Evidence from some, but not all non-randomised studies suggest the possibility that cognitive training may influence cognitive functioning in older people. Due to the differences among cognitive training interventions reported in the literature, giving a general overview of the current literature remains difficult.
Some psychosocial interventions appeared to have beneficial effects on behaviour in patients with dementia, but the effects were mainly small to moderate and short-term and there were limitations in terms of significant within-group differences in response. Given limitations with the included studies and potential for bias in the review, the authors' conclusions should be treated with caution.
The authors concluded that some evidence existed for the effectiveness of certain multifactorial interventions for fall prevention in older acute-care patients, particularly those that increased patient education or targeted fall risk factors. Recommendations for further research accurately reflected the limitations presented in this review. The authors' conclusion reflected the evidence presented and their cautious interpretation is likely to be reliable. (Source: NLH Later Life Specialist Library Newsfeed)

The review concluded exercise resulted in improvements in postural stability and balance task performance for people with mild to moderate Parkinson's disease. However, the number and quality of the trials and outcomes were limited. The authors' cautious conclusions reflect the evidence presented, but they were based on few trials, some with small sample sizes, and diverse interventions and outcome measurements.
The authors concluded that evidence for the impact of community-based heart disease intervention on depression in older adults was mixed and further research was needed with different follow-up periods, common depression outcome measures and subgroup analysis by demographic characteristics. Given the possibility of publication bias and uncertainties regarding the analysis, the authors' cautious conclusions are justified. (Source: NLH Later Life Specialist Library Newsfeed)

Most disabling strokes are due to blockage of a large artery in the brain by a blood clot. Prompt removal of the clot with intra-arterial thrombolytic drugs or mechanical devices, or both, can restore blood flow before major brain damage has occurred, leading to improved recovery. However, these so-called percutaneous vascular interventions can cause bleeding in the brain. (Source: NLH Later Life Specialist Library Newsfeed)

The review did not find definitive evidence of effectiveness of methylphenidate in medically ill older adults with depression, fatigue or apathy, although it appeared to be tolerated in the short term. Due to variation in outcomes, methodological flaws in the included studies and shortcomings in the review process, the reliability of the author's conclusions is not clear.
The objective was to compare the cost-effectiveness of a walking programme against the best usual care for the management of moderate depression or moderate obesity in women aged 60 years or older. The authors concluded that the walking programme was a cost effective addition to the best care for depressed or overweight elderly women. Overall, the economic evaluation was well conducted and valid sources of data were used. These facts make the authors' conclusions robust.
This review assessed the effectiveness of prevention programmes for community-dwelling older adults aged 60 years and over in terms of risk and rate of fall. The authors concluded that multifactorial fall prevention programmes appeared to be more effective, especially for high-risk older adults with a previous history of fall. These conclusions appear to be broadly reliable. (Source: NLH Later Life Specialist Library Newsfeed)

This review concluded that an increased risk of falls was probable when elderly individuals were exposed to NSAIDs.
This study evaluated the incremental cost-effectiveness of cholinesterase inhibitors and memantine for mild to moderate vascular dementia. The authors concluded that, compared with standard care, cholinesterase inhibitors and memantine had small benefits in cognition and were all more costly. If an active treatment was approved, donepezil 10mg was the most cost-effective. There were a few limitations to the study and the authors' conclusions should be considered with caution.
The use of statin therapy in established Alzheimer's disease (AD) or vascular dementia (VaD) is a relatively unexplored area. In AD ß-amyloid protein (Aß) is deposited in the form of extracellular plaques and previous studies have determined Aß generation is cholesterol dependent. Hypercholesterolaemia has also been implicated in the pathogenesis of VaD. Due to the role of statins in cholesterol reduction it is biologically plausible they may be efficacious in the treatment of AD and dementia.
Falls by older people in nursing care facilities and hospitals are common events that may cause loss of independence, injuries, and sometimes death as a result of injury. Effective interventions are important as they will have significant health benefits.

This review includes 41 trials involving 25,422 participants, with about three quarters being women and having an average age of 83 years. Many of the participants had cognitive problems. (Source: NLH Later Life Specialist Library Newsfeed)

This Annual Evidence Update (AEU) on hip fracture has been released by NHS Evidence - emergency and urgent care in conjunction with NHS Evidence - trauma and orthopaedics. The scope of the literature search covers fractures affecting the proximal femur. This AEU will appeal to all health professionals, reflecting the multidisciplinary approach required across both primary and secondary care. However, there may be clinicians who are involved in the management of polytrauma and high energy injuries that affect the hip joint who will find that our update is offset from their area of interest.
Easing the shortage in adult primary care - is it all about money

As Americans debate health care reform, it is easy to forget that success may depend as much on the availability of primary care physicians for adults as on the specifics of the reforms themselves. Access to health insurance does not ensure access to timely medical care, particularly in places where doctors are in short supply, are not accepting new patients, or are not accepting patients with some types of insurance. Effective primary care can improve the quality of care and health outcomes and save money. But to the extent that easing the shortage of primary care physicians will require additional funds, the initial costs of reform will increase.
The review addressed a clear question along with clear inclusion criteria. However, only one database was searched for studies, the literature search was restricted to publications in English and there was no specific search for unpublished studies. These restrictions may mean that some studies were missed and both publication and language bias could have been present. In addition, the likelihood of reviewer bias and error could not be evaluated as details of the methods employed to select studies and extract the data were not given. The quality of the included studies was not reported and only limited details of the studies were presented, which made it difficult to judge the appropriateness of using a narrative synthesis. Owing to the limitations highlighted, the reliability of the revie...

