Association of childhood family environments with the risk of social withdrawal (‘hikikomori’) in the community population in Japan

Conclusions:  Our findings suggest that hikikomori cases are more likely to occur in families where the parents have high levels of education. Maternal panic disorder may be another risk factor for children to develop hikikomori. (Source: Psychiatry and Clinical Neurosciences)

Aripiprazole may be free from tachyphylaxis: Preliminary findings

Loss of antipsychotic efficacy (tachyphylaxis), possibly linked to an increase in D2 receptor number and sensitivity, is a significant impediment to treatment in chronic schizophrenia patients. Animal studies, however, suggest that aripiprazole may be free from tachyphylaxis. The aim of the present study was to investigate this hypothesis. In this preliminary study, aripiprazole‐treated patients were retrospectively investigated for the presence or absence of tachyphylaxis. Clinical Global Impression‐Improvement (CGI‐I) scores did not significantly change and there was no significant association of CGI scores with fixed dose duration. This suggests that aripiprazole may be free from tachyphylaxis. (Source: Psychiatry and Clinical Neurosciences)

The aim of the present study was to investigate, for the first time, the relationship between circadian preference and mood seasonality in adolescents. To this end, 1539 participants (881 female; 658 male) completed the Morningness–Eveningness Questionnaire for Children and Adolescents and the Seasonal Pattern Assessment Questionnaire for Children and Adolescents to determine circadian preference and mood seasonality, respectively. Evening types achieved significantly higher mood seasonality scores than intermediate and morning types, as intermediates did than morning types. Agreeing with previous studies on young adults and adults, the present data indicate a significant relationship between eveningness and higher mood seasonality in adolescents.
Reminiscence therapy workshops for health‐care professionals (total, n = 105: four trainees in their 20, seven in their 30s, 13 in their 40s, 30 in their 50s, 41 in their 60, five in their 70s, one in their 80s, and four of unknown age) were held, and group work with reminiscence therapy was provided to local residents (total, n = 151; mean age, 73.6 ± 10.6 years). Comparison of pre‐ and post‐workshop questionnaires showed that rates of interest and understanding of trainees increased after the workshop. Concerning group work, 89.2% of the participants felt that reminiscence therapy would prove helpful in their life. Reminiscence therapy may be useful in improving mental health in local residents.

Conclusions:  The low dosages of amisulpride elevate serum prolactin level in the majority of patients. This finding indicates that the dose‐reduction of amisulpride has little effect to relieve amisulpride‐induced hyperprolactinemia at therapeutic dosages. Clinicians should monitor serum prolactin level even when low dosages of amisulpride are administered. (Source: Psychiatry and Clinical Neurosciences)

Conclusions:  As age increases, the chances of developing dementia increase. Dementia, contrarily, is not selective between genders. The likelihood of developing dementia increases as the stage of disease advances. Further research is required in order to understand underlying mechanisms of dementia in Parkinson's disease.
The objective was to examine effects of active interventions on physician's prescribing of antipsychotic polypharmacy. Prescriptions for patients with schizophrenia at the Centre for Addiction and Mental Health, Canada were collected in 2006 (n = 648) and 2008 (n = 778). During the intervening period, a pharmacist monitored prescriptions with antipsychotic polypharmacy and contacted corresponding prescribers to provide education on risks of polypharmacy. Moreover, educational sessions on polypharmacy were presented to inpatient and outpatient teams. A three‐fold decrease in the prevalence of antipsychotic polypharmacy was observed between 2006 (18.3%) and 2008 (6.6%). Thus, active monitoring of prescriptions with educational interventions could reduce antipsychotic polypharmacy. ...

