Colorado House Republicans vowed to help businesses create jobs by cutting regulation this legislative session. And on Monday, they proved their commitment to that tenet by killing a professional licensing bill that was brought by one of the members of their own caucus.

Stock ownership by members of Congress has been an issue recently after several media reports that were critical of what some viewed as politicians capitalizing on inside information about the financial crisis, including a featured report on CBS news program "60 Minutes."

On Thursday the U.S. Senate approved a bill that prohibits legislators and their family members, as well as staffers, from trading stocks if they have non-public information... (Source: bizjournals.com Health Care:Hospitals headlines)

A June 5 ballot initiative designed to boost the tax on cigarettes by $1 a pack could create 12,000 new permanent jobs right away and add nearly $2 billion in economic activity in the Golden State annually, according to a new study by UC San Francisco.

A four-year-old girl visited the Eye Department of Hippokration, General Hospital of Thessaloniki due to reduced visual acuity of her left eye. She was diagnosed with PFV and underwent surgery (lensectomy, capsulorhexis of the posterior capsule, insertion of an intraocular lens in the posterior chamber, and posterior vitrectomy) in order to dissect the PFV. Along with the postoperative medical care, she underwent intensive treatment for amblyopia. The postoperative course was uncomplicated, and the visual acuity of her left eye improved from hand movement to 20/25 with proper correction.
Conclusion Hospitalizations of patients with a secondary diagnosis of AF are significantly longer and more costly than those of patients without a secondary diagnosis of AF, especially when warfarin is used during the hospital stay. Among patients with a primary diagnosis of AF, warfarin therapy during hospitalization is associated with significant increases in mean LOS and hospital costs.

Discussion
Nutritional problems can occur in all parts of the world and in all socioeconomic strata. Caloric and/or protein inadequacy unfortunately plagues too many people because of inadequate supplies or availability. Supplemental food programs around the world attempt to provide appropriate nutrition, but can be stymied because of war, political instability, economic instability and many other social factors.
The most common specific nutrient deficiencies are iron and Vitamin D deficiencies. Minerals important for essential nutrition include copper, iodine, selenium and zinc. A typical mixed diet usually provides enough minerals. In the United States, mineral deficiencies are usually uncommon unless there is an underlying disease process or abnormal food restrictions or diet. Vegan an...

Hawaii lawmakers on Friday passed legislation for about $1.8 million in emergency funds for kidney and liver transplants in the state.

Talon Therapeutics Inc. must hurdle a Food and Drug Administration advisory panel March 21 before its experimental leukemia drug can be approved.

San Mateo-based Talon (OTCBB: TLON) said Friday that its drug Marqibo will be assessed by the FDA’s oncology drugs advisory committee for treating adult Philadelphia chromosome-negative acute lymphoblastic leukemia, a rare blood cancer.

“We believe Marqibo has the potential to help a near end-stage leukemia population of patients without good treatment options,” Talon President and CEO Dr... (Source: bizjournals.com Health Care:Hospitals headlines)

The Susan G. Komen for the Cure Foundation on Friday said it has reversed its decision about funding grants for Planned Parenthood.

The organization, which has Cincinnati affiliate, said earlier this week that it would not fund grant applications to groups under federal investigation, which includes Planned Parenthood.

Culture Map reports that after the initial Komen announcement, Planned Parenthood received more than $400,000 in donations in 24 hours.

A Friday statement from Nancy Brinker, the founder of the foundation, apologized for upsetting its supporters and partners with the funding removal, and said the foundation will “ensure that politics has no place in our grant process...
Conclusions&nbsp;&nbsp;Appropriate management requires aggressive medical treatment and, at times, surgical intervention. If recognised early and
treated appropriately, a full recovery can often be expected. Therefore, clinicians should be aware of the clinical presentation
of such infections to improve patient outcome. A review of the literature was done to advance our understanding of the diagnosis,
treatment, prevention and outcome of these infections.

This report provides a detailed analysis of the impact of CQUINs in London, examining which issues were incentivised in 2010/11, how providers performed against their targets, as well the financial impact of this performance. It found that in 2010/11, London hospitals missed out on nearly £22 million of revenue as a result of failing to meet CQUIN goals – some 23% of the total available.

