with caution. Specifically, patient satisfaction results are not risk-adjusted, and thus, are not appropriate to represent a provider's quality of care. Consequently, neither patients nor physicians should yet use the information provided to make their final decision for or against an individual physician.

Conclusions: A variety of frameworks and models were found in the biomedical and life sciences literatures. These frameworks and models drew upon and extended frameworks, models and theoretical perspectives that have emerged in other literatures. These frameworks and models are informing an emerging line of research in health and biomedical informatics involving technology-induced errors in healthcare.

Conclusions: SNOMED CT can represent most (80%) of the ICNP version 1 concepts. However, improvements in the ICNP version 1 in terms of concept naming and definition, and the addition of missing concepts to SNOMED CT, would lead to a greater harmonization of the ICNP seven-axis model version 1 concepts with SNOMED CT.

Conclusions: Active RFID tags provide potentially useful information on LOS at a low cost and with minimal impact on the work environment. Machine learning techniques may be employed to handle the variable nature of RFID's radio signals. Work on mapping LOS data to activities will involve integration with other sensors and task analysis techniques.

CONCLUSIONS: Evaluation and comparison of results in medical informatics, and specifically EHR systems, must address technical and non-technical aspects. It is challenging to condensate in a single framework all potential views of such a field, and any chosen approach is bound to have its limitations. That being said, any well structured comparison approach, such as the framework presented here, is better than no comparison framework at all, as has been the current situation to date. This paper has presented the first attempt known to the authors to define such a framework.

    Abstract
APAMI 2009 was held in Hiroshima, Japan, on November 22-24, 2009. This issue includes two selected papers recommended by the programming committee co-chairs. They are "Lessons Learned from Data Mining of WHO Mortality Database" and "Survey on Medical Records and EHR in Asia-Pacific Region - Languages, Purposes, IDs and Regulations". The theme of APAMI 2009 was "What are the medical records for?" A Hiroshima episode; Medical Records at Dr. Ban's Clinic, 9 km from the Epicenter of A-Bomb, is included, which lets us think of the fundamental purpose of medical records.

Conclusions: Interruptions are a complex phenomenon where multiple variables including the characteristics of primary tasks, the interruptions themselves, and the environment may influence patient safety and workflow outcomes. Observational studies present significant challenges for recording many of the process variables that influence the effects of interruptions. Controlled experiments provide an opportunity to examine the specific effects of variables on errors and efficiency. Computational models can be used to identify the situations in which interruptions to clinical tasks could be disruptive and to investigate the aggregate effects of interruptions.

    Methods Inf Med. 2011;50(1):53-61
Authors: Hackl WO, Hoerbst A, Ammenwerth E
Progress in the medical sciences, together with related technologies, in the past has led to higher specialization and has created a strong need to exchange health information across institutional borders. The concept of electronic health records (EHR) was introduced to fulfill these needs. Remarkably, many EHR introduction projects ran into trouble, not least because they lacked the acceptance of EHR among physicians. Negative emotions, such as anxiety and fear due to a lack of information, may cause change barriers and hamper physicians' acceptance of such project...

Conclusion: Classification tools produced the poorest results in predicting cause of death. Given the inadequacy of variables in the WHO database, creation of a classification model to predict specific cause of death was impossible. Clustering and association tools yielded interesting results that could be used to identify new areas of interest in mortality data analysis. This can be used in data mining analysis to help solve some quality problems in mortality data.

Local control in adjuvant/definitive RT of adenoid cystic carcinoma (ACC) is largely dose-dependent leading to the establishment of particle therapy in this indication. However, even modern techniques leave space for improvement of local control by intensification of local treatment. Radiation sensitization by exploitation of high EGFR-expression in ACC with the EGFR receptor antibody cetuximab seems promising.

We report the case of a 71 year-old male patient undergoing carbon ion therapy for extensive local relapse of ameloblastic carcinoma. Treatment outcome was favourable with a complete remission at 6 weeks post completion of radiotherapy while RT-treatment itself was tolerated well with only mild side effects. High dose radiation hence is a potential alternative for patients unfit or unwilling to undergo extensive surgery or in cases when only a subtotal resection is planned or the resection is mutilating.

Conclusions: We developed a robust system to monitor the dressing activity. Given the importance of monitoring the dressing activity as an indicator of both cognitive and motor skills the system allows for the possibility of long term tracking and continuous evaluation of the dressing task. Long term monitoring can be used in rehabilitation and cognitive skills evaluation.

Conclusion: We produced a ranked list of reporting items from STARE-HI according to their prioritized relevance for inclusion in space-limited conference papers. The priority adjusted completeness scores demonstrated room for improvement for the analyzed conference papers. We believe that this prioritization framework is an aid to improving the quality and utility of conference papers on health informatics evaluation studies.

