For years, it has been widely assumed that older adults may experience memory loss and other cognitive problems following surgery. But a new study from researchers at Washington University School of Medicine in St. Louis questions those assumptions. In fact, the researchers were not able to detect any long-term cognitive declines attributable to surgery in a group of 575 patients they studied.
In a step toward using human saliva to tell whether those stiff joints, memory lapses, and other telltale signs of aging are normal or red flags for disease, scientists are describing how the protein content of women's saliva change with advancing age. The discovery could lead to a simple, noninvasive test for better diagnosing and treating certain age-related diseases in women, they suggest in a report in ACS' Journal of Proteome Research, a monthly publication.
BETHESDA, Md. (MedPage Today) -- With members citing unanswered safety questions, an FDA advisory panel today narrowly rejected approval of the nation's first cell-based influenza vaccine -- an alternative to vaccines produced by the traditional, time-consuming method of growing the virus in eggs.
Involvement in a hospice program is important because it may allow individuals with dementia to delay or prevent institutionalization as well as provide psychosocial support for their families. Once used mostly by patients with a terminal cancer, now more than one half of the hospice patients have diagnoses other than cancer. Yet hospice is still underused for individuals dying with advanced dementia. We conducted a pilot study of hospice agencies to determine barriers and characteristics of dementia hospice enrollment. Using a mailed questionnaire and interview, we looked at demographics, accessibility, training, referral sources, and marketing. Our analysis divided the agencies based on dementia census and availability to non-Medicare eligible individuals. Results showed hospices having ...

This study investigated whether aspects of family caregivers&rsquo; prosody (pitch and loudness) would be associated with successful or unsuccessful conversations with spouses who have Alzheimer&rsquo;s disease (AD). Secondary analysis was conducted of 12 caregivers&rsquo; speech when interacting with spouses who have AD. Acoustic analyses were conducted to calculate the fundamental frequency (pitch) and intensity (loudness) of caregivers&rsquo; speech. The results showed no significant overall differences between the caregivers&rsquo; pitch or loudness in either successful or unsuccessful conversations. However, for 1 subgroup of caregivers, an increase in pitch variation and loudness was associated with unsuccessful communication, whereas for another subgroup the opposite pattern was obs...

Obesity is now a global health hazard. It not only predisposes to an array of risk factors leading to increased morbidity and mortality amongst adults but it also has a major negative impact on children&rsquo;s health. The deleterious effects of obesity on cardiovascular system have now been well acknowledged. It causes insulin resistance that in turn leads to diabetes, hypertension and cardiovascular abnormalities. The vascular effects of obesity may have a role in the development of a rapidly growing disease of late life, Alzheimer&rsquo;s disease. The precise mechanisms of the association between adiposity and impairment of cognitive performance remain to be elucidated. However, negative impact of obesity on cognitive function may be, at least in part, due to vascular defects, impaired ...

A novel influenza A partly of virus of swine origin (2009 H1N1) emerged this spring, resulting in an influenza pandemic. This pandemic is anticipated to continue into the next influenza season. Given that the 2009 H1N1 and seasonal influenza A appear to be somewhat different in the human populations affected and that two influenza vaccines will be recommended this fall, those who manage long-term care facilities and treat patients in them will be faced with many uncertainties as they approach the 2009/10 influenza season. Ten specific suggestions are offered to those responsible for the care of patients in long-term care facilities regarding the upcoming influenza season. These practical suggestions are the clinical opinions of the authors and do not represent official recommendations of t...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Background:
Prior evidence suggests that the renin angiotensin system and antihypertensives that inhibit this system play a role in cognitive, central vascular, and endothelial function. Our objective is to conduct a double-blind randomized controlled clinical trial, the antihypertensives and vascular, endothelial, and cognitive function (AVEC), to compare 1 year treatment of 3 antihypertensives (lisinopril, candesartan, or hydrochlorothiazide) in their effect on memory and executive function, cerebral blood flow, and central endothelial function of seniors with hypertension and early objective evidence of executive or memory impairments.
Methods:
The overall experimental design of the AVEC trial is a 3-arm double blind randomized controlled clinical trial. A total of 100 community eligibl...

