January 27, 2012 - A recent study published by the American Journal of Obstetrics and Gynecology has found the rate of spontaneous abortion in nurses who handle chemotherapy drugs to be twice that in nurses who did not handle these drugs. Past studies have yielded similar results, but this study, done in partnership with National Institute for Occupational Safety and Health (NIOSH) and the Nurses’ Health Study 2, used a significantly larger sample size of nearly 7,000 nurse pregnancies. The study looked at five potentially hazardous exposures: x-rays, chemotherapy drugs, sterilizing agents, anesthetic gases and antiviral drugs. Abstract
The aim of this study was to explore whether work characteristics (job demands, job control, social support) comprising Karasek and Theorell's (1990) Job Demands-Control-Support framework predict employee safety performance (safety compliance and safety participation; Neal and Griffin, 2006). The majority of staff believe that therapeutic time improves patient recovery and reduces risks while increasing patient satisfaction with care received and enhancing nurse–patient relations.• Staff co...

Conclusions.  Specified discharge worker roles, multi‐professional care coordination teams, and information technology systems promote better service satisfaction and subjective quality of life for older people when compared with standard hospital discharge. Improved multi‐professional communication reduces rates of re‐admission and length of stay indicating greater cost effectiveness and efficiency for the health and social care systems. Systems of care emphasizing information exchange, education and negotiation between stakeholders facilitate communication in transitional care contexts for older adults. Conversely, lack of dialogue and lack of understanding of others’ roles are barriers to communication in transitional care.Relevance to clinical practice.  Enhanced multi‐pr...

Bereavement care for older people in healthcare settings: qualitative study of experiences

Conclusions.  The study identified the interactions of healthcare staff with bereaved older people in terms of the bereavement journey. Staff demonstrated awareness of difficulties the bereaved person may encounter and showed commitment to providing support. However, lack of flexibility in services restricts meaningful interactions.Implications for Practice.  (i) Healthcare staff may identify gaps in services in terms of preparing relatives and follow‐up post‐bereavement; (ii) Therapeutic relationships between staff and relatives enable ongoing support; (iii) Development of practice guidelines is a key consideration.
Ambiguities: residents’ experience of ‘nursing home as my home’

Conclusions.  Ambiguities concerning the nursing home as a home and place to live, a social environment in which the residents experience most of their social life and the institution where professional health service is provided were uncovered. High‐quality care was when ambiguities were managed well and a home could be created within the institution.Implication for practice.  Achieving quality care in nursing homes requires reconciling the ambiguities of the nursing home as a home. This implies helping residents to create a private home distinct from the professional home, allowing residents’ personal habits to guide institutional routines and supporting meaningful activities. Using these resident developed quality indicators is an important step in improving nursing home service...

According to a new analysis of hundreds of recorded office visits, doctors and nurse practitioners typically issued orders and asked closed or leading questions when talking to their HIV-positive patients about adherence to antiretroviral therapy. Attempts at problem-solving with patients who had lapsed occurred in less than a quarter of visits. Take your medicine, Doctor's orders. It's a simple idea that may seem especially obvious when the pills are the antiretroviral (ARV) drugs that add decades to the lives of HIV-positive patients...
Tetrapods with highly different morphologies occupy ecological niches of the canopy making them ideal for testing the evolution of structures and performances under similar environmental selective pressures. We compared leap up strategies between two distantly related amniote species, Anolis carolinensis (Squamate) and Microcebus murinus (Lemuriform) known to use leaping as their major locomotor mode for predator avoidance. Our comparative analysis and model show that leaping strategies (flat jump trajectory in horizontal leaps, use of forelimbs in landing) are similar in both species. The most striking divergence concerns only the temporal joint sequence accommodation to leaping height, although an identical proximal-to-distal sequence is observed when both taxa leap to maximal height.
Copepods are important grazers on microplankton in marine food webs and are, in turn, preyed upon by a wide range of predators with diverse feeding adaptations. Although copepods have evolved numerous adaptations to help them avoid predation, their escape behavior sets them apart from many other planktonic organisms. Mechanoreception is widely used by copepods to detect hydrodynamic disturbances created by approaching predators. When these disturbances are detected, copepods respond quickly with escape jumps that can accelerate them from a stationary position to speeds of over 600 body lengths per second within a few milliseconds. Myelinated nerves may improve the escape behavior of some copepods through faster conduction of nerve impulses. The differences in response latencies between mye...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

Schooling behavior in fish has been recognized to confer antipredator advantages. However, the mechanisms that lead to various patterns of escape maneuvers in fish schools are largely unknown. Here, we investigated the effect of startle stimulus characteristics (distance and orientation) on the escape maneuvers of schools of a highly gregarious fish species, the Atlantic herring Clupea harengus. We quantified information transfer of the onset of startle responses by analyzing the speed of the waves of their propagation (U w = 6.7 ± 1.6 m/s; mean ± SD). U w was found to be positively correlated with the number of early responders (i.e., individuals reacting with a latency <50 ms). In addition, we identified the rules determining two different evading maneuvers...

