Cen Eur NeurosurgDOI: 10.1055/s-0031-1291206The superoanterior portion of the third cervical vertebra may need to be rimmed during anterior odontoid screw fixation procedures. We, therefore, retrospectively evaluated radiological data to analyze the anatomical relation between the second and third cervical vertebra of the patients who were operated by an anterior cervical approach with respect to the question if odontoid screw fixation would have been possible without rimming or not.Patients in whom the anterior approach for cervical disc prolapse and/or cervical stenosis was used between 2008 and 2010 were included in this study. The odontoid screw angle, and the angle between the lower second and the upper third cervical vertebral endplate were measured on intraoperative cervical lateral...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1291207Nimodipine is primarily used in subarachnoid hemorrhage (SAH). Clinical trials revealed also a beneficial effect of prophylactic nimodipine treatment on cranial nerve functions following vestibular schwannoma surgery. The unknown pharmacokinetics of prophylactically administered nimodipine were investigated.
Samples were taken from 27 patients with skull base lesions. Prophylactic intravenous nimodipine infusion was started 5.8–25.8 h (mean 17.9 h) before surgery. Nimodipine concentrations were determined in serum (intra- and postoperatively), cerebrospinal fluid (CSF) (intraoperatively), and tissue samples.Wide interindividual differences were observed. Mean concentrations for nimodipine were 46.9 ng/ml (SD: 6.4; min. 4.1 and max. 92.7...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1283120Extensive tentorial meningeomas which are growing supra- and infratentorially are a surgical challenge, and a distinct surgical strategy is required for good results.3 patients, 2 with asymptomatic tumor and 1 with signs of increased intracranial pressure and visual disorder underwent microsurgery. In 2 patients, the semi-sitting position was taken. A lateralized occipital/suboccipital craniotomy with exposure of the confluens sinuum, transverse and superior sagittal sinus was performed. First, the dura was opened infratentorially allowing good cerebrospinal fluid (CSF) release. In a second step the dura was opened supratentorially sparing the sinus. The tumor was then removed completely without brain swelling or significant venous bleeding. One ...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1286262The aim of the present pilot study was to test the feasibility of an innovative Short Psychological Intervention (SPI) for back pain patients as part of an acute inpatient neurosurgical treatment. Fear and fear-avoidance beliefs have been shown to influence the functional outcome in chronic back pain (CBP) patients. Therefore, a reduction of fear and fear-avoidance beliefs should improve the functional outcome and reduce pain in the acute neurosurgical setting.39 patients were studied in a randomized prospective longitudinal study. The patients had severe degenerative spinal disease and had undergone posterior lumbar interbody fusion. All patients enrolled in the study were investigated in the immediate preoperative period and 6 weeks postoperativ...

The chamber count has been the current reference method to date, yet it is problematic due to its subjectivity depending on the examiner’s skill and experience. Therefore, as a reference method, we used an impulse cytophotometric measurement with Epics XL owing to its improved objectify ability and compared this method to the measurement of CSF samples performed with the ADVIA 2120 and XE-5000.101 CSF samples were measured with the ADVIA 2120, XE-5000, and Epics XL. For impulse cytophotometric measurement, CD235a was used for identification of erythrocytes; CD45 for the entire leukocyte population; CD56, CD16 and CD...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1286261The purpose of this study was to analyse the differences between patients with frontal (FEDH) or parieto-occipital (POEDH) epidural haematomas and evaluate possible statistically significant prognostic factors. In this retrospective study of a group of 41 patients with a FEDH (17) or POEDH (24 individuals), the authors analysed the influence of gender, age, type of injury, clinical presentation, Glasgow coma scale (GCS) score on admission, radiological findings, and time interval from trauma to surgery on outcomes. A good recovery and moderate disability were considered a “good” or “favourable outcome”, whereas severe disability, a vegetative state or death was a “poor outcome”.In the POEDH subgroup, a higher GCS score on admission and...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1284415Balloon kyphoplasty (BKP) has been established as a standard procedure for treatment of osteoporotic compression fractures of the spine in elderly patients, as it results in pain relief for most of the patients. Moreover, BKP may result in a partial realignment of the sagittal shape of the vertebra and the spine. However, it is unclear whether BKP may result in long-term improvement of the sagittal alignment of the spine. The aim of the current study was to investigate if there is a significant loss of correction of the sagittal alignment of the thoracolumbar spine following BKP.A prospective, noncontrolled, nonrandomized study, consisting of 71 patients treated for single level osteoporotic compression fracture by BKP at 1 institution and by 1 s...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1279751AbstractCerebrospinal fluid (CSF) leaks are well-known and frequent complications of intracranial procedures with their clinical aspects covered by numerous studies. Little, however, is known about the pharmacoeconomical aspects of this specific complication. 545 patients with a variety of intracranial procedures (elective and trauma) were recruited for a multicenter, prospective, observational study over a 13-month period. A specific pharmacoeconomic analysis was performed in 168 of these patients from the institution of the first author covering all clinical and pharmaco-economical aspects of this specific complication. Of all patients, 5.36% developed a postoperative CSF leak. Treatment of the leak required numerous diagnostic and therapeutic ...