Phonological fluency is uniquely impaired in Japanese‐speaking schizophrenia patients: Confirmation study

In alphabet‐based language‐speaking patients with schizophrenia, category fluency is disproportionately disturbed as compared with phonological fluency. Deficits in category and phonological fluency observed in Japanese patients, however, were similar. The aim of the present study was to replicate these findings by modifying the task to minimize the influence of lack of motivation and concentration in the patients. Similar deficits were found in both types of fluency in Japanese patients. Patients who speak Japanese have deficits in phonological fluency, compared with patients who speak alphabet‐based languages, suggesting that the pattern of impairment in verbal fluency in schizophrenia is dependent on the specific language system.
Conclusion:  Hyperhomocysteinemia was more frequent in bipolar I patients independent of C677T polymorphism. Patients had reduced levels of folate, which modulates homocysteine metabolism. Indeed, this finding indicates that folate supplementation may be appropriate for bipolar patients with hyperhomocysteinemia. (Source: Psychiatry and Clinical Neurosciences)

Conclusion:  As a life‐changing experience, an intractable disease may influence an increase in self‐transcendence. The results also showed that there was a strong correlation between self‐transcendence and respondents' subjective well‐being. Our results suggest that patients with life‐changing intractable disease can have a high level of self‐transcendence, which may lead them to regain mental well‐being, and increase their psychological health even in situations that cause physical and mental suffering.
Conclusions:  Without comprehensive postgraduate psychiatric education, the accuracy of recognizing psychiatric symptoms does not improve year by year.
The purpose of the present study was to investigate regional cerebral blood flow (rCBF) changes in a patient with very‐late‐onset schizophrenia‐like psychosis (VLOS) with catatonia. A 64‐year‐old woman developed catatonia after experiencing persecutory delusions. The patient's rCBF was examined using single photon emission computed tomography (SPECT) with easy Z‐score imaging system. Before treatment, hypoperfusion was observed in the striatum and the thalamus, whereas hyperperfusion was observed in the left lateral frontal cortex and the left temporal cortex. After treatment, the disproportions in rCBF disappeared, and hyperperfusion was observed in the motor cortex. Sequential SPECT findings suggest that rCBF abnormalities may be correlated with the symptomatology of catatoni...

Non‐clinical schizotypy was found to be related to poorer Wisconsin Card‐Sorting Test (WCST) performance, but the results were inconsistent. Two subgroups, the higher negative–higher positive and the lower negative–lower positive (15 vs 16), were selected from the top and the bottom quartiles of negative and positive scale scores of the Schizotypal Personality Questionnaire (SPQ) completed by 177 healthy volunteers, respectively. The higher negative–higher positive SPQ score subgroup had significantly poorer performance regarding the completed categories of WCST than the lower negative–lower positive SPQ score subgroup. Subjects with higher non‐clinical schizotypy trait showed relatively mild cognitive dysfunction. (Source: Psychiatry and Clinical Neurosciences)

This study examined the psychosocial and psychiatric features of 16 Japanese suicide completers with unmanageable debt compared with 23 suicide completers without such debt at time of death, using a psychological autopsy method. The individuals with unmanageable debt were more likely to have been self‐employed and to have experienced divorce. They were less likely to have engaged in help‐seeking behavior, despite having mental health problems. Our findings suggest that providing comprehensive support and promoting help‐seeking behavior may be important for suicide prevention in middle‐aged men.
Corticosteroids are widely used in modern medicine but can result in troubling psychiatric side‐effects. Physicians and other medical professionals should be aware of the potential for these side‐effects, possible means of prevention, and efficacious treatments. Herein, we review adult case report data published during the past quarter‐century on adverse corticosteroid‐induced psychiatric effects, and present a case of corticosteroid‐induced psychotic depression. PubMed and PsychLit databases were searched using the terms ‘corticosteroids’, ‘steroids’, and the generic names of corticosteroid medications with terms for psychiatric symptoms or syndromes, including psychosis, mania, hypomania, depression, apathy, anxiety, panic, depersonalization, delirium, confusion, halluc...

