You can watch it hereLisa Power, Head of Policy at Terrence Higgins Trust said “We’re delighted that Gordon Brown is making a commitment to increase HIV testing. Thousands of people in the UK don’t know they have the virus and many find out very late – sometimes years after infection.
The risks of malaria infection in Kenya in 2009

Conclusion:
Model based geo-statistical methods can be used to interpolate malaria risks in Kenya with precision and our model shows that the majority of Kenyans live in areas of very low P. falciparum risk. As malaria interventions go to scale effectively tracking epidemiological changes of risk demands a rigorous effort to document infection prevalence in time and space to remodel risks and redefine intervention priorities over the next 10-15 years.
In 2001, Rian Malan wrote an article in Rolling Stone questioning the accuracy of HIV tests in order to disparage the evidence of a growing HIV epidemic in South Africa. In 2003 he published similar articles in the Spectator and Noseweek. All these articles were replete with errors. I subsequently debunked the latter two in a January 2004 article.
Yesterday (18th November 2009) marks the second annual European Antibiotic Awareness Day, established to highlight the problem of increasing antibiotic resistance and the need for prudent use of antibiotics. Figures from the European Surveillance of Antimicrobial Consumption project, which monitors antibiotic use and antimicrobial resistance in countries across Europe, suggest that antibiotic use in many countries remains too high.
Although much work remains to be done, a United Nations global reporting system on HIV/AIDS has already yielded an "unequaled wealth of data" on progress toward meeting UN targets for responding to the global HIV epidemic. An update on the development of the UN General Assembly Special Session on HIV/AIDS (UNGASS) global reporting system appears in a special supplement to JAIDS: Journal of Acquired Immune Deficiency Syndromes.
Conclusions. These are the first prospective data obtained from homosexual men to assess circumcision status as a risk factor for STIs. Circumcised men were at reduced risk of incident syphilis but no other prevalent or incident STIs. Circumcision is unlikely to have a substantial public health impact in reducing acquisition of most STIs in homosexual men.

Conclusion: The burden of T. vaginalis infection at 8.5% is relatively high among a community sample of young reproductive aged women. Because this infection increases the risk of HIV transmission and is associated with adverse pregnancy outcomes, there is a need for increased screening and treatment of this easily curable sexually transmissible infection in India.

(University of Alabama at Birmingham) In what has eluded discovery for 30 years, scientists have identified a cellular receptor for the immune system's first-response antibody immunoglobulin M (IgM). The proposed name is the Fc mu receptor gene; it describes a key region of the IgM antibody that binds this receptor and fills in a crucial gap in understanding the science behind immune deficiencies and allergy diseases, says UAB researchers.
Conclusions:
Despite suggestions that family-based approaches to preventing adolescent HIV infection may be culturally inappropriate, our results suggest that a family-based intervention to prevent adolescent HIV infection is feasible for rural Indian families if it (1) provides families with relevant family-based prevention strategies and knowledge about HIV/AIDS, (2) addresses barriers to participation, (3) is adolescent-friendly, flexible, and convenient, and (4) is developmentally and culturally appropriate.
This paper documents the progress of human papillomavirus (HPV) vaccine introduction in Belgium. Information on vaccine use is based on sales statistics and reimbursement claims. From November 2007 to November 2008, the National Institute for Health and Disability Insurance reimbursed the HPV vaccine for girls aged between 12-15 years. In December 2008, the age limit was extended to include girls up to the age of 18. In November 2008, the total number of HPV vaccines sold exceeded 530,000 doses. The number of vaccines reimbursed in Belgium, for the period November 2007-November 2008, corresponds to the amount required to fully vaccinate 44% of all girls aged between 12-15 years. However, the trend was decreasing over the last 10 months. By the current reimbursement policy, we can expect th...

