CONCLUSIONS: Treating ICS with electronic alternate occlusion reduced suppression periods, increased binocular periods, and improved symptoms as measured in the COVD QOL questionnaire. Positive changes also occurred in reading scores. These data suggest ICS should be considered a probable cause for symptoms of reading problems.

CONCLUSION: The lenticular increase was additive in nature, but only in the late-onset and early-onset myopes. This confirmed and extended objectively earlier studies that suggested increased nearwork accommodative susceptibility of a primarily lenticular nature in myopes.

CONCLUSION: In older male patients with AMD, Zx-induced foveal MPOD elevation mirrored that of L and provided complementary distinct visual benefits by improving foveal cone-based visual parameters, whereas L enhanced those parameters associated with gross detailed rod-based vision, with considerable overlap between the 2 carotenoids. The equally dosed (atypical dietary ratio) Zx plus L group fared worse in terms of raising MPOD, presumably because of duodenal, hepatic-lipoprotein or retinal carotenoid competition. These results make biological sense based on retinal distribution and Zx foveal predominance.

CONCLUSION: This case report confirms that toric soft contact lenses are a good first choice in fitting patients with high and irregular astigmatism from postoperative LASIK corneal ectasia. It also confirms that excellent vision and comfort with toric soft contact lenses is possible in these patients.

CONCLUSIONS: These presentations are consistent with myopic traction maculopathy. Few myopic traction maculopathy cases have been studied with SDOCT. The time domain optical coherence tomography and SDOCT analysis suggests that the etiology of this condition may be secondary to preretinal and extraretinal traction exhibited in degenerative high myopia.

CONCLUSION: This case focuses on the utilization of a wavefront-optimized LASIK treatment profile to eliminate a high magnitude of astigmatic refractive error without inducing higher-order aberrations. Wavefront-guided treatments are not required in most cases but should be considered if the magnitude of preoperative root-mean-square higher-order aberrations is greater than 0.35 μm. Wavefront-optimized aspheric corneal ablations attempt to avoid reducing the prolate eccentricity of the average cornea, and optimized treatments have shown improved visual outcomes compared with conventional LASIK treatments. A thorough knowledge of refractive surgery is important for any treating eye care practitioner to provide patients with the full range of options to correct all types of refractive error...

CONCLUSION: Microtropia can defy detection, resulting in a reduced chance of achieving near-normal visual potential in children. It is important for clinicians to understand the possible etiologies, related entities, proper testing, differential diagnoses, and the goals of treatment. This case report reviews the clinical findings, diagnoses, and management of patients with microtropia.

CONCLUSION: Confrontation visual field testing in many places is the only method used for examining the visual field. A laptop computer system for visual field screening could be a better method for visual field screening in the primary eye care setting. For clinical relevance, it is necessary to perform the method with 2 different screening programs. The method can be useful as an important part of a routine examination and for directing further examinations.

    Abstract
Peripapillary subretinal neovascular membranes (PSRNVM) are most commonly associated with age-related macular degeneration and idiopathic causes in older patients. In younger patients, it has been linked to a wide variety of other conditions. As with the more commonly occurring macular form of choroidal neovascular membranes, PSRNVM can also lead to severe vision loss. Therefore, clinicians must take care to avoid overlooking this event to provide appropriate management and treatment. Current knowledge of PSRNVM suggests the importance of regular examinations of the affected eye in both treated and untreated cases to watch for progression and recurrence, which are unpredictable, and also of the fellow eye because there is a high risk of bilateral...

CONCLUSION: The low percentage of optometrists in the state of Michigan who practice low vision rehabilitation may lead to a shortage of providers for the aging and visually impaired Michigan population seeking low vision rehabilitation services. Even fewer are certified low vision providers through the voluntary Michigan Optometric Association certification process.

CONCLUSION: A bilateral and progressive appearance of a retinal pathology in a middle-age male patient, with no family ocular history, suggests systemic causation. One theory includes a back flow of porphyrin byproducts from PCT. This is exacerbated by a less-than-productive liver caused by high iron stores, chronic HCV, and a history of alcoholism, which prevents the normal filtering process to occur. We believe this is the first report of a case of presumed bilateral, geographic, peripapillary chorioretinal atrophy in a patient with PCT, complicated by high iron stores, HCV, and alcoholism.

