Burns Patients Awaiting Skin Grafts: Human Embryonic Stem-Cells Could Be Used To Generate Temporary Skin

A study in this week's edition of The Lancet reports that the use of human embryonic stem-cells (hESCs) is a promising alternative for producing temporary skin substitutes for patients awaiting skin grafts after, for example, serious burn injuries. The article is the work of Dr Christine Baldeschi, INSERM and Institute for Stem Cell Therapy and Exploration of Monogenic Diseases, Evry Cedex, France, and colleagues.
Conclusions We report here the largest series to date of French patients with BHDS. We noted a high prevalence of thyroid nodules and renal cysts. However, the lack of a control group does not allow assessment of whether or not such association with BHDS is fortuitous. (Source: British Journal of Dermatology)

This is the first new book devoted to epidermolysis bullosa (EB) to hit the shelves since Jo-David Fine's masterpiece that came out about a decade ago; the book is packed with epidemiologic data from the National EB Registry in the United States, which he founded.
When asked to review this book, I wondered why someone would write another “general dermatology,” book and I further wondered how I could sit down and read another textbook of dermatology. Then I read the forward. The purpose of the series is to present topics—in this case, diseases—with an emphasis on the key points essential for residents and practicing clinicians. Their purpose has been fulfilled. (Source: Journal of the American Academy of Dermatology)

To the Editor: Circumscribed palmar or plantar hypokeratosis (CPH) is a new entity recently described by Perez et al in 2002. To the best of our knowledge, only a few cases have been reported in the literature. It may be underdiagnosed, because most of the cases are asymptomatic, and sometimes it is misdiagnosed as palmar porokeratosis or Bowen disease. A 47-year-old woman presented to our department with an asymptomatic, solitary, circumscribed patch of depressed skin on the left thenar palm that had been present for approximately 10 years. There was no history of trauma or other notable incident. Her medications consisted of hydroxychloroquine, 200 mg daily, 5 days a week, for chronic cutaneous lupus. The physical examination revealed a 1 × 1 cm, annular, well demarcated erythematous de...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

To the Editor: A 5-day-old female was seen in consultation by the pediatric dermatology service for a bluish discoloration over the right medial infraorbital area. This resembled a bruise at 2 days of age. On the fifth day of life, a gradually enlarging nodule was noted in the same area that caused some distortion of her lower eyelid. On examination, there was a 9- × 7-mm, very firm blue nodule present inferior to the right medial canthus (). There was no purulent drainage. The differential diagnosis included a deep infantile hemangioma or a midline developmental defect; however, a diagnosis of dacryocystocele was favored because of the classic location and presentation within several days of birth. The pediatric dermatologists recommended an ultrasound and ophthalmologic evaluation, but ...

To the Editor: Heald et al described seven patients with what they called persistent agmination of lymphomatoid papulosis (LyP) and classified them as a distinct clinical subset within CD30+ lymphoproliferative disorders. Patients had waxing and waning papules and nodules within a persistent erythematous and scaly plaque, histopathologically compatible with a LyP, but with a different clinical behavior. We observed a similar case with the same clinical presentation but a distinct clinical evolution. A 27-year-old male without medical history had a 4-month history of several asymptomatic, stable nodules between 10 and 20 mm in size that appeared on the anterior aspect of his right thigh within a 15-cm erythematous, scaly, noninfiltrated patch (). A biopsy of one of the nodules was performed...

