CONCLUSIONS. The characteristics of our series were similar to others that have recently been published. We propose that the next criteria in the IHC should include the possibility of the pain not being dull and that it occurs on fewer than 15 nights per month. The lower limit for the age of onset could be reduced to 40 years.

CONCLUSIONS. The resulting scores show that the MFE-30 is a useful instrument in clinical practice. Findings are in line with those from previous studies, thus suggesting that there is a close relation among the appearance of cognitive complaints, the presence of prefrontal symptoms and perceived stress.

CONCLUSIONS. The portable bladder ultrasound was an effective method that allowed greater control of the patient, it detected urine retention accurately, preventing further discomfort to the patient, and we recommend their incorporation in nursing care of acute stroke, especially in the first days of hospitalization.

CONCLUSIONS. The frequency of pharmacoresistance in the population analysed was estimated to be 56%. Identification of the factors associated with pharmacoresistance makes it possible to consider surgical treatment or to optimise the treatment with AED. There is a tendency to use the new AED, above all in combination therapy. Our findings are consistent with those described in the literature.

    Abstract
Stroke is a leading cause of mortality and the main cause of severe and long-term disability in adults. Following treatment during the acute phase, there is a need to continue the treatment of the patients in the rehabilitation phase, in order to improve the outcome and daily life activities. This is the role of rehabilitation programs. Rehabilitation is focused on increasing brain plasticity to recover some of the lost functions, based on different methodologies, including pharmaco-therapy. In this context, the role of citicoline in the rehabilitation of patients with stroke is reviewed.

CONCLUSIONS. The patient, in accordance with current criteria, presented idiopathic hypereosinophilic syndrome with an undetermined subtype. To our knowledge, the association with eosinophilic fasciitis and multineuritis has not been reported to date. There is no proven infiltrating mechanism in multiple mononeuritis, which corroborates the poor control of the neuropathic clinical symptoms with oral corticoid therapy. Association with gabapentin, which stabilises the axonal membrane, also backs up the neurotoxic pathogenetic hypothesis.

    Abstract
Non-convulsive status epilepticus is a significant issue for a neurologist because, despite its low prevalence, it mimics other pathologies, with therapeutics and prognostic outcomes. Diagnosis is based on clinical features, mainly mental status or impaired consciousness and electroencephalographic changes, so electroencephalogram is the first exploration we must perform with clinical suspicion. There are three clinical forms: generalized or absence status, with diffuse epileptiform discharges; focal, with epileptic discharges located in a specific brain area and may not affect consciousness; and subtle, with diffuse or local epileptic activity after a tonic-clonic seizure or convulsive status and limited or no motor activit...

CONCLUSIONS. Factorial analysis with Varimax rotation obtained as the main factor resulting in the emergence of social interaction problems such as in the validation performed for teens population, keeping the three factors of the original test, although with some differences in their composition. It also had a high internal consistency by Cronbach alpha coefficient (0.86) which confirms the reliability of the scale. In terms of demographics variables analyzed, the variable number of failures significantly explains the variance of all factors that make up the scale, being the only significant variable in the factor of attention deficit and problems with social interaction. The sex variable is significant in the factor behavior problems and hyperactivity whereas age is only in the hyperacti...

CONCLUSIONS. In Mexico, ICH represents about a third of the forms of acute cerebrovascular disease, and the majority of patients present severe disability or death at 30 days of follow-up. Hypertension is the main cause; hence, control of this important cardiovascular risk factor should reduce the health burden of ICH.

CONCLUSIONS. The level of compliance of epileptic patients is rather poor. Patients with refractory epilepsy are more aware of the severity of their disease, they are more critical with the health care system and with their social setting, and they tend to comply better with their treatment. Hence, there are no reasons to believe that therapy non-compliance can explain a high proportion of the resistance that exists in epilepsy.

