Abstract
Radiology with oral contrast, or enteroclysis, have traditionally been the techniques of choice in the examination of the small intestine, due to the excellent visualisation of the mucosal pattern. However, the absence of extra-luminal information and the use of ionising radiation have replaced these examinations with sectional techniques which enable the abdominal cavity to be viewed with good resolution. Magnetic resonance enterography is a simple technique, with no ionising radiation, provided quality images, distends the intestinal lumen well by the administration of non-reabsorbable oral substances, minimises peristalsis, and establishes a protocol which includes sequences with intravenous contrast. Thes...

    Abstract
Magnetic resonance imaging is the main structural imaging in epilepsy. In patients with focal seizures, detection (and characterization) of a structural lesion consistent with electroclinical data allows therapeutic decisions without having to resort to other more expensive or invasive diagnostic procedures. The identification of some lesions may provide prognostic value, as in the case of Mesial Temporal Sclerosis (MTS) or may contribute to genetic counseling, as in the case of some Malformations of Cortical Development (MCD). The aim of this paper is to review the current state of structural MRI techniques, propose a basic protocol of epilepsy and mention the indications for structural MRI. Also, review the semiology of the main causes of ...

    Abstract
The Seventh Edition of the TNM Classification for non-small cell bronchogenic carcinomas include a series of changes in the T and M descriptor, in particular a re-classification of malignant pleural and pericardial effusions and of separated tumour nodes, new tumour size cut-off values and sub-divisions of the T1-T2 and M1 categories. We review these corrections that led to the changes in the staging system that affects stages II-III. Furthermore, we describe and illustrate the role of the different imaging techniques in tumour staging (CT, PET, PET-CT and MRI), highlighting their respective indications, advantages and disadvantages, as well their complementary function.

    Abstract
Heart failure is not always due to an alteration in systolic function, and a diastolic dysfunction could explain many cases of heart failure with a normal systolic function. Diastolic function depends on the left ventricular filling capacity to ensure a normal stroke volume. It is routinely measured with transthoracic echocardiography, as it is an easily accessible non-invasive test. The MRI, using flow sequences, shows good agreement with the echocardiography, analysing the diastolic function in a practical way, by the flow into the mitral valve and pulmonary veins. In this sense, the analysis of diastolic function should be added as part of a routine cardiac MR examination.

    Abstract
Cavernous malformations (cavernomas) are hamartomatous lesions formed by sinusoidal vascular spaces, with no cerebral parenchyma between them. Seizures are the most usual clinical presentation. They are dynamic lesions, producing changes throughout their evolution. The majority are located in the supratentorial region, but up to 20% of cases they are found in the posterior fossa. In computed tomography (CT) and in magnetic resonance (MR) their typical presentation is as a well defined round or oval lesion, with or without a minimal mass effect or oedema, with little or no contrast enhancement. Their appearance in MRI will depend on the stage of the haemorr...

    Abstract
Acute pain in the right iliac fossa is common in children. It can arise from a wide variety of gastrointestinal and genitourinary processes that make up the differential diagnosis with acute appendicitis. In this article, we describe the most representative findings of these processes on ultrasonography. We emphasize the characteristics that enable these processes to be differentiated from acute appendicitis.

    Abstract
Infantile myofibromatosis, despite being considered a rare condition, is the most common fibrous tumour in infancy. It is characterised by the presence of benign fibroblastic-myofibroblastic lesions. It usually occurs in children under two years-old, but it can appear at any age. The solitary form (myofibromas) may affect the skin, subcutaneous cellular tissue, muscle or bone. In the multi-centred form (myofibromatosis), there may also be visceral lesions. The lesions usually regress spontaneously in one or two years, with the prognosis being excellent in these cases. However, when there is visceral involvement, the prognosis is poor and treatment with chemotherapy is indicated. Lung involvement is more ass...

CONCLUSION: We believe that US is a useful procedure in the diagnosis of acute penile fracture and that it could be proposed as the diagnostic method of choice to confirm the clinical suspicion of penile fracture. Identifying the exact site of a tear in the tunica albuginea facilitates the surgical procedure.