To the Editor: Balanitis is an inflammatory condition of the glans penis that may be caused by a wide range of dermatologic conditions, including psoriasis. Psoriatic balanitis is clinically characterized by erythematous, nonscaling plaques most commonly located proximally on the glans and under the prepuce. Most often, genital psoriasis is part of a more generalized disease process, but in the case of exclusively penile involvement, the correct diagnosis may be difficult to make, and several investigations, including a skin biopsy, are often necessary.
To the Editor: In recent years, multiple sources have reported long wait times for new patient appointments in some major metropolitan cities across the United States. However, little information has been collected pertaining to patient perceptions of these wait times and consumer preferences for dermatology visits compared to other providers in terms of access. In the report, Thiboutot et al present combined data from two randomized controlled trials (RCTs) as if the two RCTs were in fact one large RCT. Unfortunately, such an approach is invalid.
To the Editor: In the April 2009 issue of the Journal, Cordoro et al stated that physiologic vascular changes in infants may alter the appearance of some vascular birthmarks; they incorrectly suggested that these changes contribute to the efficacy of pulsed dye laser (PDL) treatment of port wine stains (PWS) in infancy. We have recently shown that high energy PDL of PWS before 6 months of age offers the best chance of eradication of these lesions. Factors related to laser–tissue interactions are far more responsible for this high cure rate than are physiologic changes in the vasculature.
To the Editor: In the June 2009 issue of the Journal, Hexsel et al reported the incidence of nonmelanoma skin cancer (NMSC) in a cohort of patients with vitiligo. In the “Introduction” section, they noted that the frequency of skin cancer in patients with vitiligo seems to be uncommon, with only a few reports in the literature. They identified 477 patients at their hospital with vitiligo, six of whom (42% of the study group) had a NMSC. Four were basal cell carcinomas, and two were squamous cell carcinomas (SCCs). The pathology specimens were stained for the presence of melanocytes. Two of the six cancers arose in vitiliginous skin, the other four in skin containing melanocytes. They calculated the incidence of skin cancer in white patients with vitiligo and concluded that NMSC is as c...

To the Editor: The Organizing Committee of the 6th Biennial International Sentinel Node Society (ISNS) Meeting in Sydney, Australia, held from February 18-20, 2008, refused to allow any presentations from the podium that did not support sentinel node biopsy (SNB) in melanoma. The members of the international panel who prepared the subsequent “consensus statement” all represented the same opinion. All but one of the references cited in the May 2009 commentary by Balch et al in the Journal were written by the authors themselves. As a result, it is my opinion that both the commentary and the consensus statement are biased.
Congenital dermatofibrosarcoma protuberans (DFSP) is a rare tumor with varying clinical presentations that is commonly misdiagnosed. Treatment of congenital DFSP is complicated by delays in diagnosis and its propensity for subclinical spread. Of 61 reported cases, 11 (18%) were treated with Mohs micrographic surgery (MMS) and 46 (75%) were treated with wide local excision (WLE). One case was treated with imatinib, and the remaining 3 did not differentiate between receiving MMS or WLE. In the cases of congenital DFSP treated with MMS the clearance rate was 100% with an average follow-up of 4.3 years. The clearance rate seen with WLE was 89% with an average follow-up period of 1.9 years. The average margins taken during MMS (1.7 cm) were smaller than those taken with WLE (2.8 cm). Fifty perc...

This review will discuss the role of angiogenesis in specific cutaneous diseases. Scientific evidence now points to the role of angiogenesis in tumor development and many other cutaneous disorders. Angiogenesis is a complex process that involves angiogenic growth factors and inhibitors, many of which could be a potential target for pharmacologic intervention. Antiangiogenic agents have recently been applied to dermatologic diseases with promising efficacy.Learning objectives: After completing this learning activity, participants should be able to recognize cutaneous diseases where angiogenesis is likely to be an important factor, recognize scenarios where angiogenic therapy may be useful in conjunction with traditional therapies, and be able to use angiogenic-mediating agents in the treatm...

Angiogenesis is an important process in normal physiology and disease pathogenesis. Angiogenesis is controlled in a healthy body by a system of angiogenic growth factors and angiogenesis inhibitors. When angiogenic growth factors are predominantly expressed, blood vessel growth occurs and disease may result. Successful therapies have been developed that target growth factors, their receptors, or the cascade pathways that are activated by growth factor/receptor interactions. There is good evidence that angiogenesis plays an important role in a wide range of cutaneous maladies, and angiogenesis-targeting therapies are playing an increasing role in the management of dermatologic disease. Cutaneous angiogenesis offers an exciting new arena for targeted dermatologic therapeutics.Learning object...

