Many studies have shown that the number of new dementia diagnoses in Germany is increasing yearly. Thus, two social tasks are important: the adequate support and care of dementia patients, now and in the future, as well as covering the costs thereof. The survival period of dementia patients has a central meaning - especially for health policy planning. Therefore, the question of our 8-year follow-up study was whether living conditions affect the survival period of dementia patients? A total of 173 dementia outpatients (ICD-10 numbers F00 and F01) were screened for survival time and living conditions. For deceased patients, a close reference person was interviewed, and the exact date of death was recorded. For...

Beneficiaries from the German long-term care insurance living at home have the option between cash payments and care-in-kind services. For those who opt for cash payments, there is an obligation to receive expert advice and 'counseling visits' by a professional nursing service at least twice a year. Although more than 2 million of these 'counseling visits' are conducted each year in Germany, hardly anything is known about these visits and whether they are useful for care recipients and their family caregivers. The purpose of this project was to gain insight into the 'counseling visits' in order to develop a useful approach for the performance of these visits. Data collection took place in two regions of Nordrhein-Westfalen (North Rh...

Authors: Dienel HL, von Blanckenburg C
Demographic changes in the former East Germany are starting earlier and are more dramatic, and the serious financial difficulties of former East German municipalities are forcing a radical modernization of institutions for public services in the direction of a civil community. Municipalities profit, if citizen initiatives can partially make up for the decline of standards in public services. This saves community resources. An especially important group supporting community engagement is the group of"young old." Community engagement needs publically supported structures. A municipality that would like to activate the young old to support public services, which means to improve the quality of life in the community, cannot simply shift the services p...

Despite the large number of studies dealing with dementia caregivers in Europe, a valid German version of the most widely used measurement of caregiver burden, the Zarit Burden Interview, has not been published. The purpose of this study was to evaluate the psychometric properties of the German Zarit Burden Interview (G-ZBI). A sample of community-dwelling older couples (n=37) with the husband suffering from dementia and the wife being the primary caregiver participated in this study. The G-ZBI and related constructs were assessed in order to test for reliability and construct validity. The G-ZBI revealed psychometric properties comparable with those of the original instrument and empirically validated translations. Results indicate high ...

CONCLUSION: The DiNA proved to be a valuable instrument to assess the specific priorities of elderly patients suffering from dizziness. Regarding the limited therapy options for dizziness in old age, a doctor-guided shift of patients' attention from causes to symptom-related implications could be a promising approach.

In order to evaluate the perceived living conditions, i.e., satisfaction with the scope and quality of municipal infrastructure, from a senior citizen's perspective, a questionnaire, differentiating six areas and 39 sub-areas and the corresponding testing scales for satisfaction with the quality of life, was developed analogous to the grading system used in the German school system. This questionnaire is suitable for evaluative city walks and comparative analyses of the municipal infrastructure to gain indications of the quality and to identify or shape future infrastructure actions. In 2007/2008, the questionnaire was used by the local senior citizen's representatives in the federal states of Germany. The data obtained were condensed into indices of citizen sati...

CONCLUSION: The majority of clinical trials investigating contracture as outcome parameter do not provide a clear definition. However, the success of contracture prevention and treatment in clinical trials and practice can only be judged in the presence of a clear definition. Thus, it appears especially difficult to use contracture as a quality indicator for nursing homes.

Authors: Falkenberg I
Humor and laughter are fundamental elements of human communication throughout the lifespan. The understanding of humor, humor production (e. g. joking) and functionality of humor (e. g. as a coping strategy) evolve in the course of a lifetime and are essentially determined by cognitive, verbal and social abilities. This review outlines the landmarks in the development of humor and laughter from early childhood to older age. In all stages of life, humor fulfils important functions; in the elderly, however, the function of humor as a coping mechanism gains in importance. Yet, humor processing, such as understanding of humor, humor production and emotional reactions to humor in the elderly, is an underresearched area.

This article presents the field of activity of a clown doctor in geriatric settings as well as an overview of current scientific research on the topic and the effects on persons with dementia and nursing staff. It will be become clear that the clown doctor is a supporting therapeutic intervention. Through the clown representation, it is possible to obtain access to and interact with a person with dementia, thus, maintaining social contact of the patient with his/her environment. This effect leads to an increase of well-being and contributes to a reduction of problematic behavior. In addition to reduced workload and relief for the nursing staff, it has a positive effect on the working atmosphere.

This paper reviews recent literature on gelotophobia (i.e., the fear of being laughed at) with an emphasis on age-specific aspects. Research with two instruments, the GELOPH and PhoPhiKat questionnaires, is presented with special attention being given to sociodemographic correlates and differences in intelligence, character strengths, personality, emotion, and humor. Quite consistently gelotophobes tend to misread positively motivated smiling and laughter (e.g. in social interactions, photographs or auditorily presented) and have lower values in many, but not all, components of humor. They have a low propensity to joy and a disposition to experience shame and fear. More generally they tend to describe themselves as being introverted and neurotic, and...

This article provides an overview on the structures involved and findings concerning the neuropsychological faculties necessary for joke comprehension, e.g., working memory and mental flexibility. On the other hand, there are also many aspects of humor, like joke production, that are not neuropsychologically well studied; an overview of current knowledge will be given. In addition, there is little research on the effect of aging on the different aspects of cerebral humor processing.

