Conclusions
To reduce health disparities in urban communities, it is essential to
continue program evaluation of the critical elements of peer-led programs for
multiethnic adults with diabetes to promote self-management support in a
cost-effective and culturally appropriate manner.
Practice Implications
A diabetes self-management support program can be successfully implemented
in the community by peers, within a model including remote supervision by a
CDE. (Source: The Diabetes Educator)

Conclusions
Pedometers can be a helpful strategy to motivate persons with diabetes to
increase physical activity. However, it appears that attendance at DSMEP alone
may be enough to increase physical activity for persons with type 2
diabetes. (Source: The Diabetes Educator)

Conclusions
This randomized controlled pilot study suggests that the inclusion of a
medical assistant self-care coach as part of the diabetes care team holds
promise in improving outcomes and should be further examined in a large-scale
study. (Source: The Diabetes Educator)

challenges of conducting a tailored telephone intervention to improve rates of
screening in an underserved, diverse urban community. (Source: The Diabetes Educator)

Mothers in the experimental arm valued the mentors' help in adjusting to
the diagnosis, but this value was not measured by the study instruments. Focus
group research is under way to clarify the concept of parent mentor social
support and to develop a social support measurement tool. (Source: The Diabetes Educator)

Conclusions
Among individuals with diabetes, RLS can impair mental health, increasing
the risk for anxiety and depression. Since RLS consequences on nocturnal rest
and psychological status may impair glycemic control in this population,
diabetologists and diabetes educators should investigate for the presence of
RLS in their patients and treat them. (Source: The Diabetes Educator)

This article aims to present how RT-CGM is integrated into
diabetes clinical practice at the Yale Children's Diabetes Program (YCDP). The
authors provide factors to consider when choosing one of the commercially
available RT-CGM systems and a discussion of key strategies for successful use
of RT-CGM for families. Careful training and troubleshooting strategies will
ensure the most positive experience possible for a family using RT-CGM. (Source: The Diabetes Educator)

Conclusions
Plant-based meal planning is an acceptable and effective strategy that
educators can use to improve diabetes management and reduce risk of
complications. Studies show that dietary changes that are effective for
reducing cardiac risk may also be helpful for management of type 2 diabetes. A
low-fat, plant-based diet reduces body weight and improves glycemic control,
and has been shown to be remarkably well accepted by patients. (Source: The Diabetes Educator)

The purpose of this article is to describe how self-monitoring of blood
glucose (SMBG) data is a useful tool for identifying and managing postprandial
hyperglycemia (PPHG). PPHG and postprandial glucose excursions occur
frequently in patients with diabetes even when hemoglobin A1C is controlled
below 7.0%, and convey increased risk of cardiovascular morbidity and
mortality. Consequently, effective management of diabetes must include control
of postprandial glucose levels. Postprandial plasma glucose (PPG) depends on
the composition of meals, specifically the amount of carbohydrates.
Reduced-carbohydrate diets offer short-term improvements in glycemic control
and other metabolic parameters, but await the support of long-term efficacy
and safety studies. Glucose profiling and paired-meal SMB...

Conclusions
Patients should be encouraged to discuss their injection-related concerns,
and providers should regularly ask about injection-related problems. Providers
should offer patients information about tools to reduce injection-related
worries, preferably by having them available to show and demonstrate. (Source: The Diabetes Educator)

Conclusions
An intensive MNT program, including counseling, education, and capillary
glucose self-monitoring, has a positive effect over preeclampsia, maternal
hospitalization, and neonatal death in women with diabetes in pregnancy. MNT
guidelines should be implemented in Mexican health care facilities treating
diabetes in pregnancy. (Source: The Diabetes Educator)

Dancing 2 times per week for 12 weeks produced significant group
differences in systolic BP and body fat and significant intraindividual
changes. The implications are that dancing in a supportive environment with
peers may be an effective strategy for diabetes educators to help those with
diabetes to become more physically active and improve diabetes outcomes and
overall health. (Source: The Diabetes Educator)

