            <title>A Rare Case of Aplastic Anemia Caused by Temozolomide.</title>

            <description>A 65-year-old female with glioblastoma multiforme (GBM) who developed aplastic anemia from temozolomide is reported. The patient presented with rapidly developing slurred speech, apathy, memory difficulties, an ataxic gait, and right-sided weakness. She was found to have GBM. She received standard external beam radiotherapy concomitant with temozolomide, followed by adjuvant temozolomide. On day 14 of adjuvant treatment, she presented with profound fatigue and spontaneous bruising and was noted to be severely pancytopenic. After an extensive workup, she was found to have aplastic anemia on bone marrow biopsy. In contrast to the obvious findings of pneumothorax on chest radiograph, the ECG manifestations are sometimes difficult to identify.             <description>We describe an extremely rare case of emphysematous cystitis in a nondiabetic, nonimmunocompromised elderly patient and we summarize the pertinent literature. The only risk factors for emphysematous cystitis in our patient were old age and female sex. Emphysematous cystitis can present asymptomatically. In such instances, the diagnosis should be suspected based on imaging findings. Physicians should be aware of this potential lack of typical symptoms and the presentation of this clinical entity in nonimmunocompromised individuals.             <description>In this report, we discuss some of the basic principles of diffusion-weighted imaging (DWI), a sensitive magnetic resonance imaging (MRI) technique that detects the mobility of water molecules within the brain parenchymal tissue. Although the most common cause of what is known as hyperintense lesions on DWI is ischemic stroke, we will present multiple cases that may also have hyperintense lesions but have a diagnosis other than ischemic stroke. While usually seen in patients with underlying cardiovascular diseases, atrial flutter can also exist in patients with no obvious risk factors. Typical atrial flutter is characterized by a classical saw tooth appearance of P waves on electrocardiogram.             <title>Hospital-Acquired Thoracic Empyema in Adults: A 5-Year Study.</title>

            <description>Little is known about the etiology, microbiology and outcome of hospital-acquired thoracic empyema (HATE) in adults. In our study, we classified HATE into two categories: hospital-acquired pneumonia (HAP) related and non-HAP related. We found that HAP-related empyema had a significantly higher incidence of aerobic Gram-negative organisms (76% vs. 38.7%, P = 0.005), polymicrobial pathogens (40% vs. 9.7%, P = 0.008), and anaerobic pathogens (20% vs. 0%, P = 0.009) than non-HAP related empyema. Thus, choice of antibiotic treatment for HATE should be based on the etiology of the pleural infection.             <description>The coexistence of multiple and synchronous primary neoplasms in the genitourinary system has rarely been described in the literature. The case presented here is the first report of a very rare tumor, a tubulocystic carcinoma, simultaneously occurring with micropapillary urothelial carcinoma in the bladder and clear cell type renal cell carcinoma in the same kidney. Tubulocystic carcinoma is a recently entitled tumor. It had been thought to arise from the distal collecting duct for a period of time and was therefore named as a low-grade collecting duct carcinoma. Tubulocystic or tubulopapillary architecture and low nuclear grade with hobnail morphology defines the tumor microscopically.             <description>We report a rare case of pancreatic adenocarcinoma presenting as a symptomatic colon carcinoma in a young man with transplanted kidney. The patient presented with abdominal pain and weight loss. A colonic mass was visualized via colonoscopy and biopsy showed a poorly differentiated adenocarcinoma. Surgery was performed and pathologic examination revealed a metastatic pancreatic carcinoma.             <description>This report details the successful utilization of nitazoxanide and probiotics to treat multi-recurrent Clostridium difficile infection (CDI) in a peritoneal dialysis patient. A 58-year-old female was admitted with hypotension, nausea and vomiting attributed to metronidazole therapy for Clostridium difficile. This was her third CDI treatment regimen in 3 months. During her admission, the patient developed CDI and was started on a 6-week regimen of nitazoxanide and probiotics to assist in re-establishing the colonic flora.             <description>Cardiovascular magnetic resonance imaging (CMR) has advanced in the last ten years and has been proven a reliable and flexible method for cardiac diagnosis. Stress perfusion magnetic resonance imaging has been shown to accurately detect coronary heart disease. The role of CMR is also increasing for evaluation of congestive heart failure and cardiomyopathies. Late gadolinium enhancement highlights areas of myocardial scar and helps predict prognosis. CMR usually complements, but does not fully replace other diagnostic modalities. Careful patient selection is required to ensure safety in the presence of this powerful magnetic field. Stamps with medical themes have also been used to educate the general public about different aspects of medicine. These include stamps to celebrate the careers of prominent medical scientists, and raise awareness about other aspects of medicine, such as disease prevention, medical technology, immunization, medical instrumentation and the history of medicine. To our knowledge, this is the first study of concentrations of uranium decay products (radium and radon) concurrently in the soil, tap water, and indoor air in the northern part of the Texas-Mexico border. Using field data collected over a period of four years, we analyzed radon gas in the indoor air and compared it to the levels in the tap water and soil. Other factors that were analyzed included housing characteristics, air temperature, the US or Mexico side of the border, and urban or rural residence. The basis for vitamin D/TB interaction and a brief literature review are discussed. Grade 3 neutropenia was the only toxicity observed.             <description>Ultrasound-guided fine needle aspiration cytology (US-FNAC) represents the most effective test available to distinguish between benign and malignant thyroid nodules. The major limit is the rate of inadequate specimens, which generates a much higher cost. The adoption of a stylet needle significantly improves diagnostic performance, reducing the expenditure per diagnostic procedure by about 14%.             <description>Sodium polystyrene sulfonate (SPS, Kayexalate) has been implicated in the development of intestinal necrosis, especially when administered with sorbitol. Previous studies documented intestinal injury primarily in postoperative, dialysis, and transplant patients. We conducted a retrospective review of all patients at our institution from December 1998 to June 2007 to further describe characteristics of patients with probable SPS-induced intestinal necrosis. Findings from our study indicate that patients may be susceptible to SPS-sorbitol intestinal injury even in the absence of end-stage renal disease, surgical intervention, or significant comorbidities. All adult trauma patients using aspirin, clopidogrel bisulfate (Plavix&amp;#x00AE;), warfarin (Coumadin&amp;#x00AE;), or heparin admitted to the hospital with computed tomography (CT) scan evidence of brain injuries were included in the study. Patients were classified into three groups; the anticoagulation group, antiplatelet group (29 patients total), and the control group (63 patients). Although most body packers swallow the drug packets, the insertion of packets into body cavities such as the rectum and vagina has also been reported. Radiological imaging plays an important role in detecting body packing in potential drug carriers. Plain abdominal radiography is imperfect as a screening method because it is difficult to delineate the margins of the packets and differentiate them from residual bowel contents, especially after enemas and multiple bowel movements.