Empirical studies on humor among the elderly are lacking. Based on the model of sense of humor by Paul McGhee, different age groups are compared (n=979) in the present study. Data suggest that playfulness and sense of humor are stable across different age groups (in a cross-sectional design) but that elderly participants were highest in positive mood. However, they also indicated that they laughed less and less easily than younger participants. Among the elderly, those who frequently (more than once a week) meet with friends and colleagues yielded higher expressions in different aspects of the sense of ...

Positive psychology is a scientific approach within psychology that focuses on research on what is best in people. Within this framework, humor is understood as a character strength. The article gives a brief overview on positive psychology and implications for aging (positive aging, successful aging) with an emphasis on the contribution of humor. Humor as a strength of character and its measurement by various approaches are discussed. It is argued that there is a lack of empirical data about humor in the elderly. Potential benefits of considering humor in research but also in practice are discussed.

This contribution refers to the 2009 Council of Experts Report on health system development and discusses the special care requirements of old people. This includes the geriatric phenomenon of multimorbidity, polypharmacy in old people with multiple diseases, and the need for care. The probability and complexity of multiple diseases increase with age. About half the German citizens who are over 65 have three or more relevant chronic diseases. Multimorbity often causes elderly people to concomitantly consume many different pharmaceuticals. Twenty percent of the insurance holders aged 70-99 who were prescribed drugs in 2005 received 13 or more pharmaceutical agents. However, multimorbidity also has a negative effect on the quality of life, subjective state of health an...

The aim of this article is to provide an overview of the current state of research concerning health inequalities among the aged. First, health inequalities are introduced as a general phenomenon. Then, the magnitude of inequalities in elderly populations is described for different health indicators. In particular, analyses of the association between education and depressive symptoms based on a European dataset are presented. Subsequently, different hypotheses on age differences in health inequalities and possible explanations for social inequalities in health in elderly populations are discussed. Finally, some consequences for health policy, which can be derived from current research, are outlined.

This contribution deals with the question of how existing health care systems can be sufficiently and adequately adapted to ageing patients in general, and to patients with special needs for nursing care and assistance. Simultaneously, it advocates explicit consideration of the dimensions of social inequality and health hazards related to biographical factors. It demonstrates that health care involves much more than just adequate medical care in the event of illness. The reasons that change is needed are given and reform options for relevant segments of health care (such as medical practitioners, hospital care, health care for patients in need of nursing care, palliative care, prevention and rehabilitation) are presented. Special emphasis is placed on standards and g...

The present investigation aimed to assess the health-related QoL (HRQoL) of elderly people living in two settings: (i) rural community and (ii) homes for the elderly in a district of South India. The data are drawn from elderly (>60 years of age) sampled from both settings. The short form 36-item health survey (SF-36) was administered to all respondents. The average scores for several domains, including total physical health, total mental health and overall health (total SF-36 score) were around 50, which can be interpreted as a moderate level of health-related QoL. Residents living in a home for the elderly scored better in all domains except for role-physical and role-emotional. Though univariate analysis revealed some associations between cha...

CONCLUSION: Underweight at the time of death is observed more often in the very old. This is associated with the need for professional nursing care and also the occurrence of pressure sores. Less than 5% of persons with tumors were included. The results underscore the need of early detection of people at risk of malnutrition when growing older.

Since the mid-1990s, many German companies have used the part-time retirement scheme (Altersteilzeit) as an opportunity to release older workers. This was often consistent with the interests of the workers themselves. This paper explores whether workers who participate in this early retirement scheme would like to continue to work. A survey of all the workers in one company who participated in the part-time retirement scheme revealed that those already retired would more often like to continue working than employees who are still awaiting retirement. To a greater extent, early retirees would also like to return to work with shorter working hours or for a time-limited project. Results are explained with the affective fo...

Medical arbitration boards ("Schlichtungsstellen", expert panels for extrajudicial malpractice claim resolution) try to settle claims of suspected malpractice between patients and their physicians and to avoid court trials. Numerous studies found an increasing incidence of adverse events with rising age. Injuries that occur in the hospital are frequently beyond the specialty of the treating physician. Therefore, the physician has to broaden his diagnostic view beyond the borders of his own specialty to recognize injuries in his patients and to prevent malpractice claims. In this paper, we report on adverse events in elderly patients where the geriatrician/internist was accused of negligence for not having promptly recogni...

Sarcopenia is linked to an increased risk of morbidity and mortality in the aging. Whole body vibration (WBV) exercises are currently discussed as a "gentle" alternative to conventional exercises to improve muscle mass. The present study scrutinized whether a multipurpose (exercise) training program using WBV can improve muscle mass and neuromuscular capacity, while lowering fall risk. A total of 151 postmenopausal women were randomized into three groups: exercise group (TG), exercise group with vibration (VTG), and fitness control group (CG). The TG group participated in an exercise program including leg strengthening training twice a week over 12 months, while the VTG carried out an identical program wi...

Using empirical evidence, this paper provides conceptual approaches to prevention and health promotion in long-term care - an area that has gained only limited attention in German discussions on prevention. First, a short analysis of long-term care in Germany is provided; existing problems but also resources with regard to prevention and health promotion in order to identify starting points for the development of interventions are presented. The existing pressure and problem accumulation in long-term care that calls for action on maintaining and promoting the health of long-term care professionals as well as long-term care users are illustrated. Considerations on programs for health promotion in long-term care focus on the users, in particular the o...