Conclusions Our results confirm that phototherapy is a safe, effective and well-tolerated, first-line therapy in patients with early-stage cutaneous T-cell lymphoma, with prolonged disease-free remissions being achieved. It suggests that narrowband UVB is at least as effective as PUVA for treatment of early-stage MF. (Source: Journal of the European Academy of Dermatology and Venereology)

Conclusion Specific dermatoses as skin tags, striae distensae and plantar hyperkeratosis, could be considered as a cutaneous stigma of severe obesity. The low permeability of the skin to evaporative water loss is observed in obese subjects compared with normal weight control. Although the physiological mechanisms are still unknown, this finding has not been previously described and we believe that this may constitute a new field in the research on obesity.
Conclusions The most frequent cutaneous side effect reported in this series was acneiform eruption. The authors observed that all women with acneiform rash had only limited facial involvement, whereas all but one man experienced more widespread lesions of the face, the back and the chest. We found no association between the extent and severity of cutaneous eruptions (grade 1 vs. grade 2) and patients' response to therapy.
Please cite this paper as: A simple assay for the study of human hair follicle damage induced by ionizing irradiation. Experimental Dermatology 2009.Abstract: Due to its rapidly proliferating matrix keratinocytes, the hair follicle is highly sensitive to ionizing irradiation (IR)-induced skin damage and thus an instructive and clinically relevant model organ for investigating the effects of IR on rapidly dividing epithelial-mesenchymal interaction systems. Here, we have assessed the impact of IR on organ-cultured human scalp hair follicles. We show that IR significantly inhibits the proliferation and induces apoptosis of hair follicle matrix keratinocytes, disrupts normal hair follicle pigmentation, and upregulates a number of quantitative toxicity and viability markers (oxidative stress i...

Please cite this paper as: The mineralocorticoid receptor as a novel player in skin biology: beyond the renal horizon? Experimental Dermatology 2009.Abstract: The mineralocorticoid receptor (MR) and its ligand aldosterone regulate renal sodium reabsorption and blood pressure and much knowledge has been accumulated in MR physiopathology, cellular and molecular targets. In contrast, our understanding of this hormonal system in non-classical targets (heart, blood vessels, neurons, keratinocytes[hellip]) is limited, particularly in the mammalian skin. We review here the few available data that point on MR in the skin and that document cutaneous MR expression and function, based on mouse models and very limited observations in humans. Mice that overexpress the MR in the basal epidermal keratino...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

As soon as uveal melanoma has metastasized to the liver, response rates to systemic chemotherapy are low. It can be improved by development of special locoregional procedures. A 24-year-old woman suffered from inoperable hepatic metastases which grew to life-endangering size despite both systemic chemotherapy with gemc-itabine/treosulfan and conventional intrahepatic chemoembolization with fotemustine and starch particles. We subsequently performed two angiographic C-arm CT-guided, superselective chemoembolizations of the hepatic arteries feeding the tumor, using cisplatin, starch microspheres and ethiodized oil. Following this treatment, no vital tumor tissue was detectable by MRI. This remission lasted for more than 6 months and the patient's quality of life was good. A subsequent local ...

People who received daily text messages reminding them to apply sunscreen were nearly twice as likely to use it as those who did not receive such messages, a new study led by a UC Davis Health System dermatologist has found. Researchers hope their findings, which appear in the November issue of the Archives of Dermatology, will inspire other health-care providers to use text messaging to encourage healthy habits in their patients, such as taking prescribed medications properly.
Researchers from the universities of Bristol and St. Andrews in the UK have found that the color of a person's skin affects how healthy and therefore attractive they appear, and have found that diet may be crucial to achieving the most desirable complexion. The work will be published in the December issue of Springer's International Journal of Primatology. (Source: Dermatology News From Medical News Today)

Quality of life and health-related utility analysis of adults with moderate and severe atopic dermatitis treated with tacrolimus ointment vs. topical corticosteroids

Conclusion Treatment with 0.1% tacrolimus ointment rather than a standard topical corticosteroid ointment regimen was associated with clinically significant, incremental improvement in QoL, sustained over a 6-month period. A within-trial cost-utility estimate based on study medication cost alone suggests that tacrolimus ointment is highly cost-effective given existing willingness-to-pay thresholds.
Background Despite its nature as a systemic vasculitis, renal involvement is known to occur infrequently in Behçet's Disease (BD).Objectives Our aim was to investigate proteinuria, microhematuria and microalbuminuria in 24-h urine and evaluate subclinical or symptomatic renal involvement in BD patients. Methods Two hundred and eleven patients who fulfilled the International Behçet's Disease criteria were included in the study. After urine analysis, five of 12 patients who were found to have proteinuria underwent renal biopsy, while 199 patients without proteinuria were investigated for microalbuminuria (MA).Results A total of 34 (16.1%) patients were found to have renal involvement including 22 (11.1%) with MA and 12 with proteinuria (5.6%). Renal biopsies resulted as focal glomeruloscler...

