Clinical Practice Guidelines for Feeding Behaviors and Weight‐for‐Age at 12 months: A Secondary Analysis of the Infant Feeding Practices Study II

Conclusions and Implications: Modifiable infant feeding behaviors contributed to weight‐for‐age at 1 year. Replication studies with economically and culturally diverse samples are needed. Further, testing longitudinal interventions that address infant feeding practices is likely to yield the evidence needed for shaping future clinical practice guidelines that reduce the risk for childhood obesity and related pediatric comorbidities.
Recognizing and Responding to Uncertainty: A Grounded Theory of Nurses’ Uncertainty

Conclusions: This theory advances the nursing perspective of uncertainty in clinical practice. The theory is relevant to nurses who are faced with uncertainty and complex clinical decisions, to managers who support nurses in their clinical decision‐making, and to researchers who investigate ways to improve decision‐making and care delivery.
Conclusions: Involving both ward and ICU staff in the redesign process may have contributed to a shared situational awareness of the problems, which led to more timely and effective ICU discharge processes. The use of a change agent, whose ongoing role involved follow‐up of patients discharged from ICU, may have helped to embed the new process into practice.
Barriers and Enablers to Implementation of a New Zealand‐Wide Guideline for Assessment and Management of Cardiovascular Risk in Primary Health Care: A Template Analysis

Conclusions: The lack of facilitation of the guideline into practice was a major barrier to implementation. Implementation plans that address the concerns and complexities of everyday practice are an essential aspect of guideline development. The PARIHS framework was found to be comprehensive and accommodating of the complexity of everyday practice associated with guideline implementation in primary health care. The pertinence of the framework confirms its usefulness as a tool to guide implementation. (Source: Worldviews on Evidence-Based Nursing)

Systematic Review of Text‐Messaging Interventions to Promote Healthy Behaviors in Pediatric and Adolescent Populations: Implications for Clinical Practice and Research

Conclusions: This review supports previous literature suggesting that mobile phones are uniquely positioned to bridge gaps in health disparities and reach across demographics. Interventions using short messaging service may be most effective as a reminder system to support disease management behaviors. Existing recommendations for tailored, interactive, and family‐centered care are supported with mobile technology recommended to augment clinical practice and health behavior change efforts. However, more rigorous, theory‐based intervention research using mobile technology is warranted in pediatric and adolescent populations. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: Researchers and clinicians should take note that CBGT had a moderate effect on the level of depression and a small effect on the relapse rate of depression. The results of this study suggest that the patient should receive a course of therapy at least every 6 months. (Source: Worldviews on Evidence-Based Nursing)

ABSTRACTBackground: Necrotizing soft tissue infections (NSTIs) are rare, rapidly spreading infections that occur in the soft tissue compartments. The mortality rate is high and has been found to decrease if patients are treated early and aggressively with surgical debridement and broad‐spectrum antibiotics. Unfortunately these infections present similarly to other types of skin and soft tissue infections (SSTIs) making diagnosis difficult.Aims: This paper reviews the evidence surrounding the early diagnosis of NSTIs. This was used to develop a clinical practice guideline (CPG) for implementation in the emergency department (ED) setting to assist the provider in distinguishing NSTIs from SSTIs to potentially decrease the time from presentation to diagnosis. Methods: A review of the literat...

ABSTRACTBackground: Over the past decade, the need for healthcare delivery systems to identify and address patient safety issues has been propelled to the forefront. A Canadian survey, for example, demonstrated patient safety to be a major concern of frontline nurses (Nicklin & McVeety 2002). Three crucial patient safety elements, current knowledge, resources, and context of care have been identified by the World Health Organization (WHO 2009). To develop strategies to respond to the scope and mandate of the WHO report within the Canadian context, a pan‐Canadian academic‐policy partnership has been established.Approach: This newly formed Pan‐Canadian Partnership, the Queen's Joanna Briggs Collaboration for Patient Safety (referred throughout as “QJBC” or “the Partnership”...