CONCLUSION: Idiopathic bilateral, multiple, tiny, serous PEDs in middle-age healthy people are rare entities that may be a variant of central serous choroidopathy in which retinal pigment epithelium is predominantly involved. The visual prognosis seems to be good without any intervention unless complicated by hemorrhages.

CONCLUSION: Although the effects of vitreoretinal traction are most commonly associated with the macula or peripheral retina, residual attachment can occur at the optic nerve head. Peripapillary vitreoretinal traction can cause optic nerve head elevation with the appearance of edema that must be distinguished from sight- or health-threatening etiologies. OCT is a valuable diagnostic tool in confirming peripapillary vitreoretinal traction.

CONCLUSION: Although lipemia retinalis does not typically result in vision loss, it is a sign of a systemic condition that can have potentially fatal consequences. While the retinal appearance normalizes soon after resolution of the acute lipid imbalance, a multidisciplinary approach is necessary to obtain the desirable systemic outcome. Optometrists play a critical role in prompt referral of these patients for appropriate management of their lipids.

Vision affects daily activities, but of particular importance is the impact upon the learning process. Many studies have been conducted to establish the relationship between vision problems and academic performance. The results are varied, however, and suggest additional research is needed with particular care given to study design.

Smoking is causally associated with certain prevalent visually impairing eye diseases, including age-related macular degeneration and cataract. Studies have found that people are afraid of "going blind" and may be motivated to quit smoking if they know that vision loss is associated with smoking behavior.

CONCLUSION: Management for these hallucinations includes treatment of the actual ocular disease as well as optimizing vision for the patient using appropriate low vision devices. As the population continues to age, more patients will be seen with reduced vision caused by a myriad of ocular diseases, increasing the likelihood that more patients may present with CBS in the future. It is therefore prudent to become familiar with the syndrome so primary care optometrists can properly identify CBS and help their patients deal with it.

CONCLUSION: The presentation of a BRVO in young patients with minimal observable retinal effects from classic BRVO risk factors may indicate the necessity of a hematologic evaluation to determine possible compounding causes. Here we show a methodical inspection of the cause of an atypical BRVO and, more specifically, consideration for hypercoagulable pathologies and their treatment in such patient encounters.

Development of professional expertise is the gradual transition from novice to expert within a profession. Studies on expertise in the profession of optometry have never been published. However, many studies have been performed in other health professions (e.g., nursing, medicine, physical therapy, occupational therapy).

Authors: Oltz M, Check J
Rosacea is a chronic condition affecting the facial and ocular surface tissues. It is historically underdiagnosed and affects people of all ethnicities. The exact pathophysiology of rosacea is poorly understood. A variety of medical therapies are available to treat the various signs and symptoms of rosacea. Currently, there is no cure for the condition.

CONCLUSION: Monocular partial/sector occlusion therapy to inhibit intermittent diplopia in Brown syndrome offers a sensible alternative to surgery for those with normal or near-normal alignment, fusion, and stereopsis in primary and down gaze. For those who contemplate surgery, had failed surgery, or require treatment of underlying disease, monocular partial/sector occlusion therapy can serve as an excellent adjunct. Also, it may be attempted in other incomitant strabismic deviations with bothersome diplopia limited to specific positions of gaze.

Hyaluronan (HA) is a naturally occurring, biocompatible polysaccharide with unique viscoelastic and hygroscopic properties. Its role as a natural lubricant and its excellent water-retaining properties make it well-suited for use in ophthalmic products. Many reports have been written describing the various uses of HA. The purpose of this report is to review the uses of HA in the anterior segment of the eye, more specifically, its use in the treatment of dry eye syndrome, and in contact lenses and contact lens care systems.

CONCLUSIONS: Use of IT in optometry practices is widespread and likely to continue to increase in the years ahead. IT may be a key element in the future success of optometry practices in an increasingly competitive marketplace. Perceptions of high investment and training costs are barriers to further adoption. Optometrists may benefit from increased exposure to IT as part of their training.

CONCLUSION: HAART has significantly reduced HIV-related morbidity and mortality, but new complications have emerged. Recognition of immune reconstitution inflammatory syndrome and the correlation with ocular inflammation is critical because treatment is based on protecting the patient against the effects of the inflammatory response without compromising antimicrobial or antiviral therapy.