An experimental device for removing blood clots in stroke patients dramatically outperformed the standard mechanical treatment, according to research presented by UCLA Stroke Center director Dr. Jeffrey L. Saver at the American Stroke Association's 2012 international conference in New Orleans on Feb. 3.
&nbsp;
The SOLITAIRE Flow Restoration Device is among an entirely new generation of devices designed to remove blood clots from blocked brain arteries in patients experiencing stroke. It has a self-expanding, stent-like design and, once inserted into a clot using a thin catheter tube, it compresses and traps the clot. The clot is then removed by withdrawing the device, thus reopening the blocked blood vessel.

In the first U.S. clinical trial of SOLITAIRE, the device opened blocked ve...

CONCLUSION: Despite the smaller caliber of the instrument, RFA is not associated with a decrease in the overall complication rate for selective termination procedures. The technique used for selective termination should still be determined by technical considerations but patients should be informed of the survival associated with each technique.
Conclusions.  Physical restraint is still prevalent in Japanese long‐term care hospitals, and nurses need to develop effective intervention approach to redesigning practice related to physical restraints. Specialised intervention approach seems needed depending on the types of restraint.Relevance to clinical practice. Specific approach should be developed to minimise the physical restraint in long‐term care hospitals in Japan.
AbstractChronic diseases are now the largest cause of mortality in Thailand, and form an increasingly large portion of the healthcare landscape. In the Thai health system, many patients with chronic conditions receive care and disease management services from nurses, yet specialized training in chronic diseases is not currently part of standard nursing degree programs. Given the evolving epidemiology of the Thailand population, we questioned whether practicing nurses remain confident in their knowledge and skills in chronic disease management. We conducted a cross‐sectional, self‐efficacy survey of nurses in eight randomly‐selected provinces in Thailand, receiving 468 responses. Nurse self‐efficacy was analyzed in prominent chronic disease types, including cancer, hypertension, dia...

Abstract:  Acute genital ulcers rarely occur in nonsexually active young girls. When present, they can cause significant physical and emotional distress for the patient and her parents, and prompt an evaluation for sexual abuse and sexually transmitted diseases. With this review, we aim to further characterize acute genital ulcers in nonsexually active young girls by reviewing the medical records of patients with this disorder and to offer an approach to the diagnosis, evaluation, and treatment of acute genital ulcers based on our understanding and knowledge of this condition. We retrospectively review our understanding and knowledge of acute genital ulcers in nonsexually active girls at a pediatric hospital. Angry or aggressive behaviour can have serious repercussions, including loss of residential or day placements, admission to hospital and reduced quality of life.
Authors: Morrison I, Riha RL
    Abstract
Excessive daytime sleepiness is a common presentation to physicians both in general practice and hospital settings. In this review, we provide an update on the latest theories on the pathogenesis of the condition, and discuss the approach to investigation of the sleepy patient, with particular reference to narcolepsy. Recommended therapy is reviewed for both narcolepsy and cataplexy, to provide physicians with an important reference on the investigation and management of these troubling conditions.

D.C. Mayor Vincent Gray has tapped Elaine Crider, an independent project management consultant from Ward 8, to join the United Medical Center board of directors. He has also asked Virgil McDonald to remain on the board for another term.

D.C. Council must approve both appointments, which would run through 2014.

If confirmed, Crider will replace Robin-Eve Jasper, the former director of the D.C. Department of Real Estate Services, who was first appointed to the board by then-Mayor Adrian Fenty shortly after the District seized the hospital in 2010...
Conclusion
&nbsp;&nbsp;
The Lodox/Statscan can provide benefits for surveying extracranial injuries in patients with diminished level of consciousness.
The Lodox/Statscan also emits a notably low dose of radiation and appears to be a relatively inexpensive adjunct to screen
torso or extremity injuries in TBI patients.

Clinicopathological Profile, Airway Management, and Outcome in Huge Multinodular Goiters: An Institutional Experience from an Endemic Goiter Region

Conclusions&nbsp;&nbsp;Long-standing huge goiters are common in iodine-deficient endemic areas. The majority of patients have symptomatic or clinicoradiological
evidence of airway involvement. The incidence of RSE, airway deformity, intubation difficulty, and tracheomalacia is high
with huge goiters. The surgery is technically demanding with greater associated chances of injury to native structures. Malignancy
influences the presentation and outcome in smaller goiters. In centers with experienced endocrine surgeons and dedicated anesthetists,
huge goiters can be successfully managed with minimal short-term and long-term morbidity.