Conclusions:  Elevated serum cortisol levels in schizophrenic patients might be associated with the role of cortisol in the pathophysiology of schizophrenia. Also, the elevation of serum cortisol levels in first‐degree relatives compared to controls suggests that similar pathophysiological processes might have a role in individuals without any disease symptoms, but with a genetic predisposition for schizophrenia. Elevated serum DHEA‐S levels might be the result of a compensatory response to elevated cortisol levels. Serum cortisol and DHEA‐S levels may be used as a biological marker for the diagnosis of schizophrenia; however, further studies with larger sample sizes are warranted to support this finding.
The present study sought to determine whether the co‐occurrence of problem drinking heightens suicide risk in individuals with depression in Japan, using a sample of 784 outpatients (287 men and 497 women) with depressive disorder. Female subjects with at least a moderate problem drinking showed significantly more severe depression and suicidality than those without, but no such difference was identified in men.
Plasma substance P level in patients with schizophrenia: A cross‐sectional study

In this study, we examined clinical and demographic variables that could impact the plasma substance P level, which is a useful predictive biomarker of aspiration. Thirty‐four patients were included (mean age ± SD: 70.9 ± 10.8 years). A greater number of cigarettes/day and a higher antipsychotic dosage were found to be associated with a lower plasma substance P level, while age showed a trend‐level effect. This finding suggests the need for intensive observation for prevention of aspiration pneumonia in heavy smokers who are receiving a higher antipsychotic dose in this senior population.
Conclusion:  These findings obtained from newly hospitalized major depression patients support the previous notion that pain interference, depression severity, and early improvement can be the predictors for remission. Patients with high pain interference, a greater depression severity or a less early improvement are likely to require aggressive treatment early. These data require confirmation and extension to outpatients and other antidepressants. (Source: Psychiatry and Clinical Neurosciences)

Conclusions:  Most patients who were transferred to another hospital were readmitted within 3 months. Some women were readmitted in the late stage. They had relatively high cognitive and physical functions and most of their caregivers had care distress. The causes of readmission were not due to a decline in cognitive function. It is important to prevent complications in dementia patients and to establish a caregiving system for dementia patients that decreases the burden on caregivers in order to reduce the rate of readmission.
Successful aging in individuals with schizophrenia dwelling in the community: A study on attitudes toward aging and preparing behavior for old age

Conclusion:  Quality of life and the length of the hospital stay significantly contributed to forward‐looking factors of successful aging. Avoiding long hospitalization periods for patients with schizophrenia may lead to more active preparing behavior, but the improvement of quality of life may not be a sufficient condition. As schizophrenia patients have an optimistic attitude and insufficient preparing behavior, support to prepare such individuals for old age is required as part of community‐based psychiatric care strategies.
Conclusions:  Episodic course appears to be typical of OCD with BD. Bipolarity has a pathoplastic effect on OCD and it is possible that some forms of OCD and BD are pathophysiologically related. Bipolar OCD is associated with a higher rate of depressive episodes, higher suicidality and more frequent hospitalizations, suggesting greater morbidity. Long‐term prospective follow‐up studies and studies addressing pathophysiology and genetic basis are needed to understand the complexity of such comorbidity.
There is a need to reduce stigma and increase awareness in order to prevent social exclusion of people with mental illness and to facilitate the use of mental health services in young people. The purpose of this review was to examine the effects of educational interventions to reduce stigmatization and improve awareness of mental health problems among young people. An electronic search using MEDLINE, PsycINFO and Academic Search Complete was carried out for studies that evaluated the effectiveness of educational interventions. Forty eligible studies were identified. There were three types of educational interventions (Educational condition, Video‐based Contact condition and Contact condition). Eighteen of 23 studies reported significant improvements in knowledge, 27 of 34 studies yielded...

