Occlusal rehabilitation requires a correlation of biologic factors, mechanical principles, and esthetic requirements with treatment procedures. A combination of new generation materials, along with improved clinical procedures, works in tandem to produce an esthetic, long-lasting, and functional outcome, satisfying both the clinician as well as the patient. This clinical report describes the full mouth rehabilitation of a severely worn out dentition in functional harmony with the stomatognathic system.
Kumar Sandeep, Kamath Giridhar, Gupta SeemaThe Journal of Indian Prosthodontic Society 2009 9(3):160-163The use of immediate postextraction implants presents several advantages in terms of reduction of surgical steps and time required to conclude the therapy. Particularly important is the possibility to minimize bone loss that otherwise would occur in the physiologic healing of the alveolus. Immediate implant placement has helped solve issues with regard to bone quality, quantity and esthetics and also treatment time, as opposed to delayed implant placement. Here, a case report has been presented in which immediate extraction Frialet implant has been placed in mandibular premolar region. (Source: The Journal of Indian Prosthodontic Society)

Prasad Sanjay, Giri Tapan KThe Journal of Indian Prosthodontic Society 2009 9(3):156-159The aim of this clinical report is to describe the prosthodontic management of a young boy aged 7 years affected by ectodermal dysplasia (ED). Dental treatment can vary depending on the severity of the disease (tooth size, morphology, and the amount of available alveolar bone). New technologies, such as adhesive dentistry, overdenture, and complete denture represent some of the options in the management of the rehabilitation of the patient affected by ED. The conical shaped central incisor was modified using composite resin followed by complete denture. Prosthodontic and restorative treatment was provided for the psychological and social comfort of the young patient. (Source: The Journal of Indian Prost...

Naik Amit VThe Journal of Indian Prosthodontic Society 2009 9(3):148-150The main purpose of the study was to determine the causes for the fracture of complete dentures of patients reporting to the Department of Prosthodontics, Teerthanker Mahaveer Dental College, Moradabad, Uttar Pradesh, India. Data collected from 200 patients reported for repair of their complete dentures. Data was collected from patients, aged between 30 to 80 years (mean 55 plus/minus 25 years), from both the genders. Investigations were done on factors causing the fracture. After the analysis it was observed that the ratio of fracture of upper to lower denture was 1:3. Most fractures were common among males (55%). The most common reason being accidental dropping of the denture in case of the lower and improper ...

Conclusions</b> : It was concluded that: the occlusal plane is parallel to Camper's plane posterior reference point as the superior point of tragus in females and middle point in males. The occlusal plane is not generally parallel to the interpupillary line. Intra-orally, the level of buccinator groove is reliable, while variations in retromolar pad area make it an unreliable landmark for orienting the occlusal plane.
Conclusion</b> : Microwave post-polymerization irradiation can be an effective method to increase the flexural strength of Kooliner (at 550 W for 5 mins) and Denture Liner (at 650 W for 5 mins) by reducing the residual monomer content by further polymerization at free radical sites. The Kooliner reline resin had superior flexural strength compared to Denture Liner reline resin. (Source: The Journal of Indian Prosthodontic Society)

Conclusion</b> : Custom-made dynamic visco-elastometer has proved to be reliable, low cost and a convenient instrument to evaluate silicone maxillo-facial prosthetic material. No single material was found to have all the desirable dynamic mechanical properties necessary for maxillo-facial prosthesis applications. Therefore, layering of two or more such kind of materials is recommended to achieve desirable properties. (Source: The Journal of Indian Prosthodontic Society)

Prakash Vijay, Gupta RuchiThe Journal of Indian Prosthodontic Society 2009 9(2):116-118Patients have many restorative options for changing the appearance of their teeth. It is difficult to mask dark underlying tooth colour and retain a natural appearance of the tetracycline stained tooth. While conservation of tooth structure is important, so is selecting the right treatment modality for each patient based on clinical findings. This case is an example to approach a tetracycline stained tooth to achieve a desirable esthetic result. (Source: The Journal of Indian Prosthodontic Society)

Shah Farhan K, Rajput Geeta, Lall Saurabh, Bey AfshaThe Journal of Indian Prosthodontic Society 2009 9(2):112-115The temporo-mandibular joint (TMJ) consists of three basic components: The TMJ, the teeth, and the neuromuscular component. The neuromuscular system consists of nerves and muscles which help in the functioning of the TMJ. The nerves transmit messages to the muscles for jaw functioning. They also transmit pain signals to the brain causing discomfort. Temporomandibular disorders (TMD) occur in many forms and varying degrees of severity. The symptoms of TMD can be an obvious jaw pain in the muscles, but can also exhibit as headache at the temporal region. Patients often feel this symptom as a tension or a sinus headache. Patients understandably fail to make this connection between ...

