Hailey–Hailey disease (HHD) (OMIM 169600), also known as familial benign chronic pemphigus, is a rare autosomal dominantly inherited dermatosis. It is characterized by suprabasal separation (acantholysis) of the epidermis resulting in recurrent blisters, crusted erosions, and warty papules that occur mainly on the body folds, particularly the groin, axillary region, the neck, perianal, and submammary region. Painful erosions in the intertriginous areas may become hypertrophic and malodorous, and even mild disease can affect the quality of life . In occasional cases, the lesions may develop into squamous cell carcinomas . Additionally, liver injury and contact allergies have also been described in patients with HHD .
Skin cancer is the most common form of cancer in the United States among Caucasians. DNA damage induced by ultraviolet (UV) exposure has been shown to be involved in the etiology of melanoma, squamous cell carcinoma (SCC), and basal cell carcinoma (BCC) . Under normal conditions, UV-induced DNA damage is repaired by DNA repair mechanisms, or cells with damaged DNA undergo apoptosis . Apoptosis can be triggered by one of the two pathways: an intrinsic pathway or an extrinsic pathway. In the extrinsic pathway, a death-ligand binds a death-receptor that is displayed on the cell surface of the damaged cell. FAS ligand (FASL [also known as CD95LG and TNFSF6]) is one such death-ligand, and FAS (also known as CD95, APO-1, and TNFRSF6) is the corresponding death-receptor. Without the ability to un...

Skin aging is the result of both intrinsic and extrinsic factors. Extrinsic aging is mainly due to photo-damage caused by ultraviolet (UV) irradiation. A characteristic feature of photo-damaged skin is the deterioration of the dermal connective tissue. On the histological level fragmentation of interstitial collagens is observed which substantially contributes to the reduction of skin's tensile strength . While young dermal fibroblasts exhibit a sufficient capacity to adequately maintain the homeostasis of the extra cellular matrix (ECM), (photo)-aged fibroblasts not only display a decrease of their synthetic activity but are also reduced in number . Additionally, the interaction between fibroblasts and the surrounding ECM is impaired. As a consequence, the feedback-loop between matrix and...

X-linked ichthyosis (XLI) and ichthyosis vulgaris (IV) are the most prevalent skin disorders of cornification in humans and share a similar phenotype of generalized fine scaling of the skin . XLI is due to deletions/mutations in the steroid sulfatase (STS) gene on Xp22.32 . A lack of STS, secreted from lamellar bodies into the interstices of the horny layer, correlates with the preservation of corneodesmosomes and impaired permeability barrier homeostasis . IV is caused by mutations in the filaggrin (FLG) gene on 1q21–23 . The resulting absence/decrease of the precursor protein profilaggrin, the main constituent of the keratohyalin granules, and incomplete proteolytical cleavage of profilaggrin to FLG monomers perturbs epidermal barrier formation and stratum corneum hydration . Recently,...

Conclusion: The number of ASCs per unit of tissue weight decreased in aged mice. However, the cells possessed proliferation and differentiation potentials almost equal to those of young mice even though the differentiation potentials showed a tendency of decrease. These results raise the possibility that stem cell functions, self-renewal and multipotency, are maintained regardless of aging. (Source: Journal of Dermatological Science)

Abstract: Atopic dermatitis (AD) can be categorized into the extrinsic and intrinsic types. Extrinsic or allergic AD shows high total serum IgE levels and the presence of specific IgE for environmental and food allergens, whereas intrinsic or non-allergic AD exhibits normal total IgE values and the absence of specific IgE. While extrinsic AD is the classical type with high prevalence, the incidence of intrinsic AD is approximately 20% with female predominance. The clinical features of intrinsic AD include relative late onset, milder severity, and Dennie-Morgan folds, but no ichthyosis vulgris or palmar hyperlinearity. The skin barrier is perturbed in the extrinsic, but not intrinsic type. Filaggrin gene mutations are not a feature of intrinsic AD. The intrinsic type is immunologically char...