Conclusion:  Increased physical activity by pedometer walking plus individual motivational interviewing may be an effective program for the reduction of bodyweight and BMI in Thai schizophrenic patients who are obese or overweight. Its efficacy may be comparable to other cognitive/behavioral programs. Further studies in larger sample sizes are warranted. (Source: Psychiatry and Clinical Neurosciences)

Conclusion:  Yamashita (1977) reported that the majority of offensive subtype patients were doted on by their parents, although current offensive subtype patients are more likely to have had a troubled childhood, show severer forms of SAD, and more frequently exhibit an inflexible personality. This study suggested that the offensive subtype might not be essentially different from the non‐offensive subtype (quantitative rather than qualitative).
Kraepelin's system of mental diseases has contributed substantially to the foundation of modern psychiatric diagnosis in the DSM‐IV and ICD‐10. Considering Kraepelin's influence in the creation of these diagnostic tools, the question arises as to how Kraepelin's contemporaries and successors reacted to his nosology. Historical libraries and archives in Munich, Berlin and Paris were searched for European and American psychiatric literature using the keywords ‘Kraepelin’, ‘dichotomy’ and ‘manic‐depressive disorder’. Literature from 1900 to 1960 was examined and compared to the current literature regarding Kraepelin's impact on modern classification systems. The sixth edition of Kraepelin's ‘Lehrbuch’ (1899) presented the dichotomy in its entirety and led to three differ...

The purpose of the present study was to provide preliminary data on the usefulness and safety of blonanserin for patients with delirium in the intensive care unit (ICU). The charts of 32 consecutive patients with delirium in the ICU were retrospectively reviewed. These patients were treated with blonanserin. A total of 96.6% had reduction in Memorial Delirium Assessment Scale score. The proportion of patients with side‐effects was 24.1%. Blonanserin may be effective and safe in the treatment of delirium in the ICU. (Source: Psychiatry and Clinical Neurosciences)

Lithium‐induced polyuria, although common, often goes unrecognized. The purpose of the present study was therefore to investigate the complaints of polyuria, and distress and functional impairment associated with polyuria, in 56 patients with bipolar disorder on long‐term lithium treatment. All participants underwent 24‐h urine collection, and renal function tests. Polyuria (24‐h urine volume > 3 L) was found in 70% of subjects. Unless directly enquired about, polyuria was underreported. Impairment in work and daily routine due to increased urine output/frequency was associated with 24‐h urine volumes. Polyuria is a highly prevalent, distressing and impairing side‐effect of long‐term lithium treatment, requiring due attention. (Source: Psychiatry and Clinical Neurosc...

Cross‐correlation was examined for the volume of suicide‐related Internet searches and suicide death rate. Analysis of Google data and figures released by the Ministry of Health, Labour, and Welfare indicated that the volume of searches using the search terms jisatsu (suicide) and jisatsu houhou (suicide method) are not correlated with the suicide death rate. In addition, a rising suicide death rate might be related to the increase in suicide‐related search activity (particularly utsu[depression]), but an increase in suicide‐related search activity itself is not directly linked to the rise of suicide death rate.
Aripiprazole, a dopamine D2 receptor partial agonist, has been used to treat schizophrenia and might be effective for alcohol dependence and craving. We treated a 53‐year‐old woman with refractory medication overuse headache, which was successfully treated with aripiprazole. Our experience suggests that aripiprazole may be effective for patients with medication overuse headache. (Source: Psychiatry and Clinical Neurosciences)

We report two cases suffering from visual and auditory hallucinations: Case 1 abruptly stopped taking paroxetine (20 mg/day), and Case 2 discontinued paroxetine after reducing the dose from 20 mg/day to 10 mg/day for 5 months. Both cases experienced visual and auditory hallucinations in addition to dizziness, headache, insomnia, and nausea a couple of days after paroxetine discontinuation. These observations suggest that hallucinations are a part of the discontinuation syndrome that results from paroxetine discontinuation. Physicians should be aware of this symptom.
Conventional wisdom has not laid out a clear and uniform profile of schizophrenia as a unitary entity. One of the key first steps in elucidating the neurobiology of this entity would be to characterize the essential and common elements in the group of entities called schizophrenia. Kraepelin in his introduction notes ‘the conviction seems to be more and more gaining ground that dementia praecox on the whole represents, a well characterized form of disease, and that we are justified in regarding the majority of the clinical pictures which are brought together here as the expression of a single morbid process, though outwardly they often diverge very far from one another’. But what is that single morbid process? We suggest that just as the uniform defect in all types of cancer is impaire...

