Abstract
The primary endodontic treatment goal is to optimize root canal disinfection and to prevent reinfection. Successful root canal therapy relies on the combination of proper instrumentation, irrigation, and obturation of the root canal system. In this review of the literature, various irrigants and the interactions between irrigants are discussed and new delivery systems are introduced.

    Abstract
A successful outcome of endodontic treatment depends to a large extent on accurate intraoperative findings. Conventionally, micromirrors and microprobes have been used for this purpose. The dental operating microscope (DOM) has been implemented to enhance visibility during dental procedures. However, the microscope, a sizable tool, remains between operating field and the dental practitioner, making his ability to manipulate more complicated. Also, the interference of the hands and the handpiece with the visualization of the surgical field and inaccurate observation of the endodontic instruments during the procedure. Endoscopy reportedly provides the dentist with excellent vision and ease of use. It also provides a better intraoperative visua...

    Abstract
3D cleaning, shaping and obturation of root canals has always been the desired goal of endodontic treatment which in many cases is difficult to attain. The introduction of NiTi rotary files made a major change in endodontic practice, making treatment easier, safer and faster. Nevertheless, after 16 years of intensive development, most of these instruments still share several drawbacks, the major one being the inability to three-dimensionally clean and shape oval root canals. The Self-Adjusting File (SAF) System was designed to overcome many of the current drawbacks of rotary file systems. It is based on a hollow, highly compressible file that adapts itself three-dimensionally to the shape of a given root canal, including its cross section. The file is op...

    Abstract
Recently, two new clinical concepts have emerged for the management of endodontically compromised immature permanent teeth. One involves a revitalization approach to achieve tissue generation and regeneration in the root canal system. In this method, new living tissue is expected to form in the cleaned canal space allowing continued root development in terms of both length and thickness. The other is the active pursuit of pulp/dentine regeneration via tissue engineering technology to implant or re-grow pulps. Although the technology is still at its infancy, it has potential to benefit immature pulpless teeth by allowing continued growth and maturation. Evidence has shown that using dental stem cells, pulp and dentin can be regenerated in the root canal sp...

Authors: Chenin DL
With the advent of cone beam computed tomography (CBCT), the diagnostic foundations of dentistry have been forever changed and advanced into a new era of 3-dimensional (3D) possibilities. In the case of implant planning, which was among the first applications of this technology, the exact 3D location of the implant could be preplanned and even guided during surgery. This was a major advancement in implant planning above and beyond traditional radiographic images, such as panoramic, bitewing, and periapical films, all of which are subject to magnification errors and dimensional distortion. However, with CBCT imaging, the images are captured in their true anatomic size and shape, and therefore offer the most accurate treatment planning potential. For these reasons, CBC...

Authors: Friedland B
With the introduction of conebeam computed tomography (CBCT) in the early 2000s, oral and maxillofacial radiology fully entered the modern world of 3-dimensional (3D) radiographic imaging. Although conventional or medical computed tomography (CT) had been available since the 1970s, with few exceptions, it had not been widely used in dentistry. In the early days of conventional CT, the machines were limited in number, restricting their use to only the most beneficial of purposes--for example, imaging of the brain. The cost of a CT examination was also prohibitive. For these reasons, the modality in dentistry was used almost exclusively in oral and maxillofacial surgery and sparingly at that. Even as the limitations of access and cost disappeared, the cost-benefit ra...

Authors: Scarfe WC, Farman AG, Levin MD, Gane D
Cone beam computed tomography (CBCT) is a diagnostic imaging modality that has shown rapid adoption in clinical dental practice over the past 10 years. CBCT images provide high quality, accurate 3-dimensional (3D) representations of the osseous elements of the maxillofacial skeleton. The purpose of this article is to provide (1) an introduction to maxillofacial CBCT technology, (2) an understanding of the relative patient radiation dose, and (3) to underline the appropriate use of CBCTas a diagnostic imaging modality with specific clinical applications.

Authors: Tsiklakis K
Radiographic examination and imaging play an important role in the diagnosis and management of most temporomandibular joint (TMJ) disorders. Several techniques have been used for the examination of the TMJ, including conventional tomography, magnetic resonance imaging (MRI), arthrography, computed tomography (CT), and, recently, cone beam computed tomography (CBCT).