Conclusion: Surgical treatment of patients suffering from neurological deficits caused by spinal metastases results in neurological function improvements. The probability of neurological improvement does not correlate with the severity of the preoperative neurologic deficit.
Conclusions: In spinal surgery, a trend towards homogenization is observed with the early use of heparin. In intracranial procedures, practice is more heterogenous. The heterogeneity is due to the fact that the data available in the literature does not allow for the identification of an optimal protocol.[...]© Georg Thieme Verlag KG Stuttgart · New YorkArticle in Thieme eJournals:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Cen Eur NeurosurgDOI: 10.1055/s-0030-1261907AbstractThe institution of German neurosurgery as an autonomous surgical specialty, starting in Würzburg in 1934, is closely linked to the names of Fritz König and Wilhelm Tönnis. They were acting at a time when the global economic crisis and a consolidating Nazi dictatorship caused a cascade of alarming changes in political and social life. On the one hand it is fascinating to see how the restless work and energy of Tönnis managed to build up the first independent neurosurgical unit in Germany and to tighten efficient international connections all over the world within a few years. On the other hand – from a present-day perspective – it is difficult to understand how his strive towards a specialist's success, in contrast to that ...

The objective is to bridge standardized small animal models and actual patient medical care, since the results of experimental small animal studies often cannot be transferred to brain-injured humans. For improved standardization of high-velocity trauma, new trauma devices for initiating closed traumatic brain injury in sheep were developed. The following new devices were tested: 1. An anatomically shaped rubber bolt with an integrated oscillation absorber for prevention of skull fractures; 2. Stationary mounting of the bolt to guarantee stable experimental conditions; 3. Varying degrees of trauma severity, i. e., mild and severe closed traumatic brain injury, using different cartridges; and 4. Trauma analysis via high-speed video recording. Peritraumatic measurements of intracranial...

Minimizing Brain Shift during Functional Neurosurgical Procedures – A Simple Burr Hole Technique that can Decrease CSF Loss and Intracranial Air

Conclusion: This new and simple burr hole technique was associated with a significant reduction in postoperative intracranial air. Reduction of intracranial air will ultimately reduce brain shift. That total operation time does not influence intracranial air is discussed as well as the limitations of this pilot series. In the authors’ opinion, this straightforward and cost-effective technique has the potential to reduce brain shift and to increase DBS placement accuracy during functional stereotactic neurosurgical procedures performed in the seated or half-sitting position. A larger more standardized patient series is necessary to substantiate the findings.[...]© Georg Thieme Verlag KG Stuttgart · New YorkArticle in Thieme eJournals:Table of contents | Abstract | Full t...

Cen Eur NeurosurgDOI: 10.1055/s-0031-1271730AbstractThe application of lesioning procedures in the basal ganglia and, more recently, of deep brain stimulation (DBS) has revolutionalized dystonia treatment. However, our understanding of the mechanism of action of DBS is only minimal. This is largely due to a rudimentary understanding of dystonia pathophysiology itself, which in turn reflects an insufficient understanding of the functional significance of the cortico-striato-pallido-thalamocortical loops. The initial dystonia pathophysiology concept was one of changes in oscillation rate. Soon, it was realized that not only rate but also the pattern of basal ganglia activity is crucial in the etiology of the disease. The observations of altered somatosensory responsiveness and cortical neuro...

Cen Eur Neurosurg 2011; 72: 90-98DOI: 10.1055/s-0031-1271800AbstractCubital tunnel syndrome (CuTS) is the second most common peripheral nerve compression syndrome. In German-speaking countries, cubital tunnel syndrome is often referred to as sulcus ulnaris syndrome (retrocondylar groove syndrome). This term is anatomically incorrect, since the site of compression comprises not only the retrocondylar groove but the cubital tunnel, which consists of 3 parts: the retrocondylar groove, partially covered by the cubital tunnel retinaculum (lig. arcuatum or Osborne ligament), the humeroulnar arcade, and the deep flexor/pronator aponeurosis. According to Sunderland 122, cubital tunnel syndrome can be differentiated into a primary form (including anterior subluxation of the ulnar nerve and compress...

Cen Eur NeurosurgDOI: 10.1055/s-0030-1268500AbstractBackground: In 95% of patients with an apparently normal distribution of blood using unenhanced computed tomography (CT), no ruptured aneurysm for a perimesencephalic subarachnoid hemorrhage (PMSAH) will be detected. In general, the clinical course of these patients is more favorable than that of patients with a detected ruptured aneurysm. We wanted to assess whether vessel variants of the vertebro-basilar circulation are more common in patients with PMSAH than in patients with SAH caused by intracranial aneurysms. Furthermore, we wanted to investigate whether CT angiography (CTA) as a sole diagnostic modality in PMSAH is sufficient.[...]© Georg Thieme Verlag KG Stuttgart · New YorkArticle in Thieme eJournals:Table of contents | A...