This article reviews the anatomy and physiology of the right ventricle, as well as the manifestations of most common diseases affecting this chamber of the heart: infarction, cardiomyopathy, masses, and right heart failure. Knowing the distinctive features of the right ventricle with respect to the left and the particularities of the MR imaging protocol results in better technical performance in cases in which the reason for the examination or imaging findings point to the right ventricle. The importance of the right ventricle in the management of cardiopulmonary disease is growing and MR imaging can provide clinicians with the support they need.

CONCLUSIONS: The new protocol improved the management of these patients, reducing the time of immobilization, improving joint rigidity, and reducing the time off work. The limited MRI study makes it possible to diagnose other radiologically occult lesions. The cost of the new protocol is similar to that of the traditional protocol and even lower in some cases. The new protocol results in a reduction in the dose of radiation.

We present the case of a nine-year-old boy with Tourette syndrome and reading disorder with a history of a severe infectious process in the late neonatal period. Brain MRI showed a left parietal malacotic cavity and diffusion tensor imaging and tractography showed a striking disruption of the white matter bundle that joins the left parietal region with the ipsilateral frontal region with involvement of the left superior longitudinal fasciculus and of the left arcuate fasciculus. Although Tourette syndrome and reading disorder are fundamentally hereditary neuropsychiatric disorders, they can also occur secondary to cerebral alterations like those existing in this boy. The introduction of modern neuroimaging techniques in patients with neuropsychiatric disorders (or the risk of developing th...

Authors: Aguilar-García JJ, Alcaide-León P, Vargas-Serrano B
    Abstract
The processes that course with intraabdominal fat necrosis often manifest with acute or subacute abdominal pain; these clinical findings can be caused by various conditions, including epiploic appendagitis, omental infarction, encapsulated fat necrosis, mesenteric panniculitis, appendicitis, diverticulitis, and certain neoplasms. In this context, although the anatomic location of the pain and accompanying symptomatology can help orient the diagnosis, there is a risk of unnecessary surgery. Imaging tests like ultrasonography and especially computed tomography are essential for diagnosing intraabdominal fat necrosis. Radiologists must be familiar with the characteristic findings for all the conditions that can ca...

CONCLUSION: Direct digital mammography has improved the detection of microcalcifications, increasing the number of DCIS diagnosed without decreasing the positive predictive value of the invasive procedures indicated for microcalcifications. However, direct digital mammography has had a negative effect by increasing the recall rate and indication for short-term follow-up, possibly due to the difficulty of comparing the findings with those of earlier analog mammograms.

We describe the magnetic resonance imaging (MRI) findings for the spine in patients with seronegative spondyloarthropathy (SNS) and discuss the indications for MRI in the diagnosis and follow-up of this type of patients. We describe the pathological aspects of four patients diagnosed with SNS (Crohn's disease, ankylosing spondylitis, and psoriasis) with spinal involvement. The MRI findings in SNS vary in function of the type and stage of disease. Osteitis of the anterior vertebral bodies is a very early sign of spinal involvement in this group of diseases. Inflammatory involvement of the discovertebral complex that involves the adjacent vertebral bodies to a greater or lesser extent occurs later. MRI of the spine makes it possible to evaluate incipient signs of disease that are characteris...

CONCLUSION: Pancreatic involvement of renal cell carcinoma was detected in 4.5% of patients, ranking fifth in frequency in patients with disseminated disease. The arterial phase is necessary to detect pancreatic involvement of renal cell carcinoma. The pattern of presentation is nearly constant, helping differentiate pancreatic metastasis from primary pancreatic adenocarcinoma.

    Abstract
Enchondroma protuberans (EP) is an uncommon exophytic cartilaginous tumor described as an intramedullary osseous lesion that protrudes through a cortical defect in one of the sides of the affected bone and expands to the adjacent soft tissues. Due to its peculiar features on imaging studies, the main differential diagnosis is with osteochondroma, chondrosarcoma, and periosteal chondral tumors. In this article, we describe the imaging findings in two patients with EP in the humerus. We review the literature and discuss the imaging features that can enable a definitive diagnosis. The identification of the connection between the two components of the lesion through a cortical defect is a key ...

