Authors: Alfsen GC, Lyckander LG, Lindboe AW, Svaar H
Background. Deaths at Akershus University Hospital were systematically reviewed to evaluate the quality of death certificates and to improve reporting of deaths by unnatural causes. Material and method. Death certificates and medical records from the 496 patients who died at Akershus University Hospital in the period 1 May-31 December 2008 (8 months), were reviewed prospectively. Doctors were contacted when death certificates had an illogical set-up, when important clinical findings were not reported, and upon suspicion of unnatural death or a lethal adverse drug reaction. For comparison, 134 deaths that occurred in March 2007 and March 2008 were evaluated retrospectively. Results. 27 % of death certificates in the control period an...

Authors: Størdal K
Background. HIV (human immunodeficiency virus) may be transmitted from mother to child during pregnancy, delivery and breast-feeding. Transmission can be prevented, and efficient methods have caused vertical transmission to become rare in Europe and the USA. The article discusses the global HIV epidemic among children and challenges in reducing transmission from HIV-positive mothers. Material and methods. The article is based on literature retrieved through a non-systematic search in Medline and experience from working as a paediatrician in Botswana. Results. HIV transmission from mother to child can be reduced from about 30 % to 4 % with relatively simple measures. In many high-endemic areas in Sub-Saharan Africa, little is done to prevent HIV transmission to n...

Background. A consensus meeting at the Norwegian Institute of Public Health decided to change the limits for significant bacteriuria (used in microbiology laboratories) from 104 to 103 colony-forming units per ml. Such a low threshold value is difficult to read on dip-slides; NOKLUS (Norwegian quality improvement of laboratories in primary care) therefore wished to review the use of dip-slides in general practice. Material and methods. The article is based on literature retrieved through a non-systematic search in PubMed and on the authors' experience and research in the field. Results. Escherichia coli (E coli) is the most common agent in both lower and upper urinary tract infections, and in asymptomatic bacteriuria in pre...

Authors: Marienhagen K, Helland A, Nordberg T, Guren MG, Hofsli E
Background. After receiving worrying signals from its members concerning their workload, the Norwegian Oncology Society decided to review how Norwegian oncologists perceive their working conditions. Material and methods. A questionnaire was sent to all doctors working in departments of oncology. They were asked to provide background information about themselves, how they experienced the working conditions, and what could be done to improve work satisfaction. Descriptive statistics were used for analysis of results. Results. 174 of 298 doctors (58 %) answered the questionnaire. The median number of working hours per week was 52 hours for oncologists, and 45 hours for doctors in training. 72 % of oncologists and 56 % of do...

Relapses in multiple sclerosis, associated with significant functional impairment, should be treated with methylprednisolone. In case of repeated relapses or onset cases of multiple sclerosis there is an indication for preventive treatment with interferon-beta/glatiramer acetate (first-line treatment), natalizumab (second-line treatment) or mitoxantrone (third-line treatment).

Conclusion. Familial Mediterranean fever is a rare disorder in Norway but frequent in many Mediterranean countries. Common symptoms are recurrent episodes of abdominal pain, chest pain, joint pain and fever. Treatment with colchicine reduces inflammation and the risk of developing amyloidosis.

This article focuses on frequency, causes, and prevention of the most important anaesthetic complications. Material and methods. The article is based on literature identified through a non-systematic search in Pub-Med, and the author's research and experience in this field. Results. The risk of death associated with anaesthesia is closely related to patient age and physical status. In otherwise healthy patients (ASA 1), the risk of such deaths is approximately 1:250 000. Medication errors occur in approximately 1:1 000 anaesthetic procedures. The risk of awareness during general anaesthesia is approximately 1:650. Neural injury from epidural and spinal anaesthesia is rare, even more so in obstetrics. Anaphylaxis caused by muscle relaxant drugs is more common in Norway than in many other in...