Conclusion&nbsp;&nbsp;RICH should be suspected in neonates who present with a solitary liver lesion and normal-for-age serum alpha-fetoprotein.
Serial US scans should be used to confirm a progressive shrinkage of the lesion. Corticosteroids and β2-adrenergic antagonists have no proven effect in treating RICH. If the lesion grows, percutaneous needle biopsy is recommended
to exclude a malignant tumour and to direct further management. Infants with cardiac failure should be treated medically.
Embolization (with or without needle biopsy) should only be performed when this strategy fails.

ConclusionsAustralian Indigenous patients with stroke received a reduced quality of care in hospitals and experienced worse outcomes than non‐Indigenous patients. Indigenous patients require the provision of evidence‐based care to increase their opportunities for optimal health outcomes following stroke. Further research to explain the differences is needed.
Conclusion:  Outpatient GEM could effectively reduce growth of BOM and medical utilization for relatively healthy elderly patients in Taiwan where elderly patients haverelatively free access to general and specialty care. The results of this study implied the potential benefits of systematically implementing GEM in primary health‐care settings to improve the quality of care and slow down the growth of BOM for elderly patients.
Abstract&nbsp;&nbsp;Surgical intervention for congenital heart disease (CHD) can be complicated by pulmonary hypertension (PH), which increases
morbidity, mortality, and medical burden. Consequently, postoperative management of PH is an important clinical consideration
to improve outcomes. Inhaled nitric oxide (iNO) is a widely accepted standard of care for PH and has been studied in the context
of cardiac surgery for CHD. However, large randomized, double-blind, placebo-controlled, multicenter clinical trials in pediatric
patients are limited. This review will provide an overview of the clinical studies in this setting and will discuss general
treatment considerations to facilitate a better understanding of the clinical use of iNO for PH after pediatric cardiac surgery.

Abstract&nbsp;&nbsp;Olfaction is an essential chemosensory system in the living world. Although less appreciated in humans, smell impairment significantly
affects many aspects of quality of life. Smell disorders may be caused by an impaired nasal airway or by lesions in the olfactory
system, leading to reduced or distorted smell perception. The most common causes of smell disorders are aging, upper respiratory
tract infection, sinonasal disease, and head trauma. Recovery is rarely complete. Counseling is important in progressive or
severe smell loss. In patients with distorted smell perception, antidepressant medication is sometimes necessary. Best response
to treatment is achieved for nasal obstruction and sinonasal inflammatory disease. Treatment of olfactory impairment caused

Abstract&nbsp;&nbsp;Repair of congenital diaphragmatic hernia (CDH) continues to pose a dilemma for the pediatric surgeon. While the management
of CDH has evolved from surgical urgency to delayed repair after medical optimization with substantial improvements in mortality,
morbidity continues to perplex clinicians. Significant morbidity occurs with recurrence, re-recurrence and complications such
as obstructions, principally with mesh repair. When primary closure is not possible, mesh repair is indicated. While there
are several non-absorbable prosthetic, absorbable biosynthetic and composite mesh types available, the ideal mesh remains
elusive. In this article, we reviewed the current materials, reported advantages, and animal and clinical studies of non-absorbable

CONCLUSION. An effective electroencephalogram service was being provided, though abuses were common. These were mainly because of misconceptions regarding the role and limitations of standard electroencephalograms. Through an educative, non-confrontational approach, and with time to explain guideline recommendations to clinicians, sustainable change in practice could be achieved so as to benefit patients, clinicians, and service provision.