Two new studies of pedestrian safety found that using a cell phone while hoofing it can endanger one's health. Older pedestrians, in particular, are impaired when crossing a busy (simulated) street while speaking on a mobile phone, the researchers found. The studies, in which participants crossed a virtual street while talking on the phone or listening to music, found that the music-listeners were able to navigate traffic as well as the average unencumbered pedestrian.
People with dementia - who occupy a quarter of all hospital beds - are staying far longer in hospital than people without the condition who go in for the same treatment at a cost of hundreds of millions of pounds to the NHS, an Alzheimer's Society report found today (Tuesday, 17 November 2009). Based on research involving 2,400 people, Counting the Cost: caring for people with dementia on hospital wards reveals large, costly variations in the quality of care for people with dementia.
To examine the association between body mass index (BMI) and mortality in older people. A longitudinal cohort study of an age-homogenous, representative sample born in 1920/21. Community-based home assessments. West Jerusalem residents born in 1920/21 examined at baseline in 1990 (n=447), with additional recruitment waves in 1998 (n=870) and 2005 (n=1,086). Comprehensive assessment of health variables including BMI (m/kg2) at ages 70, 78, and 85. The primary outcome of mortality was collected from age 70 to 88 (1990[ndash]2008). Adjusted Cox proportional hazards analysis was used to calculate hazard ratios (HRs) for mortality according to unit increase in BMI. A unit increase in BMI in women resulted in HRs of 0.94, (95% confidence interval (CI)=0.89[ndash]0.99) at age 70, 0.95 (95% CI=0.9...

Conclusion: The results suggested the stronger involvement of families' preferences regarding the patients' place of death over patients' own preferences. Therefore, factors affecting families' preferences need to be clarified for the dissemination of death at home for terminally ill cancer patients.
Conclusion: Vascular dementia in patients is characteristic of increased levels of oxidative stress, especially lipid peroxidation markers. This finding is relevant to determining the pathophysiology of dementia, particularly in the light of the recently suggested importance of the vascular component in dementia development, in addition to aiding in the diagnosis of VaD following clinical presentation. The study will be continued to compare the character and level of decline in both groups. (Source: Geriatrics and Gerontology International)

Hypertrophic cardiomyopathy (HCM) is a relatively common genetic cardiac disorder with heterogeneous morphological, functional and clinical features. Although the risk of sudden death and incapacitating symptoms in young patients has been focused upon, the disease has been found with increasing frequency in elderly patients. However, there have been few studies on clinical features of HCM in the elderly. We established a cardiomyopathy registration study in Kochi Prefecture, which is one of the most aged communities in Japan, to provide detailed descriptions of the clinical features of HCM in a community-based patient cohort. The unselected regional HCM population consisted largely of elderly patients (70% of the study cohort being [ge]60 years of age at registration), although HCM has bee...

Conclusion: Employing actigraphs, sleep[ndash]wake assessments were conducted on aged people in three different lifestyle environments. This lifestyle difference was clear from the activity rhythm detected. Then, from a comparison of both countries, we were able to offer suggestions on life patterns of aged people brought about by the differences embodied in culture, environment and economic background.
Etiology and predictors of urinary incontinence and its effect on quality of life

Conclusion: Urinary incontinence commonly occurs in dementia subjects. Poor mobility and presence of nocturia increase the risk of urinary incontinence. Correction of the possible reversible factors may help to reduce the prevalence of urinary incontinence in patients with dementia and reduce carer stress.
This article reviews publications dating back more than a century describing investigations of the endometrium, including those examining the relationship between endometrial hyperplasia and carcinoma, the influence of estrogens on the endometrium, and strategies for protecting the endometrium from unopposed estrogen stimulation. Endometrial hyperplasia and carcinoma studies date from before 1900. The influence of endogenous estrogens on the endometrium became evident with observations of endometrial hyperplasia and/or carcinoma in women with estrogen-secreting tumors or polycystic ovarian disease. Later, observational studies and randomized, controlled trials suggested a relationship between unopposed estrogens and endometrial cancer and hyperplasia. The first, and to date only, effective...

