    Abstract
    This overview portrays the salient physiological mechanisms being involved in the clinical manifestation of chronic pain in traumatized patients. A «hypermnesia-hyperarousal-model» is purported to support the neurophysiologic plausibility of the trauma-pain-relationship. We discuss seven characteristic clinical pain entities which alone or in combination can be found in patients with a previous psychological trauma.

    Abstract
    Intraoperative Neurophysiological Mo-nitoring (IONM) identifies eloquent areas or nerves fibers during neurosurgical interventions and monitors their function. For several interventions IONM has become mandatory in neurosurgery. IONM increases patient safety during surgery as the risk of neurological deficits is reduced. Safer surgery reduces the time needed for the intervention and thereby reduces risk. IONM contributes to complete resection of tumors, which in turn prolongs patients' survival.     Abstract
    The Wernicke Encephalopathy (WE) as a result of a (sub)acute thiamine deficiency remains, unfortunately, still under-diagnosed, especially among non-alcoholics. It should be considered in each occurrence of delirium as well as in any ocular motor disorder and ataxia, in particular if there is a history of weight loss with or without gastrointestinal symptoms. In cases with suspected WE an immediate intravenous substitution of 3×200 mg/d is recommended.

            <description>The objective behind this quality control (performed for the time period from January 2006 through June 2010) was to analyse differences between employed and unemployed patients. Patients were interviewed in a longitudinal profile with a follow-up of three months after the interdisciplinarian evaluation (including medical, physio/occupational therapy as well as psychological evaluations). Client management for the group of currently unemployed persons was simplified for 63.8% of referring doctors following these consultations hours. With the group of currently employed persons it even facilitated client management for 79.2% of referring doctors, subsequent to consultation hours. 92.5% of these referring doctors would recommend the RIPA to colleagues and interested parties.

    Approximately 550 million people worldwide are chronically infected with hepatitis B (HBV) or hepatitis C (HCV) virus. The clinical course of HBV and HCV infection is variable and can lead to a chronic but mild hepatitis or to liver cirrhosis and hepatocellular carcinoma (HCC)1. Therefore, antiviral therapy should be considered in patients with chronic hepatitis to prevent progression of disease. Here, we review the current indications and guidelines for antiviral therapy in chronic HBV and HCV infection.

    Abstract
    Ischemic stroke is an important disease with high morbidity and mortality. Systemic fibrinolysis is well studied and has become the accepted international standard of acute therapy. Intraarterial lysis and intravascular mechanical treatment options are becoming increasingly important. The time window for acute therapies is limited to a few hours after the onset of symptoms. A substantial proportion of stroke is preceeded by a transient ischemic attack (TIA). The immediate identification of the cause of such a TIA and initiation of treatment can prevent as much as 80% of subsequent strokes. Recent clinical trials attempt to treat more patients with more individualized strategies.

    Abstract
    A 36-year-old woman with anorexia nervosa and resulting malnutrition decided to change her life and eat properly. One day after the refeeding start, instead of feeling better she felt much worse: fatigue, nausea and leg swelling occurred. She consulted our emergency department. A «refeeding syndrome» has been diagnosed. In this case report we discuss causes, clinical presentation and treatment of this illness. The aim is to make practitians aware about the existing of this disease even with oral refeeding. Warning signs have to be interpreted correctly in order to avoid dramatic consequences.

    Abstract
    Inflammation and neoplasia lead to an alteration of the normal tissue structure causing hardening of the tissue and an alteration of its elasticity. The elasticity modulus is a measure of the stress which is applied to tissue structures relative to the strain or deformation produced. Acute inflammation (softer tissue) will be compressed more than harder (neoplastic) tissue under the same pressure. This different deformation can be detected from the ultrasound data, analysed (autocorrelation) and displayed similar to a conventional Colour Doppler examination colour overlay of the conventional B-Mode image. In the following article elastographic applications are discussed.

    Confirmation and clarification of an adnexal mass is a domain of transvaginal ultrasound. The highest hit ratio is achieved by experienced sonographers relying on a dedicated set of sonomorphologic criteria. Less experienced may consult validated scores, colour doppler findings and the tumor marker CA125. The assignment to one of the three groups simple cyst, benign tumor, malign tumor should lead to an adequate procedure (control, laparoscopy, laparotomy).     Abstract
    A 65 year old HIV-infected patient (CDC A2, diagnosed in 07/2008) presented with facial, erythematous infiltrated papular lesions. Consistent with progressive immunodeficiency a low CD4+ T-cell count and an increase of the viral load was seen simultaneously and an eosinophilic pustular folliculitis (EPF) was assumed. Though, the lesional biopsy revealed a follicular eosinophilic infiltration and endotrichial hyphae, proving for an infiltrating dermatophytosis. This shows how an infiltrating Tinea faciei is mimicking clinically and histologically an HIV-associated EPF of the face.