The Plan's purpose is to enhance resilience in the event of a heatwave. It is an important component of overall emergency planning and will become increasingly relevant in adapting to the impact of climate change. Additional guidance from 2009 is provided for the role of Strategic Health Authorities and Government Offices at Regional Level for each of the heatwave levels from one to four. This provides clarity about governance and risk management. (Source: NLH Later Life Specialist Library Newsfeed)

Several studies, including work from the Parkinson's disease (PD) non-motor group and others, have established that the non-motor symptoms of PD are common, occur across all stages of PD, are under-reported, and are a key determinant of quality of life. Research suggests that the non-motor symptoms of the disease are frequently unrecognised by clinicians and remain untreated. Even when identified, there is a common perception that many of these symptoms are untreatable. The role of dopaminergic drugs in treating the various non-motor problems of PD, although clinically recognised, has received little attention. In this Review, we investigate the dopaminergic basis of the range of non-motor symptoms that occur in PD such as depression, apathy, sleep disorders (including rapid-eye movement s...

“Scientists are closer to developing a more accurate test for prostate cancer which could save hundreds from surgery every year,” reported the Daily Mail. It said that a urine test could identify those who need further treatment and those whose cancer is dormant. The newspaper continued that, “tiny globules of fat in men's urine contain RNA molecules which could indicate whether the cancer is aggressive”.
This study compared the cost-utility of chemoprevention using daily finasteride in the prevention of prostate cancer compared with no chemoprevention. The authors concluded that chemoprevention for men, when started at age 50, was not cost-effective in terms of life-years saved. However it approached cost-effectiveness when quality-adjusted life-years were considered. Overall, the conclusions made by the authors are hard to evaluate as the study methods and results were not transparently and comprehensively reported.
The aim was to assess the cost-effectiveness of adjuvant memantine in patients with moderate-to-severe Alzheimer’s disease, who were receiving stable doses of donepezil. The authors concluded that memantine plus donepezil improved the clinical outcomes and reduced the total costs of care in comparison with donepezil alone. The study appears to have been based on valid methodology and uncertain areas were satisfactorily investigated. The authors’ conclusions appear to be valid. It is for NHS primary care and other professionals who have a direct or indirect role in, and responsibility for, promoting older people's mental wellbeing. This includes those working in local authorities and the wider public, private, voluntary and community sectors. It will also be relevant for carers and family members who support older people and may be of interest to older people themselves.
“A fatty acid, an ingredient found in foods considered healthy, could harm brain cells and raise the risk of getting Alzheimer's disease” The Guardian reported. It said a study has found that high levels of arachidonic acid, an Omega 6 fatty acid, are linked to brain changes commonly found in people with Alzheimer's. The substance is found in a variety of foods, such as vegetables and nuts, and is generally considered as vital to a healthy diet. (Source: NLH Later Life Specialist Library Newsfeed)

It says the new research has demonstrated that the supplements actually make very little difference to the decline of mental functions, contrary to what previous studies have suggested.
The DH commissioned two literature reviews and two research studies on the costs and benefits of eliminating age discrimination in the provision of health and social care. This was to inform decisions on whether to pursue legislation to outlaw discrimination in the provision of health and social services. One of the reviews is the following: Cost-effectiveness analysis and ageism: a review of the theoretical literature (University of Leeds, Leeds Institute of Health Services).
This is a NICE guideline: A cataract is an eye condition in which the lens becomes cloudy over time, and if untreated, cataracts can lead to blindness. During cataract surgery, the clouded lens is removed and replaced with an artificial lens, which provides clearer vision. Unlike standard intraocular lenses, a multifocal intraocular lens has areas with different focusing power with the aim of allowing near and distant objects to be seen without the need for spectacles.
This AEU is an update to the 2007 NKW and so the same scope has been followed. The term Age Related Macular Degeneration is used to cover both wet and dry forms of the disease AMD. The dry form of the condition is also known as geographic atrophy. The wet form of the condition is also known as neovascular AMD. Whilst an overview on early disease (which may not be visually impairing) is important to include for the epidemiology of this condition, the emphasis of this AEU is on its sight threatening or visually impairing forms. The developments in therapeutic and surgical interventions available for AMD that are featured in the NKW relate to the wet form of AMD.
“Longer limbs 'mean less risk of dementia'” reads the headline in The Guardian today. It goes on to report that a study in the US followed 2,798 people, with an average age of 72, over a period of five years. It found that women who had longer legs and arms were less likely to develop dementia, while the women “with the shortest arms were 50% more likely to develop the disease than those with the longest arms”.