While some weapons have evolved in response to natural selection for defense against predators, others may have evolved to serve some other primary function (e.g., intraspecific aggression) but still have defensive uses. In this comprehensive review of extreme morphological weaponry in mammals, I explore specific hypotheses regarding the ecological, morphological, and behavioral correlates of different types of weaponry in order to explain why some taxa have evolved elaborate weapons while others have not. I provide evidence demonstrating that several types of defensive weaponry (e.g., spines, quills, a...

Food acquisition mode in lizards (i.e., ambush vs. widely searching) has been intensely scrutinized for the past decade to identify correlations between food acquisition mode, diet, sprint speed, and other aspects of phenotypic diversity. To begin to understand these correlations, we studied foraging mode variation in natural foraging behavior and attack speed in three ambush predators and two widely foraging species in the field. Sequential analyses revealed considerable variation in the temporal structure of behavioral repertoires associated with acquiring food. Ambush and wide-foraging species use unique combinations of behaviors prior to prey attack with differences among and between food acquisition modes. Attack speeds were well below maximum sprint speed for these species. Thus, the...

In this article I briefly discuss the role that artificial (robotic) models can play in the study of competing co-evolutionary dynamics, the main results obtained in the research works addressing the evolution of predator and prey robots, and the implications of these studies for robotics. In particular I discuss the factors that cause the convergence toward a cyclical dynamic and the factors that enable prolonged innovation phases eventually leading to open-ended processes.
Predator-prey interactions paradigm: a new tool for artificial intelligence

Predator–prey interactions are probably one of the key mechanisms for explaining the evolution of organisms in their ecosystems. Scientific fields relevant to understanding the mechanisms of these interactions are as diverse as evolutionary biology, behavioral ecology, ecomorphology, molecular biology, phylogeny, neurosciences, physiology, biomechanics, and robotics. The difficulty in understanding these mechanisms lies therefore (1) in the multi- and interdisciplinary nature of this issue, and (2) in keeping up with very rapid developments in various scientific fields. This Special Issue provides an interdisciplinary approach to predator–prey interactions to identify how phenotypic traits of both types of organisms interact and how each can act as a selective pressure on the e...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

Facilitation of evidence-based practice is a multifaceted process and a team effort. Communication and relationship-building are key components. The practical aspects of facilitation explicated in this study validate what has been previously noted in the literature and expand what is known about facilitation process and activity.
The Daily Mail (U.K.) has a fascinating story today about Mary Ann Cotton (1832 – 1873), Britain’s first serial killer. According to the author, Professor of Criminology David Wilson, a serial killer is someone who kills more than three people over a time period of more than thirty days. Cotton more than met this definition:
Few have heard of the so-called “Black Widow” killer who posed as a wife, widow, mother, friend and nurse to murder perhaps as many as 21 victims [over a period of almost 20 years], living off her husbands before eventually claiming their estates. Two decades before Jack the Ripper would terrorist the streets of Whitechapel in London, Mary Ann Cotton had already become a killing machine, perhaps murdering as many as eight of her own children, se...

In addition, they request a public debate regarding the NHS's future to "salvage some good" from the government's "damaging" reforms.
Abstract Hypertension affects a large proportion of urban African-American older adults. While there have been great strides in drug
development, many older adults do not have access to such medicines or do not take them. Mindfulness-based stress reduction
(MBSR) has been shown to decrease blood pressure in some populations. This has not been tested in low-income, urban African-American
older adults. Therefore, the primary purpose of this pilot study was to test the feasibility and acceptability of a mindfulness-based
program for low income, minority older adults provided in residence. The secondary purpose was to learn if the mindfulness-based
program produced differences in blood pressure between the intervention and control groups. Participants were at least 62 year...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

Abstract Two major statistical issues confronting comparative analyses of hospital outcomes are adequacy of case-mix adjustment and
proper accounting for random variation. Hierarchical modeling has been proposed to improve precision and reduce the impact
of random variation but becomes difficult to implement when there are numerous case-mix factors to control. In this paper
we formulate the problem of hospital performance comparisons within the framework of potential outcomes and illustrate an
approach to hospital comparisons which combines multiple category propensity score methods for the control of case-mix variations
with hierarchical Bayesian modeling of case-mix adjusted summaries. The approach is similar to that proposed by Huang et al.