Surveillance of HAI is under the auspice of the national institute for public health surveillance through a central coordinating structure, the Réseau d'alerte, d'investigation et de surveillance des infections nosocomiales (RAISIN), which consists of five regional coordinating structures, two national advisory committees of the Ministry of Health and public health agencies. Surveillance includes the performance of national prevalence surveys every five years (latest in 2006), specific surveillance networks to follow trends and characterise HAI that are national priority, and mandatory reporting of HAI that meet specific criteria for alert purposes.
Analysis of all complete genome sequences of the pandemic influenza A(H1N1)v virus available as of 10 September 2009 revealed that two closely related but distinct clusters were circulating in most of the affected countries at the same time. The characteristic differences are located in genes encoding the two surface proteins - haemagglutinin and neuraminidase - and four internal proteins - the polymerase PB2 subunit, nucleoprotein, matrix protein M1 and the non-structural protein NS1. Phylogenetic inference was demonstrated by neighbour joining, maximum likelihood and Bayesian trees analyses of the involved genes and by tree construction of concatenated sequences.
We report a case of a 64-year-old veterinarian working in a state camel veterinary laboratory who was diagnosed with and treated for acute brucellosis with complicating epididymo-orchitis. Genomic tandem repeat analysis (MLVA-16) revealed identical Brucella strains in patient cultures and from different dromedary milk samples positive for Brucella melitensis, thereby confirming the diagnosis of a laboratory acquired infection. The case illustrates the high (airborne) infectivity of brucellosis in laboratory settings and the need to implement vigorous bio-safety measures in veterinary laboratories handling camel specimen diagnostic veterinary laboratory.
Population structure analyses and demographic history of the malaria vector Anopheles albimanus from the Caribbean and the Pacific regions of Colombia

Conclusions:
The data provide evidence for limited past gene flow between the Caribbean and the Pacific regions, as estimated by mtDNA sequences and current gene flow patterns among An. albimanus populations as measured by MS loci which may be mainly influenced by semi-permeable natural barriers in each biogeographical region that lead to the genetic differences and effective population sizes detected. The relatively high genetic differentiation in the port city of Buenaventura may be the result of specific ecological conditions, human migration and activities and/or differences in effective population sizes. This knowledge could serve to evaluate and coordinate vector control strategies in these regions of Colombia. (Source: Malaria Journal)

Hypothesis of snake and insect venoms against
Human Immunodeficiency Virus: a review

Conclusion: With reference to mechanisms of action, properties and components of snake venom such as sequence homology and enzymes (protease or L- amino acid oxidase) may have an effect on membrane protein and/or act against HIV at multiple levels or cells carrying HIV virus resulting in enhanced effect of anti-retroviral therapy (ART). This may cause a decrease in viral load and improvement in clinical as well as immunological status. Insect venom and human Phospholipase A2 (PLA2) have potential anti-viral activity through inhibition of virion entry into the cells. However, all these require further evaluation in order to establish its role against HIV as an independent one or as a supplement. (Source: AIDS Research and Therapy)

Fever in a returned traveller may be the manifestation of a self-limiting, trivial infection but it can also presage an infection that can be rapidly progressive and lethal. We studied the diagnostic accuracy of procalcitonin (PCT) as a biomarker for a bacterial cause of fever in a cohort of 157 consecutive travellers with fever after a stay in the (sub)tropics. Elevated procalcitonin levels were observed not only in about 50% of travellers with proven bacterial infection, but also in a significant proportion of travellers with a likely infection. Using a cutoff point of 0.5 ng/mL, procalcitonin had a sensitivity of 0.52 and a specificity of 0.76 for a bacterial cause of fever on admission. Interestingly, only 1 out of 16 patients with a proven viral infection had a marginally e...

A landmark, 25-year review of cases in which surgeons had to remove the lower portion of the body from the waist down for severe pelvic bone infections shows the therapy can add years and quality of life to survivors, say researchers at UT Southwestern Medical Center. The rarely performed surgery is called a hemicorporectomy or translumbar amputation, and involves removing the entire body below the waist, including legs, pelvic bone and urinary system.
With 900,000 assisted reproduction treatments annually such as IVF and intrauterine inseminations in Europe the Commission's proposal to screen both partners before each treatment could lead to costs of over EUR 140 million annually. These figures do not include the additional overhead costs such as administration, personnel and documentation that the hospitals would have to carry on top of that.
Abstract The purpose of this study was to identify the psychosocial factors which influence transitioning HIV positive clients without
primary medical care to a case management agency within 6 weeks by a city brokerage agency. People living with HIV who reported
being in a social support group and those who requested assistance with meeting their basic needs were significantly more
likely to attend their first case management appointment within 6 weeks (adjusted OR 1.91 95% CI 1.22–2.97 and OR 1.24 95%
CI 1.01–1.54, respectively). Individuals requesting medical care or substance abuse treatment were less likely to accomplish
transitioning (adjusted OR 0.75 95% CI 0.59–0.95 and adjusted OR 0.70 95% CI 0.53–0.91, respectively). The implications of

