            <description>We thank the authors for their response and views on randomization in clinical trials. Although their study was a randomized trial, it was primarily designed to answer research questions on nutritional changes, not dental caries rates. Therefore, it would be prudent to demonstrate comparability between the control and intervention groups with regard to well-known risk indicators for dental caries, including enamel hypoplasia and oral hygiene. Comparability between the cohorts at the beginning and end of the study should also be shown, as there was loss of research subjects during the period of investigation.             <description>As reviewed by Seow in a recent edition of the Journal of Evidence-Based Dental Practice, we conducted and reported a randomized clinical trial that demonstrated that nutritional advice during the first year of life decreases the incidence and severity of caries at 4 years of age in a low-income community. As randomization led to homogeneous comparison groups, we were surprised at the commentary accompanying the review, which criticized our study for its “lack of control for confounding variables, which affects the interpretation of the findings.” (p. 61). Further, this critique gave special emphasis to the fact that we did not adjust for hypoplasia and plaque control in our analyses.