Pavaiya Arpan, Tyagi Vinod K, Tripathi Arvind, Singh Saumyendra Vikram, Chand PooranThe Journal of Indian Prosthodontic Society 2009 9(2):109-111Cranial defects result either from trauma or after intentional osteocraniotomies or external decompression craniectomies. These lead to mechanical vulnerability of the brain, esthetic disfigurement, and transmission of vibrations and pulsation of the brain that cause disconcerting sensations to the patient. Subsequent cranioplasty may be required to compensate for the defect and alleviate various signs and symptoms. A referred posttraumatic patient of a cranial defect that was restored with an alloplastic heat polymerizing methyl methacrylate cranial implant is presented in this case report.
This report discusses the esthetic and functional rehabilitation of a pair of fraternal twins with amelogenesis imperfecta. Both patients presented with esthetic disharmonies and sensitivity of teeth. The treatment plan was custom made for both the patients and executed successfully. <b> Clinical Implications:</b> Cosmetic rehabilitation of patients with AI has been open to a variety of treatment options. There is no set rule in planning the treatment for such cases. The aim is to properly diagnose the case and provide durable functional and esthetic management of these patients, where the unaesthetic appearance has a definite negative psychological impact. The design of the prosthetic tooth will be dictated by aesthetics, function, ease of cleaning, maintenance of healthy tissue on the edentulous ridge and patient comfort. Proper design is more important for hygiene and good tissue health than the choice of material. An ovate pontic has been recommended to fulfill both aesthetic and functional requirements. The convex design of this pontic was intended to form a concave soft tissue outline in the site of the alveolar ridge mucosa. Clinically healthy conditions can be established at pontic sites if appropriate plaque control is performed. As the ovate pontic suffer...

Prithviraj D R, Gupta Ankit, Singh Vishal, Shruti D PThe Journal of Indian Prosthodontic Society 2009 9(2):96-99This clinical report describes the use of alveolar ridge augmentation procedure using connective tissue graft, cast post, and metal-ceramic fixed partial denture for a patient with ridge defect to enhance the esthetic result for a metal-ceramic fixed partial denture (FPD). This ridge augmentation procedure significantly increased the functional and esthetic outcome of the final FPD by restoring the alveolar ridge defect to its original dimension. It may present itself as a local symptom, as part of a systemic disease, as side effects of medications or following therapeutic radiation to the head and neck regions. Xerostomia can lead to difficulties during management in patients receiving prosthodontic treatment. The comfort and retention of removable dentures depends largely on the physical properties and the lubricating ability of the saliva and dry mucosa. To help overcome this problem, various techniques have been proposed for incorporating reservoirs that contain salivary substitutes into dentures. These techniq...

DR Prithviraj, Vishal Singh, Sarvanan Kumar, DP ShrutiThe Journal of Indian Prosthodontic Society 2009 9(1):55-58Amelogenesis imperfecta has been described as a complex group of hereditary enamel defects that disturbs the enamel structure and exists independent of any related systemic disorder. A 24-year-old male patient presented with discolored and hypersensitive teeth. The aim of the treatment was to reduce dental hypersensitivity and restore esthetics and masticatory function. The maxillary and mandibular teeth were prepared and metal ceramic crowns were cemented on the prepared teeth. The final treatment result provided the patient with improved dental esthetics and functional efficiency that enhanced his self-image. As a result, mandatory clinical procedures like jaw relations, intra or extra-oral tracings and interocclusal records are made complicated, and the accuracy of such records, questionable. Hence every maxillofacial patient must be considered on an individual basis and rehabilitation procedures customized to suit the clinical situation, rather than strictly adhering to theoretical principles. This involves procedures like customizing the record bases, occlusal schemes and individualiz...