The Pantheon in Rome survives today as one of the triumphs of Western architecture. It was erected between 120 and 124 AD by the Emperor Hadrian as a temple dedicated to the multiple gods of Rome. An eighteenth century depiction of it, in masterful perspective, by Giovanni Paolo Panini, now in the National Gallery of Art in Washington, DC graces the cover of this unique tribute to the “deities” of dermatology. It is fittingly entitled Pantheon der Dermatologie. (Source: Journal of the American Academy of Dermatology)

To the Editor: A 1-year-old white male with a 6-month history of “raised bumps” overlying the dorsal surface of his neck was brought to our dermatology clinic for evaluation. An agminated patch of white papules was identified on the suboccipital neck (). Nuchal ultrasound was unremarkable. A 3-mm punch biopsy demonstrated spindle cell proliferation within the papillary dermis, without significant atypia (, A). Vascularity was slightly increased. The overlying epidermis was unremarkable. Laboratory tests for Ulex europaeus, S-100, CD68, muscle-specific actin, and smooth muscle actin were negative. The spindle cells and visualized vascular structures stained strongly positive for CD34 (, B). After a thorough Web-based literature search, we were unable to find any reports of CD34+ stainin...

To the Editor: A 57-year-old African American male with a history of a seizure disorder caused by a traumatic head injury presented to the emergency department with a recent onset of multiple pustular lesions that started on his face and spread to his trunk and upper extremities. He had presented to the same emergency department 5 days earlier in status epilepticus. He was treated with 1 g phenytoin for the first time in addition to increased doses of his other medicines (lorazepam and levetiracetam). His seizure resolved and he was discharged 2 days later on increased doses of levetiracetam and valproic acid. (Source: Journal of the American Academy of Dermatology)

To the Editor: Nutritional deficiency, a prevalent problem worldwide, remains rare in developed countries. In the United States, acquired nutritional deficiencies have been reported in patients with anorexia nervosa, malabsorption syndromes, those on long-term parenteral nutrition, and patients with food allergies. Given the significant morbidity and sometimes mortality associated with certain nutritional deficiencies, their prompt recognition, diagnosis, and treatment by clinicians is of great importance. Many nutritional deficiencies have classic cutaneous presentations that are valuable clinical diagnostic tools. However, combined nutritional deficiencies often put forth a mixed clinical picture easily mistaken for another condition. There is a paucity of reports of cutaneous presentati...

To the Editor: Myxoinflammatory fibroblastic sarcoma (MIFS) is a slow growing, low grade tumor reported as a soft tissue tumor affecting the distal extremities. It was first described in 1998 by Meis-Kindblom and Kindblom and Montgomery et al. Although it was described by different names—acral MIFS and inflammatory myxohyaline tumor of distal extremities with virocyte or Reed-Sternberg–like cells—it described the same entity of histologic features. The reported histologic similarities included numerous inflammatory cells (plasma cells, granulocytes, eosinophils, and lymphocytes), fibrosis, and scattered, large, bizarre tumor cells resembling Reed-Sternberg–like cells in an abundant myxoid extracellular matrix. Clinically, it is described in all age groups (4-91 yrs of age), occurs...

To the Editor: We read with interest the article by Rose et al in the August 2009 issue of the Journal. Ultraviolet B light phototherapy (UVB) is generally considered the safest form of systemic psoriasis therapy in pregnancy. We commend the authors for creating a study to examine the photodegradation of serum folate after exposure to narrowband UVB (nbUVB), given that folate deficiency in the first trimester of pregnancy could predispose to the development of neural tube defects.
To the Editor: The International Sentinel Node Society (ISNS) opinion, that sentinel node biopsy (SNB) “is standard of care,” is not supported by clinical evidence and warrants scrutiny. Their commentary contradicts recent multidisciplinary Australian guidelines that state that “Patients with a melanoma greater than 1.0 mm in thickness be given the opportunity to discuss SNB to provide staging and prognostic information” but deliberately do not state that SNB should be considered standard of care.
To the Editor: I read with interest the article by Nijsten et al in the January 2009 issue of the Journal that describes three minimally invasive techniques in the treatment of saphenous varicose veins: foam sclerotherapy, endovenous laser therapy, and radiofrequency ablation. Another procedure—the 3S saphenectomy (the 3S technique)—should be added to theses techniques. The 3S technique associates the surgical sectioning and sclerotherapy of saphenous veins under local anesthesia. It has been developed in France mainly by Vin et al. Many angiophlebologists learned this technique during the educational program of the European School of Phlebology in Paris. It is useful in the following instances: (1) when surgery is not recommended because of an advanced age and/or in case of a leg ul...