CONCLUSIONS: Adrenocortical tumors are rare in children. Adrenocortical tumors include adenomas and carcinomas, and in the absence of vascular infiltration and/or metastases it is difficult to differentiate between the two types by imaging and histology. The combination of an adrenal mass and clinical signs of adrenocortical hyperfunction in a child is virtually diagnostic of an adrenocortical tumor.

    Abstract
The outcome of treatment for rectal cancer in recent years has been improved by diverse advances in the field of surgery and in neoadjuvant oncologic therapies. Heald's introduction of the concept of the mesorectum as an anatomical unit (total mesorectal excision) in 1982 and the generalization of preoperative radiochemotherapy have improved the prognosis in a significant number of patients. Owing to these advances, it has become necessary for imaging studies to define a series of prognostic factors for tumors, both before and after neoadjuvant treatment, to make it possible to tailor treatment for individual patients with rectal tumors. On the other hand, the advent o...

    Abstract
A review is presented on the histological and radiological findings in idiopathic interstitial pneumonias, which are included among the diffuse parenchymal lung diseases. Although they may affect other compartments, the lung interstitium is the initial substrate of the parenchymal lesion due to different patterns of inflammation and fibrosis. The current classification, proposed in 2002 as an international multidisciplinary consensus document promoted by the American Thoracic Society and the European Respiratory Society, includes 7 conditions. Based on histological criteria, each histological pattern is associated with an image pattern. They are a group of conditions of unknown origin with common charact...

    Abstract
Most patients with articular or periarticular tumors present with chronic monoarthritis and nonspecific alterations on laboratory tests; thus, it is important to know the changes on magnetic resonance imaging (MRI) that can suggest their diagnosis and the differential diagnosis of these tumors. MRI defines the anatomic origin and the tissue characteristics of articular and periarticular tumors, making it possible to reach a specific diagnosis in most cases. It is important to evaluate the MRI findings together with the plain-film findings because this helps in the characterization of mineralized tumors.

    Abstract
Inflammatory pseudotumor is the most common primary lung mass in children. In many cases, it mimics organizing pneumonia on imaging tests. Another site often affected by inflammatory pseudotumors is the orbit, although they can be found in any part of the body. Inflammatory pseudotumors are rare and quasi-neoplastic, as radiologically and clinically they behave like malignant tumors. Consensus about their pathogenesis, natural history, imaging findings, and treatment options has yet to be reached.

    Abstract
Neuroimaging studies play a fundamental role in the diagnosis and evaluation of epilepsy. Technological advances in neuroimaging techniques have made it possible to obtain functional as well as structural information. The pathophysiology of epilepsy consists of an abnormal increase in cerebral activity that can be appreciated on neuroimaging techniques like functional magnetic resonance imaging (fMRI), PET, and SPECT. In patients with epilepsy that is refractory to drug therapy, the main aim of surgery is to control seizures and thus to improve the quality of life. In the preoperative workup of these patients, fMRI has an increasingly important role, evaluating the location of functional areas that must be safeguarded during s...

    Abstract
Baastrup's disease is not usually considered among the possible causes of low back pain in children. Classically, Baastrup's disease is characterized by degenerative phenomena secondary to friction between adjacent spinous processes, with sclerosis of the margins and decrease in the interspinous space. Baastrup's disease becomes more prevalent with age and is usually accompanied by degenerative changes in the vertebral column, such as lumbar facet hypertrophy or disc disease. Certain activities like dance or gymnastics can make Baastrup's disease more likely to appear at an earlier age. In children, Baastrup's disease can manifest in a different way, with increased interspinous spaces an...

We present the case of a patient with a pacemaker and a sarcoma lung metastasis treated with microwave ablation. Although the treatment of tumours with microwave ablation is a successful and minimally invasive approach, there are concerns about the safety of this procedure for patients with implanted cardiac devices, such as a pacemaker. After careful planning between radiology and cardiology, microwave ablation was indicated in the patient since it is safer and shorter than the radiofrequency technique. The lesion was treated without complications. It is important to communicate the procedures performed, as well as any complications in order to formulate guidelines for the use of microwave ablation in patients with pacemakers.

