The regulatory pressure just keeps mounting on Invacare.
Invacare, a manufacturer of homecare durable medical equipment products – wheelchairs and respiratory, announced yesterday that the FDA had asked them to negotiate and agree to a consent decree of injunction. This relates to previous findings and disclosures at the Invacare’s Elyria facility which also serves as it’s world headquarters.
One of the potential results of this is that Invacare may be forced to suspend shipping certain products.
This is not the first run in with the FDA who issued a warning letter to Invacare in December last year for issues related to it’s Florida facility.
Invacare’s stock plummeted to $14.70, a drop of 29%, however it is up at time of writing to $16.12 (+9%).

The cost of the knowledge that is being lost, possibly forever.
The cost of being unable to innovate in future, because critical knowledge has been lost.
The consequent cost of its current business being destroyed by competitors emerging who can make a better product at lower cost.

Frederick Banting and Charles Best, were instrumental in the discovery of insulin in 1922, and have saved millions of lives since then.
So while there is cause to celebrate there is also the realization that there is no cure for this life altering disease which often attacks the young with type 1 and catches others at a later stage with type 2.
As usual lower income countries have the highest death rate by far – 80%.
In fact it is so big that it has it’s own market segment in homecare.
Time to get focused and get rid of it. (Source: the international medical market place)

Healthcare is often a forced lifestyle choice – if that makes sense. Patients often have a choice on what level of healthcare and certainly what healthcare products they they use. Of course some people unfortunately have none.
If however you do have a choice then the choices are getting better as manufacturers start to produce products that are more consumer friendly. While most products rely on functionality rather than design and aesthetics, there are those that recognize that you really can have both, mostly at a higher price.
Here are recent examples from the Wall Street Journal (where else) of a redesigned walker and nebulizer.

Guest post from FG:
Steady growth in 2011 for EUROMEDIS and 12% expected in 2012:
After heavy restructuring, the manufacturing & distribution group for disposable medical supply and which operates the PARAMAT franchises in France, is not considering any new acquisitions, and is looking forward to grasp the benefits of both a 1,5 MEUR investment in a new 12,000 m² logistic center and focusing on its sourcing policy in Asia.
A subsidiary was set-up in Germany, and EUROMEDIS which is planning to double it’s shops from 100 today, is also growing its export business from 14% today, and expanding into non-woven medic tissues.

Despite the rush to Asian outsourcing trend and the “stereotype” of expensive French manufacturing, HMS-Vilgo a French based manufacturer makes “100% made in France” medical / homecare beds.
Not only that, CEO Patrick Jude has grown the business from about 1000 beds to 24000 beds per year from 1997 and remarks that they have used robots to avoid having to outsource their production to Asia. This despite fairly recent reimbursement reorganization in France.
Their main market is France with overall revenues of 26 million Euro (about 35 million US Dollars at today’s rate) with export revenues accounting for up to 12% of sales.

In this latest news, Invacare has released 50 employees, most from their Elyria headquarters. They are not the only company in the homecare / mobility industry to do so recently.
Invacare maintains that dismissed staff are “office workers” and spokesperson Lara Mahoney is quoted as saying that they are “just ensuring that we’re running as efficiently as possible”. She went on to say that they were “hiring in some positions and having to make tough decisions in others.”
Perhaps not directly related, my take is that while companies continue to cite reimbursement as a major issue (it is), often neglected is the shift to, and opportunity in, the “private” retail market which has tremendous upsides both in the US and Europe.

Smoot and Hawley 1929
If it does not happen now, then it is just a question of time?
China has threatened the US with a trade war if the US law-makers pass a bill currently under discussion that will force the value of the Chinese RMB to rise against the US dollar, thus increasing the price of imports to the US. This follows a long fight in Washington to get the Chinese to allow the RMB to float unrestricted.
This is yet one more reason why we continue to advise our clients (disclosure – Sibaya both advises and provides services in off/near shore locations) to be cautious about unfettered outsourcing to a single location like China in this case, as opposed to taking a portfolio approach to outsourcing and dividing the economic and political risk over several locations – and mor...