Authors: Greenwall LH
White lesions occur commonly on the anterior teeth. There are numerous reasons why these lesions appear in the anterior dentition, and these reasons will be discussed in detail. For some patients, these white lesions may be considered unsightly and the patients may seek treatment to have them removed; other patients are not concerned about their presence on their anterior teeth. Some of the white lesions are caused by demineralization and occur because of poor oral hygiene. For the purpose of this article, developmental white lesions will be discussed in detail, along with the options for treatment of these lesions.

Creating a smile makeover is a complex process. It is a challenge that requires attention to detail in all areas. The most critical step in the process is the seating of the porcelain laminate veneers (PLVs). Both appearance and durability can be affected if mistakes are made in this phase of treatment.

For years now, dentistry has been in the aesthetics business. We have been talking about and offering dental restorations that look as close as possible to natural teeth-or perhaps look even better. We have been bleaching, veneering, and crowning in an effort to meet patient demand and expectations of that elusive "perfect smile." As these restorative treatments evolved, we became better at creating more translucent porcelains and composites, brighter shades, better color matching tools with the advent of electronic color matching, and more talented dental technicians-we are literally creating works of art. Yet some of this "art work" still falls short of the mark (Figure 1). Why is this? In many cases it is because we have forgotten to choose a frame that is deserving...

The success of dental restorations should not only be measured by immediate outcomes, such as patient comfort and appearance, but also by long-term survival and stability of the definitive restorations. Long-term success is dependant on the level of precision at which treatment is carried out. The three main areas in which this precision should be focused are precision of fit, occiusal precision, and aesthetic precision.

For years, restorative practices and traditional orthodontics stood largely separate from one another save for the most extreme instances. Orthodontics has evolved over the years, and a new breed of orthodontic technology appeared in the late 1990s. A clear alternative to traditional orthodontics emerged, and Invisalign (Align Technology, Inc., Santa Clara, CA) was born. Historically, orthodontics and restorative work, within a practice, rarely coincided on the same patient, but the advances within both disciplines have afforded a new and innovative way of considering conservative dentistry. No longer does there have to be aggressive preparation to attain a desired result. With patient understanding, a negligible amount of patience, and the desire to achieve the v...

Authors: Laskin DM
Internal derangements of the temporomandibular joint (TMJ) are characterized by displacement of the intra-articular disc, which acts as an obstacle to normal joint movement and results in clicking and popping sounds or locking and an inability to open the mouth widely. These conditions may be painless or they may be associated with pain, especially during function. The most common causes are trauma, which results in an immediate displacement of the disc, or chronic parafunction, which results in degenerative changes in the articular surfaces, increased friction, and gradual disc displacement.

Temporomandibular joint (TMJ) reconstruction presents unique problems because of the integral and complex roles this joint plays in establishing and maintaining proper form and function of the stomatognathic system. The TMJ not only acts as a secondary growth center for the mandible in prepubescence, but it is also essential for the functions of mastication, speech, airway support, and deglutition in both the child and the adult.

Authors: Nahlieli O
Obstructive sialadenitis, with or without sialolithiasis, represents the most common inflammatory disorder of the major salivary glands, and sialolithiasis is one of the major causes of sialadenitis. Calculi in the salivary glands can be found in 1.2% of the general population. Other common causes of salivary gland obstruction are ductal strictures and kinks. The diagnosis of these problems has been traditionally hampered by the limitations of standard imaging techniques. Satisfactory management depends on the surgeon's ability to reach a precise anatomic diagnosis and to accurately locate the obstruction.

Conventional wisdom advises that when a tooth needs to be extracted, the whole tooth should be removed, usually with as little surrounding bone as possible. However, the evidence to support this is not compelling, and every dentist has experienced cases where the apices of teeth are not removed for a variety of reasons and, in most cases, the patient seems to suffer no ill effects. If one extrapolates from this, it is evident that there might be instances where it is actually preferable to leave the apical part of the root rather than remove it, and this can be carried out deliberately. The usual time that one would consider this is when the inferior alveolar nerve is intimately related to the roots of the lower molar teeth, and this occurs most often in relation to ...

Obstructive sleep apnea is characterized by sleep deprivation and daytime fatigue and somnolence. It is conservatively estimated to occur in up to 3% of the adult population. However, the incidence is probably higher in industrialized countries, where sleep patterns are disrupted by abnormal work shifts. The incidence is also slightly higher in middle-aged males and may be as high as 67% in elderly males.