Conclusions
Family support, specific to diet, is significantly associated with glucose
outcomes in Korean immigrants with type 2 diabetes. That is, more perceived
family support was associated with better glucose control. The positive impact
of family support on glucose outcome was significantly stronger in men than in
women, even after other factors were taken into consideration. (Source: The Diabetes Educator)

Conclusions
Few type 2 diabetes interventions address emotional well-being in Latinos.
More attention has been directed toward designing culturally sensitive
community-based programs for improving behavior and physical outcomes. Because
some Latino groups believe that negative emotions cause diabetes and because
depression and anxiety are associated with poor self-management, programs
should address emotional well-being as an important aspect of diabetes
management. (Source: The Diabetes Educator)

Conclusions
Paired SMBG (testing before/after specific events) promotes diabetes
knowledge and self-management skills and facilitates assessment of the impact
of behavioral changes, medical nutrition therapy, and pharmacologic
interventions on glycemic levels. New tools have been developed to assist in
using paired testing in clinical practice. (Source: The Diabetes Educator)

Conclusions
In this setting, hypoglycemia occurs infrequently in all groups, but at
higher rates with more intensive treatment. Nurse case management, whether
with or without medication adjustment, is effective in improving short-term
glucose control. (Source: The Diabetes Educator)

Conclusions
Health care providers and diabetes educators in primary care clinics should
consider how organizational characteristics of the clinic might influence
self-management behaviors of patients. The focus should be on better access to
evidence-based information at the point of care and self-management needs and
activities. (Source: The Diabetes Educator)

Conclusions
The focus group discussions identified both barriers to diabetes management
and opportunities for improving care for underserved patients with diabetes.
The results are useful to improve the delivery of care and to develop
quantitative studies to explore particular areas of interest. Based on these
results, the current system needs to provide more support and education to
patients with diabetes. (Source: The Diabetes Educator)

Conclusions
The collaboration between diabetes educators and physicians yields positive
clinical quality and cost savings. The analysis indicates that quality can be
improved, and cost reduced, by increasing referral rates to diabetes education
among low-referring physicians, specifically among men and people in
disadvantaged areas. More needs to be done to inform physicians about ways to
increase access to diabetes education for underserved populations. (Source: The Diabetes Educator)

Conclusion
Currently, 3 incretin-based therapies are available and widely used in
clinical practice. Several more agents are either under review by the Food and
Drug Administration (FDA) or are in the very late stages of development. For
diabetes educators trying to help their patients understand the differences
among their antidiabetic medications, a comprehensive understanding of these
agents and their role in therapy is imperative. (Source: The Diabetes Educator)

Conclusion
Diabetes educators will find the DiaBEATes Nurse Champion program useful
for structuring educational programs within various practice settings. The
case studies are useful tools for evaluating specific application of diabetes
knowledge.
Background
The nearly 24 million people with diabetes strain the health care system in
terms of resources and health care dollars. According to the American Diabetes
Association (ADA) a hospital stay for a patient with diabetes costs almost 3
times the stay of a patient without
diabetes.1 The
higher costs associated with caring for a patient with diabetes is partly
attributed to inadequate knowledge regarding current diabetes treatment
options and the failure of some health care workers to accept professional
responsibility for diabetes
managemen...

Conclusions
Certification examination specifications were developed directly related to
the important activities that diabetes educators perform. Future forms of the
certification examination will continue to be matched to job-related,
criterion-referenced test specifications and will have strong evidence of
content validity. Future forms of the exam will contain 200 items at specified
cognitive levels with a representative sampling of tasks within 3 core areas
from the detailed content outline. (Source: The Diabetes Educator)

Conclusions
Although CDEs knew relevant information, they lacked some specific
knowledge about PC, and they lacked confidence in their knowledge and in their
ability to counsel patients. Diabetes educators can benefit from an education
program to provide PC to their female patients, including adolescents.
Computer or Web-based accessibility could make this a low-cost and easily
disseminated program. (Source: The Diabetes Educator)

Conclusions
These findings suggest that people with diabetes without elevated A1C can
benefit from a community-based, peerled diabetes program. Given the large
number of people with diabetes and lack of low-cost diabetes education, the
DSMP deserves consideration for implementation. (Source: The Diabetes Educator)