Invacare announced their Q2 2011 results yesterday and the stock dropped in the region of 5%, why?
What didn’t the market like, especially in view of the fact that:

Organic net sales growth was up 3.8% vs Q2 last year.
Adjusted EPS improved 15% vs Q2 last year to $0.45.
Invacare generated free cash flow of $21.3 million.

Looking at the international markets, Europe sales were up 15,2% to $141.9 million (organically up 5,1%) despite a tough market and competitive environment. EBIT was at $9,9 million (excluding restructuring $ 0,4 million).
In Asia while sales grew 8,4%, organic sales decreased 7,3% due largely to the New Zealand businesses.

So despite this the question remains what longer term effect will the higher sales of low end products to lower end customers continue to ef...

A manual wheelchair system that automatically senses current usage and shifts gears automatically has been developed. Use on any wheelchair.
Enter Scott Daigle, together with the team at IntelliWheels, has developed a product that automatically shifts gears according to speed and force, allowing the driver, for example to climb slopes easier, without changing gears manually.
Up until now several enhanced mobility (or assist) systems have been tried and the one that has been most successful is in the form of the electronic assist device. While motorized versions definitely have their place in certain wheelchair driver categories, the automatic manual gearing approach may well suit those looking for a more active product.
The manual gear wheel will be a lot lighter (only 6 pounds or 2.72 kg)...

Dutch Competition Authority NMa provides clearance for acquisition of HandicareFollowing the announcement of the anticipated acquisition of Handicare by Nordic Capital Fund VII in June 2010, the Dutch Competition Authority NMa initiated an investigation, because of another Nordic Capital Fund’s holding in power wheelchair manufacturer Permobil. Although Handicare and Permobil are two independent companies and have no intention to merge their activities, the NMa concluded that the intended transaction would be harmful to competition in the Dutch power wheelchair market.
To resolve this situation we have explored possible scenarios with the NMa, and we are pleased to announce that the NMa has now approved the ‘remedy proposal’ submitted and decided to provide merger clearance provided...

Welzorg, one of the largest homecare providers in the Netherlands has been acquired by Dutch family company investors Louwman Group. Louwman, primarily known in the auto business acquired Welzorg from their former owner IK Investment Partners.
Welzorg, together with rival Harting Bank are the largest competitors in this space in Holland and it will be interesting to see how this develops. Harting Bank themselves were acquired by the NPM Venture Capital group, as we reported in 2006.
While Welzorg has built up a presence in Denmark, neither company is significantly active in other countries.

It is common knowledge (is it?) that homecare is more cost efficient than homecare.
While this is normally true for the reimbursement systens or insurance companies, it may well not be true for the patient.
In fact it can be very expensive for the patient, and getting the right information (a very complex process) to deal with your homecare – may necessitate special homecare consultants that will bill you for an evaluation … by the way the insurance company does not reimburse the consultant fees.

Press Release:
Our society is faced with a disturbing reality: According to the Alzheimer’s Association, nearly half of all people age 85 and older will develop Alzheimer’s — a disease that currently has no cure. Caring for a person with Alzheimer’s costs $1 million over the course of a lifetime, yet the NIH funds only two to three percent of submitted Alzheimer’s grants.
Four experts recently traded thoughts on this disparity and dug deep into the science of Alzheimer’s as part of Breakthroughs, a new Big Think video series made possible by Pfizer
One topic they discussed is the inevitability of Alzheimer’s. Will everyone get the disease if they live long enough? Mental decline, on some level, is inseparable from aging. With more people living ...

As expected the new drug rules were approved in Germany today. Pharmaceutical companies will have to demonstrate that new products are worth it in order to get better pricing.
If not products will fit into a benchmark category of peers. This will influence pricing across the European union as neighboring countries watch cross border pricing.
To what degree this will trickle over into the device or homecare product areas is yet to be seen, although the precedent has now been set with pharma.

