CONCLUSION: The primary care optometrist can play an important role in the diagnosis and continued management of a patient with a cholesteatoma, carefully monitoring early signs of progression or change. This case illustrates how an intracranial lesion, away from the eye, can impact a number of ocular and visual functions.

CONCLUSIONS: An individualized treatment plan is indicated for most patients who wear an ocular prosthesis. Despite this, some commonalities in responses were noted, specifically that the ocular prosthesis needs to be cleaned with every removal, and a silicone oil lubricant is recommended.

CONCLUSIONS: Pain is historically considered a cardinal sign of acute inflammation and a predominant symptom of IOIS. The authors present a case of acute IOIS, atypical because of its presentation in the absence of pain. In addition, a review of the literature is conducted to identify predominant clinical and radiologic characteristics of IOIS along with current accepted and proposed treatment options. Differentials are discussed to distinguish this disease from systemic immunologic and inflammatory conditions such as syphilis, sarcoidosis, and Wegener's granulomatosis as well as potentially fatal neoplastic conditions.

Authors: Ciuffreda KJ, Vasudevan B
The purpose of the current study was to assess the effect of nearwork-induced transient myopia (NITM) on retinal defocus patterns during distance viewing. An empirically derived, conceptual model of human blur perception and related retinal defocus patterns has been extended to determine the effect of NITM on the relative contributions of myopic and hyperopic retinal defocus during distance viewing. Under the normal hyperfocal refractive condition during distance viewing with isolated stimulus conditions, there is very small myopic defocus ( approximately 0.25 diopters), and no hyperopic defocus, present. After sustained nearwork generating NITM, a moderate increase in this myopic defocus contribution takes place. In the normal isolated distance viewi...

CONCLUSION: Diplopia and disc edema in a young, nonoverweight patient with an altered mental state is very concerning. Meningitis and complications of spinal anesthesia, although rare, need to be considered in patients who have recently given birth. However, other causes of increased intracranial pressure, including subdural hemorrhage secondary to trauma, also have to be considered, even if the patient denies any history of trauma. Abused patients often deny trauma.

CONCLUSION: Schools using the highly sensitive and specific MCT identified more visually at-risk children than schools using alternative, less sensitive vision screening techniques, and the percentage of grade 1 children referred to an eye care provider was higher for school corporations with lower median family incomes. Although statistically insignificant, the results indicate that students who fail the vision screening in grade 1 tend to be more at risk for poorer academic performance on standardized testing in grade 3.

CONCLUSION: Corneal abrasions are one of the most common ocular conditions presented to eye clinics or emergency departments. Although there are different etiologies of abraded corneal epithelium, current clinical management for most corneal abrasions involves a bandage contact lens, use of topical antibiotics, and cycloplegics. Large and central corneal abrasions, however, warrant a consultation with a corneal specialist.

CONCLUSION: Proper eye selection chosen for implantation with the telescope prosthesis appears to be an important if not critical factor in determining patient satisfaction for visual processing and functional success. Based on the author's experience with the IMT, optometrists can aid the multidisciplinary team by preoperatively determining which eye, if implanted, offers the optimal potential functional benefit for appropriate candidates.

CONCLUSION: This study confirmed the findings of prior studies in identifying a significant presence of uncorrected refractive errors and ocular pathology in the Down syndrome population. Screening with the Lea symbol chart found borderline sufficient sensitivity and specificity for the test to be used for screening in this population. The better sensitivity and specificity of the CVA, if adjusted normative values are utilized, appear to make this test sufficient for testing Down syndrome children for identifying both refractive errors and ocular pathology.

For nearly 2 millennia, signs of hypothalamic-related vision disorders have been noticed as illustrated by paintings and drawings of that time of undiagnosed Horner's syndrome. It was not until the 1800s, however, that specific connections between the hypothalamus and the vision system were discovered. With a fuller elaboration of the autonomic nervous system in the early to mid 1900s, many more pathways were discovered. The more recently discovered retinohypothalamic tracts show the extent and influence of light stimulation on hypothalamic function and bodily processes. The hypothalamus maintains its myriad connections via neural pathways, such as with the pituitary and pineal glands; the chemical messengers of the peptides, cytokines, and neurotransmitters; and ...