    Abstract
The growing demand for imaging tests and the progressive aging of the population have led to a progressive increase in the detection of indeterminate adrenal lesions. Once an adrenal incidentaloma is detected, clinical and hormone tests together with a battery of imaging techniques (CT, MRI, PET/CT…) make it possible to determine the cause in most cases. In this article, we discuss the advantages and limitations of each technique. We show the imaging characteristics of the most common adrenal lesions and propose a diagnostic algorithm to enable their diagnosis.

    Abstract
A perimesencephalic subarachnoid hemorrhage refers to the presence of bleeding around the midbrain with a normal angiography, meeting the well established clinical criteria and radiological criteria. Unlike the aneurysmal subarachnoid hemorrhage, it has a good prognosis, the recovery in most cases being complete and satisfactory. On the other hand, between 2 and 5% of the population will develop an intracranial aneurysm in the course of their life, most of them asymptomatic, with the likelihood of finding an incidental aneurysm in patients who have suffered a perimesencephalic hemorrhage. The importance of a proper diagnosis governs the course to follow, being a challenge for vascular treatme...

    Abstract
Sarcoidosis is a multisystemic granulomatous disease of unknown etiology. It mainly affects the thoracic lymph nodes and the lungs. The staging of sarcoidosis, which classifies patients according to their probability of spontaneous remission, is based on the plain chest film findings. Plain chest films are not as sensitive as high resolution computed tomography (HRCT) at detecting involvement of the lymph nodes, lungs, or bronchi. The high resolution CT findings can be typical, practically pathognomic, or atypical. High resolution CT provides information about the activity of the disease and detects incipient signs of fibrosis and other complications. To reach the diagnosis, it is necessary to correlate the clinical and radiol...

CONCLUSION: High grade astrocytomas originating in the supratentorial cortex can have an atypical neuroradiological presentation in the early stages. High grade astrocytoma should be included in the differential diagnosis of a cortical lesion that enhances slightly or moderately after the administration of contrast material in a patient with a new onset epileptic seizure.

    Abstract
Imaging-guided percutaneous biopsy techniques have been developed to diagnose the lesions detected in breast cancer screening programs based on mammography. Although traditional fine-needle aspiration cytology continues to be indicated in some cases, in many others it has been supplanted by more modern techniques like core biopsy or vacuum-assisted biopsy guided by ultrasonography, stereotaxy, or magnetic resonance imaging. These highly reliable techniques have minimized the need for surgical biopsy. Radiologists play a key role in the histological diagnosis of breast cancer in the early stages of disease and in the evaluation of its local and regional extension through magnetic resonance imaging and sentinel node biopsy.

    Abstract
Gastrointestinal bleeding represents a diagnostic challenge both in its acute presentation, which requires the point of bleeding to be located quickly, and in its chronic presentation, which requires repeated examinations to determine its etiology. Although the diagnosis and treatment of gastrointestinal bleeding is based on endoscopic examinations, radiological studies like computed tomography (CT) angiography for acute bleeding or CT enterography for chronic bleeding are becoming more and more common in clinical practice, even though they have not yet been included in the clinical guidelines for gastrointestinal bleeding. CT can replace angiography as the diagnostic test of choice in acute...

CONCLUSIONS: Our results corroborate previous reports. The exceptions can be explained by aspects related to variability and nonradiological factors that might influence the classification and PPV. Mathematical models should be developed to enable the objective classification and these should include factors not related to imaging.

    Abstract
Crohn's disease is a chronic disease with an unpredictable course. Patients with Crohn's disease will have to undergo numerous imaging tests. Crohn's disease often affects young people, who are more vulnerable to the harmful effects of repeated exposure to ionizing radiation. The high resolution of tissues on MR enterography give it a diagnostic accuracy similar to that of CT; however, MR enterography does not have the drawback of ionizing radiation. The clinical indices used to assess Crohn's disease are subjective and not very accurate; thus, enterographic techniques are becoming more common in clinical practice as a means to follow up patients objectively. Abstract
The reviewer's job is fundamental to the editorial process in a scientific journal. The quality of the review will determine the quality of the journal. Therefore, it is very important for the reviewers to make this mission their own and to contribute to the development of science and of the journal. On the other hand, the focus of a review will depend largely on the type of journal for which one is reviewing. In this article, we explain what the journal Radiología, the scientific journal of the Spanish Society of Medical Radiology, expects of its reviewers and their reviews of original articles. We aim to establish some basic rules to lay the groundwork for the development of more detailed guidelines for reviewing articles for the journal Radio...