In this article I explore the relationship between symbolic interactionist theory and interpretive description methodology. The two are highly compatible, making symbolic interactionism an excellent theoretical framework for interpretive description studies. The pragmatism underlying interpretive description supports locating the methodology within this cross-disciplinary theory to make it more attractive to nonnursing researchers and expand its potential to address practice problems across the applied disciplines. The theory and method are so compatible that symbolic interactionism appears to be part of interpretive description’s epistemological foundations. Interpretive description’s theoretical roots have, to date, been identified only very generally in interpretivism and th...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

Grounded theory was employed to elucidate how public health nurses (PHNs) develop therapeutic relationships with vulnerable and potentially stigmatized clients, specifically, single mothers living in low-income situations. We named the emerging theoretical model Targeting Essence: Pragmatic Variation of the Therapeutic Relationship, after discovering that although PHNs strove to achieve relational goals, their attention was primarily focused on the goal of ascertaining concerns foremost on the hearts and minds of mothers, and that PHNs had to accomplish these goals within short practice timeframes. The study’s focused context elicited a nuanced explanation of the dynamic relationship-building process derived from subjective relationship experiences of PHNs and single mothers living i...

[Original Articles] Effect of Chronic Dementia Caregiving and Major Transitions in the Caregiving Situation on Kidney Function: A Longitudinal Study

Conclusions
Kidney function did not differ between caregivers and controls over time. However, GFR had impaired at 3 months after a major caregiving transition. Because the effect of placement of the spouse with Alzheimer’s disease on the decline in GFR was moderated by BP, it might be confined to caregivers who experience increased sympathetic activation after placement.
Conclusions.  Physical restraint is still prevalent in Japanese long‐term care hospitals, and nurses need to develop effective intervention approach to redesigning practice related to physical restraints. Specialised intervention approach seems needed depending on the types of restraint.Relevance to clinical practice. Specific approach should be developed to minimise the physical restraint in long‐term care hospitals in Japan.
AbstractChronic diseases are now the largest cause of mortality in Thailand, and form an increasingly large portion of the healthcare landscape. In the Thai health system, many patients with chronic conditions receive care and disease management services from nurses, yet specialized training in chronic diseases is not currently part of standard nursing degree programs. Given the evolving epidemiology of the Thailand population, we questioned whether practicing nurses remain confident in their knowledge and skills in chronic disease management. We conducted a cross‐sectional, self‐efficacy survey of nurses in eight randomly‐selected provinces in Thailand, receiving 468 responses. Nurse self‐efficacy was analyzed in prominent chronic disease types, including cancer, hypertension, dia...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

AbstractMilitary spouses frequently cope with separation, but limited research reviewing the impact of an overseas relocation when a spouse accompanies their serving husband/wife has been conducted. A search for studies reviewing the impact of foreign postings on these accompanying spouses was undertaken utilizing 12 databases and other resources. Ultimately, 12 studies were analyzed and four key themes produced: functioning of a military family on an international posting, loss, wellbeing and support. Overall, additional stressors are associated with an overseas posting and experiences are specific to an individual and their circumstances. Further research is required to examine the potential relationship between a spouse's experiences overseas and the impact on their health and wellbeing...

AbstractThe worldwide academic workforce is ageing. At the same time, health and human services workforces are expanding. The preparation of educators to fill gaps in expertise and to position the health sciences for future growth is an urgent need. The findings from a recent action learning project that aimed to enhance the professional growth and development of higher degree researcher student supervisors in a School of Health and Human Sciences are presented. Seven early career researchers and the facilitator met for two hours every two to three weeks over 4 months between April and July 2010, in a rural and regional university in New South Wales, Australia. The processes initiated were a combination of experiential knowledge, referral to relevant published reports, use of an effectiv...

This study determined the impact of a computerized care documentation system on client outcomes, regulatory compliance, and staff workloads after 3 years of use. The survey was conducted at an 800‐bed aged care facility, and staff using the computerized care system were invited to participate (n = 112). The survey was an adapted version of the Nurses Computer Attitudes to Technology Inventory, which was refined to make it relevant to the aged care workplace. Four multiple regression models were produced, assessing the impact of the computerized care management system on staff and workload; time; accuracy, and regulatory data; and resident care. The analysis showed that the perceived benefits of the computerized system were influenced by personal attitudes towards computer use and f...