CONCLUSION: Asthenopic symptoms score was correlated with convergence amplitude as measured, whereas accommodation is controlled and the ratio score calculated based upon DEM results. Further study is needed to evaluate the usefulness of the integration between symptom survey and objective reading examinations as screening tool for the diagnosis of convergence insufficiency.

CONCLUSIONS: These data indicate that in the assessment of normal eyes, Orbscan gives consistently higher measurements for anterior chamber depth compared with Galilei and Pentacam. Because the differences between Orbscan and either Galilei or Pentacam were not within clinically acceptable levels, they are not interchangeable in every clinical situation. However, the differences among anterior chamber depth values measured with Galilei and Pentacam were within clinically acceptable levels. Thus, these 2 systems can be regarded as interchangeable.

CONCLUSION: The use of the OCT has enhanced the evaluation of the vitreal-retinal interface, leading to a better understanding of VMT. Once thought to be a rare distinct clinical entity, VMT is now considered a spectrum of macular diseases. The understanding of VMT and its role in the pathophysiology of various macular conditions may facilitate diagnosis and management of these conditions.

CONCLUSION: To facilitate early diagnosis and treatment, eye care professionals should be aware that craniopharyngiomas can present with decreased visual acuity and bitemporal hemianopia. Visual deficits can arise from both the aggressive nature of craniopharyngiomas as well as the therapeutic intervention. Patients treated for craniopharyngiomas are at risk for recurrence and need to be followed up on a long-term basis.

CONCLUSION: The subjective DCC test as performed here does not provide an accurate measurement of the AR to a near target in a young population. We recommend that alternative techniques such as using an objective, open-field optometer or Cross-Nott retinoscopy be adopted for determining the within-task AR.

Vision and visual impairment have a significant impact on older adults' daily function and safety within their homes and communities. Yet many do not seek out the necessary care to address their visual health. Many older adults want to age in places that require a supportive environment to compensate for changes in abilities and to promote healthy aging and quality of life. The Elder's Right to Sight Collaborative (ERTS) was established to gain insight into the role of the environment in maintaining the functional independence and safety of older adults with visual impairment and to understand the barriers to obtaining the necessary eye care services. The ERTS provides education, screening, environmental assessments, and simple tools to complete...

CONCLUSIONS: Anti-inflammatory pharmacologic agents have shown great success in patients with moderate to severe dry eye when compared with alternative treatment modalities. A deeper understanding of the link between inflammation and dry eye validates the utilization of anti-inflammatory therapy in everyday optometric practice.

CONCLUSION: The primary care optometric setting, by the way a patient's visual symptoms present or the appearance of fundus abnormalities, can offer the capability of first detection and referral for the discovery of metastatic lesions. Although orbital metastasis is considered a terminal finding in these cases, timely diagnosis enables, while limited, the best options for management.

Authors: Baxstrom CR
A 14-year-old girl with diplopia and esotropia secondary to Arnold-Chiari I malformation was surgically treated with Arnold-Chiari I malformation decompression (suboccipital craniectomy), C1 and partial C2 laminectomy, and duraplasty. The residual esotropia was treated with compensatory prisms and vision therapy more than 1 year after Arnold-Chiari malformation surgery. The esotropia was resolved after approximately 3.5 months of treatment. Five years later, the patient continued to maintain fusion without compensatory prism.

CONCLUSION: Automated vision therapy delivered by the HTS system improved convergence and divergence amplitudes with a concomitant reduction in symptoms. The HTS system should be used on those patients with symptoms associated with an accommodative/vergence anomaly when in-office vision therapy supplemented with home therapy is not practical.

CONCLUSION: The majority (88%) of glaucoma cases show concordance between structural loss and functional deficits. The primary proposed explanations for discordance include (1) visual field sampling and test selection limitations (i.e., the SS24-2 samples only axons that are anatomically connected to photoreceptors in the central retina, whereas the GDx samples virtually all axons) and (2) GDx measurement limitations in the papillo-macula bundle (i.e., the GDx has difficulty differentiating the normally thin RNFL from the pathologically even thinner RNFL). Tests of both structure and function are recommended in glaucoma suspects and patients, as neither mode is capable of identifying all glaucomatous deficits. Due to the apparent diagnostic benefit that the peripheral functional data may p...