In Tanzania, limited laboratory services often preclude routine identification of microorganisms that cause infections in persons with diabetes. Thus, we carried out this study to determine the utility of a Gram stain alone versus culture in guiding appropriate antimicrobial therapy. During February 2006 to December 2007 (study period), deep tissue biopsies were obtained from persons with diabetes presenting to the Muhimbili National Hospital (MNH) with infected limb ulcers. Specimens were Gram‐stained then cultured for bacteria and fungi. Biopsies were obtained from 128 patients. Of 128 cultures, 118 (92%) yielded bacterial or fungal growth; 59 (50%) of these 118 cultures yielded mixed growth (80% included Gram‐negative organisms); 38 (32%) and 20 (17%) yielded Gram‐negative and Gra...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> blue sky scrubs medical uniforms are not only <a href="http://www.blueskyscrubs.com/">cheap scrubs</a> but also stylish and of the finest quality.</p></div>

Maternal use of certain drugs during pregnancy can result in transient neonatal signs consistent with withdrawal or acute toxicity or cause sustained signs consistent with a lasting drug effect. In addition, hospitalized infants who are treated with opioids or benzodiazepines to provide analgesia or sedation may be at risk for manifesting signs of withdrawal. This statement updates information about the clinical presentation of infants exposed to intrauterine drugs and the therapeutic options for treatment of withdrawal and is expanded to include evidence-based approaches to the management of the hospitalized infant who requires weaning from analgesics or sedatives.
Although sepsis is one of the important etiologies of illness in hospitalized infants, it is often difficult to determine if an infant is truly infected and, moreover, how to treat these infections. To address the first issue, researchers have begun to examine techniques to shorten the amount of time it takes to culture and identify organisms. On the clinical side, the development of biomarkers may help physicians to better identify infants who are likely ill from infection versus those infants who are unstable from other processes. The ability to distinguish between these cohorts will help to curtail excessive use of empirical antibiotics. Even if infants are determined to truly have infection on the basis of a positive culture, it is becoming more challenging to appropriately treat causa...

How does lean work in emergency care? A case study of a lean-inspired intervention at the Astrid Lindgren Children's Hospital, Stockholm, Sweden

Conclusions:
Drawing on Spear and Bowen's theoretical propositions, this study explains how a package of lean-like changes translated into better care process management. It adds new knowledge regarding how lean principles can be beneficially applied in healthcare and identifies changes to professional roles as a potential challenge when introducing lean thinking there. This knowledge may enable health care organizations and managers in other settings to configure their own lean program and to better understand the reasons behind lean's success (or failure).
Effectiveness of a multidisciplinary care program on recovery and return to work of patients after gynaecological surgery; design of a randomized controlled trial

DiscussionThe discrepancy between expected physical recovery and actual return to work after gynaecological surgery contributes to the relevance of this study. There is strong evidence that long periods of sick leave can result in work disability, poorer general health and increased risk of mental health problems. We expect that this multidisciplinary care program will improve peri-operative care, contribute to a faster return to work of patients after gynaecological surgery and, as a consequence, will reduce societal costs considerably.Trial registration: Netherlands Trial Register NTR2087 (Source: BMC Health Services Research)

    Abstract
The subjective illness burnout is often described as the combination of workload-related suffering and job dissatisfaction, thus, leading to depressive symptoms. Burnout is a serious model of personal illness perception, but not useful as a diagnosis term because of its lack of specification and reliability. In this respect, burnout therapy cannot be regarded as a specific form of psychotherapy or any other form of therapy, but rather a pragmatic procedure focusing on the burnout clients' needs. When applying scientific standard...

    Abstract
Due to increased economic challenges in the management of hospitals head surgeons do not only need excellent surgical expertise but also significant management qualities. The personality of head surgeons should include authenticity, sincerity, fairness and the ability to cooperate. Visionary leadership, strategic thinking and strategic management of the personnel are further prerequisites for success. Besides good abilities in communication head surgeons need knowledge of the operating figures for interpretation. To keep up with the own capabilities time and self-management is essential. A survival as head surgeon is likely if these qualities and abilities exist.

Several months ago, I saw a young mother with a baby strapped to her in a sling walk through the front doors of the Bill & Melinda Gates Foundation, wander up to the front desk and ask, “What do you guys do?”

The people at the front desk stifled a laugh and clearly weren't sure what to say. They told her the foundation does a lot of things. They handed her one of the pamphlets sitting on the counter and she wandered away, apparently no more enlightened than when she walked in.

Should the woman come back to visit the foundation next week, she would probably walk away with a better answer after a tour through the brand-new Gates Foundation visitors’ center, opening Feb...
The Kaiser strike became a numbers game Tuesday afternoon, with different numbers tossed from different sides.