Conclusions: The research demonstrates the applicability of archetype patterns to ontology development in HIS. While numerous ontologies exist in biomedicine, there are few well-developed ontologies for general healthcare. Properly modeled, archetype patterns have potential to reconcile the differences in high-level design views across health care inter-enterprises. Future research can focus on governance, standards and tools for archetype patterns, as well as development of a comprehensive set of high-level healthcare archetype patterns.

Conclusions: There is a continuous and, as it seems, ever growing overlap in the research methodology and application areas of the mentioned disciplines. As there is a continuing and even growing need for such a publication forum, Methods of Information in Medicine will keep its broad scope. As an organizational consequence, the journal's number of associate editors has increased accordingly.

Conclusions: In high CPOE installation rate (85% in 400+ bed hospitals), though most of them only capable of exporting data in proprietary format, prefecture and ministry projects were effective to promote healthcare information exchange between providers. The standardized storage became an infrastructure for many useful applications, and many hospitals started using them. Ministry designation of proposed healthcare standards was effective so as to allow vendors to conform their products, and users to install them.

Conclusions: Our results suggest that accelerometer data may be used to predict falls in an unsupervised setting. Furthermore, the parameters used for prediction are measurable with an unobtrusive sensor device during normal activities of daily living. These promising results have to be validated in a larger, long-term prospective trial.

Conclusions: The hierarchy of risk factors provides further insights into clinicians' perceptions of risk factors for falls. This hierarchy helps understand the relative importance that clinicians place on risk factors for falls in older people and why evidence-based guidelines are not always followed. This information may be helpful in improving intervention programs and in understanding how clinicians prioritize multiple risk factors in individual patients. The AHP method allows the opinions of HCPs to be investigated, giving appropriate weight to their coherence, background and experience.

Conclusions: A number of factors affected the successful implementation of NHIS-T. First, all stakeholders have to adopt the specified standards. Second, the UN/CEFACT CCTS approach was applied which facilitated the development and understanding of rather complex EHR schemas. Finally, the comprehensive testing of vendor-based hospital information systems for their conformance to and interoperability with NHIS-T through an automated testing platform enhanced substantially the fast integration of vendor-based solutions with the NHIS-T.

Conclusions: These results suggest that cancer symptoms co-occur in a nested pattern as opposed to distinct clusters, thereby demonstrating the value of exploratory network analyses to reveal complex relationships between patients and symptoms. The research also extends methods for exploring symptom co-occurrence, including methods for quantifying the degree of symptom overlap and for examining nested co-occurrence in co-occurrence data. Finally, the analysis also suggested implications for the design of systems that assist in symptom assessment and management. The main limitation of the study was that only one dataset was considered, and future studies should attempt to replicate the results in new data.

Conclusions: Developing high-quality guidelines requires a skilled team of people and sufficient budget. The guidelines should give personalized advice. Computer-interpretable guidelines (CIGs) that have access to the patient's EPR are able to give personal advice. Because of the costs, sharing of CIGs is a critical requirement for guideline development, dissemination, and implementation. Until now this is hardly possible, because of the many models in use. However, some solutions have been proposed. For instance, a standardized terminology should be imposed so that the terms in guidelines can be matched with terms in an EPR. Also, a dissemination model for easy updating of guidelines should be established. The recommendations should be based on evidence instead of on consensus. To test th...

Conclusions: In general, careful modeling correlation is encouraged when considering estimation efficiency and other implications, and a comparison of choosing instruments in GMM and generalized estimating equations (GEE) would be worthwhile. Some theoretical drawbacks of GEE need to be further addressed and require careful analysis of data. This particularly applies to the situation when data are missing at random.

CONCLUSION: Our main conclusion is that rather than having individual testing tools for each standard or initiative, a generic and modular test framework exploiting the commonalities in the testing processes and fostering reusability of modular, pluggable testing components will improve the efficiency of testing. Through the TestBATN framework, we describe how this modularity can be achieved by providing common interfaces facilitating the development of adaptors which allows different testing components to be plugged into the system.

CONCLUSIONS: For two particular kinds of archetype prescriptions, individual transformations are required for the corresponding EHR documents. Otherwise, a fully generic validation is possible. In general, we consider W3C XML Schema as a practicable solution for the semantic validation of standard-based EHR documents.

Conclusions: Local codes for two institutions could be mapped to LOINC codes with 99% or better concept token coverage, but mapping for a third institution (a reference laboratory) only achieved 79% concept token coverage. Our research supports the conclusions of others that not all local codes should be assigned LOINC codes. There should also be public discussions to develop more precise rules for when LOINC codes should be assigned.

