            <title>Pancreatic cancer and glucose metabolism.</title>

            <description>CONCLUSIONS: Our results showed that tumor removal in pancreatic cancer patients improved glucose metabolism. This occurred despite a postoperative reduction in endocrine pancreas mass, which may suggest the presence of insulin resistance and diabetogenic effect of pancreatic cancer. The elucidation of the mechanism is of immense importance for providing an early tumor marker and preventative and therapeutic modalities.

            <description>CONCLUSIONS: Patient safety, outcomes, and satisfaction are similar in colonoscopy procedures performed under sedation protocols using either midazolam and meperidine or midazolam alone. Although endoscopists favor the use of both medications together, adding meperidine to midazolam before the colonoscopy does not seem to have additive beneficial effect for the patients.

            <description>We describe a rare presentation of acute cardiac tamponade in a patient with gastric cancer in pregnancy. A 32-year-old woman developed dyspnea and tachycardia during the 28(th) week of her pregnancy. These symptoms were interpreted initially by the patient as a condition related to the normal state of pregnancy; however, her symptoms persisted over the next two weeks, and she was hospitalized for evaluation.     Meckel's diverticulum is a remnant of the vitelline duct and a congenital anomaly of the gastrointestinal tract. Hemorrhage from a Meckel's diverticulum is common in children but extremely rare in adults over 50 years of age. Very few cases have been reported to date and all prior cases were in men. Meckel's diverticulum is commonly overlooked as a possible cause of a lower gastrointestinal hemorrhage in adults. Here, we present the rare case of a 58-year-old woman with massive hemorrhage from a Meckel's diverticulum that was diagnosed by repeated emergency angiographies and treated with elective laparoscopic surgery.

            <description>We report herein a 50-year-old man with a 6-mm sigmoidal polyp; he had been operated previously for rectal carcinoma. The polyp was characterized by benign-appearing spindle cells in the lamina propria leading to a wide separation and disorganization of the colonic crypts, accompanied by focal erosion and restricted areas suspicious for inflammatory fibroid polyp. The histologic features were found consistent with the disease spectrum of 'benign fibroblastic polyp of the colon' defined by Eslami-Varzaneh et al. The case is presented with a review of the literature and differential diagnostic considerations.

A 46-year-old woman with a history of recurrent mild upper abdominal pain and diarrhea for 10 months was admitted to our hospital. Multiple-phase spiral computed tomography revealed a 10 cm x 8 cm, ill-defined, elliptic mass in the body and tail of the pancreas. There was a slightly heterogeneous enhancement on hepatic arterial phase and isodensity to the pancreatic parenchyma with small dotted necrosis within the middle region of the mass on hepatic portal venous and parenchymal phase, with patent splenic vein, dilated collaterals at the splenic hilum and no dilated pancreatic duct, resembling a diffuse infiltration tumor.             <description>Conclusions: This is the first study investigating CYP2C19 polymorphism in dyspeptic Turkish patients. Our investigation revealed that the most common CYP2C19 genotype was wild type CYP2C19 in dyspeptic Turkish patients. Dyspeptic Turkish patients are extensive metabolizers for proton pump inhibitors. This finding might have impact on the clinical consequences for the treatment strategies in dyspepsia.

            <description>We present three cases with giant intra-abdominal cysts (one mesenteric cyst, two ovarian) that were initially misdiagnosed as refractory ascites in another hospital. On ultrasound examination, displacement of bowel loops to outside the fluid without dispersion of fluid around the liver and spleen were the main findings suggesting the possibility of giant intra-abdominal cysts in our patients. In all three patients, the diagnosis of giant intra-abdominal cysts was confirmed after their total excision. Clinicians must consider processes other than ascites in the differential diagnosis of large abdominal fluid accumulation.

In our study, there was no evidence that Celiac disease is a pre-malignant condition for lymphoma. Serological screening for Celiac disease in lymphoma patients does not seem to be necessary.

In comparison with the pharmacological action of octreotide, we believe that baicalin, as a new drug, has similar protective effects on the intestinal mucosa of severe acute pancreatitis rats, and therefore deserves further study and development.