PURPOSE: The purpose of the study was to develop and validate the depression in old age scale (DIA-S). The DIA-S is a new screening scale for depression, which is comparable to the GDS-15 in terms of practicality but which is more appropriate for the use in geriatric patients. METHODS: The DIA-S and the GDS-15 were validated among a sample of n=331 geriatric inpatients (n=151 depressed subjects and n=180 not depressed subjects), using the Montgomery and Asperg Depression Rating Scale as gold standard criterion for depression. For the comparison of the two scales, item analysis (missing values, difficulty, discriminatory power) was considered as well as test performance (internal consistency, correlation with the MADRS, ROC curves, sensitivity, specificity)...

Social relationships are an important factor for maintaining life satisfaction in elderly people. Little is known, however, about the influence of increased social support towards the well-being of cognitively impaired people. This is why two comparative studies in Switzerland and Austria, with 84 cognitively impaired individuals (aged between 65 and 98) with in-patient treatment, used a control group design to examine whether increased social support would have a positive effect on the well-being of these individuals. To this end, interventions in the form of emotional social support through volunteer visitors for dementia patients were carried out. In both studies, a positive association between increased social support by volunteer assistan...

The prevalence of peripheral arterial occlusive disease (PAOD) is high in elderly patients and its clinical manifestation is often atypical. Comorbidity and morbidity as a consequence of PAOD are significant. Therefore, standardized primary diagnostics are required among geriatric patients. Drawn from a search of the literature, evidence-based recommendations are provided. Pulse palpation and the evaluation of a patient's medical history are obligatory components of primary diagnostics, even in the absence of typical symptoms. In the case of pathological and ambiguous findings, measuring Doppler ankle pressures is suggested as the next diagnostic step. Further measures depend on the following factors: the presen...

For many years, Germany has been regarded in international comparisons as an example of a generous early retirement culture, resulting in a low labor market participation of older employees. Recently, however, employment rates of older employees have increased remarkably. Reasons are the demographic structure of older persons in Germany, a long-term trend of increasing female labor market participation, and reforms in labor-market policies and pension policies during the last 10 years. Despite an increasing labor market participation of older employees, traditional labor market risks for older persons partly remained, but some new risks evolved as well. Therefore, social differentiation among older employees increased.Although detailed macro descriptions exist, the c...

The article provides a short overview on cognitive strengths and weaknesses of older workers. The older people who are under the age of 65 years already exhibit changes in performance and neurophysiological measures in laboratory tasks that test fluid intelligence. Apart from deficits, the physiological data of the older people show clear evidence for compensatory strategies. Cognitive performance on the current job is usually less impaired. This can be explained by practice, learning, and selection, which are, however, often linked with higher effort. The cognitive competence of older employees varies exorbitantly, which is due to the influence of various factors like education, lifestyle, and in particular the type of work. Thus, the co...

During recent years, a discussion on work activity of retirees has commenced also in Germany. First, this contribution presents a survey of the North American phenomenon of bridge employment and, second, complements leadoff empirical results on individual motives and favored basic conditions in Germany. Individual and structural preconditions for work activity during retirement are presented as well as possible manifestations of retirement activities. This paper quantifies the extent of work activity of retirees in Germany into their upper seventies. Potential follow-up actions for the three levels individuals, organizations, as well as politics and society are recommended.

CONCLUSION: Six parameters proved to have excellent reliability and are recommended to be used in further investigations. Narrow stand with eyes closed should be used as the test position. The tested protocol proved to be reliable. Whether these parameters can be used to predict falls in elderly persons remains to be investigated.

This report presents age-, education- and gender-specific reference values for the subtests verbal fluency, word list memory, word list recall and word list recognition as well as the word list savings score of the CERAD-NP battery. The study sample consists of 2891 general practitioners' patients from Germany aged 75 years and older. The study participants had a mean age of 80.2 years (SD=3.6); thus, this report provides reliable reference values for the neuropsychological diagnosis of dementia in older age groups.

Spatial disorientation is among the first manifestations of dementia and a prime reason for institutionalization. However, the autonomy of residents and their quality of live are strongly linked with their ability to reach certain places within the nursing home. Also affected is the efficiency of the institutions and the quality of care provided. The physical environment has a great potential for supporting resident's residual wayfinding abilities. Until now little systematic research has been carried out to identify supportive architectural characteristics. For this exploratory study, extensive data on resident's spatial capabilities were collected in 30 German nursing homes. The architectural structure of the buildings was also analyzed. Within the nursi...

In the present study, 188 first year and 120 sixth year students of the University of Zurich were questioned about their attitudes towards older people, their knowledge concerning aging specific developments, their experiences with older people and their own expectations concerning old age. Structured and standardized questionnaires were used. The data were analyzed using univariate and multivariate statistical methods. The results show a positive image of old age independent of gender and point in time of education. Their positive experiences with older people and their positive expectations concern their own aging refer, above all, to their own mental health.
This study could not clarify in 10.9% (n=111) of all cases whether dementia was present in a patient at the time studied. It was mainly limited due to multiple concomitant morbidities and reduced patient communication skills. We recommend that structured diagnostic procedures and treatments should be performed at specialized centers in order to avoid missing any treatable underlying diseases. In addition, the patients should be followed up at regular intervals.

The new Retirement Age Adjustment Act, enacted by the Federal Government in spring 2007 to raise the statutory retirement age to 67 years in Germany, poses a challenge for the jobholders concerned as well as for the companies. Especially older female jobholders are exposed to particular risks regarding their ability to continue working due to the cumulative concurrence of age-specific but also additional gender-specific employment patterns and risks. Their employment situation is influenced by age discrimination in terms of recruitment and human resource management. Furthermore, women were and are confronted with specific obstacles in the course of their working lives, such as, the problem of reconciling work and family life and care giving, lesser...