MedWorm Sponsor Message: Have a look at the quality evident in these nursing scrubs. blue sky scrubs provides the best customer service for nursing professionals.

Area: Evidence > Medicines Management > References
Background: Medication reconciliation has been recognised as an important process in care transitions to prevent adverse health outcomes. Because older adults have multiple comorbid conditions and use multiple medications, they are more likely to experience complicated transitions between acute and long-term care settings. Hence, it is important to develop effective interventions to protect older adults at transition points of care.
Objective: To systematically review the literature and evaluate studies performing medication reconciliation interventions in patients transferred to and from long-term care settings.

Tobacco use is the number one preventable cause of death and disability in the United States today. In 2003, the Interagency Committee on Smoking and Health recommended to establish a federally funded national tobacco quitline network by 2005. Quitlines are telephone-based programs that assist tobacco users to quit. The combination of health professionals referring patients to an accessible, evidence-based, cost-effective cessation resource can produce a substantial reduction in the number of tobacco users in the United States. Initiatives to increase knowledge and working relationships between nurses and quitlines need to be created, implemented, and evaluated. (Source: Nursing Clinics of North America)

Nursing faculty practice groups can play a vital role in tobacco cessation in academic medical centers. Outcomes from the Georgia Health Sciences University Nursing Faculty Practice Group Tobacco Cessation Program revealed 64% abstinence outcomes at the end of treatment (N = 160) over a 2-year period from the campus-wide tobacco-free policy initiation. A nurse-led, evidence-based, interdisciplinary approach can be an effective strategy to make a difference in the lives of tobacco-dependent individuals, while at the same time integrating practice with education and research. fracture.

Conclusions Depression and fatigue symptoms increased during radiotherapy treatment, while QoL levels decreased. This demonstrates that
these symptoms are strongly correlated and that their presence negatively influenced QoL. At the start of treatment, nurses
need to advise patients and plan care, offering interventions to decrease these symptoms and improve QoL.

Conclusions: The responses revealed several missed opportunities for supporting health care professionals with disabilities in the workplace. These findings should inform the continuing debate regarding what defines “reasonable accommodation” and how to create a workplace that is welcoming for nurses and physicians with disabilities.
Conclusion:  Here, we provide the first direct evidence, derived from intervention, that components of the early‐life environment present on farms profoundly affects both local development of regulatory components of the mucosal immune system and immune responses to food proteins at weaning. We propose that neonatal piglets provide a tractable model which allows maternal and treatment effects to be statistically separated. (Source: Pediatric Allergy and Immunology)

This is the second part of the article based on the thesis work for a Masters of Science in Wound Healing and Tissue Repair, Cardiff University, to develop an evidence‐based protocol for the care of pilonidal sinus wounds (PSWs) healing by secondary intent, using a modified Reactive Delphi process. The sample included surgeons, clinic physicians, nurses and enterostomal therapy nurses experienced in the care of these wounds. Item generation involved an extensive review of the literature to identify key aspects of evidence‐based wound care essential to wound healing, infected wounds and pilonidal wounds healing by secondary intent and drawing on clinical experience. The participants responded via an electronic Delphi website, using a 4‐point Likert rating scale and a ranking system. C...

A member of the Tubal Reversal Message Board asks what steps she needs to take to have her tubes untied at Chapel Hill Tubal Reversal Center. It is helpful for patients to obtain the records from their tubal ligation procedure. If tubes were cut and tied, a pathology report from the hospital should also be available. Once your records are received at Chapel Hill Tubal Reversal Center, a Tubal Reversal Nurse will contact you within 24 hours. If you are not able to obtain your records right away, you can still schedule your tubal ligation reversal with Dr. Gary Berger or Dr. Charles Monteith, the top two tubal reversal doctors in the world.
Women with a tubal ligation who want to have more children have two treatment options: tubal ligation reversal or in-vitro fertilization (IVF). The best option to become pregnant for most women is tubal ligation reversal. At Chapel Hill Tubal Reversal Center, Tubal Reversal Nurses are available 7 days a week to assist potential patients with scheduling tubal reversal surgery with Dr. Berger or Dr. Charles Monteith. It is often possible to schedule surgery within 1-3 weeks. The tubal reversal doctors at Chapel Hill Tubal Reversal Center are the only doctors in the world who specialize solely in reverse tubal ligation surgery.