Conclusions: APNs need to be supported to address the individual, interpersonal and organisational factors, which influence their ability to promote EBP. Organisational commitment at the highest level is key to APNs’ ability to fulfil this aspect of their role. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: Findings illuminate the multiple challenges in translational research. SNRI appeared to reduce fall rates initially, but fidelity to the SNRI implementation and documentation was variable and fall reduction gains appeared lost 1 year later. Nurses expressed the importance of balancing intervention fidelity and individualizing patient interventions. (Source: Worldviews on Evidence-Based Nursing)

ABSTRACTBackground: The Patient Self Determination Act of 1990 mandates healthcare providers (HCP) to speak with patients about end‐of‐life preferences and advance directives (AD). HCP work with patients of varying cultures, and standard ADs do not address cultural differences. In order to understand various cultural beliefs, cultural sensitivity is important especially when discussing advance care planning (ACP).Aims: Individuals from differing ethnic backgrounds are likely to turn to their traditional norms of practice when ill or treatment choices must be made. An AD that addresses varying cultural values and beliefs was sought. Methods: A comprehensive review of the literature was conducted. Articles selected for review included qualitative and quantitative studies. The evidence was...

Conclusions: Smoking cessation interventions in later life are important, as older smokers generally have long‐term conditions caused or complicated by smoking. The delivery of brief smoking cessation interventions is known to be highly cost‐effective; however, research demonstrates that practitioners often fail to raise the issue of smoking cessation with older adults. This study has demonstrated the effectiveness of a 1‐day training course for practitioners. Further research is recommended. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: As prompt eye irrigation with tap water immediately after alkali burns had better outcomes, it would be important to commence eye irrigation immediately after burns are sustained. In this review, irrigating fluids including normal saline, lactated Ringer's, normal saline with sodium bicarbonate added, BSS Plus, and diphoterine solutions all yielded positive ocular outcomes suggesting for its use in hospital settings.
Preferred Information Sources for Clinical Decision Making: Critical Care Nurses’ Perceptions of Information Accessibility and Usefulness

Conclusions and Implications for Practice: The social exchange of clinical information may meet the needs of nurses working in a complex, time‐pressured environment but the extent of the evidence base for information passed through verbal communication is unclear. The perceived usefulness and accessibility of information is premised on the ease of use and access and thus the variability in information may be contributing to clinical uncertainty.
Conclusions: This study provides an evaluated clinical pathway for generalist community nurses to screen for mental health difficulties, make appropriate referrals as required and to support war veteran and war widow clients. However, the study also shows how research can be used to develop and evaluate practical, evidence‐based clinical pathways. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: Although there seems to be scholarly agreement that leadership is a vital part of the process of implementing EBP, more rigorous research is needed concerning the possible role of the leader. Our findings also indicate that leadership cannot be studied in isolation or without being clearly defined. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: The EXTRA program has been structured to reduce barriers and to enhance the facilitators found in the literature on the implementation of evidence‐based practices (EBP) in health care settings. Overall, nurse executives perceived that the benefits of participating in the EXTRA program were both individual and organizational. (Source: Worldviews on Evidence-Based Nursing)

An Exploration of the Roles of Nurse Managers in Evidence‐Based Practice Implementation

Conclusions: The study reveals that NMs are only involved in evidence‐based practice implementation in a passive role, not the full engagement described in the literature as being necessary. This study adds previously lacking detail of the roles of NMs. It elucidates why exhortations to NMs to become more involved in evidence‐based practice implementation are ineffective without action to address the problems identified.
Conclusions and Implications: Our findings highlight the importance of psychological distress in dealing with cancer‐related fatigue in addition to the need to be attentive to a patient's symptom distress. Of the symptom distress, nausea/vomiting should be prioritized by nurses when managing cancer‐related fatigue. This study provides sound empirical evidence that can be used to draft guidelines for the management of cancer‐related fatigue. (Source: Worldviews on Evidence-Based Nursing)

ABSTRACTBackground: Evidence‐based practice (EBP) models provide a framework to guide organizations and their clinicians to implement evidence‐based policies, protocols, and guidelines. A historical review of evidence‐based models is presented. The revised Colorado Patient‐Centered Interprofessional EBP Model supports use of research evidence and nonresearch evidence and adopts a patient‐centered approach to EBP.Aim: The purpose of this article is to present a framework that can be used to transform an organization and foster the use of evidence by interdisciplinary team members.Approach: An evidence‐based intervention to decrease catheter associated urinary tract infections (CAUTI) is presented to show how the model is operationalized. The EBP model is supported by the five st...

