Stefan Schilling presents his study results in Deutsches Arzteblatt International (Dtsch Arztebl Int 2012; 109[3]: 33-8). In order to establish the vitamin D status in geriatric patients in Germany, the researchers measured 25-OH vitamin D in 1578 patients in the geriatric rehabilitation hospital in Trier after they had been examined on admission. Insufficiently high concentrations were found in 89% of patients, and 67% had severe vitamin D deficiency...
Conclusions Compared with never smokers, middle-aged male smokers experienced faster cognitive decline in global cognition and executive function. In ex-smokers with at least a 10-year cessation, there were no adverse effects on cognitive decline. (Source: Archives of General Psychiatry)

Suicidal Thoughts and Behavior With Antidepressant Treatment: Reanalysis of the Randomized Placebo-Controlled Studies of Fluoxetine and Venlafaxine [Meta-analysis]

Conclusions Fluoxetine and venlafaxine decreased suicidal thoughts and behavior for adult and geriatric patients. This protective effect is mediated by decreases in depressive symptoms with treatment. For youths, no significant effects of treatment on suicidal thoughts and behavior were found, although depression responded to treatment. No evidence of increased suicide risk was observed in youths receiving active medication. To our knowledge, this is the first research synthesis of suicidal thoughts and behavior in depressed patients treated with antidepressants that examined the mediating role of depressive symptoms using complete longitudinal person-level data from a large set of published and unpublished studies.
It can be difficult to distinguish between people with normal age-associated memory loss and those with amnestic mild cognitive impairment (aMCI). However people with aMCI are at a greater risk of developing Alzheimer's disease (AD), and identification of these people would mean that they could begin treatment as early as possible. New research published in BioMed Central's open access journal BMC Geriatrics shows that specific questions, included as part of a questionnaire designed to help diagnose AD, are also able to discriminate between normal memory loss and aMCI...
The pain suffered by older adults is the shared focus of the two newest entries in The Gerontological Society of America's (GSA) From Publication to Practice* series. Together they address both pain management and new labeling changes for one of the most popular pain medications, acetaminophen. Both issues aim to provide readers with information on how new advances in pain prevention, treatment, and management may improve care and quality of life for older adults... Abstract
As populations age, the prevalence of geriatric neurodegenerative diseases will increase. These diseases generally are multifactorial, arising from complex interactions among genes, environment, concurrent morbidities, treatments, and time. This essay provides a concept for the pathogenesis of Lewy body diseases such as Parkinson disease, by considering them in the context of allostasis and allostatic load. Allostasis reflects active, adaptive processes that maintain apparent steady states, via multiple, interacting effectors regulated by homeostatic comparators-"homeostats." Stress can be defined as a condition or state in which a sensed discrepancy between afferent information and a setpoint for response leads to activation of effectors, reducing th...

(BioMed Central) It can be difficult to distinguish between people with normal age-associated memory loss and those with amnestic mild cognitive impairment. However people with aMCI are at a greater risk of developing Alzheimer's disease. New research published in BioMed Central's open access journal BMC Geriatrics shows that specific questions, included as part of a questionnaire designed to help diagnose AD, are also able to discriminate between normal memory loss and aMCI.
We examined the GNRI of 120 maintenance HD patients and followed these patients for 120 months. Predictors for all‐cause death were examined using life table analysis and the Cox proportional hazards model. Life table analysis revealed that subjects with a GNRI < 90 (n = 19) had a marginally lower survival rate than did those with a GNRI ≥ 90 (n = 101) (Wilcoxon test, P = 0.048). Multivariate Cox proportional hazards analyses demonstrated that the GNRI was a significant predictor of mortality (hazard ratio 0.966, 95% confidence interval 0.945–0.995, P = 0.018), after adjusting for age, sex, presence of diabetes mellitus, and body weight. These results demonstrate that the GNRI may be a significant predictor of mortality in Korean HD patients. (Source: The...

Area: Evidence > Medicines Management > References
Opioids are the mainstay of treatment for moderate to severe pain. However, opioid therapy in the elderly is often associated with significant morbidity because of excessive ventilatory depression. The large amount of interindividual variability in opioid dose-response relationships makes it difficult to individualise the dose and dosing interval to provide safe and effective analgesia. By examining how aging affects the pharmacokinetics (PK) and pharmacodynamics (PD) of opioids, it is possible to provide a rational basis for age adjustment in opioid dosing.
Authors conclude that 'simulations with PK-PD models confirm the clinical impression that elderly patients n...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Area: Evidence > Medicines Management > References
Objective: To assess the impact of medication reconciliation interventions on medication error rates when elderly patients are discharged from hospital to community care or nursing homes.
Setting: Elderly patients (older than 65 years) in Sweden, living in nursing homes or in their own homes with care provided by the community nursing system.
Method: All medical records containing information on drug treatment were collected from hospital departments, the community care service and GPs. We then identified whether there were any changes in the transfer of information i.e. if the drugs were not the same as before the transfer. Two different persons independently...