    Abstract
Perfusion MRI makes it possible to evaluate the cerebral microvasculature through changes in signal due to a tracer passing through blood vessels. The most commonly used technique is based on the magnetic susceptibility of gadolinium in T2*-weighted sequences, and the most commonly evaluated parameters are cerebral blood volume, cerebral blood flow, and mean transit time. Diverse technical aspects, like the sequence used, and the dose and speed of contrast material injection, must be taken into account in perfusion MRI studies. It is also essential to consider possible sources of error like contrast material leaks due to changes in the permeability of the blood-brain barrier. The most widely u...

Teleradiology is an established fact in developed countries. It has been the subject of intense professional and even legal debate in recent years because the quality of care and the role of the radiologist as a medical specialist are at stake. The opportunities and the dangers involved in teleradiology are discussed in this article. The use of teleradiology in certain circumstances in the emergency department can result in significant benefits when done right; however, it is evident that poorly implemented teleradiology services can lead to significant decreases in the quality of care (for example when the aim is to provide a low cost reporting service through outsourcing). Radiologists must use their knowledge and collaboration to en...

Authors: Morales Santos A, Artigas Martín JM
Urgent imaging accounts for more than 50% of the overall activity in diagnostic imaging departments. The enormous impact of this workload makes the appropriate organization and management of urgent imaging essential. Providing urgent imaging services aims to meet the needs of urgent and critical patients from both within the hospital (wards) and outside it (emergency department). To achieve these aims, all hospitals should have a specific unit for urgent imaging. This unit should be large enough to deal with the volume of activity and it should be provided with the human, technological, structural, and organizational resources necessary to fulfill this mission.

Stroke results in significant morbidity and mortality. Recent years have seen a revolution in the diagnosis and treatment of stroke, with advances in diagnostic imaging and improvements in early and specific treatment. Multimodal CT (unenhanced CT, perfusion CT, and CT angiography) is widely available, fast, and efficacious, all of which give it a key role in the early diagnosis of stroke and in the selection of patients who will benefit from thrombolytic treatment. Unenhanced CT is useful for ruling out hemorrhage or lesions that simulate stroke and for evaluating the presence of early signs. Perfusion CT enables us to know the presence and extension of infarcted ischemic tissue (irrecoverable) and of penumbra (tis...

Multidetector computed tomography (MDCT) has significant advantages over plain-film X-rays in the study of the extremities. Acute trauma poses diagnostic challenges, especially in joints in which the overlapping of bony structures impedes the view of the fractures or luxations. Each major joint has at least one classification system for fractures that aims to include the fundamental aspects necessary to orient the most effective treatment. These are the cases in which MDCT's capabilities for multiplanar and three-dimensional reconstructions provide information that is crucial for the management of trauma to the joint.

Facial trauma occurs relatively often in traffic accidents, work accidents, and aggressions, causing repercussions in the functionality of the structures involved. Infectious processes require early and accurate diagnosis because they can progress quickly, leading to unexpected complications, or because of the severity of the infections themselves.

MDCT has become a fundamental tool for determining the causes of acute abdomen. CT is considered the imaging technique of choice in the diagnostic workup of both localized and diffuse acute abdomen, except in patients in whom acute cholecystitis or acute gynecological disease is suspected and in children, adolescents, and pregnant patients, in whom ultrasonography is the imaging technique of choice. Plain-film X-ray examination has been relegated to the initial management of renal colic, suspected foreign bodies, and intestinal obstruction. One of the drawbacks of MDCT is its use of ionizing radiation, which makes it necessary to filter and direct the examinations as well as to ensure that the most appropriate protocols are ...

We present the cases of two adolescents (13 and 18 years old) with fast-growing palpable breast nodules. Both underwent breast ultrasonography and core biopsy. The ultrasonographic findings of nodular pseudoangiomatous stromal hyperplasia were indistinguishable from those reported for juvenile fibroadenoma and for phyllodes tumor. Thus, histological study was required for the definitive diagnosis. Treatment depends on the size of the nodule. In our two patients, the lesion was surgically excised through tumorectomy and mastectomy.