The aim of the present 12‐week, open‐label study was to investigate the effect of olanzapine augmentation in outpatients with depression with melancholic features who demonstrated partial response to standard antidepressants but who were reluctant to change antidepressants. The subjects consisted of 22 outpatients meeting the DSM‐IV‐TR criteria for major depression. Olanzapine was initially added at 2.5 mg/day and the dose was adjusted according to the clinical condition. Data were analyzed using an intention‐to‐treat methodology. A paired t‐test was used to compare total Montgomery Asberg Depression Rating Scale (MADRS) scores before treatment, at baseline (prior to olanzapine), and 4, 8, and 12 weeks after starting olanzapine. Of 22 enrolled patients, 20 completed the t...

Conclusions:  Paroxetine single acute administration diminished brain activity induced by motivation in healthy subjects. This may partially explain the increased lack of motivation seen in patients with relatively mild symptoms after taking a dose of paroxetine for the first time.

Conclusions:  Despite a general interest, general psychiatrists in Japan tend to possess insufficient knowledge of this subspecialty and lack experience in and opportunities to work in this subspecialty. The reluctance of psychiatrists to work in forensic mental health might be partly responsible for this situation. These results suggest that the enrichment of education systems for forensic psychiatry is necessary for the development of forensic psychiatry in Japan.
Conclusions:  It is possible that the increase in 5‐HT in response to glycine and d‐serine was mediated by N‐methyl‐D‐aspartate receptors. The transient increase in 5‐HT in the PFC might be associated with the alleviation of negative symptoms in patients with schizophrenia and the amelioration of sleep quality in patients with insomnia.
Mitochondria provide most of the energy production in cells. They are involved in the regulation of free radicals, calcium buffering, and redox signaling and take part in the intrinsic pathway of apoptosis. Mutations or polymorphisms of mitochondrial DNA, mitochondria‐mediated oxidative stress, decrease of adenosine triphosphate production, changes of intracellular calcium and oxidative stress are concerned in various diseases. There is increasing evidence that impaired functions of mitochondria are associated with mood disorders. It is suggested that disturbed energetic metabolism and/or reactive oxygen species production take part in the pathophysiology of mood disorders and could participate in the therapeutic effects or side‐effects of antidepressants and mood stabilizers.
Cognitive therapy/cognitive behavior therapy was introduced into the field of psychiatry in the late 1980s in Japan, and the Japanese Association for Cognitive Therapy (JACT), founded in 2004, now has more than 1500 members. Along with such progress, awareness of the effectiveness of cognitive therapy/cognitive behavioral therapy has spread, not only among professionals and academics but also to the public. The Study Group of the Procedures and Effectiveness of Psychotherapy, funded by the Ministry of Health, Labor and Welfare, has conducted a series of studies on the effectiveness of cognitive therapy/cognitive behavior therapy since 2006 and shown that it is feasible for Japanese patients. As a result, in April 2010 cognitive therapy/cognitive behavior therapy for mood disorders was adde...

Twin, family, and adoption studies have consistently shown that genetic factors play an important role in the pathogenesis of alcohol dependence. Numerous studies have aimed to identify genes that contribute to susceptibility to alcohol dependence. Whole‐genome linkage studies have identified several chromosomal regions that are linked with alcohol dependence. Association studies have also identified genes associated with alcohol dependence. Alcohol‐metabolizing enzymes, such as alcohol dehydrogenase‐1B and aldehyde dehydrogenase‐2, are the most well‐established genes that have polymorphisms associated with the risk for alcohol dependence. Polymorphisms in gamma‐aminobutyric acid receptor genes are also reported to be associated with alcohol dependence. The polymorphism of opio...