In this contribution the results of a descriptive explorative study are presented, in which the significance of Pre-Therapy techniques for dementia care is examined. The approach was developed by Garry Prouty. In the project the approach was taught by the author to 111 geriatric care staff members and subsequently tried in geriatric care everyday life. The article first presents the theoretical concept of Pre-Therapy as developed from the person-centred approach (Rogers). The experience of the geriatric care staff and the results of transfer to practice are systematically described. Ten everyday examples of dementia care show the practical procedure with this method and demonstrate the possible applications. The result of the project is that Pre-Therapy techn...

Conclusion The treatment of dysphagia in the elderly requires a multi-professional setting, differentiated assessment and functional training of oral motor skills and sensation and swallowing techniques. Compared to patients with severe dysphagia, however, patients with mild to moderate dysphagia benefit most from functional training. The results reveal the importance of precise and early diagnosis of swallowing disorders in order to introduce PEG supply early in cases of persistent dysphagia.

As part of a research project, a behavior therapy-oriented cognitive training program, designed to improve the cognitive ability of middle-aged employees (50 to 59 years), was implemented and evaluated. The goal of the training program was to analyze the cognitive, affective and behavior-related conditions of memory deficits in the workplace. The training focused on two topics: (1) development of new job tasks, (2) lack of time and deadline constraints. A total of 316 patients in the psychosomatic clinic of Bad Neustadt participated in the study. Those who showed cognitive impairments took part in the cognitive training program during the intervention phase. During the control phase, patients with cognitive impairment...

CONCLUSION: Significant differences between fallers and non-fallers among geriatric in-patients can be detected for several assessment subscores as well as parameters recorded by simple accelerometric measurements during a common mobility test. Existing geriatric assessment data may be used for falls prediction on a regular basis. Adding sensory data improves the specificity of our test markedly.

Quality of life is a multidimensional construct commonly used in geriatric health care. The patient's view is increasingly incorporated in its determination. Pain and satisfaction with life are two important subjective facets of quality of life. Potentials and limits of an integration of the subjective view in seemingly heavily disadvantaged patient populations are discussed using three approaches: 1) Self-reports on pain in aphasic patients can often be obtained by means of nonverbal communication or verbal communication adapted to the disorder. 2) Even in people suffering from dementia, pain can be assessed in self-reports into the middle stages of the disease. In severe dementia, observational methods have been developed, but their si...

In higher age, various diseases are more frequently present and occur in combination, with diabetes mellitus representing the comorbidity seen most frequently. Within the framework of a retrospective study, it has been evaluated whether the degree of metabolic compensation has an influence on the activities of daily life in geriatric patients after an acute event and following rehabilitation treatment. For this purpose over a period of 27 months (01/02/2000 - 03/31/2002), diabetic patients of a Saxonian geriatric center (n = 644, mean age 77.86 years/SD 7.115, 30.3% males) were examined before, directly after, and 2 - 3 years after rehabilitation. Independent of age, rehabilitation resulted in a significant increase in activities of daily living (AD...

CONCLUSION: These findings support the notion that measurement of the corpus callosum and cortical grey matter atrophy may identify the underlying causes of cortical disconnection in AD and VD and may be helpful to differentiate between both conditions.

Considering the incidence of dementia, the development of procedures that allow correct differential diagnosis is gaining increasing importance. The analysis of spinal fluid from 632 patients, who were admitted with suspected dementia, diagnosed dementia or dementia of unclear etiology, showed that there was high differential diagnostic power for tau protein, but not for beta-amyloid.

Delirium is a common, serious medical and often life-threatening condition in elderly in-patients. Delirium can develop primarily or secondarily related to other medical conditions and lead to hospital admission. The pathogenesis is still not fully known and is usually addressed as multifactorial. Alterations in neurotransmitters have a key role in this process. The incidence varies by setting up to 90%. Delirium is associated with increased short- and long-term mortality, iatrogenic complications, functional decline, and future development of cognitive impairment or dementia. Delirium is also associated with longer hospital stays, higher hospital and total health system costs, and an increasing rate of nursing home admissions. A structured diagnostic and therapeutic p...

All states of confusion with acute onset and alteration of thinking, perception and awareness are defined as "delirium". Delirium is a common problem in older patients admitted to the hospital. It is combined with a negative prognosis and complications (falls, infections, etc.). Diagnosis and management need special efforts. Delirium is mostly associated with an underlying disease. In the elderly patient, nearly every disease can be accompanied by delirium, and the fragile older patient is especially affected.Delirium should be evaluated using a systematic approach according to frequency and impact of possible causes. An accurate diagnosis can be challenging. Symptoms have to be distinguished from other cerebral alterations, such as preexisting dementia or depression. D...

The treatment and care of individuals with dementia, who are admitted to general hospitals due to somatic diseases, often leads to crisis situations which not only compromise the therapeutic benefit, but also can be followed by avoidable institutionalization in nursing homes or psychiatric hospitals. With the model project "Gerontopsychiatric Consultation-Liaison Service" in Kaufbeuren a comprehensive solution to this problem has been developed. A multiprofessional team consisting of a medical specialist, nurses specialized in psychiatry and an occupational therapist supports the health personnel by giving assistance and advice. It also provides contact persons for patients and their families. The effectiveness analysis proved that the work of this service has...