    Patients not having a general practitioner will more likely use the emergency departments (ED) of hospitals for primary care. Crowding of the ED due to patients with minor health problems is a growing burden. The present work was aimed to analyze data of ED consultations at a Swiss regional hospital. Leading diagnoses of ED consultations covered a broad spectrum of internal medicine. The majority of patients seen in the ED are «walk-in» patients with minor problems that after initial evaluation and treatment in the ED could be managed as outpatients. Pediatric patients made a considerable part of the workload. Elderly patients (&amp;gt;65 years) were hospitalized more often.

    Resistant hypertension is defined as blood pressure that remains above therapeutic goal despite the use of 3 antihypertensive drugs including a diuretic. After exclusion of secondary causes and medical incompliance, the estimated prevalence of resistant arterial hypertension ranges from 4% to 19% in hypertensive patients. Treatment requires a multimodal therapeutic approach. Hyperactivation of the sympathetic nervous system plays a key role in the genesis of hypertension. Targeting renal sympathetic nerves, percutaneous catheter-based renal denervation is a new therapeutic option for the treatment of resistant arterial hypertension in selected drug resistant patients.

            <description>This article reviews the applications of high resolution ultrasound (HRUS) which were discussed at the SGUM congress in Davos in the talk «pearls and crystals». Sonography is an excellent and validated tool in the diagnostics and assessment e.g. of spondylo-arthritis and crystal deposition diseases. Enthesitis is a typical feature of SpA. The Sensitivity of HRUS diagnosing enthesitis is higher compared to the clinical examination. High frequency US probes allow furthermore an accurate assessment in crystal deposition diseases. Sonography is able to detect various typical pathologies in gout, pseudogout and apatite depo-sition disease. Will HRUS in the near future be able to replace microscopy in the diagnostics of crystal deposition diseases?

            <description>The objective of the LARA program was to identify the comorbidities and characteristics of asthma (A), intermittent or persistent rhinitis (IPR) and physician defined atopic dermatitis (AD) in 6- to 16-year old asthmatic Swiss children and adolescents. Overall, 126 general practitioners and paediatricians collected the data of 670 asthmatics. Approximately one third of the asthmatic children in Switzerland had well-controlled asthma. Almost two thirds of these asthmatics suffered from concomitant IPR. The latter presented with significantly less symptoms while the treatment rates with inhaled corticosteroids (approximately 90%) and leukotriene-receptorantagonists (approximately 50%) were comparable.     Abstract
    Erythropoietin is a glycoprotein predominantly produced in the kidney. It is an essential regulator of erythropoiesis in the bone marrow. Although cancer-associated anemia is caused by multiple factors, recombinant erythropoietin (rhuEpo) was widely used to treat and prevent this condition. Several clinical studies showed that the use of rhuEpo results in an efficient reduction of red blood cell transfusions in cancer-associated anemia. However, over the past twenty years, Epo and its receptor EpoR were found to be expressed also outside the hematopoietic system and in malignant tumors. This led to a discussion concerning potential risks associated with the application of erythropoiesis-stimulating agents in oncology.

    Abstract
    Stroke prophylaxis in patients with non-valvular atrial fibrillation is becoming an increasingly dynamic field. The new guidelines from the European Society of Cardiology (ESC) recommend a stroke- and bleeding risk-assessment in patients with non-valvular atrial fibrillation using the CHA2DS2VASc and the HAS-BLED scores, respectively. Furthermore, new drugs for stroke prophylaxis such as dabigatran, rivaroxaban and apixaban are undergoing approval in Europe and will undoubtedly challenge the well-established vitamin K antagonists. Hence, stroke prophylaxis is likely to become a much more individualized treatment in the future. This review should help to critically weigh the pros and the cons of the new therapeutic options.

            <description>This study concerns itself with consultations, what they are about, how they are structured and how their achievements can be described. In total, 127 recorded consultations were analysed. The 12 recorded general practitioners were themselves involved in the data analysis. The analysis confirms that in comparison to the European average, the Swiss consultation is rather long with a consultation length of 12.5 minutes. 80% of the consultation time is taken up by mere conversation. Per average, three medical topics are discussed within that time. Thus, about three minutes are at disposal per topic. In every consultation we can also find one non-medical topic. The structure of a consultation is complex and changes continually depending on the performed tasks.

    According to most studies, persons living or being born at higher altitude have a lower risk of fatal myocardial infarction or stroke than lowlanders. The altitude effect is more pronounced for myocardial infarction than for stroke and generally stronger in men than in women. Possibly, exposure to a certain altitude impacts on cardiovascular risk for the entire life span. It is unlikely that classical cardiovascular risk factors substantially influence the altitude effect. In contrast, environmental conditions that depend on altitude could play a role.

    Abstract
    Cardiac monitoring with ECG and blood pressure recording is routinely performed during a stress test and blood pressure response is an important part of its interpretation. Unfortunately, the latter is not straightforward, partly because of the inconsistent definition of a pathologic blood pressure response, the different populations included in the studies and divergent test modalities. An exaggerated blood pressure response in normotensive subjects without coronary heart disease is associated with an increased risk of future arterial hypertension, whereas in patients with known or suspected coronary heart disease, prognosis is not worse than in patients with normal blood pressure response.
