            <description>CONCLUSION:            The implementation of fall and fracture prevention measures based on the National Expert Standard led to additional costs. Cost figures can be used by decision-makers with respect to decisions on resource allocation for different prevention programs (e.g., different National Expert Standards), to determine the main cost components, and finally for model-based cost-effectiveness analyses of fall-prevention programs in nursing homes.

            <description>CONCLUSION:             A coordinated, multidisciplinary model for the treatment of fragility fractures has the potential to improve the quality of patient care. Several international studies report superior outcome and our own findings are promising as well. We could show that our major goals, e.g., reduction of complications, shortening the length of stay, and restoration of the prefracture residency, can be improved by implementing such a model.

            <description>CONCLUSION:             Our results show that the management of osteoporosis in postmenopausal hip fracture patients has significantly improved over the last decade. From our point of view, the main impact on this development is due to the implementation of a GFC with corresponding guidelines and treatment steps. In addition to structural changes, the improved evidence for osteoporosis treatment, especially of geriatric patients, has a positive influence on osteoporosis therapy.

    Abstract
    The case of a geriatric patient with total hip arthroplasty for coxarthrosis and an inpatient fall 12 days after the first operation is reported. Six weeks after the first operation, the patient reported new pain in the area of both hips and thighs. X-ray and scintigraphy confirmed the diagnosis of bilateral heterotopic ossification. NSAID therapy was started, and rapid improvement was observed. At discharge, the patient was able to walk with aids in- and outside. At the 12-month follow-up, x-ray control showed Brooker state 3 for the right and 4 for the left hip. Walking ability did not change during follow-up.

            <description>CONCLUSION:            The data demonstrate that the German DRG system does not sufficiently consider the difficult management caused by patients without the ability to give consent to treatment and without a valid power of attorney. The time required until a professional legal guardian is appointed is too long for patients in a hospital. The necessity of a power of attorney has to be promoted more intensely to the public. Currently, the only two ways to minimize the problem is to identify the patients without, but needing a power of attorney as quickly as possible and to remain in close contact with the legal authorities.

    Personality traits, e.g., resilience, which is defined as emotional strength, have a strong impact on lifespan development. In this paper, a cross-sectional study examining the protective character of resilience in aging is presented. In a sample of persons ranging in age from 30-80 years, a significant negative association between resilience and depression was observed for the oldest group of our sample (age &amp;gt; 70 years). Thus, resilience might serve as a &quot;safe-guard&quot; against depression over the life course that, however, was found to disappear when people are faced with a transition into dependency and being in need of care.

The distribution of the risk index CRB-65 shows that these patients were at higher risk, were more often cognitively impaired (not caused by pneumonia), and time to starting antibiotics was longer. However, none of these differences had an influence on total mortality. The results are limited by the number of patients, potential differences of the treatment groups, and the quality of data in general as a result of a quality improvement program.

    An 85-year-old man with myasthenia gravis was successfully treated with methotrexate (10 mg/week), pyridostigmine and prednisolone (0-30 mg/day) for over 10 years. Then, he developed dysphagia and lost weight. Gastroscopy revealed Candida esophagitis. The patient received nystatin for 2 weeks. Methotrexate was stopped, and immunosuppressive therapy was continued with prednisolone alone. The patient has now remained in good condition for over 1 year. Although dysphagia is a typical symptom of myasthenia gravis, swallowing disturbances should not be attributed hastily to this disease, since they may also be a complication of therapy.

            <description>CONCLUSIONS: The mortality rate in the early rehabilitation course in this preliminary and observational study was relatively low and may be explained by both the exclusion of patients not surviving the first week of treatment and an effective inclusion algorithm. The high overall 6 month mortality rate may also be explained by the selection of patients focusing on those severely functionally impaired. This subpopulation of elderly inpatients in acute care units reveals a high vulnerability to critical incidents. Besides scale effects there is also a remarkable variability in the 6 month postdischarge course pointing to heterogeneity and different patterns concerning the time course of functionality.

    The slow procession of the primary mechanisms of aging lead, at first, to unnoticed changes in the vascular system. Endothelial dysfunction is one of the earliest markers in aging vessels, caused by oxidative stress via the reduction of the availability of NO, on the one hand, and the nitrosylation of proteins, on the other hand. At the same time, glycation of the proteins of the extracellular matrix leads to stiffening of the vessel wall. Together with the loss of elastic fibers, e.g., elastin, this leads to the age-related changes of the vessels. Knowledge of these primary mechanisms of aging may lead to the development of new drugs.

            <description>This article gives an overview of the importance of biographical work in caring for people with dementia. In particular, the role and arrangement of biographical work in the DEMIAN concept are described. Within the anamnesis of the DEMIAN concept, meaningful themes are identified in conversations with different participants (person with dementia, reference persons, and care workers) and through observations. From these findings, specific interventions, aimed at supporting emotional well-being of people with dementia, are derived and integrated into everyday nursing care to promote emotional well-being. The potential of the DEMIAN nursing concept are discussed and further possibilities are highlighted.

By sharing their stories, they increased their knowledge and information. The intervention encourages empowerment and self-management of Turkish people in need of care and their family caregivers.     Positive psychology is a scientific approach within psychology that focuses on research on what is best in people. Within this framework, humor is understood as a character strength. The article gives a brief overview on positive psychology and implications for aging (positive aging, successful aging) with an emphasis on the contribution of humor. Humor as a strength of character and its measurement by various approaches are discussed. It is argued that there is a lack of empirical data about humor in the elderly. Potential benefits of considering humor in research but also in practice are discussed.

    The aim of this article is to provide an overview of the current state of research concerning health inequalities among the aged. First, health inequalities are introduced as a general phenomenon. Then, the magnitude of inequalities in elderly populations is described for different health indicators. Subsequently, different hypotheses on age differences in health inequalities and possible explanations for social inequalities in health in elderly populations are discussed.     PURPOSE: The purpose of the study was to develop and validate the depression in old age scale (DIA-S). The DIA-S is a new screening scale for depression, which is comparable to the GDS-15 in terms of practicality but which is more appropriate for the use in geriatric patients. METHODS: The DIA-S and the GDS-15 were validated among a sample of n=331 geriatric inpatients (n=151 depressed subjects and n=180 not depressed subjects), using the Montgomery and Asperg Depression Rating Scale as gold standard criterion for depression.
