            <title>Biting for attention: A case of dental discomfort manifesting in behavioural problems</title>

            <description>AbstractThere is a dearth of literature describing behaviour that expresses discomfort caused by poor dentition in patients with dementia. In this paper, we report on a patient whose behaviour only abstrusely pointed to his teeth as the source of discomfort. Although changes to his medication over a 2‐year period had little effect, eventual extraction of his caries brought about an almost immediate resolution of all antisocial behaviour. Clinicians must be mindful that poor dental care is easily meliorated but remains endemic in patients suffering from dementia. (Source: Psychogeriatrics)</description>

            <description>AbstractThe case of a 64‐year‐old patient with pica and severe mental retardation who was admitted to our hospital for treatment of recurrent cholecystitis is reported. Abdominal ultrasound showed sludge in the gallbladder, but no stones. Abdominal CT revealed a foreign body in the duodenum resembling a suction cup of the type commonly used in kitchens and bathrooms. The object could not be removed because it was deeply embedded in the hypertrophic intestinal mucosa. A nasogastric tube was inserted for feeding, since the object impeded the passage of solid foods. The patient's fever and abdominal pain subsequently resolved, and laboratory data improved. The indwelling feeding tube prevented recurrence of cholecystitis.             <title>Effectiveness of Japanese herbal medicine yokukansan for alleviating psychiatric symptoms after traumatic brain injury</title>

            <description>The Japanese herbal medicine, yokukansan, has been reported to improve behavioral and psychological symptoms of dementia and activities of daily living in patients with dementia. In the present case report, the authors report the effectiveness of yokukansan in treating psychiatric symptoms after traumatic brain injury. An 85-year-old man, who underwent surgery for hepatic portal cholangiocarcinoma, sustained traumatic brain injury after falling from bed as the result of postoperative delirium. He subsequently presented with psychiatric symptoms, showing markedly impulsive and aggressive behavior. Neuroleptics did not alleviate the symptoms.             <description>Conclusions: The total scores of the Mini-Mental State Examination (MMSE), the score of MMSE domain of registration and recall were significantly lower (P &lt; 0.05) and the Functional Assessment Staging (FAST) score, the number of different kinds of prescribed psychotropic medications (the number of prescribed psychotropic medications) were significantly higher (P &lt; 0.05) in the SAA (+) group than in the SAA ([minus]). At the same time, so much simple atrophy of nerve cells was evident under light microscopy. Electron microscopical observations of serial sections revealed that small blood vessels, including capillaries, had a deep relationship to the amyloid fibrils which formed the senile plaques and they had fallen into degenerative states. Both patients were treated successfully with behavioral intervention. In the first case, the patient's challenging behavior of repeatedly entering his wife's workplace and interrupting her work was reduced after the patient's wife changed her response to the behavior and her schedule prior to work to ensure that she spent time with him in the morning. In the second case, the patient's challenging behavior of urinating in the street was reduced by changing his walking route.             <description>Activation syndrome is a severe adverse effect caused by antidepressants. It is more frequently induced by paroxetine than by other selective serotonin reuptake inhibitors. However, there have been no descriptions of demented patients with activation syndrome derived from paroxetine. Herein, we report on three cases of dementia with Lewy bodies who suffered from activation syndrome induced by paroxetine. These symptoms rapidly disappeared following discontinuation of paroxetine. All five patients were prescribed Yokukansan (7.5 g/day) to reduce their symptoms.             <description>Herein, we report on a patient with spinocerebellar degeneration who exhibited subclinical rhythmic electrographic discharge of adults (SREDA). Thyrotropin-releasing hormone (TRH) improved the clinical symptoms and SREDA was observed only when administration of thyrotropin-releasing hormone was discontinued and the symptoms worsened. Furthermore, after resuming administration of taltirelin hydrate, a TRH analog, the improvement in motor function was accompanied by a decrease in delirium. This case report demonstrates the therapeutic effect of donepezil even for TD due to amoxapine, corroborating the view that donepezil is beneficial for the treatment of TD induced by dopamine antagonists.             <description>Conclusions: These results suggest that donepezil not only improves the cognitive dysfunction of AD patients, but may also relieve BPSD in these patients. Treatment with donepezil was also found to alleviate the burden of caregivers for approximately 60% of patients. Moreover, the results indicate that donepezil is unlikely to trigger potential risks of excessive deterioration of BPSD, which would result in a heavier burden of nursing care.