Authors: Gurlit S, Möllmann M
Due to increasing life expectancy, the number of elderly patients needing surgical care is increasing. Improvements in surgical techniques and anesthetic procedures offer the opportunity of surgical intervention even in frail patients. Delirium on admission to the hospital or in the perioperative setting is a common and often serious complication. Cognitive impairment is regarded as the main risk factor for delirium; unfortunately, under routine clinical conditions, these deficits often remain undetected. Postoperative delirium is associated with increased morbidity and mortality, as well as increased length of hospitalization, resulting in increased suffering and costs. The aim of the intervention presented here was to prevent delirium in a general h...

This paper describes the development and management of a new model of care for hospitalized patients with challenging behaviour evoked by dementia and/or delirium. To ameliorate care for patients with dementia in a geriatric acute care hospital a segregated Special Care Unit for patients with challenging behaviour was created. Environmental features allow for safe and unrestricted ambulation within the unit and create a home-like atmosphere. Day-time activities structure the day and assure additional professional presence in the unit. An intensive training program for the staff was provided. The SCU has been well accepted by the staff and is considered to be an...

Authors: Huxhold O, Schäfer S, Lindenberger U
Research on crossdomain couplings between sensorimotor and cognitive functions in older adults has gained momentum during recent years. Results of most studies point to increasing interdependencies between the two functional domains with advancing adult age. The causes of this increase are complex and show dynamic interactions across adult development. This review integrates experimental, correlational, and neurophysiological evidence, with an eye on implications for maintaining an independent and mobile lifestyle in old age.

CONCLUSION : Nursing home residents are commonly affected by continence problems which are associated with functional dependency and relevant co-morbidity. The public and expert debate on continence problems of nursing home residents has to be promoted to achieve long-term changes.

Sporting activity is proven to decline with advancing age. But do older adults replace sporting activity with physical activity like walking and cycling for transportation purposes, and are people with a history of sporting activity more likely to integrate physical activity in their day-today lives? There is a proven negative correlation between sporting activity and overweight, a major risk factor for numerous diseases. The purpose of this study was to investigate correlations between walking/cycling for transportation and sporting activity/overweight, while controlling other variables....

One of the main reasons for deficits in the care of patients with dementia is the taboo on the disease and the stigma of the people suffering from the disease, also from the side of the professional caregivers. The aim of the project was to find out whether a continuing education program for GPs and for nurses in ambulatory care was able to increase the skills of professionals to better communicate with patients and relatives on the taboo topic of dementia. The program was developed with a multi-professional team and tested with 53 GPs and 54 nurses in ambulatory care. The evaluation was performed with a 39-item questionnaire administered before and after the training. Also, to test the stability of the outcomes,...

The loss of muscle mass, muscle strength and muscle endurance-capability in the elderly is summarized under the term 'sarcopenia'. This phenomenon is widespread in the older population and is a large financial burden for the health system. As a consequence of sarcopenia, functional and metabolic consequences occur. These among other things are associated with a loss of the independent lifestyle and the appearance of various age-related chronic diseases. An intervention with whole body vibration training can increase muscle strength, especially in older people with a low level of muscle strength, similar to resistance training. A strength increase is mainly attributed to improved inter- and intramuscular coordination. A muscle hypertrophy is also possibly realizable ...

Communication is a key competence for medical and nursing health care providers alike. However, there appear to be areas of "speechlessness" regarding specific medical problems, including the "taboo" disease incontinence. There is a lack of scientific data regarding incontinence in the context of communication among patients and health care professionals. The descriptive study was designed to provide insights how female and male patients alike perceive communications about incontinence with doctors and nurses, respectively. 22 structured interviews were conducted and taped with 16 elderly female and 6 male patients (age >/= 60 years, mean: 81 years, demented patients excluded, n = 19 analysable), and five interviews each with doctors and nur...

The aim of this study was to describe the aspects of psychological and physical burden caused by incontinence. The study involved 112 women and men with urinary and/or fecal incontinence aged between 59 and 93 years. The subjective experience of burden was measured in a qualitative manner by means of a semi-structured interview. The semi-structured interviews were evaluated by applying content analysis. In the group of 59- to 93-year-old incontinent women and men, mental stress factors were predominant. Social factors and factors of the behavioural level ranked second and third, respectively. Physical and economic stress factors played only a minor role. Study results are discussed in relation to their implications for theory and practice. Findings result in r...

CONCLUSIONS : A conservative and multidisciplinary management of UI improved the impact of UI on daily life among comorbid geriatric patients. The Catalan ICIQ-SF was successfully implemented. Outpatient geriatric incontinence units seem to be effective and should be implemented to improve detection and management of this important geriatric syndrome.

The role of urinary incontinence (UI) in the disablement process model has been mainly defined according to its impact on quality of life, global wellbeing, life satisfaction, institutionalization and death, which are global outcomes of disability. Recent research focused on the "active" role of UI in the main pathway of the disablement process model, i.e. actively involved in the causes of disability. The aim of this paper is to review the complex current scientific evidence on this second active role and to define the implications for further research and for clinical work in geriatrics. The relationship between UI and disability can be classified in the following five pathways:1) UI as risk factor for functional decline and reduced ph...

Urinary incontinence can be expected to present an issue in domestic care with various conditions and ways of dealing with the situation. This pilot study investigates the kinds of coping strategies developed by family caregivers of urinary incontinent persons. Therefore, it seeks to explore how family caregivers experience and shape everyday life with an incontinent family member.Based on a qualitative design, problem-centered interviews were carried out with 10 family caregivers (4 men, 6 women) and analyzed according to Grounded Theory. The results show that family caregivers place major emphasis on maintaining continence, which is conceived as an expression of concern for the care-dependent person. The data facilitated the development of four major conce...