    Abstract
Music is a very powerful multimodal stimulus that transmits visual, auditory and motor information to our brain, which in turn has a specific network for processing it, consisting in the frontotemporoparietal regions. This activation can be very beneficial in the treatment of several syndromes and diseases, either by rehabilitating or by stimulating altered neuronal connections. We also review the peculiarities of the musician's brain and we look at how the brain adapts according to the needs that must be met in order to improve musical performance.

CONCLUSIONS. This study traces, for the first time in our setting, the profile of the psychogenic patient who is admitted to the neurology unit and examines their hospital management and their outpatient follow-up. We highlight the need not to underestimate this pathological condition and to generate an ordered way of managing it, which should always be multidisciplinary with the leading roles played by the neurologist and the psychiatrist.

CONCLUSIONS. The Dysphapark questionnaire is a suitable measure of dysphagia in PD, according to the Rasch analysis. A high proportion of patients with PD have dysphagia, although it has been observed that they have a low level of awareness of the condition, of the consequences it may have and of the possibility of using thickening agents. Given that some of the swallowing disorders in PD are asymptomatic and that the level of awareness of the disorder is low, we recommend including specific questionnaires as well as clinical and instrumental evaluation of dysphagia in clinical practice.

    Abstract
Neuromodulation for treating pain goes back to the times of the ancient Egyptians, who applied natural electric currents to modulate the painful sensation. Since then, this concept has been developed in parallel with the scientific and technological development, and various forms of neuromodulation with different indications and characteristics have originated. Chronic pain may produce significant disability, which leads to important physical, social and psychological consequences. Peripheral nerve, spinal cord, deep brain and motor cortex stimulation are safe and effective techniques that control pain and improve quality of life in selected patients.

CONCLUSIONS. Although the total number of publications and the observed increase are lower in Spain compared to Italy, France or Germany, the Spanish publications in Clinical Neurology shows good trend indicators with regard to publications in English and international collaboration. This improvement was associated with greater visibility as showed by the five-year analysis of citations received by Spanish publications.

    Abstract
A critical aspect in all models is the assessment of the final outcome of the modelling procedure. In the case of a focal ischaemic brain injury, apart from the determination of the size of the lesion, another valuable tool is the evaluation of the final functional deficit. Indeed, ischaemic damage leads to the appearance of different degrees of sensoriomotor and cognitive impairments, which may yield useful information on location and size of the lesion and on the efficacy of neuroprotective treatments after the acute injury. In addition, the magnitude of these impairments may also ...

CONCLUSIONS. In outpatient neurological pathology, it is not uncommon to come across cases in which it is difficult to reach the specific diagnosis. Clinical symptoms such as dizziness, instability or gait disorders often lack precise criteria with which to make a particular diagnosis. An approximation to certain diagnoses by means of the agreement of experts could be useful in several of these syndromes.

CONCLUSIONS. We recommend to use ketogenic diet as treatment in refractory epilepsy since there is a positive response in seizure control in some cases. The adverse effects seen could be prevented or treated without complications. It is a preferable treatment option before using other aggressive therapeutical measures or when surgery is not feasible.

CONCLUSION. Our aim is to back the proposal of epicrania fugax as a new syndrome with a well-defined clinical spectrum. It does not appear to be an exceptional condition and further knowledge about it will probably give rise to the description of new series. Treatment is often necessary and, although further information and experience are needed, gabapentin and lamotrigine both play a promising role.

CONCLUSIONS. Using serial TCD examinations in comatose patients after initial recovery from cardiac arrest, to detect and treat early changes in cerebral hemodynamics, will decrease the likelihood of secondary neurological damage. In the first 24 hours, TCD could identify patients who have progressed to irreversible neurological damage, thus avoiding therapeutic futility.

    Abstract
The aetiology of multiple sclerosis remains unknown at the present time, although the most likely explanation is that it has an autoimmune inflammatory origin. During the history of this disease a vascular pathophysiology was once proposed, and it has recently re-emerged as a result of the work by Paolo Zamboni with the name of 'chronic cerebrospinal venous insufficiency'. Following this hypothesis, Zamboni puts forward a curative treatment for multiple sclerosis by means of endovascular treatment of the internal jugular vein and the azygos vein. However, several teams have attempted to replicate his findings without success. In this review, we offer a chronological description of the studies carried out by Zamboni and the later attem...