Injury to branches of the trigeminal nerve is one of the possible complications associated with the removal of third molars. Fifty-two percent of patients referred to a university-based practice with trigeminal neurosensory complaints associated their nerve injury with third molar odontectomy. The inferior alveolar nerve (IAN) was the most commonly injured nerve (61.1%), followed by the lingual nerve (LN; 38.8%). There was a 1.5-times greater incidence in females, the mean age of the patients was 34.5 years, and the mean time to consultation from the third molar extraction date was 6.9 months. These data also showed that although the LN was the least commonly injured nerve, it was also the most likely to be severely damaged. It also showed that these patients were mo...

The use of lasers in dentistry has expanded and improved some treatment options for those clinicians who have adopted the technology. As with all dental materials and instruments, the practitioner must use clinical experience, receive proper training, become very familiar with the operating manual, and proceed within the scope of his or her practice. The potential purchaser should carefully analyze the style and type of the practice to decide how useful the device could be. Moreover, attending introductory courses, seminars, and presentations, such as those at the aforementioned Academy of Laser Dentistry's Annual Conference, or at various state and national scientific sessions, are essential before buying. Because of the varied composition of human tissue and the d...

It is essential for any practitioner who uses lasers in their clinical practice to understand the basic physics of lasers. It is this knowledge that allows for an educated assessment of the clinical outcomes that lasers produce in our patients. It is also this understanding that provides a scientific basis for the visual feedback the clinician uses to vary parameters as needed to get the desired clinical results. It is the intent of this paper to discuss the very basic reasons why lasers affect tissues the way they do, and to synthesize the plethora of information dental practitioners are seeing regularly in dental journals.

This article will describe how a hard tissue laser is used in various restorative procedures. The erbium family of lasers (Er,Cr:YSGG and Er:YAG) are highly absorbed in water and, to a lesser extent, in tooth mineral. Because a carious lesion contains significantly more water than healthy enamel or dentin, the laser can offer some selectivity in removing the diseased material. Furthermore, these lasers can offer some degree of anesthesia. In these clinical photos, an Er,Cr:YSGG laser (Waterlase or Waterlase MD, 2780-nm; Biolase Technologies, Irvine, CA) was used.

Historically, oral soft tissue surgery on infants and young children was completed in the operating room under a general anesthetic agent. Many children were referred to an oral surgeon and required a physician's physical examination and medical clearance for a hospital admission. potentially placed a child at risk during the use of a general anesthetic for an elective procedure, which may not have insurance benefits. Traditional methods of oral surgery using scalpels or electrosurgery may produce significant postoperative discomfort and require sutures and prolonged healing. Lasers provide a simple and safe in-office alternative for children while at the same time reducing the chances of infection, swelling, discomfort, and scaring.

The use of lasers in general dentistry is now an accepted--and to some extent, expected--treatment modality. Laser use can be either an adjunct to other procedures or the main form of treatment itself. For many procedures, lasers are now becoming the treatment of choice by both clinicians and patients, and in some cases, the standard of care. Though there are as many uses for lasers in general practice as the imagination can provide, this manuscript will address those indications for use that have received marketing clearance by the U.S. Food and Drug Administration (FDA).

The many procedures included in esthetic dentistry are dominant in many dental practices. The trend will continue until many dental practices will be primarily oriented toward esthetic procedures. Overall, the movement toward major involvement with esthetic dentistry has been good. Caution is advised to ensure that patients are provided with informed consent before proceeding with esthetic procedures and that they are not overtreated.

An inefficient schedule is a terrible thing to experience. Look at Dr. Sapperstein's situation--he's exhausted, frustrated and stressed out after another long day. Imagine living with that kind of schedule day-in and day-out for years. Unfortunately, too many dentists end up doing exactly that. Practice success demands a highly efficient scheduling system. These six action steps can help you take your schedule (and practice) to the next level.

The motivation for writing this article is, after 26 years of lecturing around the country and interacting with thousands of dentists a year, I have seen the frustration that dentists have with certain aspects of their practice, specifically in the areas of overhead control, marketing, trying to get new patients, and financial arrangements. I hope that I will give every practicing dentist something to think about and help redefine your priorities as to how to run your practice.