Stereotypes of ageing affect activity in older adults. However, no representative studies on the impact of "images of ageing" on sporting activity existed until now. In a telephone survey, 2002 over-50s were asked about sporting activity and their opinions about ageing. A factor analysis of the queried attitudes disclosed five dimensions of ageing stereotypes. A subsequent cluster analysis identified three categories of attitudes towards ageing: "fit and performance-hungry", "conventional", and "easy-going and committed". Physical activity was described as the key criterion for a healthy old age in all clusters. Analysis also revealed a significant correlation between cluster affiliation and amount of sporting activity. The percentage of nonexercis...

We investigated the influence of repressive coping, depression, cognition, education and age on geriatric patients' reports on health-related status in 80 geriatric patients with a history of injurious falls. For patient reports, subjective statements on activity avoidance, perception of terminal decline, falls, and fear of falling were assessed. Co-morbidity and number of medications were documented based on patient charts.Repressive coping was significantly associated with underreporting in geriatric patients in all items documented and predicted most variables of patients' reports. Because of underreporting significant health problems geriatric patients with repressive coping may therefore be at risk fo...

The literature provides conflicting results on the effectiveness of physical training in cognitively impaired older individuals. Cognitive impairment has been shown to be a negative predictor of rehabilitation outcome in these persons. However, the evidence on which this discussion is based is scarce. The methodology used in previous studies shows substantial shortcomings. The presented study protocol documents the methodology of one of the largest intervention studies worldwide in this research field with a standardized specific training program in cognitively impaired subjects including short- and long-term follow-up examinations. BACKGROUND : The early assessment of prevalent disability and functional decline is of increasing importance for prevention issues and for the evaluation of change in clinical studies. The assessment of common daily tasks and resulting disability is essential for a complete impression of a patient's capabilities. There are plenty of instruments that cover basic and instrumental activities of daily living by assessing the person's dependence at a specific functional task. However, the early stages of everyday life limitations and functional decline are usually not being captured. In this article we have examined psychometric properties of the German adaptation of the abbreviated "late life function and disability instr...

The body is more than a biological basic condition; it is provided with social meanings and used for constructing, defining and describing age(ing) in different contexts. The social significance of the body can, for example, be seen by the fact that it is a bearer of age attributes, serves as a display surface and a knowledge bearer for the "normal" and "natural" ageing process. Formal and informal chronological age limits are used to describe, divide etc. ageing, without expatiating that certain interpretations of physical attributes are the basis of this construction. In the scientific discourse and in everyday understanding, dichotomous views on the ageing body "old means ill" and "successfully old means active" prevail. After a social constructivism and phenome...

Perspectives on the ageing body as a social construct and in its function as constructing the social are hitherto less developed in (social) gerontology. Nevertheless, "age(ing) and body" contains much more than biological changes and losses. In the body as a social medium and result of actions, we can understand the classical question connecting social structures and individual subjectivity in its social-structural and individual differentiation. The interrelationship between the body and other dimensions of the life situation is more than health; it also concerns employment, social contacts and material situation. The social construction of reality can be examined through the body especially in terms of the special way of function in the modern society (e.g. as expe...

The article is concerned with the question to what extent does the body - the body itself as an object (Körper; body) and body that we live in and experience (Leib; self) - have an self-promoting influence on the process of aging. To solve this question, four physical categories are presented that are generally of substantial importance for the development and support of one's personal identity: physical biography, physical image, physical limiting experience and physical control. These categories are consequently discussed with their relationship to aging. Thereby, the extent and the diversity of physical alterations during the course of the life of an individual appears to make alterations with one's inner self mandatory.

According to the special view of natural sciences, ageing processes are connected with measurable changes in the body. At the same time we know little about how bodily change is experienced and the subjective acceptance of the body during ageing. Therefore a perspective with respect to the body has to be systematically embraced in gerontology. Knowledge perspectives and the view of the body are exemplified in theory and by analysing a case. The knowledge of experience and sense of body and motion in a person's life allows the creation of stimulating offers of growth development and health in age.

The main focus of this article is the meaning of particular experiences of the ageing body for identity and development with respect to very old age. This experience of embodiment results from a dynamic development between unpleasant experiences of physical ageing, positive bodily experiences of the pleasure of living, and reflections on this direct engagement with the ageing body. The interaction of those three dimensions requires a constant rebalancing, which, in view of the frail body, becomes increasingly demanding. Beyond one's own experiences with ageing through direct engagement with the body, established social relationships give room for an individual's physical nature and support the maintenance of identity and development. People who look after and ca...

Considering lifespan developmental psychology, engagement in sport can be seen as a dynamic process of phases of participation and inactivity that varies from individual to individual. Depending on the individual biography, specific categories and patterns of sport participation evolve over a lifetime. Changes of sport participation rates over the last 30 years are discussed on the basis of general social changes. Six patterns of reasons will be discussed. The focus is set on a heuristic model as a base to explain the individual changes of behavior in sports over the lifespan. This model depicts engagement in sports as a process of dynamic changes (aspect of process and time) influenced by specific determinants in the different phases of changes (aspect of determina...

Since the 1990s, sport and ageing is increasingly in the focus of psychological research. Besides motivational aspects the main attention lies on the positive influence of sporting activity on physical and psychological health. Thus, ageing and sports is mostly considered under functional aspects. The aim to compensate deficits through sports in the elderly reflects the socially widespread pictures of age (frailness) and sport (capability, youthfulness). In this article the deficit model with the associated clichés is questioned on the background of the latest research. The author argues for a more differentiated and constructive view on sport and ageing and to take into consideration the specific needs of older people.