AXURA®/AKATINOL® (Memantine) enhances language and communication skills in patients with Alzheimer’s disease – new data presented at the EFNS Congress
Geneva, Switzerland – [September 28, 2010] Memantine enhances language and communication skills in patients with moderate to severe Alzheimer’s disease (AD), a valuable contribution for patient independence and quality of life for both the patient and the family/caregivers. This one of those just thinking aloud posts…
If you are from the wheelchair industry you can relax for now. These are car wheels…. but it got me thinking.
What would happen if the EU placed a dumping tax on wheelchair wheels in Europe? It just so happens that wheels are often the most expensive component on manual wheelchairs. Say 10 euros each (20 euros because you need two) out of a cost of say 100 euros – 20%.
Let’s take a quick and dirty back of the envelope example for Europe annual consumption.
1 000 000 wc wheels x 10 euro = 10 000 000 euro (today from China).
1 000 000 wc wheels x 12 euro (with 20% duty) = 12 000 000
But then there would not be a dumping tax because the wheels come attached in large part to a wheelchair.
That got me thin...

Pretty shocking stats as former South African president Mbeki makes case for African universities.
Add to this under funded healthcare systems in Africa, new learning democracies – compared with a vociferous thirst for additional medical talent in treating an ever aging population in the “West”, it’s not hard to see that the struggle for scarce resources is not an even playing field right now. Especially in healthcare.
In 1998 South Africans numbered the largest group of foreign dentists in the United Kingdom, he said.
“Only 50 out of 600 doctors trained in Zambia since independence were still practising in Zambia. More Malawi doctors were practising in the city of Manchester in the UK than in the whole of Malawi.

Just trying to look at the facts here, it should be noted that Invacare is not alone as most of the large homecare and mobility companies (in the US and Europe) outsourced significant production to Asia and other locations.
The basic reasons we see:

Reimbursement is being slashed by Government agencies in many parts of the world, so costs have to be cut dramatically

We all know it happens, but the real question is how to stop it. There are trade associations and consumer groups in the UK that do help, but …
Elderly and vulnerable people are being duped into handing over thousands of pounds for overpriced mobility aids by rogue traders.

Theoretically the benefit of the above is that consumers should be getting products at competitive prices. Unfortunately all too often, as reported here, not only are the cost savings not being passed on but prices ar...

At Sibaya we are strong supporters of diabetes research -especially Type 1 (scroll all the way down bottom right).
MIT has done some great work in this area and has now come up with a device that can monitor glucose with light, potentially avoiding the several times a day that diabetics need to draw blood.
By scanning a users arm or finger with near-infrared light, the device frees users from the necessity of drawing blood, a daily routine for most type 1 diabetes patients. The laptop-size machine is the result of 15 years of research at the MIT Spectroscopy Lab. It employs Raman spectroscopy, which can determine chemical compounds based on their molecular vibration.

The as yet unnamed new company, which will focus on telehealth and independent living, will be made up of GE Healthcare’s Home Health division and Intel’s Digital Health Group. The new company will focus on three segments: chronic disease management, independent living and assistive technologies.

New Zealand (NZ) is to hold its first ever conference on Access Tourism on October 4. Access Tourism is tourism, travel, and hospitality for the disabled, seniors, and ageing Baby Boomers. NZ – unlike some other countries – seems hardly aware of how major demographic changes will impact the tourism industry. These changes will be brought about by ageing of world populations and the resulting rise in the number of people with disabilities (PwDs).
The NZ Ministry of Tourism has only just acknowledged that Baby Boomers and older people are already the most important domestic market, and their own data shows that this group is also the most important international market. And the older market is going to increase in size. In spite of this, the NZ government and industry are not plan...

News release:
A unique device based on sniffing—inhaling and exhaling through the nose—might enable numerous disabled people to navigate wheelchairs or communicate with their loved ones. Sniffing technology might even be used in the future to create a sort of “third hand” to assist healthy surgeons or pilots.Developed by Prof. Noam Sobel, electronics engineers Dr. Anton Plotkin and Aharon Weissbrod, and research student Lee Sela in the Weizmann Institute of Science’s Department of Neurobiology, the new system identifies changes in air pressure inside the nostrils and translates these into electrical signals.
The device was tested on healthy volunteers as well as quadriplegics, and the results showed that the method is easily mastered. Users were able to navigate a wheelchair aro...

