Although theatrical activities are used in a variety of therapeutic settings, little attention has been paid to the ways that theater can enhance the recovery process and community integration for people living with psychiatric disabilities. Community-based theater involving people with psychiatric disabilities offers unique opportunities for personal growth, social connection, and advocacy efforts. This Open Forum posits that theater has the power to both facilitate individual recovery and improve the social conditions of people living with mental illness. Critical elements of theatrical activities that relate to processes of recovery and community integration are examined. Implications for future research and program development are discussed.
CONCLUSIONS: The main health policy objective during the study period was increased access to mental health treatment. Both access and treatment intensity increased. The study was inconclusive about whether incentives related to CAMHS performance measurement caused treatment intensity to increase, specifically after the 2002 hospital ownership reform.
This article synthesizes the views of participants in two roundtables that were convened in Nairobi (March 2007) and London (July 2008) to identify key challenges to the prioritization of mental health in Africa and possible solutions. Participants included leading development experts and policy makers from head and country offices of international donors, national directors of mental health for several African countries, key mental health and public health professionals, epidemiologists, and an international nongovernmental organization. The challenges they identified to mainstreaming mental health include lack of understanding of the contribution of mental disorders to morbidity and mortality, competition for limited resources within health reform efforts, poor distribution of interventi...

Links between health institutions in countries with high incomes and those with incomes in the low-middle range are becoming more common, and benefits from these links are increasingly recognized. One such link is between Kings College Hospital and THET (Tropical Health and Education Trust) and their partners in Somaliland. The mental health part of the link consists of public service providers, educational institutions, and local nongovernmental organizations in Somaliland with mental health volunteers from the United Kingdom, led by the Institute of Psychiatry. The authors describe how careful planning has permitted the collaboration to evolve to allow mental health services to become embedded into activities that strengthen the Somaliland health sector.
Long-established Medico-Social Expert Commissions (MSECs) play a pivotal role in the Russian mental health system. They act as gatekeepers to pensions, rehabilitation, and employment services. This column describes their role in encouraging or impeding the social inclusion of people with mental illness, drawing on findings of a three-year project in Sverdlovsk Oblast. In Russia the emphasis remains on medical aspects of treatment, without adequate consideration of social and occupational rehabilitation. Links with local employment services are weak. To promote social inclusion, steps must be taken to encourage and facilitate cooperation and collaboration between the MSECs, employment services, and medical services.
This new column provides an overview of mental health reforms in Latin America and the Caribbean. Progress has been particularly visible in countries that have implemented policies with strong political support (Brazil, Chile, and Belize, among others). However, lessons may be learned from the collective experience of a region that has faced multiple obstacles to reform. Available resources are still insufficient and inequitably distributed, reform implementation is not complete in most countries, and high levels of unmet need exist. Countries face new challenges related to growing psychosocial problems affecting children and adolescents and increasing violence, which require new responses from mental health services.
This Open Forum illuminates shortcomings with the basis for determining degree of oversight of health services research and quality improvement activities. Using a federally regulated definition of research rather than a direct appraisal of risk to patients can misallocate effort from activities with higher risk for patients to those with lower risk. The case of the Johns Hopkins multicenter study of central line safety checklists in intensive care units is cited. Definitions of research promulgated by the Office of Human Research Protection are reviewed, and an alternative model based on patient risk is proposed. Suggestions for how quality improvement work fits into the larger paradigm of research are made. (Source: Psychiatr Serv)

CONCLUSIONS: Psychiatrists may not recognize the pervasiveness of racial inequality in psychiatric care, and they may attribute racially biased thinking to others but not to themselves. Interventions to eliminate racial-ethnic disparities should focus on revealing and modifying unconscious biases. Lack of physician interest may be one barrier to such interventions.
CONCLUSIONS: Results are consistent with U.S. clinical trials for other psychiatric disorders, which have reported similar outcomes for ratings of primary symptoms. Baseline psychotic symptoms are likely a significant contributor when African Americans with bipolar disorder are slow to recover. These results may be less generalizable to uninsured patients.
CONCLUSIONS: The DFD is a valid measure for estimating treatment outcomes that reflects the course of symptom change over time. When multiple assessments were conducted between the pre- and posttest periods, DFDs incorporated additional data yet remained easily interpreted. The DFD should be considered for reporting outcomes in depression research. (Source: Psychiatr Serv)