Physiological and pathologic changes of various body functions have a great influence on the cognitive performance of a human being in the course of aging. Knowledge-based neuropsychological abilities do not change much during the physiological aging process. However, the decreasing capacity of the working and sensory memory lead to lesser attention and learning performance in the elderly, compared to younger population groups. Pathological mental function changes correlate with the existence of certain physical diseases, the most important of which show an increasing incidence with old age. Degenerative diseases especially Alzheimer's disease and other demential syndromes as well as vascular diseases of the central nervous system, tumors, inflammation, epilepsy, tr...

This paper shows that the measurement of hand grip strength provides a non-invasive and reliable objective health indicator for social science research and is easy to collect in general population surveys. Grip strength is not only a useful complement of self-reported indicators of health, but it also exhibits a considerable predictive power with regard to a number of further relevant variables for social gerontological research, such as mortality risks. New data from the 2004 Survey of Health, Ageing and Retirement in Europe (SHARE) and the 2006 wave of the German Socio-Economic Panel Study (SOEP) allow insightful methodological and very first substantive cross-sectional analyses of grip strength in Germany. The focus of the present ...

The introduction of standardized Case Management structures to improve coordination and cooperation of all involved in care, such as cost units, service providers, voluntary organizations, families and the different occupational categories involved in nursing, is the main concern of the current reform of German long-term care insurance. In this article, demands on Case Management in care are enunciated and the basics found in expert talks, needed for efficient support of care, assembled. In doing so, the role and function of Case Management is differentiated, the different levels (case, organizational and system levels) distinguished and options and conditions needed to settle such an organization are introduced.

It is an indisputable fact in the political and specialist discussion concerning the reform of the German public long-term care insurance to give better advice and support for constant nursing care. In order to reach this goal "Community Care Access Centers", in which competent advisors work to assess and to coordinate the necessary assistance, are proposed. Nevertheless the draft of the new act is disputed with questions with respect to implementation and finance. The already existing "Service Centers for elderly people" in the city of Dortmund is a good example of how to set up the desirable structures.

We examined 834 isolates of urinary cultures (26 fungi and 808 isolates from 28 bacteria) in an academic geriatric teaching hospital for microbial spectrum and resistance analysis. Of the bacteria, 90% were associated to ten species. Detection rate of MRSA and ESBL was 1%, detection rate for pseudomonas aeruginosa, proteus species, coagulase negative staphylococci and klebsiella species were higher than in an university clinic (p < 0.001). Resistency rates were high for the 10 most frequent bacteria: 53.3% of cross-tabulations between bacterium and antibiotic agent (sulfamethoxazol- trimethoprim, amoxicillin-clavulanate, ciprofloxacin, moxifloxacin, cefuroxim, imipenem) showed resistency rate higher than 10%. We assume that specific factors of geriatric sample taking have an impact to o...

CONCLUSIONS : 1) In the clinical studies analysed external validity was taken into account only moderately, especially with respect to additional non-pharmaceutical interventions and selection of participants. 2) The evaluation according to external validity led to a different selection of studies that were used for estimation of the ginkgo efficacy without effects on the overall result. 3) Sufficient evidence of the efficacy of ginkgo biloba extracts in the treatment of dementia of vascular origin and Alzheimer disease is provided in spite of methodological limitations. Further studies should focus on effectiveness, ginkgo-sensitive subgroups, more individualised therapeutic goals and corresponding outcome measurements.

This article examines the important aspects of Turkish demographic structure and discusses the general life conditions of the elderly. All data are from a report published by the author about the life conditions of the elderly in Turkey. This report showed that, in Turkey, ageing is generally specific to female elderly. Poverty during elderly age should be considered the most serious problem both today and in the future, although this issue shows a great diversity in terms of occupational status, educational level and regions. Care for the elderly is not a major problem yet. However, Turkish society needs to prepare for future problems in elderly care by creating the necessary legal and political infrastructure. Education and research about social gerontology has not improved yet. There ne...

The need for efficacy of geriatric psychotherapy as well specialized training in geriatric psychosomatics and psychotherapy is commonly recognized amongst professionals. Despite this, this knowledge is not reflected in everyday life. One consequence of this is that psychotherapy for elderly patients is still the exception. We conducted a study in which 478 practicing psychotherapists in Münsterland (Westfalia) were surveyed by mail questionnaire regarding the number and main diagnosis of their patients of greater than 50 years of age, as well as the elderly-specific thera...

The present study investigates the psychometric properties and the factorial structure of the German adaptation of the Activities-Specific Balance Confidence (ABC) scale (Powell & Myers, 1995) for the evaluation of falls-related self-efficacy in community- dwelling older adults. The German adaptation of the ABC using a forward-backward procedure was administered to 113 older adults (age 68.9 +/- 8.5 years). The following internationally accepted instruments were used for validation: The Short Form Health Survey SF 36, the Geriatric Depression Scale (GDS), the Trail Making Test and the Letter Number Sequencing Test, and motor tests (balance, strength, mobility). The internal consistency (0.91- 0.95) as well as the test-retest reliability of the subscales was excell...

In recent studies, patients diagnosed with Alzheimer's disease (AD) showed significantly elevated CSF levels of tau protein. Tau protein was therefore regarded as a putative molecular marker for AD. Since early diagnosis of AD is warranted for appropriate therapeutic intervention, investigation of total tau protein levels in patients with Aging Associated Cognitive Decline (AACD) and AD are reasonable. In our study the CSF concentrations of total tau protein were measured by ELISA in 132 patients with AD, 29 patients with AACD and 24 healthy controls. CSF concentrations were compared between the subgroups of mild, moderate and severe AD, AACD and the control group and were correlated with age and sever...