Issues reviewed are: the decreasing dentist-to-population ratios, increasing numbers of part-time practitioners, marked growth in the size of dental establishments (including "dental spas"), anticipated continuing limitation of governmental financial support despite the ongoing needs of the poor and minority populations, growing numbers of adult individuals with special health care needs who have "aged out" of Medicaid dental coverage, as well as the increasing demand for dental services by the greater numbers of old...

If anyone told me four months ago that I would be taking out teeth and caring about the future of dental health in Uganda, the land of Idi Amin and Raid on Entebbe, I'd have told them they were crazy. I was going to Kenya for Operation Smile; a string of events led me to Samson Wamani, Medical Director for the Abayudaya community in Uganda, and helped me realize there's a huge difference between dental care here in the U.S., and what it is for fellow Jews of Uganda.

This study did not try to assess clinical efficiency of phase I treatment nor did it compare phase I appliances. but rather it attempted to survey referred early treatment case types and to illustrate successful outcomes frequently achieved with phase I therapy. Treatment acceptance of doctor recommendations (for early orthodontic intervention) was high (98%), illustrating excellent screening by referring dentists and a high level of parental awareness of the value of early intervention. Appliances selected for use were designed to be "user" friendly. Orthopedic appliances were prescribed to improve the harmony within the developing functional matrix of both the growing jaws and the dentoalveolar structures. All force systems were incorporated to use the basic principles of directional for...

Authors: Ring ME
I n 1956, workers in Rome, Italy, digging for the foundation of a building, uncovered an extensively decorated catacomb. The Vatican declared it to be a hitherto unknown Christian cemetery, dating from around the year 300. The discoverers were amazed at the fresco paintings, most of them biblical scenes. But one particular fresco was destined to set the world of medical history on its ear! The scene was interpreted as a surgeon performing an operation, and it was duly reported as such by the New York Times. It took a renowned dental historian, Dr. Curt Proskauer, using logical deduction, to disprove that conclusion. The result was to push back our knowledge of certain medical procedures by a thousand years!

In the past two decades the major advances bonding has experienced are directly related to material improvements and significant advances in adhesive technology. It is because of the superior physical properties of the posterior composite that we can provide patients with a long-lasting restoration if a meticulous bonding protocol was followed. It is possible simultaneously to preserve healthy tooth structure; reproduce its original shape, color, and translucency; and strengthen the affected tooth. Although the main focus in the past was on improving the bonding adhesive properties and the development of strength and polishability of the restorative materials, the current objective is to simplify the bonding technique and to create a shrinkage-free compo...

A pleasing final appearance is vitally important for patients who undergo extensive oral rehabilitation. This applies equally to the fields of orthodontics, restorative, prosthetic, and implant dentistry. In the field of removable prostheses there are biometric guides that aid in the replacement of teeth especially when the anatomical ridges are severely resorbed. These guides should be considered when planning the appearance and function of prostheses. This paper suggests that in view of the fact that unfavourable placement of implant fixtures is not easily rectified, knowledge of biometric guides can be critical in the treatment planning of the implant supported prosthesis especially when there are large resorbed edentulous spans.

The National Health Service (NHS) was created in 1948 to provide both medical and dental healthcare for the UK resident population based on need, not the ability to pay. Between 1948 and 2006 NHS dentistry improvement in oral health of the has made a huge contribution to the population of the UK.

Mastering temporary restorations reduces stress, improves treatment results and allows treatment of more complex cases. Good dentistry takes time, and temporary restorations are the key to allowing adequate time for individual procedures and overall treatment. Well-made temporaries will change "time" from being the eternal enemy of dentists into a useful ally.

Knowledge of the cause, prevention, and treatment of periodontal disease has increased rapidly during the past several decades; however, periodontal disease is still a prevalent infection, the treatment of which consumes vast amounts of financial and manpower resources. The intention of this article is to explore the role of nutrition in both the initiation and treatment of periodontal disease and to suggest that dental health professionals should consider nutritional guidance as a part of routine periodontal care.