Conclusion:  Outpatient GEM could effectively reduce growth of BOM and medical utilization for relatively healthy elderly patients in Taiwan where elderly patients haverelatively free access to general and specialty care. The results of this study implied the potential benefits of systematically implementing GEM in primary health‐care settings to improve the quality of care and slow down the growth of BOM for elderly patients. Abstract
The relevance of endoscopic diagnostic and therapeutic interventions is on a constant rise. However, specific aspects of the treatment of geriatric patients have only been tackled to a rather limited extent so far. The present contribution pinpoints illustratively the status quo on this issue and provides directions for further research which should be conducted in future studies supported by the working group on geriatric gastroenterology.

Conclusions:Geriatric cochlear implant patients have a similar learning curve to the younger adults and in speech tests in quiet show a comparable performance. However their performance is significantly lower in noisy surroundings. This may be due to the central presbycusis in patients older than 70 years and should be taken into account in postoperative fitting of geriatric patients. Further prospective studies are required to evaluate the role of special rehabilitation methods and cognitive training to improve the speech perception in noise in geriatric CI patients.
Conclusions:The present findings indicate that in human infants, CSF hypocretin-1 increases during the third embryonic trimester and is highest at 4 months of life. Thereafter, and consistent with previously published results, hypocretin levels are lower and stable until the geriatric age. This pattern may reflect the role of hypocretin in the dramatic process of sleep and wakefulness consolidation that occurs during early infancy.Citation:Aran A; Shors I; Lin L; Mignot E; Schimmel MS.
Abstract Early intensive therapy in type 2 diabetes can prevent complications. Nevertheless, metabolic control is often sub-optimal
in newly diagnosed patients. This web-based survey aimed to evaluate opinions of physicians about treatment, priorities, and
barriers in the care of patients first referred to diabetes clinics. Data on physician attitudes toward therapeutic preferences
for two clinical case models (same clinical profile, except HbA1c levels of 8.6 and 7.3% at the first access, respectively)
were collected. Participants were asked to rank from 1 (most important) to 6 (least important) a list of priorities and barriers
associated with the care of new patients. Overall, 593 physicians participated. In both case models, metformin and education
were primary options...

Abstract An operational definition of musculoskeletal decline in older people is needed to allow development of interventions for prevention
or treatment, as was developed for the treatment of osteoporosis. Frailty and sarcopenia are linked, but distinct, correlates
of musculoskeletal aging that have many causes, including age-related changes in body composition, inflammation, and hormonal
imbalance. With the emergence of a number of exciting candidate therapies to retard the loss of muscle mass with aging, the
derivation of a consensual definition of sarcopenia and physical frailty becomes an urgent priority. Although several consensual
definitions have been proposed, these require clinical validation. An operational definition, which might provide a threshold

    Abstract
Alzheimer's disease (AD) and age-related macular degeneration (AMD) are both neurodegenerative disorders which share common pathological and biochemical features of the complement pathway. The aim of this study was to investigate whether there is an association between well replicated AMD genetic risk factors and AD. A large cohort of AD (n = 3898) patients and controls were genotyped for single nucleotide polymorphisms (SNPs) in the complement factor H (CFH), the Age-re...

    Abstract
The biological mechanisms that give rise to age-related hearing loss (ARHL) are still poorly understood. However, there is growing recognition that oxidative stress may be an important factor. To address this issue, we measured the changes in the expression of cochlear oxidative stress and antioxidant defense-related genes in young (2 months old), middle-aged (12 months old), and old (21-25 months old) Fischer 344/NHsd (F344/NHsd) rats and compared gene expression changes with ARHL. A quantitative real-time reverse transcription polymerase chain reaction array revealed a significant age-related downregulation of only 1 gene, stearoyl-coenzyme A desaturase 1, and upregulation of 12 genes: 24-dehyd...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

    Abstract
A growing number of reports state that regular exercise enhances brain function in older adults. Recently a functional near-infrared spectroscopy (fNIRS) study revealed that an acute bout of moderate exercise enhanced activation of the left dorsolateral prefrontal cortex (L-DLPFC) associated with Stroop interference in young adults. Whether this acute effect is also applicable to older adults was examined. Sixteen older adults performed a color-word matching Stroop task before and after 10 minutes of exercise on a cycle ergometer at a moderate intensity. Cortical hemodynamics of the prefrontal area was monitored with a fNIRS during the Stroop task. We analyzed Stroop interference (inc...

    Abstract
Purpose of the Study:Our study provides a national portrait of the Baby Boom generation, paying particular attention to the heterogeneity among unmarried Boomers and whether it operates similarly among women versus men. DESIGN AND METHODS: We used the 1980, 1990, and 2000 Census 5% samples and the 2009 American Community Survey (ACS) to document the trends in the share and marital status composition of the unmarried population during midlife. Using the 2009 ACS, we developed a sociodemographic portrait of Baby Boomers according to marital status. RESULTS: One in three Baby Boomers was unmarried. The vast majority of these unmarried Boomers were either divorced or never-married; just 10% were widowed. Unmarried Boomers faced greater economic, healt...