Leprosy is usually well-controlled by multidrug therapy (MDT). However, in case of noncompliance or leprosy reactions, it may present a therapeutically challenge. A 33-year-old Brazilian woman with lepromatous leprosy was treated with MDT for one year, but then discontinued therapy because she wanted to have children. Eight weeks after stopping her medications, she developed a severe and recalcitrant erythema (nodosum) leprosum (ENL) which presented histologically with thrombosed small veins and neutrophilic inflammation in fat septa, but without arteritis. During her pregnancy and ensuing lactation period, glucocorticoids were the only suitable drug. With the use of the shortened WHO/MDT regimen (one year vs. two years of treatment), ENL will probably be seen more often after the end of l...

Conclusions&nbsp; The potential association between a specific genetic mutation and pigmentation changes secondary to imatinib therapy may account for the variety in presentation of this potential side effect. Further genetic studies paired with melanocyte-specific or c-kit&ndash;specific stains of affected tissue are warranted to better understand the relationship between the genetic mutation and the effect of imatinib on pigmentation.
Conclusions&nbsp; There are few publications that document PXE-like changes in association with various inflammatory skin conditions. Thus, the clinical significance of this finding remains uncertain. This case and previous reports are discussed in the context of current molecular and genetic knowledge. It is hoped that greater awareness of this phenomenon will promote further investigation and elucidation of the clinical and biologic significance of PXE-like changes observed in biopsies of inflammatory skin disorders.
Conclusions&nbsp; The burden of disease at sites other than the head and neck appears to be underreported in the literature and greatly affects quality of life. Differentiation between the clinical diagnoses has little prognostic or clinical utility in genetic counseling, even within individuals from the same family. Thus, we suggest an encompassing diagnosis of "CYLD cutaneous syndrome." Finally, the clinical distribution of tumors suggests that hormonal factors may play an important role in tumor induction in these patients.
Conclusions&nbsp; Elevated D-dimer level is highly specific for VMs (pure, combined, or syndromic), and therefore this easy and inexpensive biomarker test should become part of the clinical evaluation of vascular anomalies. It can detect hidden VMs and help differentiate glomuvenous malformation (normal D-dimer levels) from other multifocal venous lesions. Elevated D-dimer level also differentiates a VM from a lymphatic malformation. Moreover, slow-flow Klippel-Trenaunay syndrome (capillaro-lymphatico-venous malformation with limb hypertrophy) can be distinguished from fast-flow Parkes Weber syndrome (capillary malformation with underlying multiple microfistulas and limb hypertrophy). For these reasons, D-dimer level measurement is a useful complementary tool for diagnosing vascular anomal...

Conclusions&nbsp; Despite awareness of the benefits of sunscreen, adherence is low, even in this population, for whom adherence was knowingly monitored. Short-term data demonstrate that using existing cellular telephone text-message technology offers an innovative, low-cost, and effective method of improving adherence to sunscreen application. The use of ubiquitous communications technology, such as text messaging, may have implications for large-scale public health initiatives.

Dupuytren's contracture (Dupuytren contracture) is a condition that affects the hands and fingers. It is an uncommon hand deformity in which the connective tissue under the skin of the palm contract and toughen over time. It causes one or more of the fingers on one or both hands to bend into the palm of the hand. Dupuytren's contracture is named after the French surgeon, Baron Guillaume Dupuytren, who first described and researched the condition in 1834. The authors of this article discuss the uncertainties surrounding the use of bath emollients as part of the treatment of atopic eczema, which the NHS in England spent around £15.5m on in 2008.
&nbsp;
They were unable to identify any published RCTs assessing the clinical efficacy of bath emollients in atopic eczema from a search of the literature.&nbsp;&nbsp; In addition they state that they are not aware of any longstanding clinical experience of benefit from bath emollients to match that for directly applied emollients, and that the quantities of emollient deposited on the skin during bathing are likely to be far lower than with directly applied emollients.&nbsp; They conclude that "based on current evidence, bath emollients could be offering little or no bene...