We report the prenatal and postnatal MRI findings in a case of PHACES with involvement of the right posteroinferior cerebellar artery. This case is exceptional because, to our knowledge, it is the first to report the findings at both prenatal and postnatal MRI and because of the unique vascular anomaly that widens the spectrum of possible intracranial arterial anomalies in this syndrome.

Radiotherapy forms part of most therapeutic, preventive, and conditioning regimens in pediatric oncology. Numerous late secondary effects of cranial radiation are well known. However, radiation-induced cavernous angiomas (RICA) have been reported only sporadically and even fewer cases of earlier presentation of RICA have been reported. In this brief report, we describe a RICA that appeared in a boy treated for a CNS tumor (an infratentorial anaplastic ependymoma) after a short latency period between the end of radiotherapy and the development of the RICA. We comment on the different variables proposed to explain the formation of these lesions, as well as on their imaging features, treatment, prognosis, and follow-up.

Imaging biomarkers define objective characteristics extracted from medical images that are related to normal biological processes, diseases, or the response to treatment. To develop an imaging biomarker, it is necessary to carry out a series of steps to validate its relation with the reality studied and to check its clinical and technical validity. This process includes defining tests for the concepts and mechanisms; obtaining standardized and optimized anatomic, functional, and molecular images; analyzing the data with computer models; displaying data appropriately; obtaining the appropriate statistic measures; and conducting tests on the principle, ...

The wide availability, speed, safety, and diagnostic accuracy of multidetector computed tomography (MDCT) make it the first-line diagnostic tool in the study of acute bleeding of the lower digestive tract. The use of MDCT after the initial stabilization of the patient makes it possible to identify active bleeding and to determine its origin and possible cause, even when bleeding has stopped. MDCT provides information that is key to selecting the most appropriate treatment option: colonoscopy, embolization, surgery, or clinical follow-up. MDCT orients the surgical or endovascular intervention, minimizing the time, risks, and doses of radiation and of contrast agents involved in the intervention and avoiding "blind" resections associated wi...

We present the cases of two men with isolated spontaneous dissection of visceral arteries diagnosed by multidetector CT. In the first patient, dissection of the celiac trunk was associated with periarterial changes. In the second patient, dissection of the superior mesenteric artery was associated with stenosis at the root of the celiac trunk. Both patients presented with acute pain, which was more intense and longer lasting in the first patient. Aortic dissection was suspected clinically in both patients. Both dissections were short and had patent saccular false lumens and reduced caliber of the true lumens. This morphological type is one of the most uncommon within this rare entity. However, in recent years, the number of cases published is rising. This suggests that this entity may have...

Authors: Martínez León MI, Alcaide Martín D, García-Herrera Taillefer P, Ramos Rodríguez R
The middle aortic syndrome (MAS) is caused by stenosis of the abdominal aorta, often accompanied by concomitant stenosis of the renal or visceral arteries. Although MAS is uncommon (0.5-2%), it is an important cause of hypertension in children and adolescents. It is thought to originate in a failure of the two dorsal aortas to fuse during embryological development, and a high percentage of cases are idiopathic. MAS affects the segment of the aorta between the renal arteries in 54% of cases. Clinically, it courses with symptomatic or asymptomatic arterial hypertension. On physical examination, findings include an abdominal bruit, diminished or absent femoral pulses, and a difference between t...

Different models for training radiologists are used in different countries. Considering the trend toward international homogenization, it is important to know the differences and common traits among different countries to enable us to adapt our programs to future changes. We review training programs in radiology in Europe, the United States, Canada, and some Latin American countries. We focus on the selection process, residency programs, research, certification, subspecialization, and maintaining certification. We found a wide variability among countries, although there are more similarities within continents.

