The girl has always been tall for her age, but in the past 6 months, she has grown taller than her 8-year-old sister. The nurse practitioner notes the following on physical examination: height = 131 cm (&gt;97th percentile), weight = 23 kg (75th percentile), Tanner I breasts, Tanner II pubic hair.             <description>This study aimed to illuminate the meaning of being afraid when in contact with medical care, as narrated by children 7–11 years old. Nine children participated in the study, which applied a phenomenological hermeneutic analysis methodology. The children experienced medical care as “being threatened by a monster,” but the possibility of breaking this spell of fear was also mediated. The findings indicate the important role of being emotionally hurt in a child's fear to create, together with the child, an alternate narrative of overcoming this fear. Reasons offered by parents opposed or hesitant to allow their children to be vaccinated were fears associated with untoward side effects. Prominent among concerns was the fact that thimerosal, a mercury-based preservative is one of the ingredients of the injectable form of the vaccination; it is not in the FluMist spray (.             <title>Levels and Types of Action Selection: The Action Selection Soup</title>

            <description>Action selection (AS) is defined as the process where an action is selected among a number of alternatives. This definition, however, does not sufficiently describe what an action is. What is the unit of selection in the first place? We maintain that the artificial intelligence (AI) accounts of AS typically mix and merge two AS situations that indeed are qualitatively different. Most of the accounts actually deal only with one type of AS but purport to cover both types of AS. We propose three dimensions along which the commonalities and the differences between various AS accounts can be analyzed, and use these for a preliminary conceptualization of what we call a two-system action selection account. Experimental data from a spatially explicit dynamic commons dilemma experiment is used to empirically ground an agent-based model. Three distinct patterns are identified in the data. In-depth interviewing was conducted with six participants for a period of up to 1 year after their release from prison.             <description>As I reflect back over this year, I am amazed at the numerous care initiatives that we as nurses have led and implemented. Such evidence-based best practice initiatives include VTE protocols, surgical-care improvement programs, elder-care principles, pressure-ulcer programs, catheter-associated urinary tract infection programs, vascular catheter-associated infection programs, patient-safety programs and fall-prevention programs, to name just a few. These initiatives have demonstrated positive effects on patient outcomes. Such initiatives have improved quality of care and have helped our patients lead healthier lives. The use of a tracking sheet would allow the researcher to identify individual responses, and thus produce separate transcripts for each participant, which can be forwarded for verification or discussion. The advantage of this approach is that the researcher is able to obtain an account from individuals in addition to the group account.

            <description>Dementia care poor warns report

A report from the Alzheimers Society has found that dementia care in the NHS is poor. The survey of 2,400 NHS staff and carers from England, Wales and Northern Ireland found that many patients over 65 are being kept in hospital too long and that their condition was worsening whilst being in hospital. The report also said that the specific care needs of patients - such as being given enough food and water - were often overlooked and called for nurses to receive better training in dementia care.