To the Editor: Choosing appropriate, safe treatment for pregnant patients with psoriasis can be challenging. Although tumor necrosis factor–alfa (TNFα) antagonists are considered category B drugs according to the US Food and Drug Administration (FDA), Carter et al reported a disturbing association of infliximab and etanercept with congenital anomalies associated with the vertebral, anal, cardiovascular, tracheoesophageal, renal, limb (VACTERL) syndrome. In their review of more than 120000 adverse events of patients on TNFα antagonist therapy, they found 61 congenital anomalies occurring at higher than expected frequencies. No reports of congenital anomalies were reported in patients taking adalimumab; however, the database included entries only through 2005, which was relatively soon a...

We report herein a case of EBV-positive LELCS. An 82-year-old female presented with a red nodule on the right cheek. Histologically, the entire dermis was occupied by atypical tumor cell nests with dense lymphocytic infiltration. Neoplastic cells were strongly positive for cytokeratin 14 but were negative for cytokeratins 19 and 20. EBV genomes in neoplastic cells were detected by polymerase chain reaction analysis and in situ hybridization for EBV-encoded RNA, suggesting an association with EBV.
We present a report of a 63-year-old man with AIDS who was diagnosed with coexistent Kaposi sarcoma (KS), Cryptococcus neoformans, and Mycobacterium avium intracellulare in a single, solitary lesion. A literature review identified 11 cases of patients with AIDS and cutaneous lesions with co-existent KS and other infectious organisms. To our knowledge, this is the first report of coexistent KS, C neoformans, and M avium intracellulare in the same cutaneous lesion in a patient with AIDS.
Pyoderma gangrenosum is a rare and often painful skin disease that can be unpredictable in its response to treatment. There is currently no gold standard of treatment or published algorithm for choice of therapy. The majority of data comes from case studies that lack a standard protocol not only for treatment administration but also for the objective assessment of lesion response to a specific therapy. This review provides an update to the treatment of pyoderma gangrenosum with a particular focus on new systemic therapies. (Source: Journal of the American Academy of Dermatology)

Prospective controlled clinical and histopathologic study of hidradenitis suppurativa treated with the long-pulsed neodymium:yttrium-aluminium-garnet laser

Conclusions: Long-pulsed neodymium:yttrium-aluminium-garnet laser, together with topical benzoyl peroxide and clindamycin, is significantly more effective than topical benzoyl peroxide and clindamycin alone for the treatment of HS. Preliminary review of histopathology suggests the mechanism of action is destruction of the hair follicle. The overall success of the treatment in both clearing pre-existing lesions and preventing new eruptions, coupled with high patient satisfaction, makes the neodymium:yttrium-aluminium-garnet laser a promising treatment advance for this highly disabling condition.
There are a number of systemic disorders that can manifest with urticarial skin lesions, including urticarial vasculitis, connective tissue diseases, hematologic diseases, and autoinflammatory syndromes. All of these conditions may enter into the differential diagnosis of ordinary urticaria. In contrast to urticaria, urticarial syndromes may manifest with skin lesions other than wheals, such as papules, necrosis, vesicles, and hemorrhages. Lesions may have a bilateral and symmetrical distribution; individual lesions have a long duration, and their resolution frequently leaves marks, such as hyperpigmentation or bruising. Moreover, systemic symptoms, such as fever, asthenia, and arthralgia, may be present. The most important differential diagnosis in this group is urticarial vasculitis, whi...