Authors: García Villar C
Evidence-based radiology is defined as the decision that results from integrating clinical information to select the most appropriate imaging test on the basis of the best available evidence, the physician's experience, and the patient's expectations. The practice of evidence-based radiology consists of five steps: formulating the question, performing an efficient search of the literature, critically evaluating the literature, applying the results of the search and evaluation while taking into account our experience and the patient's values, and evaluating the results obtained within our own practice. In diagnostic imaging, the number of resources available for evidence-based radiology is increasing: apart from books, articles, and web pages on this subject, e...

Authors: Pagés Llinás M, Darnell Martín A, Ayuso Colella JR
In 2008, CT colonography was approved by the American Cancer Society as a technique for screening for colorectal cancer. This approval should be considered an important step in the recognition of the technique, which although still relatively new is already changing some diagnostic algorithms. This update about CT colonography reports the quality parameters necessary for a CT colonographic study to be diagnostic and reviews the technical innovations and colonic preparation for the study. We provide a brief review of the signs and close with a discussion of the current indications for and controversies about the technique.

Authors: Varona Porres D, Andreu Soriano J, Pallisa Núñez E, Persiva Morenza O, Roque Pérez A
Patients with oncologic disease require frequent imaging tests (predominantly computed tomography) for follow-up. These patients may have thoracic vascular disease that can influence the diagnosis, treatment, and prognosis of their cancer. Primary vascular tumors can involve the thoracic vessels, like the pulmonary arteries (pulmonary artery sarcoma), and the neoplastic disease can extend locally (lung tumor) or remotely to the thoracic vessels (pulmonary tumor embolism and pulmonary tumor thrombotic microangiopathy). Oncologic treatment results in multiple complications that involve the thoracic vessels and can even compromise the patient's life in certain cases. CT, and especially multisl...

CONCLUSIONS: The craniocaudal view was more sensitive, although this difference was not statistically significant. The sensitivity of CAD was high for microcalcifications in all four density types; however, CAD's sensitivity for masses was low in density types 3 and 4. The CAD system only failed to mark 9.1% of the cancers presenting as masses but was not sensitive for the other two radiological findings included in this marking. Half of the interval carcinomas occurred in type 4 densities and 35.7% had been marked by the CAD sysem.

Our experience in the diagnosis and treatment of cerebral pseudoaneurysms.

CONCLUSIONS: In cases with clinical suspicion of a pseudoaneurysm, the patient should undergo angiography. This is especially important in patients with inexplicable cerebral hemorrhage and in those with septicemia. CT angiography and MR angiography have good diagnostic accuracy and can replace conventional angiography. However, the treatment of choice is endovascular and treatment should not be delayed unless access to the pseudoaneurysm is impeded, usually due to severe cerebral vasospasm.

Training the best radiologist is the tutor's aim. Considering both residents features -whose intention is becoming a radiologist- and the trainer profile, the method simultaneously combines the most suitable tools, in order to reach the aim. The method knowledge and the available tools facilitate the tutor planning and his cooperation in the whole resident learning process.

We present the case of a seven-year-old girl diagnosed with an intraosseous ganglioneuroma in the mandible. She had been treated for a suprarenal neuroblastoma (stage M) five years earlier. Bone scintigraphy and computed tomography of the mandible showed a lesion in the right mandibular ramus. Many osseous lesions can affect the mandible in children and the radiological appearance of most of them is unspecific. We discuss the differential diagnosis and main imaging findings of osseous lesions involving the mandible in children.

CONCLUSIONS: Ultrasound-guided core needle biopsy of axillary lymph nodes that are suspicious for metastases from breast cancer at ultrasonography is a highly effective procedure that has low morbidity. Findings of cortical thickening and non-hilar cortical vascular flow in the same lymph node yield a positive predictive value of 100% in this series.

CONCLUSIONS: Diabetic mastopathy is an uncommon disease that should be included in the differential diagnosis with breast cancer in diabetic patients. The diagnosis is complex because it requires knowledge of the patient's history, clinical presentation, and the imaging and histologic findings, Conservative surgery is the treatment of choice because the condition does not respond to medical treatment.

CONCLUSIONS: Ischemia after carotid angioplasty and stenting does not depend solely on the embolic load and its nature. We consider that the lower prevalence of postprocedural lesions in our series compared to others suggests that appropriate patient selection and experience minimize the negative influence of some variables like age in their development.