Conclusions
Diabetes-specific enteral formulas rich in slowly digestible carbohydrate
sources can be equally effective in attenuating the postprandial blood glucose
response as low-carbohydrate, high-fat enteral formulas without elevating the
plasma triglyceride response. (Source: The Diabetes Educator)

Conclusions
Higher scores on the PAM were associated with higher rates of self-care
behaviors and ease in managing diabetes; however, the indigent urban
population reported higher activation scores than found in previous studies.
The relationship between activation and outcomes needs to be explored further
prior to expanding use of this measure in this patient population. (Source: The Diabetes Educator)

Conclusions
Patients previously unfamiliar with the meaning of A1C, using either term
(A1C or eAG) resulted in an equal improvement in knowledge. Within this study,
eAG was not a more understandable term, or an easier concept for patients to
remember. Further research is needed to test whether use of the term A1C
should be replaced by eAG. (Source: The Diabetes Educator)

Conclusions
Parenting styles are not associated with diabetes control and quality of
life in children with type 1 diabetes. However, further research should assess
the impact of the determinants of parenting on children with type 1 diabetes
and quality of life. (Source: The Diabetes Educator)

Conclusions
Family-centered interventions targeting children diagnosed with type 1
diabetes appear to be effective in enhancing health outcomes. These
interventions focused on traditional 2-parent families, and many did not
report race or ethnicity. Given the increasing number of single-parent
households and divorced parents, a need exists to explore and develop
family-centered interventions that target nontraditional family structures as
well as addressing cultural differences. (Source: The Diabetes Educator)

Conclusions
In this case study, evidence-based nutrition guidelines have been evaluated
and reviewed to demonstrate heart-healthy eating for children with
hyperlipidemia and type 1 diabetes. It is known that approximately 40% to 50%
of children with elevated lipids will continue to have abnormal lipids into
adolescence and early adulthood. Therefore, early screening is recommended by
the AHA to track lipid changes during childhood and adolescence and to begin
treating abnormal LDL-C levels to prevent the development of
atherosclerosis. (Source: The Diabetes Educator)

Conclusion
Although the most effective intervention for type 2 diabetes prevention may
not yet be identified, DPP-based interventions show promise for long-term
sustainability. The DPP intervention is effective in treating overweight and
obesity across a variety of settings and thus may prevent chronic diseases in
which overweight and obesity are risk factors. Public health practitioners can
use this successful intervention to help individuals lead healthier lives. (Source: The Diabetes Educator)

diabetes regimen and to highlight the essential role of the diabetes educator
in safely training and implementing the myriad skills in a developmentally
appropriate manner for children and adolescents. A review of literature and a
task analysis were preformed and suggest that the complexity of the regimen is
often not adequately addressed. Reviewed research assessed the regimen using
measures with on average about 25 items while the task analysis contains over
600 tasks. The article discusses implications for clinical practice, including
implications for measurement in research, targeting of interventions by
diabetes educators, and the gradual transfer of regimen control to youth. It
is argued that given the magnitu...

The familist traditions, central to the Mexican culture, had both positive
and negative consequences on diabetes management. Tailoring clinical care and
developing novel education approaches, to include family and community, is
central to improving the health of this population. Recognizing and
acknowledging the social stigma associated with diabetes, for this population,
will promote understanding and improve clinician-patient communication. The
sociocultural influences that affect diabetes management practices (eg,
include family, in particular the primary female caregiver, and establish
community- and home-based education sessions) must be integrated into clinical
practice. Future research focused on population-defined health and disease

Conclusions
Additional efforts are required to guarantee that all people with diabetes
receive the DSME they need. This will require increased referral by
physicians, increased follow-through by patients, and increased availability
of DSME in forms that make it appealing to patients and physicians. (Source: The Diabetes Educator)

Conclusion
The DLNET is composed of 24 interactive modules covering standard diabetes
care topics that can be customized to individual patient needs and used by all
members of the multidisciplinary diabetes care team. The material's content
and formatting aims to improve the ease of use for diabetes patients with low
literacy and numeracy by adhering to a lower text reading level, using
illustrations for key concepts, and color-coding and other accommodations to
guide patients through instructions for self-care. Individual sections of the
DLNET may be provided to patients for initial teaching, as well as for
reinforcement. Although designed for lower literacy and numeracy skills, the
DLNET provides unique materials to facilitate diabetes education for all
patients. (Source: The Diabetes Ed...