Background. Anaphylaxis is a serious life-threatening generalized or systemic hypersensitivity reaction. The aim of this paper is to provide knowledge on how to diagnose, treat and follow up patients with suspected anaphylaxis during general and local anaesthesia. Material and methods. The article is based on literature identified through a non-systematic search in PubMed, the Scandinavian Guidelines on anaphylaxis during anaesthesia and on own research. Results. Anaphylactic symptoms during anaesthesia vary with respect to severity. Manifestations from skin and the cardiovascular and respiratory systems are present simultaneously in approximately 70 % of patients. Early treatment with adrenaline, fluid and extra oxygen may be vital...

Background. To secure the airway has the highest priority when handling critically ill and injured patients. The aim of this article is to present simple and new devices and techniques to secure the airway. Material and methods. The paper is based on international algorithms for optimal treatment of patients with acute airway problems, and the authors' experience from clinical work and organizing of courses. Results. Unconscious patients have obstructed airways because the tongue slides back and blocks the airway. This can be prevented by lifting the jaw, possibly supplemented by insertion of an oropharyngeal airway. When these active techniques are not used, patients must be placed in the semi-prone position and air passage c...

The objective of this retrospective study was to determine, within one institution, local relapse rate and survival for women with early-stage breast cancer treated with breast-conserving surgery followed by radiotherapy. Material and methods. All women with infiltrating early-stage breast cancer who underwent post-operative whole breast irradiation at our institution in the period 14.06.99-8.03.2002 were included in the study. A CT-based 3D dose calculation was performed in all patients. Results. 222 women received 50 Gy whole-breast irradiation after breast-conserving surgery in the study period. 51 patients received adjuvant systemic therapy according to national guidelines. Median age at diagnosis was 59 years (34-82 years). Median tumour size was 12 mm (1-30 mm) and 86.5 % of the pati...

This article describes diagnosis and treatment of transsexualism with an emphasis on surgery. Material and methods. The article is based on a non-systematic review of the literature and our own clinical experience. Results. Patients are evaluated for sex reassignment surgery after at least one year of psychiatric evaluation and an additional year of endocrine treatment. Male-to-female patients are evaluated for breast augmentation if hormone therapy has given unsatisfactory results. Thereafter, genital surgery can be performed, i.e. the testis and corpus cavernosum are removed and a neovagina and neoclitoris are constructed. Female-to-male patients have the possibility to undergo breast reduction, hysterectomy, salpingoophorectomy and construction of a neopenis. If the patient responds wel...

Background. A mutant Chlamydia trachomatis variant (nvC trachomatis) has made it more difficult to diagnose chlamydia in Sweden. The proportion of nvC trachomatis has varied between Swedish counties (25-80 %) in the period 2006-07. Our goal has been to monitor nvC trachomatis among our patients from January 2007 and up to July 2008. Material and methods. In this time period, all C trachomatis samples at Fürst Medical Laboratory, Norway were analyzed twice. Cobas TaqMan 48 (Roche Diagnostics) was used to detect C trachomatis in isolated DNA and real-time PCR methods developed by us were used to both dete...

In the western world, cardiovascular disease is the leading cause of death for both sexes. These diseases show substantial differences between the sexes with respect to epidemiology, biology and clinical aspects. In recent years, more attention has been directed towards sex differences in pharmacological effects. Although both sexes are included in most pharmacological trials, the proportion of women is often too low to enable sex-specific analyses.

Background. All types of anaesthesia should ensure absence of pain, inhibit autonomic responses and provide optimal conditions for surgery. Choice of anaesthetic approach (local, regional or general anaesthesia or a combination of methods) depends on type of surgery and the patients' health and preferences. Material and methods. The review is based on literature identified through non-systematic searches in PubMed and own research and experience. Results. When selecting anaesthetic approach and anaesthetic drugs one has to consider not only the perioperative period, but also postoperative aspects such as pain relief, awakeness, functional ability and absence of nausea. After major surgery, regional anaesthesia (especially epidural an...