CONCLUSIONS: Women of climacteric age in Taiwan utilized TCM more often than other age groups. To deal with multiple symptoms and/or diseases among climacteric women, new prescription patterns of combining two or more herbal formulae have evolved. Studies on safety issues and drug-herb interactions are warranted for future research.

A team of scientists at Cold Spring Harbor Laboratory (CSHL) report their success in solving the molecular structure of a key portion of a cellular receptor implicated in Alzheimer's, Parkinson's, and other serious illnesses. Assistant Professor Hiro Furukawa, Ph.D., and colleagues at CSHL, in cooperation with the National Synchrotron Light Source at Brookhaven National Laboratory, obtained crystal structures for one of several "subunits" of the NMDA receptor.
Using Massage to Reduce Use of Sedative-Hypnotic Drugs With Older Adults A Brief Report From a Pilot Study

Despite known adverse effects, sedative-hypnotic drugs (SHDs) are widely used in institutional settings serving the elderly. Using a 2 (Intervention, Control) x 3 (Baseline, Intervention, Withdrawal) mixed design with random assignment to the intervention (n = 15) or control (n = 13) group, the authors sought to determine if a nonpharmacological sleep intervention (massage at bedtime) could reduce "as-needed" SHD (PRN-SHD) usage. Each phase of this pilot lasted 7 days and PRN-SHD usage was monitored via chart review. Results, indicating a 13% greater reduction in requests for PRN-SHD for the intervention group when it received massage, approached statistical significance for the quadratic planned comparisons (p = .17) despite limited power (.28) for the observed effect size of .07. While p...

This study examines how working memory ability in older women is related to wayfinding performance in the presence of salient (distinctive, prominent) or nonsalient cues. The degree of salient cue complexity is also examined, thus leading to the hypothesis that salient, complex cues are important in wayfinding and that working memory capacity is related to wayfinding performance. The virtual computer-generated arena is used to test this hypothesis in 20 healthy older women in three different environmental cue conditions varying in salience and complexity. Data analyses indicate that older women perform best in salient cue conditions. A greater working memory capacity is related to improved performance in the nonsalient cue condition. These findings offer preliminary evidence that cue salie...

This article explores how registered nurses (RNs) in long-term care (LTC) understand their role as charge nurses. Data are derived from 16 charge nurses employed in 8 facilities in Ontario, Canada. Qualitative methods are used to analyze audiotapings of interviews. The findings reveal a range of dimensions and subdimensions. Charge nurses experience their work as highly complex and unpredictable. Themes that captured the following dimensions of the supervisor role in LTC include (a) against all odds, getting through the day; (b) stepping in work; and (c) leading and supporting unregulated care workers. In addition, analysis within each category reveals a complex intersection between the nurses&rsquo; perceptions of the context and their consequent work strategies. The emerging demands plac...

Reconsidering Successful Aging: A Call for Renewed and Expanded Academic Critiques and Conceptualizations

This study fleshes out a more thorough critique of the sociopolitical contexts surrounding the successful aging paradigm so that it can help renew and expand existing critiques. The authors conclude that researchers need to be wary of adopting successful aging terminology without considering and expanding their understanding of the political motivations and results that accompanies it. New, expanded conceptualizations of successful aging are needed so that socially minded researchers and practitioners of gerontology do not contribute to ageism and discrimination against older adults.
Health and welfare have emerged as pivotal drivers used to position the identities that older people adopt in contemporary Western societies. The article presents a theoretical analysis of gerontology based on a critical reading of the work of Michel Foucault. It identifies the interrelationship between managerialism and older people in terms of a conceptual toolkit of (a) "medical power," and (b) "assessment," "surveillance," and "resistance"; the key point is that they are ...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Seniors who neglect themselves, risking their own health and safety, tend to be individuals with limited social networks and little social engagement, according to a study by Rush University Medical Center. The study, currently appearing online in the medical journal Gerontology, is the largest epidemiological study to date examining a wide range of sociodemographic, health-related and psychosocial characteristics associated with elder self-neglect.
