Clinical trials play a prominent role in research and development leading to innovative oral care products and treatments. Evidence from clinical trials reportedly played an important role for one new product—hydrogen peroxide whitening strips—with respect to development of the initial technology and its early market acceptance. Subsequent research includes more than 100 whitening strip studies with thousands of participants globally. Case studies on inclusivity, consistency, and other atypical research from the broader whitening strip program provide insight on “next level” evidence assessment as part of new product development in oral care.
The challenges of product development for contemporary dental companies are great. Market dynamics are changing at a rapid pace, and technological advancements need to be incorporated into a product's evolution. Our goal is always to introduce a product that fulfills the proper indications. However, quality-related guidelines and legal and operational aspects must be taken into consideration as well. Therefore, most industrial product development systems follow specific project management methods to ensure proper testing protocols for the validation of a product's efficacy and performance. Formal project management systems vary among organizations but are fundamentally organized to take a project from an idea phase and manage the process of evolvement to a product's final, marketable form....

Many dental organizations today strongly support rigorous guidelines for testing and advertising dental products. It is therefore critical that dental product manufacturing companies invest to ensure that their products are safe and effective for patients, practitioners, and the environment as well as comply with all global regulatory requirements. Companies must appropriately commit to generate credible scientific data and analysis for communicating with the dental profession as well as the public.
Evidence-based literature seeks to “inform” clinical decisions, not to “impose” them. Informing clinical decisions has been the defined focus of the Journal of Evidence-Based Dental Practice (JEBDP) for 10 years now. This special issue contains reviews with an emphasis on restorative dentistry and dental materials, as can be appreciated from the Level of Evidence Summary Matrix.
The Journal of Evidence-Based Dental Practice (JEBDP) was launched 10 years ago with the purpose of bringing authoritative reviews and analysis of current clinical studies that would facilitate decision making and result in better patient outcomes. Over the past decade, the journal has covered a wide range of topics in oral health and dental care. To celebrate our 10th anniversary, we have devoted this entire issue to relevant concepts in restorative dentistry. Under the expert guidance of our guest editor for this special issue, Dr. Robert Kelly (the University of Connecticut Health Center), a number of important topic areas in dental materials and restorative dentistry were identified. Then the most relevant articles covering these topic areas were selected for review, analysis, and comm...

This article discusses the current status of evidence-based dentistry. It describes the central role of systematic reviews in the process of clinical decision making in evidence-based dental practice. Particular emphasis is given to the correspondence of the 4 fundamental steps of the logic model—inputs, activities, outputs, and impact—to the process of actualizing and evaluating clinical decision making and policies in evidence-based dentistry.
There is some literature on how to find the best evidence for clinical practice but little is known about which evidence clinicians actually seek when they look for scientific support in changing behavior. The aim of this study was to explore which evidence has an impact on dentists' willingness to change their behavior by investigating the requirements for seeking and understanding new knowledge, as well as perceived barriers or motives for doing this.A postal questionnaire was analyzed according to demographic information, access to and use of a personal computer, postgraduate education activity, knowledge about evidence-based medicine and scientific terms, and seeking and grasping new and actual knowledge from 177 dentists. Fifteen of these dentists formed 3 focus groups that were inter...

This article focuses on dental practice-based research networks (PBRNs) as one of the solutions to the oftentimes slow and inefficient process of translating research evidence into clinical practice. This article identifies two major issues that are faced when setting up a dental practice-based research network. Using a dental practice-based research network that the author is involved with as an example, this article then discusses what is involved in undertaking a research project in your office. Also discussed are the types of projects being done in this new setting and how this author believes these studies will be one of the bridges toward developing evidenced based dentistry in private practice.
Conclusion: Generally, dentists in the United Kingdom were more familiar with the current expert recommendations on AP than dentists in Iran; however, dentists in both countries tended to overprescribe. This could contribute to the problem of antimicrobial resistance. Educational initiatives aimed at implementing updated recommendations and continuous assessment of dental practice might reverse this trend.
This is the second of 2 articles that focus on strategies to integrate an evidence-based decision-making (EBDM) approach into your practice. The previous article provided an overview of EBDM concepts and identified strategies for finding evidence to answer clinical questions, the first 2 steps in the evidence-based process. The next steps are to determine the credibility and usefulness of the evidence and to apply the evidence to patient care. The purpose of this article is to discuss those steps by reviewing the critical appraisal criteria used to assess the methodological quality of a study. A case scenario will be used to demonstrate how appraising the evidence helps to answer questions and how to translate the evidence into everyday practice. Characteristics of an evidence-based practi...

The Journal of Evidence-Based Dental Practice (JEBDP) is among the first dental journals in the United States and the first dental journal published by Elsevier to give continuing education credit to its reviewers. Peer reviewing is a newer form of continuing medical education (CME). JEBDP follows several medical journals awarding CME to reviewers, including the Journal of the American Medical Association (JAMA), New England Journal of Medicine (NEJM), Academic Emergency Medicine, Annals of Internal Medicine, Gastroenterology, and others. The straightforward rationale for this program is that by conducting a thorough analysis and review of a primary journal article or submitted manuscript, the reviewer engages in a clear and powerful learning activity. In all cases, JEBDP reviewers must ac...