Abstract The aims of this study were to understand strategies and consistency of strategy used by HIV-negative and HIV-positive men
who have sex with men (MSM) to ascertain the HIV status of their male sexual partners and their associations with unprotected
anal intercourse (UAI) and serodiscordant UAI (SDUAI) in the past 3 months. Participants (n = 640) completed an online survey in December 2007. The most commonly reported strategy was checking online profiles (85%),
followed by talking before sex (82%), talking after sex (42%), and guessing (29%). Adjusting for demographic and behavioral
factors, guessing the HIV status of sex partners was associated with greater UAI and SDUAI partners, as was using an inconsistent
strategy.
Incomplete Use of Condoms: The Importance of Sexual Arousal

Abstract The purpose of this study was to identify associations between incomplete condom use (not using condoms from start to finish
of sex) and sexual arousal variables. A convenience sample of heterosexual men (n = 761) completed a web-based questionnaire. Men who scored higher on sexual arousability were more likely to put a condom
on after sex had begun (AOR = 1.58). Men who reported difficulty reaching orgasm were more likely to report removing condoms
before sex was over (AOR = 2.08). These findings suggest that sexual arousal may be an important, and under-studied, factor
associated with incomplete use of condoms.

Conclusion:
HIV-1 affects humoral responses to AMA-1, but seems to marginally or not affect humoral responses to other merozoite antigens and VSAs. The latter were important for controlling parasite density and predict treatment outcome. (Source: Malaria Journal)

Conclusions:
The study showed specific serum IgG and IgA production in patients infected with Cryptosporidium, both HIV seropositive and seronegative as compared to uninfected subjects suggesting induction of Cryptosporidium specific humoral immune response in infected subjects. However, there was no difference in number of patients with positive response in HIV seropositive or seronegative groups indicating that HIV status may not be playing significant role in modulation of Cryptosporidium specific antibody responses. The number of patients with positive IgG, IgM and IgA response was not significantly different in patients with or without history of diarrhoea thereby indicating that Cryptosporidium specific antibody responses may not be necessarily associated with protection from symptom...

Infection with the human pathogen Vibrio vulnificus leads to the generation of reactive oxygen species (ROS) via NAD(P)H oxidase (Nox) in host cells. In the present study, we employed mutant V. vulnificus strains to identify an essential virulence factor responsible for this ROS generation. We found that repeats‐in‐toxin A1 (RtxA1) expressed by V. vulnificus acts via Nox1 to induce significant ROS generation in the intestine epithelial cells, which ultimately results in cell death. Furthermore, RtxA1 modulates the small GTPase Rac2, which is known to play an important role in the activation of Nox. When mice were infected by the oral method, in contrast with the wild‐type bacteria, an RtxA1‐defi...

Neck pain is a common complaint in the U.S. It is estimated that 30%-50% of adults will experience neck pain in a given year. Neck and back pain together account for about 65% of cases of disability, although neck pain less frequently leads to disability claims. Interestingly, neck pain has the highest frequency in those working in hospitals and offices, while those working in industrial/service and forestry sectors have the lowest frequency of neck pain. Women tend to have higher rates of trauma-related neck pain and demonstrate greater spinal motions. This may be related to men having anatomically wider and longer disk facet depth than women. Between 1993 and 1995 cervical spine conditions consumed 696,000 bed days during 161,000 hospitalizations with an average of 4.3 days per stay. Abo...

The Quebec Task Force on Whiplash-Associated Disorders defines whiplash as an acceleration-deceleration mechanism of energy transfer to the neck most often resulting from rear- or side-impact motor vehicle collisions; the impact often results in bony or soft-tissue injuries, which are referred to as whiplash injury. The varied symptoms arising from whiplash injury are termed whiplash associated disorders (WAD). (Source: Disease a Month)

Cervical Sprain/Strain Definition

Cervical sprain/strain typically refers to acute pain arising from injured soft tissues of the neck, including muscles, tendons, and/or ligaments. The most common event leading to such injuries are motor vehicle collisions. The mechanism of injury is complex. Basically, when a vehicle is struck from behind, the initial head and neck acceleration lag behind vehicular acceleration. Eventually, head and neck acceleration reaches up to 2 and a half times the maximum car acceleration, which subsequently results in dramatic deceleration at end range of motion of the neck. While such injury can also result in fracture, discogenic, or neurologic injury, cervical strain or sprain, by definition, excludes these entities. (Source: Disease a Month)