    Abstract
Despite the significant reduction in mortality, ischemic stroke is the leading cause of morbidity and mortality in adults in industrialized countries. Antiplatelet therapy has reduced the risk of stroke and other vascular events. Although most commonly used treatment is aspirin, other antiplatelet drugs with different mechanisms of action have been developed. Clopidogrel blocks platelet aggregation by inhibiting the adenosine diphosphate receptor and the subsequent activation of the complex IIb/IIIa. In addition to its antiplatelet action, it also has pleiotropic effects, reducing the inflammatory mechanisms that occur during ischemia. Clopidogrel has demonstrated a benefit over asp...

CONCLUSIONS. This protocol offers several important advantages for physicians, including systemisation, the possibility of replication and convergence among evaluators or delimitation of the sub-processes that can be evaluated by sharing the same scheme. Moreover, it can all be carried out in sessions that are short enough to allow them to be offered by nearly all services that attend to addicts who request treatment.

    Abstract
In recent years, image techniques have transformed the diagnosis in Medicine. Nevertheless, they are, by now, far from a widespread clinic and surgical use. Beyond the simple picture inspection, DICOM digital files can be explored by using image analysis/processing techniques in a better way. In practice, clinicians and surgeons usually are limited to 'see' pictures delivered by Radiology. However, image analysis/processing techniques allow the 'client' (neurologist or neuro-surgeon) to accomplish interactive image visualizations, multiplanar or 3D rendering, measurements, pathway visualizations, etc. All these tasks, offer a great advantage in diagnosis, surgical planning, teaching an...

CONCLUSIONS. To our knowledge the association of normal serum iron with high serum ferritin levels in patients diagnosed clinically and polygraphically as having RLS with periodic leg movements has not been described before. The notion of an involvement of a dopaminergic mechanism in the pathophysiology of RLS is supported by the decrease in the values of serum ferritin concentration observed in one patient during follow-up while being treated with dopaminergic agents.

CONCLUSIONS. The pattern of health of persons with ID differs from that of the general population, and they use healthcare services differently. It is important to develop programmes of health promotion and professional training that are specifically designed to attend to the needs of persons with ID. Likewise, it is also necessary to implement health surveys that include data about this population.

CONCLUSIONS. Advances in the management of stroke in recent years are associated to a decrease in the need for admission to hospital, mortality, and mean length of stay in hospital, together with a change in the clinical profile of the patients treated in emergency departments. The increase in the number of cases of strokes among young people is something that health care systems should take note of.

    Abstract
Studying the cognitive impairment of multiple sclerosis (MS) patients is fundamental for a global understanding of this disease. Neuroimaging techniques might provide crucial data about the nature of this deficit and their progression. Accordingly to this idea, over the last years there has been a marked increase in the number of studies devoted to explore the possible relationship between the cognitive alterations of this clinical population and different sorts of neuropathological indexes provided by both, classical as well as by more recently developed techniques. The results of the studies using structural information provided by structural techniques, have revealed the important...

    Abstract
Brachial artery flow mediated dilation (FMD) is a validated, noninvasive physiological measure widely used as a research tool to quantify endothelial function. FMD is diminished in patients with several coronary risk factors, coronary artery disease, peripheral arterial disease, stroke, and is an independent predictor of cardiovascular events. FMD represents a useful method for identify asymptomatic atherosclerotic subjects with raised risk of developing atherothrombotic complications and improves with risk-reduction therapy such as antihypertensive or antidiabetic drugs, antiplatelet agents and statins. FMD could be a great usefulness in ischemic stroke such as stroke subtypes classification, prognostic ...