CONCLUSION: Subjects with VMH made significantly more refixational eye movements and had higher levels of dizziness than those in the other 2 diagnostic groups. There were abnormalities of binocular function in both the VMH and vestibular dysfunction groups compared with the control group. Individuals with VMH were unable to maintain stable gaze and inhibit eye movements to background motion. The large number of subjects with diagnosis of fluctuating vestibular function in the VMH group compared with the vestibular dysfunction group may indicate that VMH is a maladaptation of the system.

CONCLUSIONS: Corneal endothelial dysfunction is a problem more commonly seen in the middle-age and elderly populations. Until recently, the only option available for these patients was a full-thickness PK. DSAEK is a promising new alternative that preserves the normal healthy cornea, allowing a faster and more stable recovery. This article discusses the indications, procedure, and management of DSAEK and comparisons to PK.

Patients today can benefit from an unprecedented array of vision correction options including: eyeglasses, contact lenses, refractive surgery, corneal shaping, and corrective intraocular lenses. However, to ensure patients receive the best possible correction, optometrists must carefully assess patients' visual requirements and specifically recommend the optimal correction.

CONCLUSIONS: It is important for optometrists to be aware of the high rates of self-reported symptoms and visual problems in military personnel returning from deployment to the wars in Iraq and Afghanistan. Post-traumatic stress disorder and depression may complicate optometric evaluation and management.

CONCLUSIONS: Optometrists should be aware of presenting signs and symptoms associated with retinal macroaneurysms as well as the many differentials and possible treatments. Given the high incidence of associated systemic disease, proper knowledge of necessary systemic workup is also critical.

CONCLUSIONS: Based on the preoperative predictions above, none of the potential vision tests was useful compared with the clinical judgement in the advanced cataract group. The super-illuminated pinhole (distance) provided additional information beyond clinical judgement in the moderate cataract subgroup. The Potential Acuity Meter and interferometer were inaccurate even in the presence of moderate cataract, and this and other recent findings suggest they should no longer be considered adequate for potential vision assessment.

CONCLUSIONS: This case illustrates discrete adenocarcinoma metastasis of an EOM, initially displaying characteristics predominantly consistent with orbital myositis. There is a paucity of epidemiologic data on EOM cancer, and clinical characteristics are derived only from a selection of case reports in the literature. Thus, the predominant features of global orbital metastatic cancer versus primary inflammation are highlighted in this presentation. This case shows that the variable characteristics of each process prohibit identification of any clinical feature that would prove pathognomonic for either disorder. The varied practice philosophies and standard of care regarding the proper time to biopsy are reviewed. This case shows the importance of early referral for orbital biopsy, even in ...

CONCLUSION: Advances in the understanding of the pathophysiology of RP are creating new opportunities for the treatment of this often visually debilitating eye condition. Optometrists, as primary eye care practitioners, should be aware of the inheritance, pathophysiology, and current treatment options for RP as well as treatments in development so that they can best care for their patients with inherited retinal disorders.

CONCLUSION: Lid varices are rare and usually visually benign. Ligation of small sections of the varix, as appropriate, may achieve the desired cosmetic effect and improve patient comfort. The varix usually does not recur after removal. Venography is used to evaluate the extent and supply of the varix before surgery. Cavernous hemangioma, a similar presentation, should be differentiated as it carries significant visual morbidity. Careful differential diagnosis with magnetic resonance imaging and computed tomography is essential with a new-onset or spontaneous varix.

CONCLUSIONS: TDS can present with visual field loss resembling that seen in chiasmal disease. Although most cases of temporal visual field loss from TDS do not respect the vertical hemianopic line and are not true quadrantanopsias, there are instances in which this does occur, likely caused by the testing modality used. It is essential that patients with suspected intracranial pathology undergo immediate neuroimaging, even in the face of TDS.

CONCLUSIONS: It has been established that MI and DD populations exhibit a higher incidence of oculo-visual anomalies (uncorrected refractive error, binocular vision anomalies, ocular pathology) than noted in the general population. They are also typically taking 1 or more neuropsychotropic medications that are frequently associated with undesirable visual side effects. Individuals with MI and DD should report numerous complaints associated with the medications they take and the oculo-visual anomalies they exhibit during the initial case history and the review of systems. The data from this study suggest that this is not the case and that only about 50% of those who should have complaints actually report them.