In a previous issue, the authors discussed the pain generators in the lumbar spine that result in low back pain. Being part of the spinal column, the cervical spine has the same pain generators: paraspinal muscles, ligaments, nerve root, outer third of the intervertebral disk, periosteum, and facet joint; these are all discussed in this issue. However, the cervical spine does have several differences from the lumbar spine, which has clinical implications. The spinal cord is within the cervical spine, unlike the lumbar spine, where the cord ends at L1 in adults. The facet joints are oriented differently, allowing for more rotational motions. The atlas and axis articulate with the occiput, allowing for flexion/extension. These anatomical structures have greater clinical significance in the c...

MedWorm Message: Get the very latest Swine Flu news via the MedWorm Swine Flu RSS news feed - updated hourly from thousands of authoritative health and news sources.

An overwhelming majority of Wyoming voters - 88 percent - support food safety legislation that would give the Food and Drug Administration (FDA) new authority to ensure the food Americans eat does not make them sick, according to a new poll commissioned by the Pew Health Group and conducted by Hart Research and Public Opinion Strategies. Support for stronger food protections is high regardless of voters' gender, income level or political affiliation.
The resistance of infectious organisms to antibiotics is particularly serious in drugs against fungi. Fungal cells are similar to human cells, which means that it is difficult to develop effective drugs that can destroy them without also damaging human cells, i.e. without causing side effects. We must therefore safeguard the effectiveness of the few antifungal drugs that are available today. Resistance to these would leave many diseases without effective treatment.
Viruses are microscopically sized parasites. They plant their genes in the cells of their victim in order to 'reprogram' them. The infected cells then no longer produce what they need to live, making lots of new viruses instead. Luckily, in most cases this hostile takeover does not go unnoticed. This is ensured by the cells' own sensors that recognise alien genetic material. One of them is RIG-I. When RIG-I encounters virus genes, it ensures that the body releases interferon.
(BioMed Central) Money available to treat HIV/AIDS is sufficient to end the epidemic globally, but only if we act immediately to control the spread of the disease, according to research published in BMC Public Health. This approach defies conventional thinking, which recommends gradual spending over 15-20 years. The study was based on a mathematical model developed by mathematicians and biologists, who recently earned acclaim for a study on how best to handle a planetary invasion by zombies.
Conclusions:
Self-reported data could provide useful quantitative information on drug intake and administration. Under strict methodological conditions this method, easy to implement, can be used to describe patients' behaviours and their use of new anti-malarial treatment. Self-reported data is a major tool for assessing compliance in resource poor countries. Blood and urine drug dosages provide qualitative results that confirm any drug intake. Urine assays for SP could be useful to obtain public health data, for example on chemoprophylaxis among pregnant women. (Source: Malaria Journal)

Summary. Recurrent hepatitis C is a common cause of graft loss in patients undergoing liver transplantation, and serial protocol liver biopsies have been used to identify patients at risk of graft loss from rapid fibrosis progression. The aim of this study was to derive a simple noninvasive index to predict fibrosis in patients with recurrent hepatitis C post-transplant. A retrospective study was performed assessing serial liver biopsies for post-transplant chronic hepatitis C infection. One hundred eighty-five patients were included in the analysis; median age 53 years (interquartile range 48[ndash]59) and 140 (76%) were male. Liver histology showed 53 (29%) had Ishak fibrosis stages F0/F1, 31 (17%) had F2, 29 (16%) had F3, 19 (10%) had F4 and 53 (29%) had F5/F6. The London Transplant Cen...

We report here that the protozoan parasite Cryptosporidium parvum induced B7‐H1 expression in cultured human cholangiocytes. Induced expression of B7‐H1 was identified in cells after exposure to infective C. parvum parasite or parasite lysate. Interestingly, the level of microRNA‐513 (miR‐513) was reduced in cells after exposure to C. parvum, which resulted in a relief of 3′ untranslated region–mediated translational suppression of B7‐H1. Overexpression of miR‐513 through transfection of miR‐513 precursor inhibited C. parvum–induced B7‐H1 protein expression. Moreover, enhanced apoptotic cell death was identified in activated human T cells after coculture with C. parvum–infected cholangiocytes. The apoptosis of activated T cells was partially blocked by a neutralizin...