    Abstract
The white matter is the main connection between different regions of the brain and helps them to work in a unified, coordinated way. Diffusion tensor imaging is an ideal technique with which to study it in order to detect the degree of integrity of these fibres. Nowadays, they are considered to play a significant role in the development and pathophysiology of different developmental disorders, and the aim of this study was to examine this role. On reviewing disorders such as autism, dyslexia or attention deficit hyperactivity disorder, certain fibres were found to be clearly involved. This was especially the case of the (arcuate) superior longitudinal fasciculus and the temporal-parietal network (related with the regulation of m...

CONCLUSIONS. At the time they were diagnosed with ADHD, one out of every three patients was in a deficient nutritional situation (subnutrition or malnutrition). Continued treatment with MTF-OROS for 30 months had a negative influence on height, which could perhaps be attenuated by improving the patients' nutrition.

DISCUSSION. The intraluminal occlusion of the MCA model, although yields in the highest mortality rate (28%), it is the more suitable for long term studies, mainly because of the absence of craniotomy. In the electrocoagulation model a non ischemic lesion region is observed which leads to an abnormal lesion evolution as compared with the other two models.

CONCLUSIONS. The IP accounts for 13% of all first visits due to headaches and their diagnoses are similar to those of the local population. Emigration is neither a precipitating nor an aggravating factor for headaches in our series. There is a significant difference in symptomatic and preventive treatment between the period prior to immigration and afterwards.

Gluten sensitivity is a systemic autoimmune disease that occurs in genetically susceptible individuals on ingesting gluten. It can appear at any age, then becoming a permanent condition. It is more frequent in women, as happens with other autoimmune diseases. Celiac disease is the intestinal form and the most important manifestation among a set of gluten-induced autoimmune pathologies that affect different systems. Neurological manifestations of gluten sensitivity, with or without enteropathy, are also frequent, their pathogenesis including an immunological attack on the central and peripheral nervous tissue accompanied by neurodegenerative changes. The clinical manifestations are varied, but the most common syndro...

CONCLUSIONS. We consider it necessary to systematize the table that has already been outlined. This will enable us to answer not only the question of whether the cerebellum plays a role in human cognition but which is its role. Perhaps the basis for understanding the cognitive functions of the cerebellum are not found in the fact that the cerebellum contains functions but that it relates the intention to the action in the emotional as well as the behavioural cognitive plane although the role of 'interface' is found closer to the output processes than processing of functions with a motor component.

CONCLUSIONS. The SBPTS syndrome has a variable presentation with a low sensitivity on the initial CT, requiring brain MRI for typification. This explains the diagnostic difficulty and the fact that its frequency is probably underestimated.

The drug used as preventive treatment, the side effects, a younger age and a lower number of seizures before starting the preventive treatment favoured higher drop-out rates. Whether the migraine was episodic or chronic, the presence of medication abuse and the drugs used to treat the seizures were not related with dropping out of preventive treatment.

Novel diagnostic and therapeutic approaches based on noninvasive brain stimulation offer some promise in the field of childhood psychiatric disorders. There are two primary methods of noninvasive brain stimulation currently available: transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS). Both noninvasive neuromodulation techniques appear to rely on modulating brain plasticity and thus open new hopes in the treatment of brain circuit and plasticity disorders. Since many childhood psychiatric disorders involve disturbances in the timing or mechanisms of plasticity within frontostriatal circuits, and the developing brain shows a greater capacity of brain plasticity, noninvasiv...

CONCLUSIONS. Magnetic resonance imaging, in all its different sequences, is the preferred diagnostic technique, since it allows 100% intracranial meningiomas to be diagnosed. Although surgery is the preferred treatment technique in most cases, radiotherapy, in its different modes, is an essential therapeutic tool, both as a complement to surgery and as the first choice.