The lesion was an unencapsulated light brown tumor (largest diameter 6 cm) without necrosis or hemorrhage.             <description>CONCLUSIONS: When tumors of the biliary system are divided as intrahepatic and extrahepatic cholangiocarcinomas, MMP-2 expression was present in the extrahepatic cholangiocarcinomas including gallbladder carcinomas. Like the intrahepatic cholangiocarcinoma, Klatskin tumors also did not express MMP-2. This can be related with its characteristic growth pattern. MMP-9 and MMP- 14 were present in metaplasia, dysplasia carcinoma sequence in all of the bile tract tumors, suggesting that MMPs play an important role in carcinogenesis. The higher expression of the MMPs with neural invasion suggests the significant role of those tumors in the invasion activity.

A 72-year-old Turkish man was admitted to the Department of General Surgery with complaints of epigastric pain, nausea and vomiting for five days. Upper gastrointestinal endoscopy evidenced a diffuse infiltrate in the stomach, causing decreased distensibility. Microscopically, superficial chronic inflammation with hyperplastic reactive changes and damaged surface was observed. Histopathological examination of the gastric mucosa showed numerous cross-sections of adult worms, eggs and rhabditiform larvae of S. stercoralis developing in the gastric crypts.

            <description>We present a 34-year-old woman who had suffered from several episodes of melena for the past three years. Endoscopic examinations were normal. Computed tomography showed a large target lesion over the right abdomen and an image representing intestinal malrotation, which was supported by enteroclysis. At exploratory laparotomy, ligamentum of Treitz was located in the mid-to-right side of the columna vertebralis, and the duodenal bulb was found to be invaginated into the proximal jejunum. In type I patients, simple cholecystectomy is generally enough, but sometimes T-tube insertion may be required, while the cases with types II-IV require more complex surgical approach, such as cholecystectomy and bilioenteric anastomosis. Roux-en-Y hepaticojejunostomy is an appropriate procedure with good outcome.

We discuss herein a patient admitted to our outpatient clinic for dyspepsia in whom a giant annular esophageal leiomyoma was diagnosed. Resection through thoracoabdominal approach is recommended instead of adjuvant treatment in similar sized tumors.

            <description>We report a 65-year-old patient with a gastric polyp of 2.5 cm in diameter located at the cardia on upper gastrointestinal (GI) endoscopy. Pathological examination of the excised polyp showed intramucosal carcinoma. Endoscopic ultrasonography (EUS) reported the lesion as early gastric carcinoma with probable submucosal involvement. On serial sections of the gastrectomy material, the lesion was an intramucosal carcinoma and surprisingly there was a leiomyoma located adjacently.

            <description>Conclusions: Determination of anti-tissue transglutaminase antibodies by simple visual system for celiac disease appeared to be as reliable as the ELISA system. It is easy to perform and interpret, costeffective, and rapid, as pointed out in other previous studies, as a screening test in large population-based studies. The prevalence of celiac disease in the overall sample of Turkish children (1:111 or 0.9%) in this preliminary study is similar to that reported in European and Middle Eastern countries and the United States.

            <description>Conclusions: Although conservative surgical procedures are considered simpler and safer to perform, the rate of postoperative complications such as biliary fistula, residual cavity and recurrence, and cavity suppuration has been reported to be about 35%. On the other hand, radical surgery can be performed with low risk of recurrence (3.2%). We believe radical surgical procedures present a lower rate of recurrence and less morbidity, and thus should be the surgical treatment of choice for hepatic hydatid disease.

Herein we present a 46-year-old female admitted to the hospital with the complaints of bloody diarrhea (25-30 times a day), fever, abdominal pain and weight loss. Eosinophilic colitis was diagnosed and she was treated with steroid successfully.

            <description>We present here a 57-year-old woman admitted to our clinic with abdominal pain, tar-colored stool and confusion. Further investigations were done because of severe anemia. Abdominal computerized tomography revealed intraabdominal hematoma. Laparotomy was performed, which confirmed that intraabdominal haemorrhagia had occurred with the rupture of a splenic artery aneurysm into a pancreatic serous cystadenoma, which ruptured into the abdomen because of high pressure. This is an interesting case diagnosed with multidisciplinary approaches.

Good user acceptance proves that both the terms and structure of Minimal Standard Terminology were consistent with usual clinical thinking. However, future work on Minimal Standard Terminology is mandatory for better coverage of endoscopic retrograde cholangiopancreatographies examinations. Technically new software development methodologies have to be sought for lowering cost of development and the maintenance phase. They should also address integration and interoperability of disparate information systems.