Background. Local anaesthetics may alleviate pain more effectively than any other anaesthetic method. In regional anaesthesia/analgesia, rare but serious complications make it necessary to always consider the risk-benefit ratio. The article discusses these issues and gives advice on effective and safe conduct. Material and methods. The article is based on non-systematic literature searches in the PubMed and Cochrane databases and our own experience from research and clinical work. Results. Regional anaesthesia is obtained by administering local anaesthetics near the spinal cord and nerve roots (spinal, epidural), spinal nerves (paravertebral), or close to peripheral nerves. Parts of the body will then become numb and paralysed. The same techniques are used ...

Background. Muscle relaxants were introduced into clinical anaesthesia for the first time in 1942. The purpose of this article is to provide an overview of the history of muscle relaxants, their mode of action and their role in current anaesthetic practice. Material and method. The review is based on clinical experience, own research and a non-systematic literature search using PubMed. Results. A muscle relaxant is either suxamethonium (curacit) or one of many curare compounds. One of the curare drugs was brought to Europe from South America in the 1700s and the active substance (called d-tubocurarine) was isolated in 1935. This type of drug paralyses striated muscles that are under voluntary control by interfering with the normal signalling system between nerve and mu...

Background. When health care leads to serious adverse events that result in (or could have resulted in) injuries to patients, hospitals are obliged to report this to The Norwegian Board of Health Supervision in the Counties. The purpose of this study was to quantify how many cases were instigated on the basis of reports sent to the supervisory authority and to what extent health personnel were sanctioned because of them. Material and methods. Reports received by The Norwegian Board of Health Supervision in the Counties (in the period 1.07.2002- 30.06.2007) and cases instigated by the National Board of Health (on basis of the reports) in the period 1.01.2003-31.12.2007. Results. The supervisory authority received 9268 reports in the period. 443 (4.7 %) of them led to i...

Background.Schizophrenia is a serious mental disease of unknown aetiology. Genetic liability is the most important risk factor. Several studies have demonstrated that pre and perinatal complications/traumas are associated with an increased risk of developing schizophrenia in adult age. The purpose of this article is to provide an overview of research on obstetric complications as risk factors for schizophrenia. Material and methods. The article is based on literature identified through non-systematic searches in the databases PubMed and Embase. Results. The putative association between obstetric complications and schizophrenia has been investigated for almost 80 years. Numerous controlled studies have found that maternal infection (influenza, rubella, toxo...

This article provides an overview of indications, contraindications, efficacy, and side effects of this so-called biological treatment of IBD. Material and methods. Based on the clinical experience of the authors, literature was selected through a non-systematic search in PubMed. Results. TNFalpha-antibodies have a documented initial effect in 60-70 % of patients with moderate to severe luminal and/or fistulising IBD. Approximately 30 % of patients achieve remission after a relatively short treatment period. However, the effect seems to decrease during long-term treatment. It is still unsettled whether these drugs prevent colectomy in ulcerative colitis. The treatment seems to increase the risk of serious infections and there are also some indications of an increased risk of malignancy. Co...

Background. A highly sensitive troponin T analysis became available in Norway in 2009. The Norwegian Society of Medical Biochemistry believed that a national strategy for implementation of the analysis would ensure a more coherent practice for diagnosing acute myocardial infarction (AMI). Material and methods. 70 laboratories were requested to identify (in a questionnaire) the type of assay used, routines for reporting results of analyses, internal quality control programmes (IQA) and local cut-off values for making the diagnosis AMI. Results. 62 laboratories responded and 57 of these analyzed troponins; 60 % used troponin T and 40 % troponin I. 82 % of laboratories were familiar with the decision limit used for diagnosing AMI at their hospital; 4...