Abstract: The American Dental Association (ADA) in conjunction with the Journal of Evidence-Based Dental Practice and a grant from Procter and Gamble sponsored a conference May 2008 at ADA headquarters. This was the first of 3 similar annual conferences at which approximately 100 dentists will be trained to become Evidence-Based Dentistry Champions. I was one of the attendees at the first Champions conference. In this article, I will share my experiences in disseminating evidence-based dentistry to my colleagues.
Evidence-based dentistry champions are committed to improving the quality, effectiveness, and appropriateness of dental care through the application of evidence-based principles and tools. They share knowledge and skills to promote evidence-based dentistry (EBD) in practice, guiding colleagues, patients and policy makers in the application of critical thinking skills and evidence-based decision-making. Being an evidence-based champion requires furthering an understanding of the full process and the challenges of evidence based dentistry, including the development of an evidence base, evidence synthesis and summary, the creation of best practice guidelines, as well as evidence implementation. Efforts to improve the quality, effectiveness, and appropriateness of dental care need to occur at,...

Time and access have often been cited as barriers to implementing Evidence-Based Dentistry (EBD). This paper describes a new web-based resource launched by the American Dental Association to enable practitioners to incorporate evidence into treatment planning. The website offers a database of systematic reviews, critical summaries of systematic reviews, evidence-based clinical recommendations and links to external resources to enable practitioners to access evidence at the point of care. In addition the site offers an online space for clinicians to suggest clinical scenarios where evidence is lacking. This could potentially be a source of topics to drive future research. With the explosion in the use of information technology within a dental office, this web-site will serve as the one-stop...

This article addresses the application of evidence-based methods for diagnosis of dental pain. Patients in pain may seek diagnosis and treatment options through Internet sources but incomplete information and lack of context are barriers to patient understanding. Dentists face similar challenges, especially when there is no strong evidence. Diagnostic error may also be precipitated by clinician overconfidence. Understanding the strengths and weaknesses of evidence and placing evidence in context are crucial skills for dentists practicing within the evidence-based paradigm.
An evidence-based dental practice and support team can easily be developed with the right materials and motivation. With the advancement of many electronic and published resources on evidence-based dentistry (EBD) this process has been made simple with limited time commitment from both dentists and their auxiliaries. The potential benefits of implementing EBD in private practice include the improvement in the overall quality of patient care and a positive and productive work atmosphere for the entire dental team. This presentation focused on 2 different approaches for accessing evidence for practice. Strategy 1 focused on using synthesized research, including systematic reviews, EBD article reviews, and critical summaries. Strategy 2 focused on using the PubMed Clinical Queries feature, “Find Systematic Reviews” to search for synthesized research, ie, systematic reviews, EBD articles, and meta-analyses. Although not discussed in the preconference session, PubMed also provides access to ind...

A profession has a fundamental obligation to its members to ensure that they embrace the process of lifelong learning and continuous education. Keeping up-to-date with new research and critically reading and understanding the literature can be a daunting task for health care professionals. Adopting an evidence-based approach can help ease the burden for busy clinicians and ensure they are staying current with the latest findings, internalizing what it means for them and their patients, and, if warranted, implementing what they have learned into their daily practice.
Dear Editor: As the director of graduate medical education (325 medical residents and 32 dental residents) I am very familiar with physician-based criteria used to evaluate the quality of research publications. When testing a hypothesis for validity, these scientists almost always frame it such that it has clinical relevance. Thus, I found it incongruous to read the article entitled “Panoramic Radiography Does Not Reliably Detect Carotid Artery Calcification Nor Stenosis” because it analyzes an article that evaluated the efficacy of panoramic radiographs to detect significant carotid artery disease among a group of patients who had already had a physician-obtained carotid artery ultrasound (CAUS) study. This is exactly the opposite of what happens in the clinical practice of dentistry...

In last decade, Dentistry has evolved tremendously, from manually operated micro-motor to the computer assisted technology has made great in roads into this specialty. Expert systems are knowledge-based computer programs designed to provide assistance in diagnosis and treatment planning. They assist the practitioner in decision making. Clinical decision support systems can be helpful in improving both clinical outcomes and adherence to evidence-based guidelines. This is one such article which reviews and gives an insight into types of system exists, how these system works and its uses related to Dentistry.
This is the first of 2 articles that focuses on strategies that can be used to integrate an evidence-based decision making [EBDM] approach into practice. The articles will focus on EBDM methodology and enhancing skills, including how to find valid evidence to answer clinical questions, critically appraise the evidence found and determine if it applies. In addition, online resources will be identified to supplement information presented in each article. The purpose of this article is to define evidence-based decision making and discuss skills necessary for practitioners to efficiently adopt EBDM. It will provide a guide for finding evidence to answer a clinical question using PubMed's specialized searching tools under Clinical Queries.

Will evidence-based dental practice (EBDP) benefit from the projected new drive for comparative effectiveness research (CER)? Optimists may answer Yes certainly, because CER and EBDP are intertwined, and benefit and feed on each other. Others may be more cautious. Any differences in opinion likely arise because the similarities and the differences between CER and EBDP are not clear cut. (Source: The Journal of Evidence-Based Dental Practice)