MedWorm Message: Get the very latest Swine Flu news via the MedWorm Swine Flu RSS news feed - updated hourly from thousands of authoritative health and news sources.

Acute urticaria is self-limiting, and a cause can be identified in many patients. Chronic urticaria is a long lasting disease, and patients are commonly examined for an autoimmune origin and for associated diseases. Although the diagnosis of urticaria is straightforward in most patients, it may pose some difficulties at times and it may require a careful differential diagnosis with a number of conditions. Urticarial syndromes comprise both cutaneous and systemic disorders. Part I of this two-part series focuses on the clinical and histologic features that characterize common urticaria and on the cutaneous diseases that may manifest with urticarial lesions and must be considered in the differential diagnosis.Learning objectives: After completing the learning activity, participants should be...

Conclusions Because long-term survivors of regionally advanced melanoma, although rare, do exist, candidates for HILP should be warned of the risk of long-term development of nonmelanoma secondary cancers. The risk-benefit balance of high-dose local chemotherapy should be carefully evaluated in the light of these findings, especially in patients with early-stage melanoma or other non–life-threatening medical conditions.
Conclusions The often unremarkable clinical presentation of NM necessitates physicians and patients to be wary of new or changing lesions. Dermoscopy may help increase suspicion in early NM because dermoscopic features are typically more suggestive of malignancy than clinical ones. We hope that secondary prevention efforts combined with prompt dermatologic consultations will allow for the timely diagnosis and management of NM. (Source: Archives of Dermatology)

Conclusions We suggest that dermatomyositis-like eruption and hydroxyurea-associated squamous dysplasia represent similar clinical manifestations of a common underlying chronic phototoxic process involving aberrant keratinocyte p53 expression mediated by hydroxyurea's antimetabolite properties and UV radiation exposure. Accordingly, we suggest that dermatomyositis-like eruption, previously considered a benign entity, may represent a premalignant precursor of hydroxyurea-associated nonmelanoma skin cancers warranting discontinuation of hydroxyurea therapy.
Conclusions We recommend surveillance for skin cancer formation in all patients who require long-term voriconazole treatment, particularly those who manifest signs or symptoms of photosensitivity or chronic photodamage. Further study of the mechanism underlying voriconazole photosensitivity and oncogenesis is warranted.

Conclusions The number of skin cancers in Medicare beneficiaries increased dramatically over the years 1992 to 2006, due mainly to an increase in the number of affected individuals. Using nationally representative databases, we provide evidence of much higher overall totals of skin cancer diagnoses and patients in the US population than previous estimates. These data give the most complete evaluation to date of the underrecognized epidemic of skin cancer in the United States. (Source: Archives of Dermatology)

Conclusions My incidence-based model indicates that the prevalence of a skin cancer history is about 5 times higher than that of breast or prostate cancer and greater than the 31-year prevalence of all other cancers combined. Despite their high frequency, population-based incidence and burden data for BSC and SCC are largely lacking. (Source: Archives of Dermatology)

Conclusions Heterogeneous changes in melanoma occur at the molecular level, and the changes are different among melanoma subtypes. Clonal abnormalities in chromosome 6 with increased copies of the short arm relative to the long arm are common in all melanoma subtypes, suggesting that isochromosome 6 is common in all variants of cutaneous melanoma subtypes. An increase in copy number of 11q13 is most frequent in chronically sun-damaged melanomas.
Conclusions Melanoma survivors have an approximately 9-fold increased risk of developing subsequent melanoma compared with the general population. The risk remains elevated more than 20 years after the initial melanoma diagnosis. This increased risk may be owing to behavioral factors, genetic susceptibility, or medical surveillance. Although the percentage of subsequent primary melanomas thicker than 1 mm is lower than for the first of multiple primary melanomas, it is still substantial. Melanoma survivors should remain under surveillance not only for recurrence but also for future primary melanomas and other cancers. (Source: Archives of Dermatology)