This article gives a short overview of the epidemiology, clinical picture, diagnostics and treatment of P. knowlesi infection. Material and methods. The article is mainly based on articles published in international journals during the last five years. Results. Most cases have been diagnosed in the Malaysian state Sarawak on the island of Borneo. The disease usually presents in the same way as other types of malaria, with the exception that fever spikes comes at 24-hour intervals. Microscopy findings resemble those for P. malariae in the early phase of the disease and many patients have been diagnosed wrongly. Without treatment a serious clinical picture may develop, blood findings may be as serious as for P. falciparum infection and death may occur. Chloroquine tablets are indicated in li...

A 42-year-old man was found unconscious in his prison cell. Investigations revealed non-recognized diabetes mellitus, possibly secondary to use of prescribed antipsychotic drugs. Second generation antipsychotics are associated with metabolic changes such as weight gain, hyperglycaemia and dyslipidaemia. Severe side effects may also occur; e.g. diabetic ketoacidosis in people without acknowledged diabetes (as presented in this case report). Providing adequate healthcare to people with psychotic disorders may be a demanding task. When these patients develop somatic conditions, a multidisciplinary approach is required. Personnel from many specialities may become involved, and they are not all familiar with drugs commonly used to treat psychiatric disord...

Background. RGPs (regular general practitioners) are obliged to take on duties in the out-of-hours services in Norway, but they actually perform less than half of this work. We wished to compare remuneration claims for out-of-hours work from RGPs with those from other doctors. Material and methods. The Norwegian Labour and Welfare administration provided information about all doctors working in the out-of-hours services in Norway in 2006 (4 729 of whom 2 220 were RGPs) and coupled this with the remuneration they had claimed in this period. Results. RGPs had 46.6 % of all patient contacts in the out-of-hours services, but more telephone contacts (57.4 %) and simple/short contacts (50.7 %) than other doctors. Other doctors had most contacts in the large...

Background. CPVT (catecholaminergic polymorphic ventricular tachycardia) is a condition characterized by syncopes and cardiac arrest that was first described in 1975. CPVT has later been classified as a genetic disease with a great risk for life-threatening arrhythmias that are mainly caused by mutations in the ryanodine receptor 2 gene. Starting with a case report, we present an overview of CPVT. Material and methods. The literature reviewed was identified through a non-systematic search in PubMed. Results. Diagnosing CPVT may be difficult, as resting ECG is normal and the syncopes may be misdiagnosed as epilepsy. Information about syncopes related to physical or emotional stress and...

Background. Neutropenic enterocolitis is a life-threatening complication that usually occurs in connection with chemotherapy for acute leukemias. Our experience with diagnosis and treatment of these patients is presented. Material and methods. Medical records from patients treated for neutropenic enterocolitis at Ullevaal University Hospital in the period 2000-2008 were retrospectively reviewed. Results. 16 patients with median age 33 years were treated for neutropenic enterecolitis. Induction chemotherapy was given for acute myelogenic (n = 9) or lymphatic (n = 4) leukemia, myelomatosis (n = 2) or lymphoma (n = 1). The patients developed aplasia five days (median) after start of chemotherapy. All patients were first treated conservative...

Background. At the turn of 2007/2008, four Norwegian men died after ingestion of commercially available supposedly denatured ethanol. Material and methods. The four deaths are presented and discussed. Results. Methanol concentrations, consistent with lethal methanol poisoning, were detected in blood and urine for all four. The imbibed mixture was subsequently shown to contain a 70/30 mixture of methanol/ethanol. Interpretation. The events emphasize the importance of investigating methanol findings from deceased to identify the source, and that investigations are instigated promptly to prevent further exposure.

Background. Acute respiratory failure has an annual incidence of 20-75/100 000 and is the most common reason for admittance to an intensive care unit. A common cause is acute inflammatory changes in lung tissue. The article reviews clinical, etiological, pathophysiological and therapeutic aspects of acute respiratory failure, with an emphasis on failure secondary to proinflammatory processes. Material and methods. This paper is not based on a comprehensive literature review, but on the clinical and scientific experience of the author, literature from a private archive and a limited Medline search. Results. Acute respiratory failure can be precipitated by agents and/or trauma that damage the lungs directly. Serious infections and tissue damage located in other parts of...