CONCLUSIONS: Outpatient commitment in New York State affects many lives; therefore, it is reassuring that negative consequences were not observed. Rather, people's lives seem modestly improved by outpatient commitment. However, because outpatient commitment included treatment and other enhancements, these findings should be interpreted in terms of the overall impact of outpatient commitment, not of legal coercion per se. As such, the results do not support the expansion of coercion in psychiatric treatment. (Source: Psychiatr Serv)

CONCLUSIONS: Although second-generation antipsychotics were frequently used for off-label indications, most of the usage was evidence based among elderly nursing home residents. However, the high level of non-evidence-based use combined with recent safety and efficacy data suggests an urgent need to address the evidence base for this vulnerable population.
This study examined medication use among Medicare beneficiaries and dually eligible beneficiaries before and after the implementation of Medicare Part D on January 1, 2006. Nationally representative 2004–2006 data from the Medical Expenditure Panel Survey were used. Two large classes of psychotropic medications (antidepressant and antipsychotic medications) and two large classes of nonpsychotropic medications (lipid-lowering and antihypertensive agents) were examined to determine whether changes in prescription patterns occurred as a result of the implementation of Part D. There was no strong evidence that Part D was associated with large changes in access to medications in the four classes of medications examined here. (Source: Psychiatr Serv)

Reaching disadvantaged adults who need mental health care is challenging, partly because of mistrust of institutions, cultural insensitivity, and stigma. Researchers from Western Psychiatric Institute and Clinic and leaders of 11 non-mental health community organizations formed a partnership to improve depression care, especially for elders and individuals from difficult-to-reach racial and ethnic minority groups. The overarching goal is to reduce disparities by providing and improving care. This column describes challenges overcome in working with a heterogeneous group of agencies to address issues of mental illness, stigma, inadequate staff training, and privacy—challenges that influenced the direction of research and ensuing projects.
One of the greatest challenges of restoring the New Orleans health care infrastructure since the post-Katrina disaster has been shortages of health care providers. Many providers had prolonged displacements or did not return to their practices, depleting the city of valuable resources. This Open Forum chronicles the displacement of Louisiana State University's Department of Psychiatry and discusses barriers to returning health care providers to their communities expeditiously. Predisaster planning and policy changes are proposed to facilitate a quicker return and decrease the attrition of health care providers after future disasters. A community's predisaster plans should include a mechanism to allow funds to follow patients instead of hospitals, to provide bridge funding that pays local h...

CONCLUSIONS: Assessment of physical illness is relatively common among mental health clinicians but was lacking in one-third of the cases in this study, until raised by patients. Because frequency of assessment differed by clinician discipline and patient ethnicity, innovations in patient assessment and clinician education are needed to address disparities in management of physical illness among individuals with mental illness. Care providers and clinicians should focus on staff training and supervision with regard to assessment and management of suicidal patients. Collaboration with referring physicians and aftercare providers is also important to help patients gain constructive coping strategies and break patterns of suicidal behavior and readmission.
Mental Health Service Use Among Suicidal Adolescents: Findings From a U.S. National Community Survey [Articles]

CONCLUSIONS: The mental health service needs of suicidal adolescents, especially those from ethnic minority groups and lower-income families, too frequently remain unmet. Larger racial-ethnic disparities were found in use of inpatient and outpatient mental health services than in use of school-based services. Mental health services offered within school settings can reach suicidal adolescents who need services but may experience barriers to standard types of care. (Source: Psychiatr Serv)

Antidepressants and Suicide Risk: How Did Specific Information in FDA Safety Warnings Affect Treatment Patterns? [Articles]

CONCLUSIONS: Release of specific risk and benefit information by the FDA was associated with changes in prescribing but not in outpatient follow-up. These results suggest that the FDA plays an important role in communicating information to the public and providers. Yet, although public health safety warnings were associated with changes in some practice patterns, not all recommendations conveyed in warnings were followed. (Source: Psychiatr Serv)

Law & Psychiatry: Liability Issues Associated With Referrals to Self-Help Groups [Columns]