Conclusions: The findings confirm the complex process of implementation and compliance of CPGs for fall prevention. A relation between experiences of high incidence of falls with negative consequences and a positive attitude and compliance to CPGs appeared. To assure compliance and a positive attitude requires an obvious benefit of the CPG in reducing falls. Factors to overcome barriers to implementation and compliance seem to be a supportive leadership, systematic evaluations of the CPG outcome, and the facilitator role.
Background: The gap between what is stated in legislation about written individualized nursing care plans (ICP) and how they are used in practice is still too wide. Aim: To explore attitudes and experiences among nurses and managers in order to capture which factors and conditions impact on the successful implementation of individual care plans within hospital care, as well as the strategies that were adopted to ensure continued use. Method: The study was carried out through directed content analysis guided by the Promoting Action on Research Implementation in Health Services framework. Interviews were conducted with 15 informants on different operational and decision levels at a hospital that is well known in Sweden for its work in the ICP area. Findings: Important factors for implementat...

Conclusions: In conclusion, users were overall satisfied with the e-Volution-TREAT system, although expressed challenges related to workload that interfered with time to utilize the system. It would be premature to conclude the change in client outcomes was related to the e-Volution-TREAT system without a randomized controlled trial with outcomes compared to a control group. Future research needs to incorporate strategies for modifying the context and engage clinicians who are in a position of influence to model change.

Conclusions and Implications: Although the prevalence of nosocomial pressure ulcers varied considerably across wards, it did not relate to organizational culture, team climate, or preventive quality management at the ward level. These results would therefore not subscribe the widely suggested importance of these factors in improving health care. However, different designs and research methods (that go beyond the cross-sectional design) may be more informative in studying relations between such complex factors and outcomes in a more meaningful way.
Conclusions: The nurse participants in this study viewed influenza vaccination as a personal health choice, not as an evidence-based nursing intervention. As a result, the decision to decline influenza vaccination was made in the context of personal health choice and/or risk of injury or illness to the nurse. Patient safety outcomes were not expressed as a factor in making the decision to decline influenza vaccination. (Source: Worldviews on Evidence-Based Nursing)

This study tested the hypothesis: Customer-oriented perception is associated with increased organizational citizenship behaviors for nurses. If nursing personnel's customer-oriented perception can increase their willingness to display organizational citizenship behaviors, it may facilitate hospital operation and enhance organizational effectiveness. Methods: A cross-sectional design using a questionnaire survey of nurses in 10 medical centers was used. Five hundred copies of the questionnaire were distributed, and 232 effective copies were retrieved, with a valid response rate of 46.4%. Structural equation modeling was performed in SPSS 11.0 and Amos 7.0 (SPSS Inc., Chicago, IL, USA) statistical software packages. Results: The main finding was that favorable customer-oriented perception is...

Conclusions: Further understanding of what facilitators are actually doing to enable changes in nursing practice based on research findings will provide the groundwork for the design and evaluation of practical strategies for evidence-based practice in nursing. Research is needed to clarify how facilitation may be used to implement change in nursing practice along with evaluation of the effectiveness of various approaches.
Conclusions: Compliance with recommended lung protective parameters in the absence of standardized monitoring and intervention protocols is suboptimal. Preliminary evidence of positive relationships between monitoring and both severity of illness and therapeutic intervention was established. Control for nursing and physician practice variation is needed to rule out the influence of surveillance and performance bias on collaborative practice outcomes. Explicit standardized protocols that address the frequency of assessments and interventions along with therapeutic targets are recommended for collaborative practice guidelines.
Conclusions: Hierarchies of nursing practice, government policies and inter-professional agendas cause barriers to meaningful assessment. Informal and formal assessments and screening processes are often conflated, resulting in confusion regarding the scope and nature of the process. Differences between the rhetoric of placing the patient at the heart of the assessment process and practice have been identified. (Source: Worldviews on Evidence-Based Nursing)

Significance: Diets high in fruit and vegetable consumption are associated with a decrease in chronic diseases. Dietary factors are linked to 4 of the 10 leading noncommunicable causes of death: cardiovascular disease, some cancers, stroke, and type 2 diabetes. Accurately measuring dietary patterns has many challenges. Dietary intake measurement has traditionally relied on self-report instruments such as 24-hour recall, food record, and food frequency questionnaires to record consumption history. These methods have inherent limitations in detecting small but important changes in fruit and vegetable consumption patterns. Promising advances in technology have made more sophisticated techniques for recording dietary intake possible. Computers and Web-based programs, handheld personal digital ...