Background. Acute renal failure is common in critically ill patients and is associated with a high mortality rate. This paper reviews current management of patients with acute renal failure admitted to an intensive care unit. Material and methods. Literature search in databases (Medline, Cochrane database of systematic reviews, UpToDate). Results. The prevalence of acute renal failure is 5-20 % in patients admitted to intensive care units; the associated hospital mortality is 30-60 %. The aetiology is usually multifactorial; inflammation (sepsis, surgery), hypovolaemia and drug toxicity commonly precipitate acute renal failure. There is no effective drug treatment, but early onset of renal replacement therapies with haemodialysis or haemofiltration is likely...

Authors: Ulvestad E, Swensen E, Simonsen GS, Schei E
During the second half of 2009 the Norwegian people experienced the first pandemic influenza since the Hong-Kong disease in 1968. The pandemic was initially believed to be severe, but it soon became evident that the disease was milder than anticipated. In spite of this the health authorities increased the level of threat and recommended general vaccination and exempt of oseltamivir (Tamiflu) from prescription requirements. In the paper we critically evaluate the evidence-base and consequences of these recommendations.

This article describes effects of these changes. Material and methods. Blood-type immunization, phototherapy and exchange transfusions were prospectively recorded in the 21-year period 1988-2008. Three cohorts comprised the basis for the analyses: infants born in the time-periods 1988-93, 1994-2006 or 2007-2008. Results. 6.2 % of the infants born 1988-93 and 6.7 % born 1994-2006 received phototherapy. Physiological hyperbilirubinaemia in term infants was the main reason for phototherapy in the first cohort, while it was hyperbilirubinemia in preterm infants in the second cohort. 4.8 % of the infants born 2007-8 received phototherapy. Exchange transfusion was performed in 0.02 % born 1994-2008 and 0.2 % born 1988-93. Such transfusions were undertaken more frequently in cases with RhD-immuni...

Background. At the Norwegian Radium Hospital, most patients who were treated for metastases (localized to an extremity) from malignant melanoma in the period 1977-99 underwent intra-arterial chemotherapy combined with radiotherapy. We have evaluated the effects of this treatment, which has now been replaced by isolated limb perfusion (with melphalan and tumor necrosis factor). Material and methods. Medical records were reviewed for patients with metastatic malignant melanoma (localized to an extremity) who had been treated at the Norwegian Radium Hospital with intra-arterial chemotherapy (5-[3,3 dimethyl-1-triazeno]-imidazole-4-carboxamide [DTIC]) in the period 1977-99. Results. 36 patients had received such treatment; in 30 of ...

Background. 2500 new cases of lung cancer are diagnosed in Norway annually. Patients with limited disease can be operated, but many will die from the disease despite surgical treatment. The aim of the study was to review survival and recurrence, and factors which affect survival, in patients operated for lung cancer. Material and methods. The risk of death and recurrence of disease was assessed retrospectively in patients who had non-small lung cancer and were operated at St. Olavs University Hospital, Trondheim in the period 1994-2001. Patient data were retrieved from medical records and a database with records from thoracosurgical procedures. Results. 190 patients (30 % women) were included i...

Background. Knowledge of biomechanics and the cervical spine's anatomy has become more topical as the incidence of whiplash neck disorders has increased. Unfortunately, injuries after traffic accidents are often brought to court, where the medical expert's knowledge is of utmost importance to ensure a correct medical evaluation. Material and methods. The article is based on information identified through non-systematic searches of PubMed and on the author's experience as a professor of anatomy. Results. The cervical spine is particularly vulnerable to forces perpendicular to the length axis. Stability depends largely on the soft tissue. Injuries of soft tissue (especially in ligaments and intervertebral discs) may lead to instability and periosteal reaction with subseq...

