            <description>CONCLUSIONS: Our analysis of long-term trends of ACSC hospitalizations in Tennessee reveals a mixed bag of good news and bad news. In 1998-2006, ACSC hospitalizations rose at a much lower rate than overall hospitalizations for all conditions. Meanwhile, the costs of ACSC hospitalization in 2006 constant dollars decreased while the costs of overall hospitalizations increased. Minority groups such as blacks and patients insured under TennCare did not experience much decline in ACSC hospitalizations, especially in the rates of chronic ambulatory-care sensitive conditions, when compared with their white and commercially-insured counterparts.             <description>This report describes a 33-year-old Caucasian male who presented with severe upper back pain for three weeks that did not respond to symptomatic outpatient treatment. Imaging studies revealed a mediastinal mass and lymphadenopathy with superior vena cava and tracheal compression. Pathology showed a poorly-differentiated malignant neoplasm consistent with seminoma. No evidence of primary testicular tumor was found. His atypical presentation of back pain was thus consistent with an extragonadal seminoma in the mediastinum.

            <description>This report describes a novel presentation of chloride resistant metabolic alkalosis in a patient with hypercalcemia related to Multiple Myeloma (MM). A 51-year-old male with newly diagnosed MM presented with widespread skeletal involvement, calcium (Ca(+2)) of 18 mg/dL, phosphorous (PO4) of 6 mg/dL, serum bicarbonate (HCO3) of 37 mEq/L, and serum creatinine (Cr) of 2.6 mg/dL Other causes of metabolic alkalosis such as vomiting, diuretics, alkali ingestion, mineralocorticoid excess and hypokalemia were excluded. Hypercalcemia and metabolic alkalosis were only partially corrected after rehydration, calcitonin and steroids.             <description>We report a hypertensive patient with symptoms consistent with a pheochromocytoma, with a left adrenal mass with a pre-contrast HU of 8 but a 70 HU post-contrast value. No biochemical evidence of catecholamine excess was noted. A MIBG scan was reported as highly suspicious for a pheochromocytoma. Laparoscopic resection of the mass confirmed the presence of a peri-adrenal hemangioma with both capillary and cavernous components. We postulate that the accumulation of MIBG because of the hemangioma was responsible for the false-positive MIBG scan.

            <description>We report accentuation of hypercalcemia and hypercalciuria with Vitamin D treatment (weekly 50,000 IU ergocalciferol x 8 weeks) in a 64-year-old Caucasian male veteran with primary hyperparathyroidism, osteoporosis and Vitamin D insufficiency. The exacerbation of hypercalcemia and hypercalciuria resolved with cessation of Vitamin D therapy. We propose that clinicians consider using modest doses of Vitamin D such as 1,000 IU daily for replacement in patients with hyperparathyroidism and Vitamin D deficiency. We recommend that serum and urine calcium be monitored if such treatment is implemented.

            <description>We report a patient with a remote history of a transient ischemic attack, significant hyperlipidemia and a positive family history of vascular disease and intolerance to multiple lipid lowering agents, including several statin agents. The patient was started on 5 mg rosuvastatin every other day with no adverse symptoms. The dose was increased to 20 mg rosuvastatin every other day over approximately four months resulting in a 39-percent decrease in LDL levels and a 14-percent improvement in HDL levels with excellent tolerability. The long half-life of rosuvastatin along with its high potency make it a good candidate for alternate-day administration. The previous case reports on the cardiac toxicity occurred mainly in combination therapy of vinorelbine with cisplatin or carboplatin. We offer evidence that acute coronary syndrome and resultant diastolic heart failure developed as a result of acute bronchospasm due to intravenous vinorelbine monotherapy.

            <description>CONCLUSIONS: Targeted interventions are needed for patients in either precontemplation or contemplation stages. Counseling should focus on helping patients resolve barriers to cessation and reasons for relapse, particularly stress and weight management. Pharmacotherapy should be utilized when patients are ready to quit. Increased intratreatment social support and counseling appear warranted to support behavior change and appropriate medication use.

            <description>We report a 44-year-old Caucasian female who presented with hypoglycemia and hypotension. The patient had a basal cortisol of 1.6 ug/dl and a peak serum cortisol of 23.3 ug/dl after 250 ug of cosyntropin. Despite this apparent normal response to cosyntropin, the patient responded to glucocorticoids with an improvement in her clinical status. Pituitary MRI revealed a 7-mm pituitary cyst. Pending the availability of free cortisol levels, it is prudent not to disregard low basal cortisol levels, even in the presence of a normal cosyntropin response. We recommend that clinicians managing acutely ill patients have a low threshold for initiating glucocorticoid replacement in the presence of hypoglycemia and shock regardless of the peak cortisol values.

            <description>We report a 29-year-old patient, with left-sided chylothorax and thymoma, who presented with truncal, lower extremities stiffness and painful cramps alleviated by diazepam and opiates. His clinical picture was suggestive of SPS secondary to invasive thymoma. This syndrome is best approached by its early recognition and prompt treatment since a missed diagnosis can have deleterious outcome. An intensive search for the underlying etiology is of utmost importance in the management of SPS. A review of the current literature is also included in this manuscript.

Despite intensive, medical, nutritional and educational efforts during that period, no discernible progress was made in achieving any improvement in glycemic control. The patient appeared to be noncompliant with recommended strategies to improve glycemia.