Press Release:
Based on current projections, ATA 2010, May 16-18 in San Antonio, Texas will, once again, be the largest meeting on telemedicine and telehealth ever held. The gathering is the 15th annual meeting of the American Telemedicine Association and serves as an international focal point for health care professionals, administrators and representatives of technology, telecommunication and healthcare service entities.
The growth in registration and participation on the exhibit floor is a strong indicator that the market for telemedicine is continuing its expansion, despite downturns in the economy. The meeting will feature over 300 educational presentations and 200,000 square feet of exhibits, double the size from the previous year.
The surge of interest in the meeting also reflects s...

Marie-Patricia Hoarau, a wheelchair user, was ejected from an EasyJet flight from Paris to Nice in France because she did not have somebody travelling with her.
EasyJet allegedly citing so called European regulations has said that they ejected her because of European regulations. Say what?? They had let her fly before with no issues. All major airlines we know let wheelchair passengers fly unaccompanied in Europe (we know because Sibaya is associated with clients that have done, or do, make hundreds of thousands of wheelchairs). Aha!! Was it the emergency exit caveat? No, does not look like it.

Handicare CEO, Per Gunnar Borhaug noted:
“We never doubted that we would receive OFT approval, but it’s great to have it now so that we can start working together in earnest. This acquisition strengthens our position in the UK, broadens our product offering and will improve our sourcing capacity too. Reversal of aging shown in human stem cells
A team of North American collaborators from both industrial research centers (Biotime Inc, Mandala Biosciences LLC and Sierra Sciences LLC) and academic institutes (Ontario Cancer Institute, Burnham Institute for Medical Research and The Scripps Research Institute) have demonstrated successful reversal of the developmental aging of normal human cells.
New German Health Minister Takes Aim At Traditional Pharma Companies.
Newly appointed German Healthcare Minister Philipp Roesler – liberal – Free Democratic Party in likely move to break big pharma stronghold and submit new products to insurance pricing scrutiny and impose an “efficiency and clinical effectiveness /outcomes” pricing performance ratio.
Not totally dissimilar to the British NICE economic and clinical effectiveness system.
Sign of the times perhaps to introduce new effectiveness and economic liberalization in the DME (durable medical equipment) sector of the ever increasing homecare industry.
Maybe not – Roesler is clearly risking his parties popularity as a junior member in Chancellor Angela Merkel’s Christian Democrat Party.

Bridgepoint will be concluding the acquisition of UK care services provider Care UK.
Care UK provides care services through it’s facilities as well as in the growing home care sector. In addition they are a provider of NHS Walk-In centers. The acquisition is valued at 309 million Euro (about 460 million US Dollars).
Jamie Wyatt, director responsible for Bridgepoint’s UK healthcare investment activity, said, “Today’s offer is a fair one that comes with the unanimous recommendation of the independent directors of Care UK to the Care UK shareholders, and which will allow Care UK to accelerate growth and undertake selective acquisitions in a way that it was unable to as a quoted business.”

Here is a thread from The BMJ where NHS physicians enter into the argument of the value (moral, ethical and commercial) of medical (pharmaceutical) sales reps.
The original letter from Dr Shane O’Hanlon starts like this:
It was switching to decaffeinated coffee that started it all. In a flash of lucidity I realised the dubious ethical basis for drug company representatives’ courtship of doctors.
Doctor – Sorry to say but so long as anybody works for money, including doctors, they could be asked the same thing. Doctors are very quick to court sales people when they “need” something. Medical Journals are quick to solicit and accept drug company advertising, and there is nothing wrong with that. It is also very ethical.
It becomes unethical when the conversation betwee...