Background. The Ministry of Health and Care Services developed an action plan for COPD (chronic obstructive pulmonary disorder) in 2006. General practitioners (GPs) have a central role in this plan and use of spirometry for measurement of lung function is important. We wished to study general practitioners' use of spirometry in the three northern counties of Norway. Material and method. A questionnaire was sent by post to all the GP offices in the region. Those who did not return the questionnaire were contacted by telephone and the researcher filled in the form based on the conversation. The Norwegian Labour and Welfare Administration provided information about the doctors' use of fee categories. Results. We found that 91 % of the GP offices had a spiromet...

Reactions to drug withdrawal may lead to unnecessary reintroduction and inappropriate prescribing. This problem is often encountered after long-term therapy with drugs acting on the cardiovascular and central nervous systems. The adverse effects include rebound effects, withdrawal syndrome, termination of drug-drug interaction, increased risk of disease and recurrence of disease. Lack of scientific evidence limits development of guidelines on how to terminate drug therapy. Slow and gradual tapering schedules for substances with long half-lives is usually recommended to avoid withdrawal syndrome and rebound effects. However, the most important measure is to avoid prescribing drugs that are difficult to withdraw. Appropriate information and involvement o...

Background. Child physiology and disease is not the same as in adults, which implies different challenges within intensive care. The aim of this review article is to shed light on special diagnostic and therapeutic problems in paediatric intensive care. Material and methods. The review is based on literature identified through a non-systematic search in PubMed, and on the authors' own clinical experience. Results. Common causes for admitting children to intensive care units are head injuries; septic shock and respiratory failure. Perfusion-directed therapy of severe traumatic head injuries is well recognized, but optimal age-appropriate cut-off values have yet to be defined for cerebral perfusion pressure and intracranial pressure. Therapeutic hypothermia i...

This article is based on data from the authors' systematic follow up of ICU (intensive care unit) patients and literature identified through non-systematic searches in PubMed. Results. In Norway more than 80 % of ICU patients survive the hospital stay. From about two years after intensive care and onwards, the overall survival rate parallels that of the total population. Pre-admission data show that ICU patients in general have more co-morbidity and reduction of life quality and functional status than the normal population. Their quality of life increases with time, and most patients become independent of others. Interpretation. Many ICU patients have rehabilitation potential despite critical illness and comorbidity and most of the long-term survivors achieve acceptable quality of life. IC...

A 20-year-old woman presented with dyspnoea in the Emergency department and subsequently suffered a cardiac arrest. The initial rhythm was PEA (pulseless electrical activity). She had intermittent return of spontaneous circulation. Transthoracic echocardiography showed a dilated hypokinetic right ventricle and a collapsed left ventricle. The tentative diagnosis was pulmonary embolism, but she remained hemodynamically unstable despite thrombolysis. 90 min after the collapse she was put on cardiopulmonary bypass and surgical embolectomy was performed. Large masses of thrombotic material were collected from central parts of the right and left pulmonary artery. Therapeutic hypothermia was applied for 24 ho...

Background. Breast milk is very important to ensure infants a well-composed and safe diet during the first year of life. However, the quality of breast milk seems to be affected by an increasing amount of environmental toxins (particularly so-called Persistent, Bioaccumulative Toxins [PBTs]). Many concerns have been raised about the negative effects this may have on infant health. Material and methods. The article is a review of literature (mainly review articles) identified through a non-systematic search in PubMed. Results. The concentration of PBTs in breast milk is mainly caused by man's position as the terminal link in the nutritional chain. Many breast-fed infants have a daily intake of such toxins that exceed limits defined for the population in general. Anima...

However, current knowledge on pharmacodynamic and pharmacokinetic properties of penicillins does not support this recommendation. A 63-year-old man was admitted to the emergency room with chest pain. He had experienced a painful, tingling sensation in the right arm for the last three months, as well as pain in the right scapula. ECG and standard blood samples were normal. An X-ray of the thorax showed a mass in the superior sulcus on the right side. Further investigation with CT and MRI identified a large tumour, about 6 cm in size, with infiltrative growth involving the upper costae. Biopsy revealed a non-small cell carcinoma of the lung. The patient underwent a tri-modal treatment regimen with induction chemotherapy (two courses of cisplatin and etopside) and concominant radiotherapy (50Gy in 2Gy fractions) before a right upper lobectomy was performed. 1 year ...