Conclusions
The PRAM-TSQ shows evidence of being a valid, reliable instrument for
assessing treatment satisfaction in patients using pramlintide. The subscales
are comprehensive and sensitive to the known potential effects of pramlintide
treatment. Diabetes educators can use patient responses to the PRAM-TSQ to
facilitate treatment adherence by reminding patients of treatment benefits
they experience and by helping patients overcome negative effects they
report. (Source: The Diabetes Educator)

Conclusion
Educators who teach RT-CGM should emphasize lag time and calibration
techniques, technical device training, and sensor insertion. Follow-up focus
should include insulin dosing adjustments and skin issues. The time and effort
required to introduce RT-CGM provided an opportunity for the diabetes
educators to reemphasize good diabetes care practices and promote

Children with T1D are not meeting dietary guidelines, and in some areas
their diets are less healthful than children without diabetes. As these
dietary behaviors may affect the risk of long-term complications, the
incorporation of behavioral approaches promoting healthy eating into routine

Conclusions
Adoption of UD principles by designers of diabetes medical devices could
benefit persons with disabilities, increase the potential market for the
manufacturer, and have unexpected benefits for people of average abilities.
Adoption of UD principles for DSME programs would not require a paradigm
change because diabetes educators already do many activities that could
contribute to UD of an education program. By replacing average-person design
of DSME programs with UD, diabetes educators can promote full participation in
DSME for individuals with the wide range of abilities normally present in
target populations without the need for added adaptations or specialized
design. (Source: The Diabetes Educator)

Conclusions
The Healthy ChangesTM program can be successfully translated into
community settings and led by trainedpeer leaders, yielding health
improvements similar to those reported in efficacy trials. Trained peer
leaders are key to effective program implementation. Peer-led groups enhance
goal attainment by giving participants a venue to discuss obstacles and

Conclusions
For people who are diagnosed with diabetes, there is a struggle to become a
person with diabetes rather than a diabetic person and to manage the lifestyle
changes that are mandated by this role/identity. One way of dealing or coping
with this new identity is to begin to "normalize" these lifestyle
changes—to view them as healthy living for all people, those with and
without diabetes. This will then have implications for
interventions—encouraging healthy lifestyles among people with diabetes
rather thanemphasizing that people with diabetes are "different
from" the general population. (Source: The Diabetes Educator)

According to parents in the current study, children receive inadequate

receive supports than white children. Parents are worried about diabetes
management in school, but most do not have the knowledge of federal laws
necessary to protect their children. (Source: The Diabetes Educator)

Conclusions
Many 2008 NPS results concur with those obtained in 2005 through 2007.
Areas of variability among programs suggest a need for standardized
interventions and practice guidelines. Educators are encouraged to report
outcomes to elucidate the contributions of their programs to patient care.
AADE can use the results and comparative data obtained from the 2008 survey
when developing practice, research, and advocacy activities. (Source: The Diabetes Educator)

Diabetes is a leading cause of cardiovascular disease. Persons with
diabetes are at greater risk for early cardiac mortality, and for repeat
events if they survive their first cardiac event. Recently, low serum
concentrations of vitamin D have been associated with increased risk for
cardiac events. Evidence indicates that persons with diabetes have lower serum
concentrations of vitamin D. In addition, persons at risk for diabetes or
metabolic syndrome have inadequate serum concentrations of vitamin D. This
review will assess the evidence relative to the impact of vitamin D in the
development of diabetes, metabolic syndrome, and diabetes complications.
Studies that address vitamin D and its impact on metabolic outcomes as well as
possible mechanisms of action are provided. Finally, the asse...