Virginia Legislature Passes Bill Requiring Reimbursement for Telemedicine
Contact: Ben Forstag, American Telemedicine Association, 202-223-3333 March 3, 2010
WASHINGTON, DC – By a unanimous vote, the Virginia State Legislature yesterday approved SB 675 mandating that health insurers pay for telemedicine services. Governor McDonnell is expected to sign the bill into law.
“This is one more indication that telemedicine is now fully accepted by providers, policy makers, consumers and insurers as an important way to increase access, improve quality and reduce costs in the delivery of healthcare” said Jonathan D. Linkous, CEO of the American Telemedicine Association (ATA).

Virginia is the latest in a string of states passing similar legislation. Eleven other states have ado...

This follows the troublesome legislative change in “les Etablissements d’Hébergement pour Personnes Agées Dépendantes (EHPAD)” (read Institutional reimbursement), the negative effect of which appears to have leveled off. In addition the regular homecare sector has continued it’s positive trend.

There is no doubt that with the strengthening dollar, costs for homecare products are set to rise in Europe.
Where does the balance between the profit motive and patient care meet? Where in the so called first world can you not go to a pharmacy and pick up the drugs you need with the minimal delay – and no, more deregulation is not the answer.

Are hospitals who limit ties with drug companies at a competitive disadvantage? | KevinMD.com.
KevinMD asks an interesting question, and goes on to summarize:
In a lot of ways, this is similar to the college recruiting of athletes. Schools that have rigorous academic requirements put themselves at a competitive disadvantage athletically. The same could be said for hospitals, who risk driving away doctors by limiting their ties with drug companies.
My own take is simply “absolutely not”.
On the related posts several other drug company / physician ethics / commercial questions are raised. (Source: the international medical market place)

National Competitive Bidding For Manufacturers – Surprise?

From Homecare Monday
So – surprise surprise – in the latest round of proposals there is one that orders an investigation possibly that manufacturers of homecare products are going to be asked to bid.
Of course we have been saying for some time that the traditional supply lines between manufacturers and dealers / distributors / buying organizations are blurring, not only as a result of this competitive pressure but the bar has been lowered because of the ease of Asian sourcing for manufacturers and distributors alike.
This is happening in several countries in Europe as well – come to think of it the British NHS – the biggest supplier of wheelchairs in the UK, has been doing it for years. (Source: the international medical market place)

Permobil has announced that they will be establishing a brand new 120000 sq ft facility in the US – 80000 of this will be dedicated to manufacturing.
This comes at a time where National Competitive Bidding, Medicare cuts and China outsourcing are the norm for several wheelchair manufacturers.

Lebanon, TN (October 20, 2009) — Permobil, Inc., an international company that is the world’s leading manufacturer of custom-made powered wheelchairs, will build its new $12 million North American Operations Center in Lebanon on a 17-acre site located in Park 840. Sunchoice gives them access to nearly 1000 UK nursing homes.

Invacare Sues Drive Medical For Patent Infringement

Invacare is seeking injunctive relief and suing Drive Medical for alleged patent infringement related to it’s bed products as well as it’s rollator.
Invacare had previously announced in one of it’s presentations that it would be focussing more on protecting it’s intellectual property and this action may well partially stem from this.
As more and more companies (like Invacare, Drive and others) seek to compete in the highly cost competitive “national competitive bidding” environment, and in addition increasingly source product from China and other Asian countries there are bound to be more disputes of this nature. Q. What do you think about the proposal for a government-backed health insurance plan?
A. I believe in the free market. But I think there are people who … aren’t faring well under capitalism. I think Americans feel that no one should be left out.
Q. How might a government health-care plan affect Invacare?
A. Invacare does business in 80 countries. Canada and England are government-run health care systems. We do very, very well well in those countries. And the home-care reimbursement [there] in many cases is better than they have in America.

We are manufacturing expandable spinal PEEK cages (Cervical and Lumbar TLIF) that are unique to the spine market. Our patented products stand out from other products as they are designed to simplify spinal surgery and facilitate treatment of difficult indications.
Our implants are very effective at retaining or expanding a existing clinician base. In addition, we can provide direct surgeon-to-surgeon support for Workshops, for the launch phase or for seminars.