Partly owing to disregard for natural and cultural history, theoretical medicine has become an insufficient guide for much medical practice. Based on theoretical elaborations by G.W.F. Hegels (1770-1831) and Charles Darwin (1809-1882) I attempt to demonstrate the relevance of such historical knowledge for the much-debated field of evidence-based medicine.

The medical faculty at the royal Frederik's University (later the University of Oslo) was established in 1814 with a combined education of medicine and surgery. This made the Norwegian medical education quite unique at the time, and the university played a main role in uniting medicine and surgery into one profession. In other European countries, medical/surgical education in universities emerged later, mainly because academic schools for surgical education already existed and because both the medical and surgical elites were reluctant to let their professions fuse into a unity. However, from the late 1700s, ...

The Gade institute in Bergen was established in 1912, at the same time as the city's new hospital was opened at Haukeland. The aim of the benefactor F. G. Gade was that the institution should function as a research institute in addition to being the hospital's pathological department and laboratory. The task of leading this work was given to a young cancer researcher, Magnus Haaland,. He succeeded in establishing an effective research school within these institutional settings through educating assistants in pathological-anatomic, bacteriological and serological themes during a two-year rotational arrangement. The institute produced a great number of scientific articles and also several doctoral dissertations. A hallmark for this research was its practical orienta...

Towards the end of the 1800s, chlorosis was a frequently used diagnosis, in Norway as well as in America and the rest of Europe. The physiological basis for the disease was assumed to be decreased levels of haemoglobin and/or red blood cells. Chlorosis was not synonymous with anaemia, but rather a subcategory with a vague symptom-based picture associated with female puberty. The different opinions on causes of the disease can be divided into three main groups: environmental, moral and biological-constitutional. At the time, it was well known that iron was an effective treatment. In a cultural critical perspective the disease was seen both as a phenomenon of degeneration and as a reaction to modern life with its stress and demands. During the first decades of the 190...

Development of medicine in the early modern period (1500-1800) formed the basis for modern medicine, in that iatrochemical and mechanistic perceptions of the human body gradually became more influential. Several different medical theories prevailed and were tested in parallel, and perceptual changes developed over time. Few studies have looked into the knowledge universe that Norwegian doctors were part of in this period. I have examined book collections owned by the physicians Jacob Woldenberg, Georg Blumenthal and Paul Dons, in order to discern how physicians practicing in Norway around 1700 responded to this particular situation of <<complexity>>. In general, these book collections covered antique medical theories and more recent debates within the medica...

Harald Frøshaug received training in scientific research by Rolv Gjessing (1887-1959), who was well known for his research on schizophrenic periodic catatonia. The Second World War brought new challenges for psychiatrists, and there was a pressing need for new treatment procedures and theories. Harald Frøshaug was inspired by English and American psychiatry, especially the psychoanalytic traditions. When he became head of the ward for women at Dikemark hospital in Asker in 1949, time was ripe to put new theories and methods into practice.

Background. Incarceration of young people should be avoided according to Norwegian policy. Even so, young people reside in our prisons at all times. Our aim was to investigate their mental health. Material and methods. Among 42 teenage boys (below 20 years of age) remanded to prison, 40 consented to participate in the study. Socio-demographic data were collected and a structured diagnostic instrument (K-SADS) was used to assess their mental health. Results. Only four participants had no mental disorder. The 36 who had mental disorders had a mean of 2.5 diagnoses. The most common diagnosis was substance abuse/dependence (n = 31). 30 of the adolescents had serious conduct disorders and nine of them fulfilled the ICD-10 criteria for a dissocial person...

A sizeable number of antihypertensive drugs from different drug classes have received market authorization.