Conclusion: The use of a collaborative nursing network provides an innovative way to address the difficulty of implementing and sustaining EBP in organizations and also for maximizing resources and knowledge. Early evaluation data are promising. This approach is offered as one possible method of moving use of EBP in nursing forward. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: Students were able to complete the EBP activity using either the mobile device or PC and reported improvements in their knowledge and skills in relation to EBP and the appraisal of clinical guidelines. Findings suggest that for undergraduate health care students, rapid access to online evidence in the clinical environment is not necessarily essential for the integration of the EBP process into practice, or for the development of EBP knowledge and skills, provided there is easy access to such evidence at some point during the placement period.
Transformative Learning and Research Utilization in Nursing Practice: A Missing Link?

Conclusions: (1) Research utilization is a learning process that involves knowledge, skills, feelings, attitudes, and beliefs. (2) Transformative learning strategies of critical reflection and discourse can facilitate insight into experiences, finding shared meanings among groups of people, and understanding/validating beliefs, attitudes, and feelings so they can more consciously guide future actions. This dimension is frequently neglected in research utilization efforts. (3) In combination with research utilization theories, Transformation Theory may be a missing link to make research utilization initiatives more effective in rendering and sustaining nursing practice change, thus enhancing client care and well-being. (4) Research and further consideration are both warranted and needed. (S...

Conclusion: With an element of caution, this review concludes that the implementation of evidence-based practice using action research is a promising approach. Caution is needed because of the lack of detailed descriptions of implementation strategies, and their intensity and frequency prevents us from drawing firm conclusions. These are important considerations for any action researcher intending to implement EBP using this approach. (Source: Worldviews on Evidence-Based Nursing)

Conclusions: The evidence-based guidelines were accepted as a reliable source of advice in patient care in Finnish primary care. It seems that implementation interventions improve attitudes toward guidelines and enhance guideline use. These interventions might also be important from another point of view; they presumably improve familiarity with guidelines, which was also associated with guideline use.
Conclusions: Findings from this study will be significant for policy makers and funders of Australian nursing in general practice. The establishment of a career structure for general practice nurses that includes salaried positions for clinical nurse specialists would assist in the translation of evidence into knowledge for utilization at the point of care.
Aim: This case study describes a quality initiative to minimize restraint in an Australian residential aged care facility. Approach: The process of improving practice is examined with reference to the literature on implementation of research into practice and change management. The differences between planned and emergent approaches to change management are discussed. The concepts of resistance and attractors are explored in relation to our experiences of managing the change process in this initiative. The importance of the interpersonal interactions that were involved in facilitating the change process is highlighted. Implications: Recommendations are offered for dealing with change management processes in clinical environments, particularly the need to move beyond an individual mind-set ...

Discussion: The study results indicate relationships between RNs' reported perception of a learning organization and EBP beliefs, and between EBP beliefs and implementation. However, findings were mixed. Overall, nurses rated their organizations the lowest in the dimensions of "promote inquiry and dialogue" and "empower people toward a collective vision." Leaders have an opportunity to offer a more supportive infrastructure through improving their organization in these two areas. RN beliefs explained 23% of EBP implementation in this study with a residual 77% yet to be identified. Implications: Acute care hospitals were perceived mid-range on learning organization dimensions by RNs, indicating an opportune area for leaders to strengthen their organizational infrastructure.
This article reports on commissioned research funded by the Swedish Council of Technology Assessment in Health Care (SBU) and the Swedish Nursing Society (SSF). The objective was to review computer-based education programs. However, as the review produced insufficient evidence of effectiveness, the publication was withheld due to a previous incident where such evidence was misunderstood by Swedish policy and health care decision makers. Aims: This article highlights the concept of evidence with regard to the consequences of insufficient evidence of effectiveness being mistaken for evidence of no effectiveness. The aim is also to present a systematic review evaluating a computer-based education program for patients suffering from severe mental illness. Methods: Systematic database searches ...

A Review of the Efficacy of the Self-Management Model on Health Outcomes in Community-Residing Older Adults with Arthritis

Conclusions: While this review suggests small efficacy in the self-management program, with improvement in certain cognitive-behavioral markers for self-management, the functional gains that can be achieved in a growing older adult population translate into a larger effect overall.