Cen Eur NeurosurgDOI: 10.1055/s-0030-1253349AbstractBackground and Aim: In the present study, we have hypothesized that volume changes of the caudate nucleus, putamen, globus pallidus, hippocampus, thalamus, and lateral ventricle in newly-diagnosed, male PTSD patients without therapy are more pronounced in those with headaches. To confirm or reject our hypothesis, we have undertaken an extensive study of forty-nine PTSD patients.[...]© Georg Thieme Verlag KG Stuttgart · New YorkArticle in Thieme eJournals:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Cen Eur NeurosurgDOI: 10.1055/s-0030-1261945AbstractObjective: Sphenoid wing meningiomas represent a difficult to access subtype of intracranial meningiomas involving important neurovascular structures such as the optic nerve, cavernous sinus or carotid artery. They cause neurological compromises by direct compression of adjacent cranial nerves. Insidious and aggressive dural, bony, and orbital involvement produces several difficulties for adequate resection leading to higher rates of recurrence.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Cen Eur NeurosurgDOI: 10.1055/s-0030-1253410Abstract In conventional MR imaging, it is often difficult to delineate the heterogeneous structure of gliomas. Proton magnetic resonance spectroscopic imaging (H-MRSI) is a noninvasive tool for investigating the spatial distribution of metabolic changes in brain lesions. The aim of this study was to assess the improvements in delineation of gliomas based on segmentation of metabolic changes measured with H-MRSI.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Cen Eur NeurosurgDOI: 10.1055/s-0030-1253350Abstract Continuous recording of electromyographic signals (EMG) is a standard method for intraoperative monitoring of facial nerve function in cerebello-pontine angle surgery. Subcutaneous needle electrodes in the facial muscles are used in different setups. The goal of this study was to compare two commonly used electrode setups concering sensitivity for pathological EMG activity.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Some patients may search for non-scientific or unconventional therapeutic options to treat the symptoms induced by the tumor itself or arising from treatment. The extent of non-scientific therapy use, the rationale behind it, and costs of these forms of therapy are unknown.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

Physicians themselves have partially adopted economic terminology, although they rarely have a profound knowledge of economics. Today game theory is one of the most important factors driving microeconomics, which is the competition for limited resources within a small group of individuals.
Cen Eur NeurosurgDOI: 10.1055/s-0030-1248256AbstractThe indication for invasive intracranial pressure (ICP) recording in comatose patients after head injury is a matter of discussion. In this review the evidence-based data are analysed. Monitoring appears useful especially in sedated patients, but there is no study which provides evidence of its overall benefit for the patient. The decision for or against ICP monitoring must therefore be based on weighing the potential benefits versus the risk for the individual patient according to the personal experience and judgement of the attending neurosurgeon.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)

In this study, the mRNA expression of 30 invasion-related molecules (twenty-one ECM components, two related receptors, and seven ECM-related enzymes) was investigated by quantitative reverse transcriptase-polymerase chain reaction. Fresh frozen human tissue samples from glioblastoma (GBM), intracerebral lung adenocarcinoma metastasis, and normal brain were evaluated. Significant differences were established for 24 of the 30 molecules. To confirm our results at the protein level, immunohistochemical analysis of seven molecules was performed (agrin, neurocan, syndecan, versican, matrix metalloproteinase 2 [MMP-2], MMP-9, and hyaluronan). Determining the differences in the levels of invasion-related molecules for tumors of different origins can help to identify the exact molecular mechanisms ...

Cen Eur NeurosurgDOI: 10.1055/s-0029-1241179Abstract Low pressure valves with ventriculoperitoneal shunts have been proven to lead to good outcomes in the treatment of idiopathic normal pressure hydrocepahlus. However, overdrainage complications are often seen with low opening pressures. Efforts have been made to obtain good outcomes without overdrainage complications by decreasing the hydrostatic pressure component using special valve constructions. The aim of this study was to ascertain whether it is possible to optimise outcome with the implantation of both an adjustable valve and a gravitational unit.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurg...

Cen Eur NeurosurgDOI: 10.1055/s-0030-1249043Abstract An elevated body mass index (BMI) is suggested to be a risk factor for a poor outcome after intracranial aneurysm rupture and is considered to be associated with cerebral infarction in patients with aneurysmal subarachnoid hemorrhage (SAH). The aim of this study was to analyze the association between permorbid BMI and neurological outcome.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Zentralblatt fur Neurochirurgie - Central European Neurosurgery)