Whether 80 percent walked the line, honored the picket — or crossed the line — depends on who’s talking.

More than 21,000 nurses, mental health providers, optical workers and others were called to strike against Kaiser Permanente Tuesday by three different unions.

The National Union of Healthcare Workers called the one-day strike to draw attention to stalled contract negotiations at bargaining tables across the state... (Source: bizjournals.com Health Care:Hospitals headlines)

The deal was announced last fall, but has been waiting approval from the state health department.

Today ADHS Director Will Humble approved the deal, but with some qualifications.

“I wanted to make sure this didn’t result in a rate increase,” Humble told the Phoenix Business Journal.

He is not allowing Rural/Metro to increase its rates for three years... (Source: bizjournals.com Health Care:Hospitals headlines)

More than 21,000 nurses, mental health providers, optical workers and others were called to strike Kaiser Permanente Tuesday, but far fewer appeared to have shown up at the picket lines by the middle of the day.

The numbers remain in dispute, however.

Two-thirds of the Kaiser nurses in Northern California crossed the picket line, Kaiser said.

HCA Virginia, which owns Reston Hospital Center, won regulatory approval in 2009 to build the full-service, 164-bed StoneSpring Medical Center at the corner of U.S. Route 50 and Gum Spring Road. However, the hospital is not expected to open until late 2015, and HCA officials wanted to begin offering care there sooner... (Source: bizjournals.com Health Care:Hospitals headlines)

The number of bioscience jobs increased by 7.4 percent during the postrecessionary period of 2009-10, compared with a 1.8 percent decline for the state’s overall private sector, according to the annual study conducted by the Battelle Technology Partnership Practice.

“Through the most trying economic circumstances of our lifetimes, bio in Arizona more than held its own,” said Walter Plosila, senior adviser to the Battelle practice... (Source: bizjournals.com Health Care:Hospitals headlines)

Oregon’s medical providers collected more than $2.5 million in federal health care grants that helped them install new health record technology systems.

So says the Portland Interhospital Physicians Association, which helps local doctors manage their practices. Some 125 state providers have received health care reform funds intended to defray costs of installing and using new electronic health records.

The money helped “early adopter” physicians add the new systems that will allow them to meet reform-driven requirements... (Source: bizjournals.com Health Care:Hospitals headlines)

Portland Archbishop John G. Vlazny has called a requirement that employers include contraception in their health care plans a "severe assault on religious liberty."

In a letter written to his "Brothers and Sisters in Christ" and republished by several media outlets, Vlazny took aim at a Jan. 20 Health and Human Services ruling that health insurance plans cover all FDA-approved forms of contraception without charging a co-pay, co-insurance or deductible.
The Louisville Federation of Musicians Local 11-637 has withdrawn its unfair labor practice claim against the Louisville Orchestra board and management.

As Business First previously reported, the musicians had filed a complaint against Louisville Orchestra Inc. with the National Labor Relations Board, alleging that orchestra management has not bargained in good faith regarding a new labor contract.

Gary Muffley, director of the labor board’s Cincinnati Regional Office, confirmed that the claim was withdrawn Monday... (Source: bizjournals.com Health Care:Hospitals headlines)

Abstract&nbsp;&nbsp; As human immunodeficiency virus (HIV)-infected patients now live longer while receiving highly active antiretroviral therapy
(HAART), chronic kidney disease (CKD) has emerged as a significant cause of morbidity and mortality among urban HIV population.
Risk factors associated with CKD in such HIV-infected population include aging, hypertension, diabetes mellitus, co-infection
with hepatitis&nbsp;C virus, low CD4 cell count, and high HIV viral load. Clinical experience has shown that HIV-infected individuals
often have one or more concurrent risk factors for CKD. The cumulative effect of multiple risk factors on the development
of CKD should be noted in this population. Glomerular disease directly related to HIV infection, so-called HIV-associated
nephropathy, rema...

Abstract&nbsp;&nbsp;Early intensive therapy in type 2 diabetes can prevent complications. Nevertheless, metabolic control is often sub-optimal
in newly diagnosed patients. This web-based survey aimed to evaluate opinions of physicians about treatment, priorities, and
barriers in the care of patients first referred to diabetes clinics. Data on physician attitudes toward therapeutic preferences
for two clinical case models (same clinical profile, except HbA1c levels of 8.6 and 7.3% at the first access, respectively)
were collected. Participants were asked to rank from 1 (most important) to 6 (least important) a list of priorities and barriers
associated with the care of new patients. Overall, 593 physicians participated. In both case models, metformin and education
were primary options...