Conclusions The prevalence of BRAF V600E in AGN suggests that ultraviolet exposure is not essential for generating the mutation. The BRAF V600E mutational status appears to be of limited diagnostic utility in distinguishing genital naevi that exhibit atypia from those that do not. Similar to oncogenic BRAF-positive common naevi without atypia, enhanced expression of the tumour suppressor IGFBP7 in oncogenic BRAF-positive AGN supports that they are biologically inert.
To determine the efficacy and safety of the 585-nm pulsed dye laser (PDL) in the treatment of recalcitrant warts in children. Retrospective survey of the medical records of children with recalcitrant warts who were treated with PDL between March 1995 through January 1999 at the Children's Memorial Hospital outpatient subspecialty center, Chicago, Illinois. Sixty-one children with recalcitrant warts were treated with PDL; 75% of them had total clearance of warts after an average of 3.1 treatment sessions. Overall success rates were 100% for both perineal and perianal and face-only warts, 93% for hands, 69% for plantar warts, 67% when both face and extremities were involved, and 60% when multiple extremities were involved. Pain and other side effects were minimal. Mild scarring occurred in 2...

Patients affected by pemphigus vulgaris will occasionally present with associated and characteristic nail changes. This manuscript was prepared as a review of this unique presentation of pemphigus. Articles describing pemphigus vulgaris of the nail were compiled and reviewed and pertinent information was extracted to provide a concise analysis. The literature on this topic is still maturing. Publications to date suggest the incidence of nail and periungal involvement may correlate with the severity of mucocutaneous symptoms and the duration of the disease. The most common nail manifestations reported were acute or chronic paronychia and onychomadesis, and involvement of the fingernails. The nail changes may precede or develop concurrently with the mucocu...

We present a retrospective study of 28 patients, admitted to Hospital de S&#xE3;o Jo&#xE3;o, EPE, between January of 1990 and June of 2008. Pyoderma gangrenosum was observed more frequently in middle age adults, in the ulcerated form, and predominantly localized in the lower limbs. Pediatric presentation was very rare. In 50 percent of these cases a systemic disease was present and the most frequent was Crohn disease (14% of patients).

We present perspectives from four fourth year medical students who matched into dermatology that highlight the factors they believed helped them most. The purpose is to offer advice to medical students interested in dermatology. We divide the paper into four areas of discussion: academics, extracurricular activities, research, and mentorship. All four factors are crucial for a strong dermatology application. We believe the paper provides valuable suggestions and guidance to students considering a career in dermatology.

We describe a patient with fatal autoimmune blistering disease with absence of mucous membrane lesions. The pattern of complement indirect immunofluoresence helped identify the prognosis prospectively. This case illustrates yet another presentation of the neoplasia-induced autoimmunity.

We describe three cases of the acquired PXE-like syndrome that often occurs in association with hemolytic anemias, in particular the hemoglobinopathies, and review the literature on the subject. The pathogenesis of the acquired PXE-like lesions is not yet completely understood. None of the mutations observed in the inherited form has been detected in the syndrome accompanying thalassemia. The cardiovascular complications could be life-threatening. Therefore, an close surveillance of these patients is mandatory.

We present two cases that were clinically diagnosed as mucoceles that were removed because of patient anxiety. However, histological examination revealed dissimilar and medically significant pathologies. Oral cysticercosis and salivary gland adenocarcinoma are rare diseases of the oral mucosa. It is important to consider these and other uncommon diagnoses when evaluating an oral nodule presumed to be a mucocele. We will discuss the natural history and pathogenesis of these disease processes.

We report a keratoacanthoma arising in a recent tattoo. A 60-year-old woman presented with a tumor on her right leg, over an area of red pigment in a professional tattoo. Histological analysis confirmed the clinical diagnosis of keratoacanthoma. Although the association between malignancy and tattoos is very uncommon dermatologists and dermatologic surgeons should be attentive to this possibility.

We describe a 50-year-old woman with a 5-month history of multiple asymptomatic papulonodular lesions on the left chest area. Biopsy was consistent with cutaneous metastases from a ductal breast carcinoma. No distant metastatic lesions were detected. The patient was referred to the Gynecologic Oncology Department. Treatment included chemotherapy, radiotherapy and surgery. At present the patient is well with no signs of recurrence. This case reports a clinically remarkable cutaneous metastatic breast carcinoma.