Authors: Steinbach S, Staudenmaier R, Hummel T, Arnold W
Loss of olfaction in old age is a frequent problem, which occurs at the same frequency as diabetes or severe sensorineural hearing loss. Problems caused by loss of the sense of smell may include weight loss due to loss of appetite, intake of rotten foods, social isolation and depression. Until now the loss of olfactory function has received relatively little attention compared to diabetes or hearing loss. In this article we review the loss of the sense of smell with age. Possible therapies are discussed to improve the quality of life in older people.

Existing physical activity questionnaires have focused either on young and middle-aged adults or on the elderly. They have mainly assessed only a portion of possible physical activities or contained nation-specific sports. As there is no gold standard for a questionnaire-based assessment of physical activity in the over-50 population, recommendations for such a questionnaire relating to German-speaking countries were developed. This work included a systematic literature research, a survey of experts, and the design of a questionnaire based on validated measuring instruments. Finally, to test its reliability and application in the field, the complete questionnaire, including a retest, was applied by telephone interview (n = 57). The test-retestcorrelation was...

CONCLUSION : Patients older than 65 years with ESRD have a low survival with and without type 2 diabetes. The mortality rate was only slightly higher in the diabetic group and was not statistically significant. The prevalence of vascular diseases and co-morbidities is high in both groups; however, the incidence of cardiovascular complications was significantly higher in our diabetic patients.

CONCLUSION : Screening for oral health performed by a physician was insufficient regarding the different clinical aspects. Training in the use of an oral health assessment tool improved the reliability of the screening. Because of the low number of examiners the results should be interpreted as a pilot investigation on this new topic.

The suicide risk in Germany increases with age. The suicide rates follow the so called "Hungarian" pattern. Especially the percentage of older females among all suicides has increased unproportionally in recent years in relation to their percentage in the population. Every second suicide of a female is today a suicide of a female older than 60 years. The main suicide method among older persons is hanging. Suicide attempts are rare among older persons; however, the rates are increasing among the oldest age groups. The suicide attempts of older persons are committed with a higher lethality and show rarely appellative components. Suicide attempt methods are often poisoning with pharmaceuticals. In total the suicide and suicide attempt rates among ...

General theories of suicidology are insufficient to explain the overproportional rate of suicidal behaviour in the elderly. The proposed transactional model represents a bio-psycho-social foundation of suicidal behavior in the elderly. The reciprocal interactions of biological, somatic, psychological and social factors in old age which may enhance suicidal behavior as a mode of problem-solving behavior are discussed.

Sexuality and suicidality in the elderly seem to underlie a taboo in science and in therapeutic practice. It is urgent to find an understanding about both aspects of human experience and behaviour by explorative studies. In a sample (N=67), the narratives of 56 persons (suicidal=43 and non-suicidal=23) on questions concerning their sexuality were transcribed and ideal types were developed with the systematic qualitative method of forming types by understanding. Four of these types consist of clearly more suicidal persons, in two the non-suicidals predominate, the rest is more heterogeneous in this aspect. Some of these types can be interpreted unequivocally. One type was surprisingly clearly suicidal dynamic, while others were a ...

This paper gives an overview about the most important aspects of crisis intervention, with special emphasis on crisis intervention with elderly people. First a review of the development of crisis intervention is given, including of some of the major concepts, with particular emphasis on psychoanalytic aspects of crisis intervention. Then a clinical case example of a crisis intervention with an elderly woman following a suicide attempt is given and discussed. The focus lies on the description of the transference-countertransference relationship, with attempts of pressing the therapist to comply with superficial, denying and minimizing fantasies. Peculiarities of crisis intervention with elderly people are highlighted: it is necessary to emphasize that elderly peo...

Recent studies have demonstrated that positive and negative stereotypes and attitudes toward age and aging may have substantial effects on the longterm development of cognitive and physical performance in old age. Relatively little is known to which degree attitudes toward one's own aging depend on (a) the current level of individual resources, (b) changes in the level of individual resources, or (c) the increasing age of older persons. The present study examined the relations between individual physical, mental, and cognitive resources and the attitudes toward one's own aging. The study was conducted with N=500 participants of the Interdisciplinary Longitudinal Study of Adult Development (ILSE) born between 1930 and 1932 (age: M=63.0, SD=0.9 at T1 and M...

Alzheimer's disease and vascular dementia still may be looked upon as distinct nosologic entities, representing the two main etiologic categories of senile dementia. However, rather recent findings suggest a comorbidity of neurodegenerative and ischemic pathology in a majority of dementia cases in later life. The effect of the vascular pathology on the survival time was studied in 173 dementia outpatients. For 147 patients with complete datasets, we were able to gain information concerning their survival time. As an indicator of cerebrovascular morbidity the 18-point ischemic scale of Hachinski (HIS) was correlated with the survival time. Thus, we did not use the HIS for its original purpose to differentiate betwe...

While research focuses mainly on support provided to the elderly, this paper deals with the very old as a support provider to his family as much as a care recipient from both his family and a formal network. We hypothesize that elders with declining health will try to maintain the provision of services, even when they require and receive help.A total of 340 octogenarians from the Swiss Interdisciplinary Longitudinal Study on the Oldest Old (SWILSOO) were interviewed up to five times over five years (N=1225 interviews). A multilevel model was applied to assess the effects of health, controlled for socio-demographic and family network variables, on the frequency of services that the old persons provided to their family and received from thei...