Our aim was to assess the internal consistency and structural/construct validity of the Center for Epidemiological Studies Depression Scale among middle‐aged employees in Japan. We conducted a cross‐sectional study of 7284 workers, aged 49.0 ± 6.3 (mean ± SD) years old. Structural/construct validity was assessed by confirmatory factor analysis. The 4‐factor structure reported in the general population was replicated, and a second‐order model with an overarching depression factor fitted well. These findings indicate that the Center for Epidemiological Studies Depression Scale is a valid and reliable measure of depressive symptoms for middle‐aged workers in Japan.
Metachromatic leukodystrophy (MLD) is an autosomal recessive lysosomal storage disease caused by a deficiency of arylsulfatase A. MLD is a heterogeneous disease with variable age at onset and variable clinical features. We evaluated a 33‐year‐old female patient who developed manifestations of disinhibitory behavior. She was diagnosed with MLD by genetic analysis, which revealed compound heterozygous ARSA missense mutations (p.G99D and p.T409I). The same combination of mutations was previously reported in a Japanese patient with similar symptoms. We performed additional, detailed neuropsychological tests with functional imaging on the current patient that demonstrated frontal lobe dysfunction. These results indicate that the mutations have important implications for genotype–phenotype...

Delirium is a complex multifactorial neuropsychiatric syndrome with a broad range of cognitive and neurobehavioral symptoms. It is associated with major adverse events and poor outcome. Little is known about delirium in the primary care setting. A retrospective chart review was carried out for the identification of ICD‐10 delirium cases presented in the regional medical office of a rural area in central Greece. Nine cases of delirium, mostly the hyperactive subtype, had been recorded. The patients tended to be elderly and suffering from dementia; infections were the underlying cause in most cases. All but one patient had been successfully treated in the primary care setting. (Source: Psychiatry and Clinical Neurosciences)

Conclusions:  The prevalence of depressive symptoms differed when assessed with CES‐D and HADS‐D. The reasons behind this difference include the fact that HADS‐D frequently detected depressive symptoms in patients with mild COPD as well as a tendency for HADS‐D to be strongly influenced by education levels. In contrast, the severity of COPD was reflected in CES‐D. It is possible that prevalence of depressive symptoms differs in accordance with the applied screening tool.
Central nervous system disorders are the third greatest health problem in developed countries, and schizophrenia represents some of the most disabling ailments in young individuals. There is an abuse and/or misuse of antipsychotics, and recent advances in pharmacogenomics pose new challenges for the clinical management of this complex disorder. Schizophrenia is a multi‐factorial/polygenic complex disorder in which hundreds of different genes are potentially involved, leading to the phenotypic expression of the disease in conjunction with epigenetic and environmental phenomena. Consequently, structural and functional genomic changes induce proteomic and metabolomic defects associated with the disease phenotype.Disease‐related genomic profiles and genetic variants in genes involved in dr...

Cortical and central white matter (WM) volumes were measured to assess short‐ and long‐range connectivity in autism, respectively. Subjects included 23 boys with autism and 23 matched controls, all without intellectual disability. Magnetic resonance imaging data obtained at 1.5 T were analyzed using BRAINS2 software (University of Iowa, Iowa City, IA, USA). Central WM volume was quantified by subtracting cortical from supratentorial WM volumes. Reduced central WM volume was observed in the autism group. IQ was higher in controls with no observed correlations between WM volumes and IQ. This preliminary evidence of reduced central WM volume in autism suggests abnormal long‐range connectivity.
This study examined prolactin levels, sexual function and clinical improvement after switching to aripiprazole from risperidone. Nine schizophrenic male Chinese patients who reported risperidone‐induced sexual dysfunction were studied. Clinical Global Impression Scale Scores for Severity (CGI‐S), Clinical Global Impression Scale Scores for Improvement (CGI‐I), Arizona Sexual Experience Scale results and serum prolactin concentrations were determined over 16 weeks. After treatment with aripiprazole, all patients showed reduced serum prolactin (26.54 ± 17.03 ng/mL to 3.71 ± 1.87 ng/mL, P = 0.008) and five reported improved sexual function. Mean baseline CGI‐S (5.11 ± 0.93) decreased to 3.78 ± 1.39 (P = 0.010) by week 16. Compared to baseline (4.0), the...