Conclusions We suggest an individualized surveillance plan, with digital dermoscopy performed at follow-up intervals of 3 months for patients with FAMMM syndrome and 6 to 12 months (depending on additional risk factors) for those with AMS. Patients with multiple common nevi and no additional risk factors had no benefit from sequential digital dermoscopy. (Source: Archives of Dermatology)

Conclusions Among melanoma seen at a tertiary referral center, histopathologic misdiagnosis is more common for melanomas that have been assessed with punch and shave biopsy than with excisional biopsy. Regardless of biopsy method, adverse outcomes due to misdiagnosis may occur. However, such adverse events are more commonly associated with punch biopsy than with shave and excisional biopsy. The use of punch and shave biopsy also leads to increased microstaging inaccuracy. (Source: Archives of Dermatology)

We describe two exceptional collision tumors, namely: a 63-year-old woman revealing a melanocytic tumor within a trichoblastoma and a 71-year-old woman with a squamous cell carcinoma colonized by dendritic cells of a melanoma. Both neoplasms showed two different tumor components with intimate relationship. The lesions are labeled in a "playful" way with the acronyms METRO (MElanocytic tumor +TRichOblastoma) and CAMEL (CArcinoma +MELanoma) to facilitate memorization.
The subcutaneous infiltration of liquid paraffin is still used for penile enlargement. The procedure has many complications. A late problem is the development of foreign body granulomas known as paraffinomnas. They may be necrotic and ulcerated, requiring acute, radical surgical excision. (Source: JDDG)

Conclusion: The study has shown the majority of negative re-excisions were from lesions on the head which had had an initial surgical procedure. However, the evidence is not strong enough to advocate a protocol for dealing with positive margins. A larger sample size that encompassed all three factors that affect outcome, that is, the location of lesion, type of lesion and type of excision carried out, would be required in order to make a more definitive statement on protocol change for treatment of basal cell carcinoma.
Conclusions 4-MU is a potent inhibitor of hyaluronan synthesis, induction of stromal hyaluronan accumulation by tumour cells, and fibroblast and melanoma cell proliferation, and results suggest that 4-MU may have potential as a tumour cell anti-invasive and antiproliferative agent.
In this study, the immediate effects of PCI on the skin were assessed, including the systemic inflammatory response and the production and gene expression of transforming growth factor (TGF) isoforms [beta]1, [beta]2 and [beta]3. Eighty rats were split into four groups: group 1 (n = 24; PCI plus skin care); group 2 (n = 24; skin care only); group 3 (n = 24; PCI only) and group 4 (n = 8; controls). Microarray analysis showed that TGF-[beta]3, an essential marker for preventing scarring, was upregulated and expressed for 2 weeks postoperatively. PCI might offer a regenerative therapy to improve skin appearance and quality and to improve or even prevent scarring.
We report a unique presentation of this syndrome in a 25-year-old male patient with prominent oral findings. This is a sporadic case, started during early childhood, progressively increasing in number and size. Oral vascular lesions were part of gastrointestinal involvement. Associated cardiac abnormalities were also observed. An early diagnosis of this syndrome is required as it gets complicated with bleeding, anemia and other systemic complications.
We report a 22-year-old man who developed severe inflammatory and keloidal, moderately bullous allergic reaction after henna paint-on tattoo. We did a patch test separately with these painting products (henna powder, PPD) and with the European standard series. PPD was strongly positive (+++) on day 2 and remained positive for following days. After treating with topical clobetasol-17 butyrate, resolution was obtained in two weeks. But some keloidal reaction remained.
Conclusions: Anti-nuc antibody detection could be a better tool for the diagnosis of SLE. Although there was no significant difference in LN and SLE without LN groups, this study suggests that anti-nuc detection can be useful as an additional disease activity marker to other laboratory tests. Antiphospholipid antibodies are a family of autoantibodies that recognize various combinations of phospholipids, phospholipid-binding proteins, or both. APS can occur in the absence of underlying or associated disease (primary APS) or in combination with other diseases (secondary APS). The exact pathogenic mechanism by which these antibodies cause thrombosis is not known; however, several hypotheses, such as activation of platelet and endothelial cells and interference with the coagulation system, have been proposed.