We report the case of a woman who required spinal surgery
3&nbsp;months after she underwent placement of two drug-eluting stents. The patient’s clopidogrel was stopped 5&nbsp;days prior to surgery
and an infusion of eptifibatide was used to “bridge” antiplatelet therapy during the perioperative period. Postoperatively,
anticoagulation therapy was reinstituted using aspirin with clopidogrel. This case serves as a successful example of bridging
therapy using a short acting and gycoprotein (GP) IIb/IIIa inhibitor as a means of maintaining antiplatelet therapy during
the perioperative period to minimize the risk of stent thrombosis and the risk of intraoperative bleeding.

Conclusion&nbsp;&nbsp;Laparoscopic choledochoduodenostomy is a useful technique in patients with benign, refractory common bile duct obstruction.
This technically demanding procedure is feasible; however, the associated comorbidities in this complex group of patients
result in a relatively high complication rate.

Abstract&nbsp;&nbsp;Since the incorporation of implantable cardioverter-defibrillators (ICDs) into the management of life-threatening arrhythmias
in the 1980s, tremendous advances in device and lead technology have allowed the implantation of ICD systems in younger and
smaller patients. The majority of these young patients with “electrical” cardiac disease and a significant number of those
with other indications for ICD placement have near normal to normal cardiac function, which has resulted in a large population
of young ICD patients with minimal to no symptoms. This population has pushed the boundaries of published guidelines on activity
restrictions for the disease state and the presence of an ICD, creating a dilemma for the patient, the family, and the health
care team with ...

    Abstract
To evaluate the effectiveness of an intensive system of case management for high end users of inpatient care in reducing psychiatric inpatient utilization. A pre-post study design with a contemporaneous comparison group was employed to determine the effects of a State designed intervention to reduce inpatient care for adults with a mental health disorder who had high utilization of inpatient psychiatric care between 2004 and 2007. Logit and negative binomial regression models were used to determine the likelihood, frequency and total days of inpatient utilization in the post period as a function of the intervention. Data from administrative reporting forms and Medicaid claims were used ...

We report a case of a 14-month-old boy with miliary tuberculosis and recurrent pneumothorax due to cavities in the left lung. This boy had no previous medical history and was referred to our hospital for a severe pneumonia. Initial chest radiograph showed bilateral miliary pattern. Direct microscopy of gastric lavage showed the presence of tubercle bacilli, providing definitive diagnosis. In spite of effective medication, his status rapidly worsened. A cardiac resuscitation was followed by intubation, and he required high-pressure ventilation for four weeks. He developed left pneumothorax, for which several drainages were performed. Computed tomography revealed a huge cavern system involving the entire lingula and surrounded by the left pneumothorax. Eventually, a massive enlargement of th...

Abstract&nbsp;&nbsp; The aim of this paper was to review the literature concerning the relationship between suicide and substance abuse behaviours
among adolescents, focusing on epidemiology, comorbidity and preventive programmes. We performed a Pubmed/Medline, Scopus,
PsycLit and PsycInfo search to identify all papers and book chapters during the period between 1980 and 2011. Adolescents
with substance abuse disorder who attempt or complete suicide can be characterized as having mood disorders, stressful life
events, interpersonal problems, poor social support, lonely lives and feelings of hopelessness. The research supports the
existence of a strong relationship between suicide and substance abuse. Preventive programmes should be based on the detection
of risk factors associated wit...

Impact of Guideline‐Concordant Antibiotics and Macrolide/β‐lactam Combinations in 3203 Patients Hospitalized with Pneumonia: Prospective Cohort Study

In conclusion, guideline‐concordant antibiotics were not associated with decreased mortality for patients hospitalized with pneumonia, but were associated with decreases in the composite endpoint of death or ICU admission. Our findings do not support any clinical advantage to macrolide/β‐lactam compared to respiratory fluoroquinolone monotherapy. (Source: Clinical Microbiology and Infection)