Conclusions: If only little information is available regarding the size of the overall event rate and the overdispersion parameter in the design phase of a trial, we recommend the use of a design with sample size reestimation as the one suggested here. Trials in COPD are expected to benefit from the proposed sample size reestimation strategy.

Conclusion: The manuscript gives comprehensive insight into the currently available, primarily non-functional, EHR requirements. To our knowledge, there are no other publications that have holistically reported on this topic. The requirements identified can be used in different ways, e.g. the conceptual design, the development of EHR systems, as a starting point for further refinement or as a basis for the development of specific sets of requirements.

Conclusions: This study discusses a new FBP-based reconstruction approach based on signal interpretation from low-dosage acquisition. This method uses multiple filtered back-projection images from projection subsets to provide clues for distinguishing underlying clinical structure from artifacts. A framework is derived for effective signal interpretation and artifacts reduction. It requires no hardware change and a minimum amount of extra software support compared with current CT systems. Clinical and simulated low-dose CT scans demonstrated effectiveness of the proposed method.

Conclusions: These data support the suggestion that pain processing in response to noxious stimulation in MD patients is different compared to healthy controls, suggesting aberrant functional connectivity. Generalized partial directed coherence is shown to be a promising method to detect changes in connectivity in both within- and between-subject designs.

Conclusion: When considering the individual ratio between the mean correlation of cry bouts during a crying sequence and their standard deviation, there seems to be a positive trend with increasing MBPS value. This might indicate that higher stressed subjects have less consistency in the investigated acoustic cry features, concluding that EMD has potential in the assessment of infant cry analysis.

Conclusions: We provide a structure for the process of identifying a domain-specific interface terminology on SNOMED CT. We use this approach to design an interface terminology on SNOMED CT for the intensive care domain. This work is of value for other researchers who intend to build a domain-specific interface terminology on SNOMED CT.

Conclusions: The simulator can simulate realistic tooth surface exploration and cutting. The accuracy of automatic performance assessment system using HMMs is also acceptable on relatively small data sets. The intelligent training allows skill transfer in a proactive manner which is an advantage over the passive method in a traditional training. We will soon conduct experiments with more participants and implement a variety of training strategies.

The impact of CPOE medication systems' design aspects on usability, workflow and medication orders: a systematic review.

CONCLUSIONS: Beside the range of functionalities provided by a CPOE system, their subtle design is important to increase physicians' adoption and to reduce medication errors. This requires continuous evaluations to investigate whether interfaces of CPOE systems follow normal flow of actions in the ordering process and if they are cognitively easy to understand and use for physicians. This paper provides general recommendations for CPOE (re)design based on the characteristics of CPOE design aspects found.

The rate of missed test results in an emergency department: an evaluation using an electronic test order and results viewing system.

CONCLUSIONS: Our rates of missed test results are lower than those reported from studies where paper ordering and reporting systems were used. This suggests that the availability of CPOE systems may reduce the risk of these events. Electronic result delivery, with electronic endorsement to allow documentation of follow-up of test results, may provide additional efficiency benefits and further reduce the risk of test results which are not followed up.

CONCLUSIONS: The paper concludes that pervasive healthcare will enable a paradigm shift from the established centralized healthcare model to a pervasive, user-centered and preventive overall lifestyle health management. In order to provide these new opportunities everywhere, anytime and to anyone, future research in the fields of pervasive sensing, pervasive prevention and evaluation of pervasive technology is inevitably needed.

Conclusions: Cluster analysis provided a unique approach for segmenting health consumers and predicting their acceptance of quantitative disease risk information. These clusters suggest that health consumers were very responsive to good news, but tended not to incorporate bad news into their self-perceptions much. These findings help to quantify variation among online health consumers and may inform the targeted marketing of and improvements to risk communication tools on the Internet.

Conclusions: Discharge letter-linked encoding by pediatricians supported by a pediatric list of diseases leads initially to increased detail and number of diagnoses with added informational value. When attention diminishes, especially the level of detail and number of secondary diagnoses that do not fall under one's own subspecialty decrease. The level of detail of principal diagnoses remains stable because of the advantage for pediatricians of having specific diagnostic codes falling under their own subspecialty.

Conclusions: Previous studies of locomotion in laboratory settings that have demonstrated gait variability increases with poor cognitive status have necessarily controlled various components of gait. The present results demonstrate that directional changes and other locomotion components can be studied by monitoring free movements in normal living settings over time. Implications for assessment and management of dementia-related wandering are discussed.

Conclusion: Selecting the appropriate integration architecture is a fundamental issue of any software development project. HIS-DF provides a unique methodological approach guiding the development of healthcare integration projects. The mediator model - offered by the HIS-DF and supported in HL7 v3 artifacts - is the more promising one promoting the development of open, reusable, flexible, semantically interoperable, platform-independent, service-oriented and standard-based health information systems.