Pravinkumar G Patil, Rambhau D ParkhedkarThe Journal of Indian Prosthodontic Society 2009 9(1):33-35Prosthetic rehabilitation with surgical obturator for total maxillectomy patients is a challenging job because of the lack of normal hard and soft tissue support. In such cases the obturator can be retained with a spring-retained appliance if mandibular dentition is normal. A patient (treated with total maxillectomy) was rehabilitated with a new spring-retained surgical obturator. The spring used in this type of appliance is made up of 23-gauge stainless steel orthodontic wire which incorporates four coils. The spring is simple in design, applying light, clinically acceptable seating force, easily cleansable and causing no soft tissue irritation.
TV Padmanabhan, Kasim Mohammad, Rajiv Kumar GuptaThe Journal of Indian Prosthodontic Society 2009 9(1):28-32The rehabilitation of patients with acquired defects of the maxilla is a challenge in terms of re-establishing oronasal separation. In most patients these goals are met by means of prosthetic rehabilitation with an obturator prosthesis. Prosthetic rehabilitation can be achieved satisfactorily if all facets of treatment planning and design consideration are taken well into account prior to the rehabilitation process. In many cases effective obturation is achieved but in the relative majority, the prosthesis is usually rejected by the patient as being non-retentive and the outcome is a failure. So, making the prosthesis hollow and the placement of attachments can have a dramatic effect...

This study compared the recorded interocclusal relationships produced using commonly used materials, determining which material accurately provides records of the interocclusal relationship for remounting. Ten occlusal registration materials were used in this study. Maxillary and mandibular casts were mounted on an articulator in maximum intercuspation. The interocclusal relationship was recorded with each specimen under a constant weight. Remounting of the mandibular cast was accomplished with the interocclusal record. The record was removed and the interocclusal contact recorded using occlusal indicator wax. The penetration of contacts was counted. This process was repeated using three specimens for each of the 10 materials used and measured. A standard deviation analysis was done on the...

Conclusions:</b> Cantilever titanium connectors with 5.28&#8197;mm2 area are insufficient to withstand 500 N loads in a molar size cantilever, but may support normal physiologic loads of 50 N. The connectors should be more elliptical than circular to better withstand vertical loads, and the CAD software should permit the design of fillets in the connector/abutment surface. Future studies should evaluate the size of this fillet. (Source: The Journal of Indian Prosthodontic Society)

SK Khindria, Sanjay Bansal, Megha KansalThe Journal of Indian Prosthodontic Society 2009 9(1):2-5Acquired and congenital defects of the face create an unfortunate condition for an individual. For the individual to lead a comfortable life requires facial rehabilitation, thus a reassessment of materials used in the field of maxillofacial prosthesis seems desirable. The materials have traveled a long way from wood, wax, primitive metal, leather, rubber, etc. to the latest biomedical material such as polymers. While the new materials have exhibited some excellent properties they have also exhibited some frustrating deficiencies. We still are in search of a material comprising all the ideal properties so as to best restore a maxillofacial defect. (Source: The Journal of Indian Prosthodontic Soc...

Prithviraj D R, Singh Vishal, Kumar Sarvanan, Shruti D PThe Journal of Indian Prosthodontic Society 2009 9(1):55-58Amelogenesis imperfecta has been described as a complex group of hereditary enamel defects that disturbs the enamel structure and exists independent of any related systemic disorder.

Patil Pravinkumar G, Parkhedkar Rambhau DThe Journal of Indian Prosthodontic Society 2009 9(1):33-35Prosthetic rehabilitation with surgical obturator for total maxillectomy patients is a challenging job because of the lack of normal hard and soft tissue support.
Padmanabhan T V, Mohammad Kasim, Gupta Rajiv KumarThe Journal of Indian Prosthodontic Society 2009 9(1):28-32The rehabilitation of patients with acquired defects of the maxilla is a challenge in terms of re-establishing oronasal separation. So, making the prosthesis hollow and the placement of attachments can have a dramatic effec...

(Source: The Journal of Indian Prosthodontic So...

It includes both white and red esthetics with much greater importance given to red esthetics. An ideal implant position in all 3 dimensions is required. These mesiodistal, apicocoronal, and orofacial dimensions are well described, defining &quot;comfort&quot; and &quot;danger&quot; zones for proper implant position in the anterior maxilla. They showed an excellent esthetic and clinical success; however, their fracture strength needed further investigation. The aim of this study was to compare the fracture resistance of these two ceromers in a situation that simulates clinical conditions. Twenty extracted human mandibular molars, similar in their dimensions, were selected and prepared. After tooth preparation, impressions were taken and casts were made. Ten molar crowns were fabricated with GC Gradia and the other 10 molar crowns were made of Signum+ at random. All the molar crowns were luted on human molars with a re...