Conclusions: The process used for the development and testing of the instrument was found to be effective. The NCI was deemed to be an effective measure of factors influenced by the implementation of person-centered nursing and would help in redressing a scarcity of quantitative evidence to examine the benefits of nurses working in a person-centered manner.
Background: Recent research has questioned the reliability and validity of the Nursing Work Index-Revised (NWI-R) instrument, raising the possibility that managers reconfiguring hospitals in line with the factors derived from the NWI-R may be misdirecting resources. Aim: To test the factor structure of the NWI-R. Approach: A population of 864 acute hospital nurses was surveyed using the NWI-R, and a response rate of 449 (52%) was achieved. The statistical factor model was examined using SPSS 11.5 and based on guidelines of successful factor analysis. Measures of internal consistency were calculated for each of the factors to emerge from the data. Findings: The findings confirm appropriateness of conducting a factor analysis on the NWI-R. A core three-factor structure emerged from the findi...

Background: It is well known that surgical site infections (SSIs) contribute significantly to the cost of nosocomial infections. Although many guidelines and recommendations exist, research has demonstrated that practices for preventing SSIs in operating theaters (OT) vary. Aim: The purpose of this study was to describe the current infection control practices for SSIs, formerly called surgical wound infections, in OTs, and compare these findings with evidence-based practice guidelines. Methods: In order to accomplish the purpose, a prospective, cross-sectional descriptive survey was conducted. Data were collected by interviewing the charge nurses in 24 OTs of 11 hospitals in the city of Izmir, Turkey. The results were tabulated and compared with an evidence-based guideline. Results: Data s...

Conclusions: Despite promising findings, primarily on cognitive and motor development, the scientific evidence on the effects of NIDCAP is limited. Shortcomings in design and methods in the reviewed studies hamper far-reaching claims on the effectiveness of the method. Scientific grounds for assessing the effects of NIDCAP would be substantially enhanced by a sufficiently comprehensive study with extended follow-up and a clear focus on a few important outcome variables.
Background: Various measures are used to summarize a diagnostic test's efficacy in deciding on the disease status of patients. Measures: Sensitivity and specificity indicate the probability of a positive or negative test result, given that the patient either has or does not have, respectively, a specified disease, and are the most commonly reported measures of test efficacy. Predictive values indicate the probability of a disease, given the patient's diagnostic test result is positive or negative, and are more useful in clinical decision making. Predictive values are useful only if the prevalence of disease in the study sample is representative of the prevalence in the population of diagnostic interest. A likelihood ratio is the sensitivity of a test divided by one minus the specificity an...

In this study, researchers investigated the extent to which textbooks were evidence based regarding preventing pressure ulcers and knowing what recommendations to make for nursing students and publishers of nursing textbooks. Method: Educators at nursing schools in Germany were contacted by telephone to identify the most often used nursing textbooks. The recommendations of the German Expert Standard of Pressure Ulcer Prevention were compared with the content of the textbooks in a content analysis. Additionally, teachers were asked what additional material they were using to help prepare lectures. Results: Only one of the four analysed textbooks complied with the recommendations of the German Expert Standard. Contents of the other books were incomplete. The authors of some books did not men...

Conclusions: The findings of the M-A were not deemed credible because of deficient information regarding the target population and outcome variable, lack of confounder control, unclear inclusion and exclusion criteria of the sample, and invalid methods of data abstraction. Because the findings were not credible, they were deemed not to be clinically applicable. Therefore, a high soy diet would not necessarily be recommended.
The Evidence-Based Practice Beliefs and Implementation Scales: Psychometric Properties of Two New Instruments

Conclusion: In this study, initial evidence was provided to support the reliability and validity of the EBP Beliefs and Implementation Scales in a heterogeneous sample of practicing nurses. Evidence to Action: Use of the scales in future research could generate evidence to guide EBP implementation strategies in practice and education. Results could establish the extent to which EBP is being implemented and its effect on clinician satisfaction and patient outcomes. (Source: Worldviews on Evidence-Based Nursing)

Registered Nurses' Adherence to Clinical Guidelines Regarding Peripheral Venous Catheters: A Structured Observational Study

Conclusions: Nurses partly adhered to clinical guidelines. Differences in guideline adherence were observed for wards with local or national guidelines, as well as for wards with different specialities. In accordance with clinical guidelines, the nurses seemed to replace or remove peripheral venous catheters before any severe complications arose. Implications for Practice, Research, and Management: Adherence to clinical guidelines has an impact for preventing patient complications and thus it is essential that nurses are aware of their existence. Feedback and discussions of guideline adherence or of complication rates might influence nurses' clinical decision-making. (Source: Worldviews on Evidence-Based Nursing)