CONCLUSION: Intravitreal bevacizumab has proven to be an effective treatment for choroidal neovascularization and shows a significant improvement of vision for wet AMD patients. However, there are risks associated with the procedure. One of the most visually significant is an RPE tear, which can occur at an incidence rate as high as 17%. Optometrists should be aware of this rare, but serious, complication associated with anti-VEGF treatment for wet AMD.

CONCLUSION: IOI is a difficult condition to diagnose and treat. After ruling out other orbital pathologies, it is appropriate to begin treatment with oral steroids. In this case report, the patient noted an upper respiratory infection before each episode of IOI. Although there is no proof of cause, there is a strong case for the consideration of a viral respiratory infection leading to IOI.

CONCLUSIONS: Any patient with proptosis and/or ptosis with a history of cancer should be evaluated for orbital metastasis. Optometrists should keep in mind that an orbital metastasis may represent the initial manifestation of undiagnosed systemic cancer. Prognosis can be poor, and thus treatment is sometimes palliative in nature, intending to slow the progression of the disease instead of providing a cure.

CONCLUSION: Difluprednate 0.05% is a new topical therapeutic option indicated for the treatment of inflammation and pain management associated with ocular surgery with an off-label potential for treatment of other anterior segment inflammatory conditions. However, clinicians need to be aware of the potential risk for significant and potentially rapid onset of IOP increase with this medication and manage patients accordingly.

CONCLUSIONS: Some glaucoma patients appear to be unaware as to how glaucoma affects their vision. Understanding one's disease can enhance patient compliance and ensure that the patient returns for continued treatment and management. Patients with poor health literacy (especially with a disease with an asymptomatic beginning stage) are at a higher risk for noncompliance.

CONCLUSIONS: The quantification of the effects on the ocular tissues of 2 years of extended wear of senofilcon A, by mainly previously successful daily soft contact lens wearers, found an excellent biocompatibility. The results support the hypothesis that senofilcon A contact lenses, when worn on a 6-night/7-day extended wear regimen, maintain excellent long-term ocular tissue tolerance.

CONCLUSIONS: The quantification of the effects on the ocular tissues of 2 years of daily wear of senofilcon A, by mainly previously successful daily soft contact lens wearers, showed an excellent biocompatibility. The results support the hypothesis that senofilcon A contact lenses, when worn on a 2-week daily wear regimen, maintain excellent long-term ocular tissue tolerance.

CONCLUSION: OCT is a noninvasive instrument that provides information on the morphology of AOVD. It not only isolates the location of the lesion but can be utilized to monitor the stability of the condition. OCT interpretation of macular lesions can aid in diagnosis and management.

CONCLUSION: CIN has a variety of clinical presentations, including gelatinous, leukoplakic, or papilliform. Because of their malignant potential, these lesions must be differentiated carefully and treated promptly. Traditional management of CIN is surgical excision with cryotherapy, but other topical treatments are being explored with promising results.

This article summarizes genetic epidemiologic methods, describing family- and population-based methods used to locate and identify genes and the advantages and disadvantages of each approach. Health care professionals are faced with more and more genetic information, both from interested patients and from the media, and understanding the principles underlying genetic studies allows such information to be placed in context.

Authors: Lack D
Several medical organizations have published yet another joint statement trivializing vision therapy and vision disorders in the learning-disabled population. A review of the references in the joint statement as well as other references find that the joint statement is misleading because of inappropriate citations and selected references, as was the case with previous joint statements. The most current joint statement ignores the results of evidence-based research and makes recommendations regarding the treatment of convergence insufficiency that have no scientific validity. Ophthalmology should not allow professional rivalry to cloud its judgment regarding optometry's involvement in the diagnosis and treatment of learning-related vision problems.

Choroidal neovascularization in a young, healthy eye after LASIK.

CONCLUSION: To date, there is anecdotal evidence of CNV after LASIK. The incidence from prior reports ranges from 0.003% to 0.33%. However, a causal link cannot be disregarded and merits further study. Each patient undergoing LASIK warrants a thorough preoperative dilated fundus examination, including a thorough examination of the macula. This occurrence, albeit rare, should be considered for inclusion in the refractive surgery consent form.

CONCLUSION: Lymphomas can occur in a variety of sites in the body. It is well documented that primary tumors can originate in the ocular adnexa. Although not typical, the first sign in this case was a recurrent subconjunctival hemorrhage. The importance of a thorough ocular examination is paramount for a patient's ocular health and possibly the patient's life.

CONCLUSIONS: Optometrists should be aware of the symptoms of palinopsia to enable them to recognize this phenomenon and minimize the chance of misdiagnosis. Learning the physiologic mechanisms behind this uncommon disorder can help the clinician correctly identify its cause. Although palinopsia itself is not a disease, it is indicative of a disease, and the symptoms of palinopsia may be a manifestation of a serious underlying systemic dysfunction that could warrant treatment. In addition, identifying the symptoms of palinopsia can put patients at ease with regard to the often disturbing visual symptoms.

CONCLUSION: This lesion was unique because of the visibly detectable alterations seen in the photoreceptor layer of the retina and retinal pigment epithelium (RPE) in the setting of what was confirmed as a neurosensory retinal detachment. Torpedo maculopathy, although benign, may be visually devastating if its neurosensory etiopatholgy involves the macula. Even so, it rarely requires intervention.

CONCLUSIONS: Because patients with undiagnosed myasthenia gravis may present initially with ocular signs or symptoms, it is important for the optometrist to be familiar with the condition and the simple "in-office" tests that can be performed to establish a tentative diagnosis and management plan. The optometrist can also participate in the management of ocular manifestations of myasthenia and should be familiar with the use of a ptosis crutch (in addition to prism spectacles or occlusion therapies if indicated) as a nonsurgical intervention for ptosis.

CONCLUSIONS: All evaluated patients who had Fabry disease had corneal verticillata, which generally does not affect vision and is readily recognizable by slit lamp examination. Greater than 60% showed conjunctival and/or retinal vessel tortuosity. The eye care provider can play a crucial role in the early recognition of ocular manifestations of Fabry disease and decrease both the time to accurate diagnosis and the delay in the institution of disease-modifying therapy.

Normal pupil findings can give a false sense of security in a presumed cranial nerve III palsy patient: A unique case report.

CONCLUSIONS: Several important points are outlined regarding CN III palsies. First, lack of pupil involvement in an incomplete palsy is not equivalent to pupil sparing in a complete palsy. Second, after initial diagnosis, if subsequent findings are no longer consistent, further investigation is warranted. Lastly, myasthenia gravis should be considered as a differential in apparent palsy cases.

CONCLUSION: Angle closure can occur in highly myopic eyes. Careful gonioscopy and ultrasonography can lead to the correct diagnosis and tailored management for these eyes. Phacomorphic angle-closure glaucoma from spherophakia is associated with Weill-Marchesani syndrome as well as a few other uncommon syndromes. Isolated pseudophakia is a rarely reported cause of phacomorphic angle closure.

CONCLUSION: A retinal astrocytic hamartoma is typically associated with TS but can also present as a spontaneous, idiopathic lesion. Prompt referral to rule out TS or other systemic associations, particularly in young children, is important. Neuroimaging, along with other diagnostic tests, may ensure the proper management and rule out need for further evaluation. Spontaneous lesions without systemic association can initially be monitored closely to rule out possible progression. Once stability has been established, annual follow-up with a primary care eye practitioner is appropriate.

CONCLUSION: Although plasmacytomas of the eye and orbit are rare, it is important to be familiar with these tumors that may be associated with multiple myeloma. Definitive diagnosis is made by biopsy and histopathologic examination of the tissue. Plasmacytomas may be treated with external beam radiation, local excision, or radiotherapy after surgical excision. Long-term follow-up, including periodic systemic evaluation, is required to establish that orbital involvement is not an early manifestation of multiple myeloma.

Post-traumatic stress disorder (PTSD) can be defined as a memory linked with an unpleasant emotion that results in a spectrum of psychological and physical signs and symptoms. With the expectation of at least 300,000 postdeployment veterans from Iraq and Afghanistan having PTSD, optometrists will be faced with these patients' vision problems. Complicating the diagnosis of PTSD is some overlap with patients with traumatic brain injury (TBI). The estimated range of patients with TBI having PTSD varies from 17% to 40%, which has recently led the Federal government to fund research to better ascertain their relationship and differences. As a result of the sensory vision system's interconnections with the structures of the limbic system, blurry vision is a common sympt...