Resin-retained bridges have been used clinically since the 1970s and offer a more conservative approach to the restoration of edentulous spaces than conventional bridgework. They are easy to place, cheap to fabricate, and have been shown to be cost-effective. Despite this, they are not frequently used in general dental practice and have an exaggerated reputation for premature failure. Since their initial introduction, they have undergone a number of changes to their method of retention and the materials used in their construction. This has resulted in a predictable, aesthetic restoration which, second only to restoration with dental implants, is often the treatment of choice when teeth adjacent to an edentulous space are minimall...

This article has taken a medical approach toward the understanding and managing of dental caries. Caries has been defined as a point in a person's life when there is an imbalance between protective and pathologic factors such that the process of demineralization of tooth structure by acid from bacteria in the tooth biofilm exceeds the patient's ability to remineralized tooth structure. The patient diagnosed with caries is out of balance. Caries is a diagnosis of a person. People have caries, teeth have lesions. Caries lesion detection, classification, and analysis are done at the level of the tooth surface. Caries is a process in time. Today we establish a diagnosis. For the future we establish a prediction. Patients can be diagnosed as caries active, caries balanced, or caries undetermine...

Today's investors have many choices. From seemingly simple investments, such as stocks, bonds and cash, to more complicated option strategies, there is a dizzying array of investment vehicles that can leave even the most seasoned investor a bit confused. In discussions with our clients, one common thread is the desire to learn more about the various types of investments available.
Authors: Christensen GJ
In years past, removable prosthodontics was a large and highly respected part of dentistry. During the past 50 years, removal of teeth has become less common, and placement of removable complete dentures has become less desirable to dentists. Tooth extraction is now limited to more legitimate reasons than in the past. The result is that removable prosthodontics has lost its appeal to many dentists. Polling large groups of dentists in continuing education courses, I have found that some dentists will not make removable complete dentures, and they do not provide dentures in their practices. Additionally, teaching complete dentures in dental schools has been diluted, because of the necessity to teach many other subjects and the reduced interest in the subject. Are ...

A daily minyan was held at 7:00 AM each morning at the recent Alpha Omega International Convention in Hollywood, Florida. That was Chanukah week, when a Torah reading is required. Thanks to a request by Allen Ressler, Torah reader, Rabbi Menachem Levy...who is connected local Chabad Congregation, joined us. As participants in the service crowded the designated prayer room, I was moved to reflect on how B'nai AO came about. At the suggestion of International Editor Dan Uditsky, I will share some facts, and especially my own personal memories.

As a child growing up in the suburbs of Chicago, I always knew I wanted to do something different. With a keen interest in biology and countless model airplanes built in my basement, I figured that somewhere down the road I would have to combine all my talents to pursue a career in the adult world. Little did I know then that my path would be so diverse and varied.

As Alpha Omegans, we are united not only by our profession but also by a mission to educate ourselves, and others, about preserving our Jewish heritage. It was with this mission in mind that the Alpha Omegan invited me to share with my fraters a very personal, and painful, account of my boyhood in Poland, where I survived the Holocaust. Among the many gruesome episodes I encountered during the war, two remain vivid in my memories. Although this is not an easy story for me to tell, it is one that ultimately gives me great strength, especially as I prepare to disclose it among my dear friends and colleagues of Alpha Omega. May we never forget what some of us lost, what we regained and why we have chosen to build our personal and professional lives in ways that honor our ...

Because dental treatment can represent a threatening event for some children, need exists to include sedation within our management arsenal. By definition, pre-cooperative children and those lacking in cooperative ability have immature cognitive skills, a highly restricted range of coping abilities, brief or negligible attention spans, and virtually no experience coping with stress. As result, they can be especially prone to maladaptive responses to anxiety provoking situations. For many of these, traditional non-pharmacologic behavior management strategies may often prove inadequate or inappropriate to overcome resistive or uncooperative behaviors. The importance of intervention with non-aversive techniques that provide a safe, child-oriented environment and timely ...

In the operatory of today, the opportunities are great. Space is at a premium and integration is a must. By consolidating the operator's primary supplies, which formerly were spread around the room, performance improves and space is gained for the successful introduction of high technology. Room cost decreases, permitting more networked rooms in less space at a lower cost. The productive potential of today's practice is greater than ever before; however, to gain the most from these opportunities, a change must occur in our perception of the treatment environment. When designing an office for the coming decades, it is vitally important to create a space for the future, not the past. Dentists must be willing to look objectively at their clinical environment and be willi...