Self-help groups, which are also referred to as mutual aid groups, are widely recognized as an important component of a comprehensive public mental health system that promotes recovery. Nonetheless, some psychiatrists and other providers have expressed concerns about these consumer-run groups and fear potential liability if they make referrals and something harmful occurs. This column reviews potential liability claims that could be made pertaining to referrals to self-help groups. The authors conclude that the likelihood of success of a plaintiff's claim is small. Recommendations are given for further lowering the likelihood of a successful suit. (Source: Psychiatr Serv)

It is well documented that African Americans receive a disproportionate share of their mental health care in the emergency room. Yet this disparate and undesirable pattern of service use has been inadequately examined and remains poorly understood. The disparity is often attributed to lack of access to outpatient care and to the low quality of available services, but these explanations represent untested hypotheses. This Open Forum reviews available data to illustrate how African Americans and white Americans are differentially affected by a broad range of social and community processes and trends, including characteristics of mental health systems and communities and changing societal conditions, and describes how these differences can lead to African Americans' disproportionate use of ps...

CONCLUSIONS: The findings imply that specialists are challenged when dealing with clients with criminal justice issues and use several approaches to overcome these challenges. Current specialist training may be deficient in preparing staff to effectively serve people with criminal justice issues. Further research should assess the efficacy of the approaches outlined in this article to give more guidance to specialists working with clients with criminal justice issues.
CONCLUSIONS: Our results suggest that failure to perceive need continues to be the major reason that individuals with alcohol use disorders do not receive treatment. On the other hand, among those who perceived a need, a majority received treatment. It is likely that high levels of unmet need for treatment services for alcohol use disorders will persist as long as perceived need is low. Efforts are needed to increase levels of perceived need among those with alcohol use disorders. (Source: Psychiatr Serv)

CONCLUSIONS: Patients with PTSD and depression had greater utilization of specialty mental health treatments and antidepressant medications and higher mental health care costs in the previous 12 months than depressed patients without PTSD. As military personnel return from Iraq, both VA and non-VA health care providers need to plan for an increase in outpatient mental health services and costs, particularly among depressed veterans who also have PTSD.
Involuntary Admission of Emergency Psychiatric Patients: Report From the Amsterdam Study of Acute Psychiatry [Brief Reports]

CONCLUSIONS: The hypothesis that outpatient treatment may help to prevent compulsory admission found some support in this study. More research is needed to understand the mechanisms of the associations so that an intervention study can be developed to test this hypothesis. (Source: Psychiatr Serv)

Population-Based Service Planning for Implementation of MBCT: Linking Epidemiologic Data to Practice [Brief Reports]

CONCLUSIONS: A large proportion of the population is eligible for MBCT introduction; however, after introduction, the rate of emergence of candidates would yield a smaller patient pool, which may limit implementation in small population centers. Treatment acceptability is a key variable. These analyses highlight the potential value of epidemiologic data and simulation modeling in planning. (Source: Psychiatr Serv)

Achieving Recognition That Mental Health is Part of the Mission of CDC [Open Forum]

For much of its history the U.S. Centers for Disease Control and Prevention (CDC) considered mental health to be outside of its mission. That assumption persisted even after CDC became a leading public health agency and began to face important mental health issues. This narrative describes how the organizational paradigm indicating that mental health was not mission related was challenged and superseded by a new paradigm recognizing mental health as part of CDC's public health mission. Even after the CDC Mental Health Work Group's establishment in 2000, CDC took eight more years to overcome powerful remnants of the old paradigm that had for so long excluded, minimized, or discouraged attention to mental health. The CDC Mental Health Work Group led the agency's mental health efforts without...

CONCLUSIONS: The findings indicate that the nurse practitioner role can potentially initiate safe and effective mental health care and treatment that is as satisfying as that initiated by a physician. Additional, larger-scale research is required to determine the generalizability of these findings. (Source: Psychiatr Serv)

This article summarizes historical perspectives on U.S. child mental health policies and their interface with education and discusses trends in educational policy relevant to children's mental health, specifically the Individuals With Disabilities Education Act and No Child Left Behind. The traditional approach of mental health research in schools, which focuses on program and intervention development, has become stagnant. New paradigms are needed. These include attending to indigenous school resources, to the organizational context of learning, and to participatory models for constructing environments conducive to mental health promotion and learning. Persistent underfunding and fragmented fiscal support, however, render new approaches meaningless. If progress is to be made, new funding s...
