            <description>CONCLUSIONS: This study shows that Balint group work in Germany is well integrated in both general practice and psychotherapy, and it is still of great interest to many specialists of other disciplines as well as a good way to realize further education. The ideas of Michael Balint are very much alive, but - because of the changed realities in the medical field - no longer as he originally foresaw, but in settings adapted to the new situations in medicine.

            <description>CONCLUSIONS: Alexithymia in patients with mental disorders is linked to specific relationship patterns and interpersonal problems at the beginning of an inpatient psychotherapy. Because interpersonal problems, and especially the therapeutic alliance, are strong predictors of outcome in individual psychotherapy, specific attention should be paid to this relationship in the treatment of alexithymic patients.

            <description>DISCUSSION: The therapeutic framework for the treatment of older patients corresponded to the rules for psychosomatic medicine and psychotherapy. There is no need for any other fundamentally different concepts. After the follow-up outpatient treatments, older patients and the therapists expressed an increased acceptance of psychotherapy. The effectiveness of treatment in older patients may result from their good resources. The results of the catamnesis show encouraging findings for the psychosomatic treatment of older patients, also in the long term from 1,5 to 2 years.

Methods/sample: 865 of the 1114 patients examined over the course of 10 months in a psychosomatic clinic agreed to participate in the study and filled in the Frankfurt Body Concept Scales (FKKS, Deusinger 1998). Results: Patients with one or more psychiatric diagnoses showed significantly more impaired body images than did healthy controls examined with the FKKS.Especially patients with eating disorders stood out.             <description>Conclusion: Patients with pain-dominated somatoform disorders are substantially impaired in their quality of life. Because 31 % of the patients with such disorders have no additional comorbid psychic disorder and the existence of a comorbid psychic disorder impacts only the psychological domain of their health-related quality of life, the results appear to strongly support maintaining the diagnostic category of somatoform disorders in ICD-11 and DSM-V. They also support the grading of the severity of the somatization similar to the grading of depressive disorders.

            <description>CONCLUSIONS: Higher comorbidity has deteriorating consequences in all bio-psycho-social respects. The aetiologic connections between medical disease and psychological disorders are complex bidirectional somato-psycho-somatic ones. Seven modalities of comorbidity for coronary heart disease (myocardial infarction) and depression/anxiety are discussed. The psychopharmacological and psychotherapeutic treatment of anxiety/depression in patients with medical diseases follows general guidelines.

Norms and a dimensional personality disorder profile provide hints concerning the presence of pathological personality traits with respect to the reference group. The ADP-IV represents the only German-language questionnaire for the assessment of DSM-IV personality disorders which has been sufficiently validated.             <description>CONCLUSIONS: The instrument is well suited for screening purposes, allowing for a reliable exclusion of personality disorders. For diagnosing personality disorders, however, clinical interviewing remains necessary. Norms and a dimensional personality disorder profile provide hints concerning the presence of pathological personality traits with respect to the reference group. The ADP-IV represents the only German-language questionnaire for the assessment of DSM-IV personality disorders which has been sufficiently validated.             <description>This study assesses the development of patients with anxiety disorders in self-help groups. An additional investigation studies the attitude of psychotherapists toward such self-help groups. METHODS: 33 participants from three self-help groups were assessed using nonstandardised and standardised questionnaires (SCL-90-R, STAI). In addition, 250 psychotherapists were surveyed. RESULTS: Subjectively, the patients experienced their group as being helpful and described their symptoms as having slightly improved. The patients showed high scores in the anxiety questionnaire, which marginally improved over time. Avoiding behaviour did not change.             <description>CONCLUSIONS: The relatively high proportion of female physicians might be the result of women's special disposition for the field. While an adequate representation of both sexes among medical psychotherapists is desirable for clinical practice, an increase of female physicians in management and research positions would be expedient in order to direct more attention to gender-specific questions in psychosomatics and psychotherapy.

Further conceptual development of structured psychosocial interventions which can be applied in the general practice for the most common emotional disorders is necessary. Considering the large number of patients with emotional disorders in primary care, these efforts will undoubtedly be worthwhile.

    OBJECTIVE: Although a majority of psychodynamic-psychoanalytic researchers and practitioners address the issue of psychic structure, reevaluation of the nature of a neurotic conflict system and its relation to psychic structure is necessary. RESULTS: A comprehensive conflict system is presented in the context of operative psychodynamic diagnostics (OPD-2), based on findings confirmed thus far. The conflict levels range from conflict tension to neurotic conflicts to conflict schemata in structural disturbances.

            <description>CONCLUSIONS: Sick leave and utilization of in-patient treatment were found to increase considerably in the year before psychosomatic treatment. Therefore, pre-post differences over the entire period were only marginal. The change in subjective criteria was more immediate, while changes in some objective parameters were delayed. Both subjective and objective criteria should be included in outcome studies.