This study examines how Baby Boomers in Quebec, Canada, perceive and play their role as caregivers and how this might differ from their parents' generation. DESIGN AND METHODS: This was a qualitative and empirical study using an interpretive constructivist design. We interviewed 39 Baby Boomers caring for a family member with a semistructured guide that examined respondents' identification with their social generation, their relationship to and values regarding caregiving, and the reality of the caregiving they offered. RESULTS: In contrast to our perceptions of previous generations, the majority of interviewees refuse to be confined to the sole identity of caregiver, as they work to juggle caregiving, work, family, and social commitments. To succeed in this juggling act, they have high ex...

This article argues for the application of this latter approach to research into the lasting impacts of HIV/AIDS on this cohort of gay men. We examine HIV/AIDS mortality within this cohort at the epidemic's height, these deaths' concentration in urban gay communities, and the growing and increasingly diverse population of HIV-positive gay men born in the Baby Boom Years. Our conclusion suggests that a fuller examination of the role of HIV/AIDS in the lives of gay male Baby Boomers, using a life course perspective, is critical to appreciating this generation's heterogeneity and to expanding knowledge of how later life is shaped by the intersection between historical events, personal biography, and social and community ties.

    Abstract
Purpose of the Study:We compare the close family resources of Baby Boomers (BBs) to previous cohorts of older adults at population level and then examine individual-level cohort comparisons of age-related trajectories of informal care availability from midlife into old age. DESIGN AND METHODS: Population data from the U.S. Census and from the Health and Retirement Study (HRS) are used to identify a cohort similar to the BBs on marital status and fertility rates. Using generalized linear mixed models and 10-year longitudinal data from Depression and WWII parents (DWP; n = 1,052) and the parents of BBs (PBB; n = 3,573) in the HRS, we examine cohort differences in the time-varying likelihoods of being married and of having a...

MedWorm Sponsor Message: Find the best January Sales in the UK.

(The Gerontological Society of America) The pain suffered by older adults is the shared focus of the two newest entries in the Gerontological Society of America's "From Publication to Practice" series. Together they address both pain management and new labeling changes for one of the most popular pain medications, acetaminophen. Both issues aim to provide readers with information on how new advances in pain prevention, treatment, and management may improve care and quality of life for older adults. Abstract
Mutations in ATP13A2 cause autosomal-recessive parkinsonism (Kufor-Rakeb syndrome; KRS). Because several other parkinsonism-associated proteins have been connected to mitochondrial function and mitophagy, we studied the impact of endogenous mutations in ATPase type 13A2 (ATP13A2) on mitochondria in fibroblasts from KRS patients compared with controls. In patients, we detected decreased adenosine triphosphate (ATP) synthesis rates, increased mitochondrial DNA levels, a higher frequency of mitochondrial DNA lesions, increased oxygen consumption rates, and increased fragmentation of the mitochondrial network. Importantly, overexpression of wild-type ATP13A2 rescued the respiration phen...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Conclusions: The survey and the focus group findings indicate that there is a strong need for (a) access to a set of nationally recognized competencies for A‐GNPs; (b) teaching/learning resources to support faculty in implementation of A‐GNP programs; and (c) formal faculty development opportunities to facilitate faculty efforts to engage in curricular redesign and innovations and assure that NP students acquire strong competence in gerontology content.Implications for practice: The transition to A‐GNP has implications not only for NP faculty but also for practicing NPs. Almost all NPs care for older adults in a variety of settings. However, most of these NPs are not specialists in geriatrics. This article provides a context for self‐assessment by these NPs of their own needs relat...

Family Perceptions of End‐of‐Life Care for Long‐Term Care Residents with Dementia: Differences Between the United States and the Netherlands

ConclusionBecause previous research favored care in the Netherlands to that in the United States, findings suggest improvement in end‐of‐life care and outcomes in the United States and stability in the Netherlands. Greater hospice use does not explain improvements in the United States directly and may relate, at least in part, to care provided by long‐term care staff themselves. Better understanding of the nature and process of these improvements may suggest areas for additional improvement. (Source: Journal of the American Geriatrics Society)

AbstractOlder adults are among the highest at risk for completing suicide, and they are more likely to seek mental health services from providers outside of traditional mental health care, but providers across the spectrum of care have limited training in suicide risk assessment and management and particularly lack training in suicide prevention for older adults. An educational program was developed to increase awareness and improve suicide risk assessment and management training for a range of healthcare providers who may see older adults in their care settings. One hundred thirty‐two participants from two Veterans Affairs Medical Centers participated in a 6.5‐hour‐long workshop in the assessment and management of suicide risk in older adults. Participants were asked to complete pre...

The objective of this literature review is to gain insight into the efficacy of nonpharmacological interventions in chronic pain management in community‐dwelling older adults. An extensive search of pertinent databases was performed to identify reports of studies of nonpharmacological (physical and psychosocial) pain interventions. The review identifies intervention studies that used randomized controlled trials (RCTs) and summarizes existing evidence of effectiveness of nonpharmacological interventions. A literature search yielded 28 RCT intervention studies (18 for physical interventions and 10 for psychosocial interventions) that met inclusion criteria and are included in this review. Twenty‐one studies (75%) identified in this review demonstrated statistically significant differenc...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors
