    A leukemoid reaction is an extreme form of reactive leukocytosis defined as granulocytic leukocytosis above 50 × 10(9)/L produced by normal bone marrow, mostly in response to systemic infection or cancer. The mechanism as to how the haematopoetic system is altered to elevate production of myeloid cells is not known.A 69-year-old man presented with phlegmasia cerulea dolens caused by massive iliofemoral thrombosis. His workout at admission revealed absolute white blood cell count of 73.4 × 10(9)/L, with neutrophil granulocyte of 68.5 × 10(9)/L.     The use of a simplified system for perivenous infusion of tumescent fluid prior to endovenous laser ablation is described. Two infusion bag systems using different combinations of needle sizes (22, 23 and 25 gauge) and pressure settings (300 and 600 mmHg) as well as hand injections were tested in vivo. The technique is simple, safe and may lead to quicker infusion, reduction in hand fatigue and reduced cost.

            <description>CONCLUSION: The distinction between PVs and OVs is not always clear in literature, and as a consequence misinterpretations may occur. Very often the terminal valve of the GSV, which is in fact a PV, is designated as an 'ostial valve'. In view of its widespread use, we suggest that the term 'ostial valve' together with its clear description be included in the consensus documents of the 'Union Internationale de Phlébologie'.

            <description>CONCLUSION: Based on the use of prophylaxis only for 65.9% of general surgery inpatients at high risk for VTE, low use of prophylaxis is assumed to remain a significant threat to public health across Turkey. Inclusion of a standard VTE-PRAF in the hospital protocol seems to raise clinical awareness of VTE risk assessment and appropriate management in VTE which otherwise well-known to be associated with significant mortality and morbidity. Impact of e-VTE-PRAF is worth investigating.

            <description>CONCLUSION: Static disorders of the foot can be considered as an important risk factor that negatively affects CVD. In daily practice, it is often underestimated. This emphasizes the crucial importance of the detection of SFD during the clinical exam of all CVD patients. Correction of static disorders of the feet will improve symptoms due to the SFD, as well as those related to venous stasis. These results can easily be explained by improvement of foot pump efficacy during walking.

            <description>CONCLUSION: Superficial venous surgery in addition to compression therapy is the most efficient treatment of venous leg ulcers. The compression therapy should be continued both surgically and conservatively treated patients with healed ulcers. In special cases after superficial venous surgery (isolated superficial reflux) compression therapy could be applied only to the time of ulcer closure without continuing it longer. HVS and ultrasound therapy are useful methods in conservative treatment of venous leg ulcers. For surgically-treated patients these physical therapies are efficient only in superficial plus deep reflux cases. HVS and ultrasound can be alternative methods, but are less effective in recurrence risk. LLLT is not an efficient physical method in treatment of venous leg ulcers.

            <description>CONCLUSION: Isolated refluxing reticular and telangiectatic vein disease may cause QoL impairment in select patients and represent far more than a cosmetic concern. Compression therapy offers relief of aching, pain, leg cramping and restlessness in patients with isolated refluxing reticular veins and telangiectasia. Sclerotherapy of reticular veins offers a statistically superior broad spectrum relief of symptoms, while additional sclerotherapy of residual telangiectasia in this cohort demonstrated additive relief of aching and pain. Symptom assessments at 12 months suggest ongoing symptom relief following sclerotherapy.

In patients with ulcer recurrence and high tissue pressures, re-fasciotomy can be helpful to promote healing, particularly in patients with primary venous disease.

The first incident occurred two days after foam UGS to treat small tributaries of a great saphenous vein (GSV).             <description>CONCLUSION: This paper demonstrates an initial rate of thromboprophylaxis use considerably less than the ENDORSE trial. However the strategies employed following initial audit resulted in a significant increase in the prescription of both mechanical and pharmacological thromboprophylaxis. This example demonstrates the role of audit education and a risk assessment in stimulating change. Such strategies could be used to ensure compliance to recently published National Institute of Clinical Excellence VTE guidelines. Furthermore this example could be generalized to improve other aspects of care.

    An 82-year-old woman was treated with endovenous laser ablation for insufficiency of the right great saphenous vein. Because of a very thick layer of subcutaneous fat, puncturing the vein and introducing the laser fibre was difficult. The patient reported pain after activation of the laser. Subsequently, the procedure was discontinued and the catheter was removed. Inspection of the disposables showed that the laser fibre had punctured the catheter and was therefore located outside the lumen. Fortunately, there were no harmful sequelae in this case, but as device-related complications of EVLA are serious, reporting them is important.

    A 58 year old female patient underwent an intervention of phlebectomy below the knee. The patient gradually developed a severe neurological event and the Glasgow Coma Score decreased dramatically. After two and a half hours from onset, general conditions gradually improved and recovered completely. Lidocaine neurotoxicity was excluded. A right side motor deficit strongly suggests a transient ischaemic attack. As to invasivity, phlebectomy can be compared with liquid and foam sclerotherapy so that the simple question arises of also comparing their adverse effects. We have the strong conviction that this case may only be a co-incidental association.

    Deep venous thrombosis (DVT) and pulmonary embolism (PE) are rare complications after diagnostic femoral catheterization. A few clinical cases have been reported, especially in the setting of venous compression with an enlarging haematoma, mechanical groin compression or prolonged procedures. Similarly, clinically evident PE after cardiac surgery is also very rare, but may be catastrophic. In this article, we present a case of PE after silent (symptomless) DVT due to femoral catheterization. The patient had critical coronary artery disease and was a candidate of coronary artery bypass grafting. A inferior vena cava filter was implanted prior to cardiac surgery for prevention of a potential pulmonary embolus.

            <description>CONCLUSION: Sclerotherapy of telangiectases and reticular veins with detergent-like sclerosants such as polidocanol (POL) or sodium tetradecyl sulphate (STS) is a well-established technique. However, evidence from clinical trials comparing these substances with a non-active solution is sparse and does not live up to expectations of modern clinical trial concepts necessary for authorisation purposes. The presented multicentre EASI study fulfils these requirements and clearly demonstrates that Sclerotherapy of C1 veins with POL is highly effective and deserves the adjunct 'gold standard'.