This study analyses the links between the Russian and Soviet neurosciences and their Spanish counterpart, especially with regard to the experiences of the Spanish Republican physicians exiled in the USSR. The Russian neurosciences, which date back to the second half of the 19th century, followed a path that ran parallel to the discipline throughout the rest of Europe and finally displayed signs of being influenced by the German and French schools. Important figures include Alexei Kojevnikov and Vladimir Bekhterev in neurology, Sergei Korsakov in psychiatry, Ivan Pavlov and his disciple Piotr Anojin in neurophysiology, Lev Vygotsky and Alexander Luria in neuropsychology, and Nikolai Burdenko in neurosurgery. When the Bolsheviks came to power, they brought with them a progressive conception ...

Epilepsy is a neurological disorder that affects 1-2% of the population. The majority of epileptic patients achieve a good seizure control with the current available treatments. However, there is a subgroup of patients that remain severely disable despite the variety of anti-epileptic drugs, the possibility of surgery for resection of the epileptogenic foci in selected patients, and vagal nerve stimulation; various lines of research are being carried out to look for new treatment alternatives. Deep brain stimulation (DBS) of the thalamus has emerged as a therapeutic alternative for patients who remain in-capacitated; the efficacy of this new therapy is subject of several studies, and its effectiveness and safety has not been establish...

CONCLUSIONS. Intracranial meningiomas are, today, the leading group of intracranial tumours. Although most of them are histologically benign, recurrence rates range between 10% and 20%, even after performing complete resections. Knowledge of risk factors and the genetic disorders that they present will make it easier in the future to introduce customised treatments to fit each case.

CONCLUSIONS. In only a few cases, such as this one and in a small number of incipient studies, have there been reports of conduction blocks that, owing to the fact that they are commonly related with demyelination, give rise to confusion when it comes to diagnoses and prognoses. The condition usually displays axonopathy due to Wallerian degeneration. It therefore seems recommendable in these cases to proceed with caution and to perform neurophysiological studies in the following weeks in order to prevent inappropriate diagnostic and prognostic judgements from being formed.

Multiple sclerosis is the most frequent chronic inflammatory, demyelinating and neurodegenerative disease in young adults, but has no definitive pharmacological treatment. It is a heterogeneous disease from the immunological, neuropathological and clinical point of view, as well as in terms of its response to different therapies. Over the last two decades, pharmacology has focused on developing drugs that are capable of modifying the course of this disease, with the aim of reducing the frequency of the outbreaks and the speed at which the disability produced by the disease progresses. Nevertheless, today, there are no drugs that are capable of offering a curative effect that can fully stabilise the disease, and ...

Authors: Mendez-Herrera CR, Cordovi-Rodriguez D
POEMS syndrome is a rare multisystemic paraneoplastic disorder. That is associated with a plasma cell dyscrasia. It is characterized by peripheral neuropathy, organomegaly, endocrinopathy, M protein, skin changes and other systemic features. The pathogenesis of POEMS is not well understood, but overproduction of vascular endothelial growth factor is likely to be responsible for most of characteristic symptoms. There are no randomized controlled trials in patients with POEMS syndrome but are therapies which benefit these patients, including radiation therapy, chemotherapy, cortico-steroids and stem cell transplant.

CONCLUSIONS. Our report supports the hypothesis that disruption of the gamma-actin-binding site located in the central rod domain plays a crucial role in the shock absorber function of dystrophin in muscle cells. Description of pathogenic variants in the DMD gene and the resulting phenotypes has important implications on the designing of molecular therapeutic approaches for DMD.

CONCLUSIONS. Focusing attention on the white matter and its disorders promises to further our knowledge of the brain as an extraordinarily complex organ in which the connectivity and processing speed provided by the white matter are key features in cognition, emotion and consciousness itself. The white matter does not appear to be the deposit for cognitive processes and functions, but rather its role would seem to be linked to two properties that are essential for a highly complex system like the brain to be effective: speed and connectivity. Further studies must be conducted to delve deeper into these processes and perhaps begin to clarify the different roles they play in the outcome of cognitive processes.

This article outlines the questions that must be included in the medical history of vertigo in order to determine whether these dehiscences are present or not. The diagnostic procedures that are best suited to confirming it are also addressed. Superior semicircular canal dehiscence can be resolved satisfactorily by surgery.