CONCLUSION: DCT provides IOP measurements that are less dependent on corneal factors than GAT, aiding in diagnosis and treatment of patients with ocular hypertension and glaucoma. Additional studies are necessary to examine the relationship between OPA, refractive error, and IOP and its possible association with increased incidence of glaucoma in myopic patients.

CONCLUSIONS: The data suggest that a PLTTT breakup time of <3.0 seconds might be suitable criterion for tear film dysfunction causing dryness symptoms. PLTTT may be a rapid and useful clinical method for practitioners to provide a glimpse of self-reported symptoms caused by contact lens-related dryness.

CONCLUSIONS: Descriptions of current practice patterns are difficult to ascertain without consensus on terminology and management criteria among low vision practitioners. This survey and accompanying literature review support the need for clinical research and education that will establish an efficacious and cost-effective model for private outpatient low vision rehabilitation for individuals with various levels of vision impairment to determine the true availability of low vision rehabilitation care in the United States.

CONCLUSIONS: Patients preferred Boston Simplus to Boston Advance with corneal reshaping lens wear when evaluated for comfort, unaided daytime vision, and care and handling. The preference of Boston Simplus to Boston Advance for care and handling is very strong and statistically significant, whereas other such advantages of Boston Simplus were not found to be statistically significant in this study. Further confirmation of these results as well as a better understanding of other visual and ocular interactions of Boston Simplus versus conventional solutions, will require using a larger sample in further studies.

CONCLUSIONS: Orange pigment was the most hyperfluorescent FAF finding. Because the presence of orange pigment is a known risk factor for malignant transformation, the use of camera-based FAF imaging may improve our ability to identify those choroidal nevi that will transform into malignant melanoma. More long-term follow-up studies will be required to determine the exact prognostic value of our findings.

CONCLUSIONS: Although the optical system of the corneal topographer was designed for measurement of convex surfaces, measurement of base curve radii from concave surfaces was performed without a correction factor using the prototype attachment. The mean SimK and Axial radii were within the tolerance for GP base curve radius, 0.05 mm, cited in ANSI Z80.20-2004 and ISO 18369-2:2006. Thus, the clinical feasibility of this prototype BC attachment to support measurement of spherical base curve radii for GP lenses by a corneal topographer was demonstrated.

    Authors:
Breakthrough research on age-related macular degeneration is stimulating the development of treatments, improving diagnosis, facilitating prevention through proper nutrition, and providing long-sought documentation on the effectiveness of low vision rehabilitation.Optometrists must take the lead in ensuring that patients reap the benefits. The February observance of Age-Related Macular Degeneration (AMD)/Low Vision Awareness Month provides a good time for optometrists to review their approach to age-related vision loss, ensure that they are ready to provide, or refer patients for, the best possible care, and prepare to conduct education efforts to ensure th...

CONCLUSIONS: Currently, there is no cure or direct treatment for progressive vision loss or complications that may develop from ONHD. Useful diagnostic tools include serial automated threshold visual fields, nerve fiber layer analysis, and fundus photography. It is suggested that ocular hypotensive agents be used to lower intraocular pressure prophylactically to prevent further nerve fiber layer and optic nerve damage.

CONCLUSIONS: Although visually impaired caregivers experience challenges similar to other caregivers, they differ in their report of added burden because of visual impairment, with extraordinary difficulty in transportation issues and the necessity of cyclical adaptations as their vision worsens. This is, to our knowledge, the first qualitative study of visually impaired caregivers and brings to light the unique problems faced by this special cohort of caregivers.

CONCLUSIONS: The Pediatric Outreach Program provides early identification and treatment of abnormal visual conditions, ensuring that children have maximal visual acuity and visual function. The educational aspects of the program have also contributed to its successes. Education for parents and early intervention agency staff promote the importance of comprehensive vision examinations for children 5 years of age and younger.

Congenital rubella syndrome is a rare disorder with devastating ocular and systemic consequences. Although efforts to eradicate the disease have been in place for some time, some areas of the world continue to be affected by this disease. The burden of the disease weighs heavily on patients and society; therefore, vaccination and other preventative strategies should continue to be strongly encouraged.