Bansal Sanjay, Palaskar JayantThe Journal of Indian Prosthodontic Society 2008 8(4):185-191The rationale of recording Centric Relation records is to establish guidelines as starting point to develop occlusion with artificial teeth in harmony with the various structures of masticatory apparatus including TMJ. It aids to maintain physiologic as well as anatomic health of tissues. When maximum intercuspation is coinciding with centric position, it provides stability to the prosthesis thereby preserving the health of remaining tissues (edentulous foundation, remaining natural teeth, musculature and TMJ) is accomplished. Cosmetic demands, functional needs, treatment sequencing, timeliness and affordability are some primary concerns that must be addressed on an individual basis. A patient will generally want a cosmetic and functional prosthesis at the earliest possible opportunity.
Bello Floriano JC, Cimini Jr Carlos A, Albuquerque Rodrigo C, Vasconcellos Walison AThe Journal of Indian Prosthodontic Society 2008 8(2):94-97The influence of the preparation and restoration of cavities by using composite resin on the stress distribution of the upper central incisor was evaluated through a three-dimensional finite element method. Nine models were constructed. Model 1 was prepared with the enamel, coronary and radicular dentin, pulp and cortical and cancellous bones. In models 2 to 5, the dentin and enamel were removed to simulate interproximal and endodontic access cavity preparations and. In models 6 to 9, the preparations were restored with composite resin. All models were subjected to a 100-N static load with an inclination of 45° at a distance of 2.0 mm from th...

Prasad Rahul, Bhide S V, Gandhi P V, Divekar N S, Madhav VNVThe Journal of Indian Prosthodontic Society 2008 8(2):83-86Dentists occasionally come across patients with constricted oral openings. As the size of the oral opening decreases, the difficulty of the required treatment increases. A maximal oral opening that is smaller than the size of a complete denture can make prosthetic treatment challenging. Several techniques have been described for such situations, where either standard impression trays or the denture itself becomes too difficult to place and remove from the mouth.
The literature contains various reports on the fabrication of sectional or hinged tray/complete dentures, utilizing various mechanisms for connecting each of the components.
A case report and technique of a simplif...

Shah Farhan K, Aeran HimanshuThe Journal of Indian Prosthodontic Society 2008 8(2):66-70It is quite usual for a person to have a natural eye removed as a result of a severe trauma; congenital abnormality; or disease such as an infection, a tumor or untreatable painful glaucoma. In such situations, the natural eye is removed by an acceptable medical procedure, for example, by enucleation or evisceration. It is also usual for a person to have a smaller than normal or phthisical eye that is blind. An ocular prosthesis is created to restore a more normal anatomical structure and the cosmetic defect created by these conditions in a person. The initial step in creating this prosthesis is the preparation of an impression of the ocular socket. From this impression, an ocular prosthesis is created ...

A dental periscope is a new device for examining the abutment preparations outside the mouth, which is otherwise difficult to judge intraorally. With the help of this device, the evaluation of relative parallelism of tooth or implant abutment preparations for fixed partial dentures will become more convenient. Axial preparations for individual inlay, onlay or crown also can be evaluated easily with this device. (Source: The Journal of Indian Prosthodontic Society)

It has been described by many artists, mathematicians, scientists and philosophers. The proportion encompasses both living and nonliving entities; even artificial components and structures appear to be directed to conform to the golden proportion. It appears that humans are genetically programmed to recognize this proportion as pleasing since it explains beauty and is related to the science of numbers. The ideal proportion is related directly to the golden proportion that ranges from 1 to 1.618. The golden proportion is considered a universal standard for facial beauty, and its...

Bhandari SK, Ravindranath T, Sachdeva S, Gurlhosur S, Bhasin SSThe Journal of Indian Prosthodontic Society 2006 6(4):202-205The choice of a suitable prosthesis for a specific case is determined to a great extent by the number, position and salvagability of the existing teeth as well as the mucosa and underlying residual bone. Also of significance are the expectations and demands of the patient from the prosthesis. The following case report discusses the rehabilitation of a partially edentulous patient with an Implant retained, tooth and mucosa supported mandibular removable partial denture against conventional maxillary removable partial denture.
A new technique to locate the external finish line in metal base maxillary denture: A clinical report

The objective information available regarding the exact location of external finish line in a metal base denture is not sufficient. In the past, placement of this external finish line was somewhat arbitrary. Improper placement of resin-metal finish lines may adversely affect phonetics.
This clinical report describes a new technique to locate external finish line precisely in metal base maxillary denture. This technique is easy to do, cost effective and reduces post-insertion visits by the patient. (Source: The Journal of Indian Prosthodontic Society)
