Conclusions:
The MSK ultrasound volume increase among nonradiologists, especially podiatrists, was far higher than that among radiologists from 2000 and 2009, with the highest growth in private offices. These findings raise concern for self-referral. (Source: Journal of the American College of Radiology : JACR)

The management of EGFR inhibitor adverse events: a case series and treatment paradigm.

Conclusion  The most common cutaneous adverse event in our cohort was papulopustular rash, followed by eczema and xerosis. Patients were managed with symptom target therapy, and suspension of the EGFR inhibitor was rarely required. As the use of EGFR inhibitors increases, it is important to promptly identify and treat adverse events. Further studies are necessary to develop targeted therapeutic and preventative measures.

The AAFP has issued a revised position paper that helps to define the professional relationship between pharmacists and family physicians in an integrated health care delivery system. The AAFP recognizes the "evolving complexity and proliferation of pharmaceutical agents and (the) important role pharmacists play in the delivery of high quality health care," says the position paper. And "The AAFP supports arrangements where the pharmacist is part of an integrated team-based approach to care." However, the paper adds, the AAFP believes "only licensed doctors of medicine, osteopathy, dentistry and podiatry should have the statutory authority to prescribe drugs for human consumption." (Source: Inside AAFP)

Conclusions: Our experience corroborates the advantages of Memory staples as described in the literature, including good approximation of bone fragments, technically easy application with fewer steps than an AO-applied screw, and an adequate source of internal fixation to achieve metatarsophalangeal joint fusion. There is also a suggestion that the time to achieve fusion is shorter.

Conclusions: The combination of increased forefoot width and the height of the second metatarsal head produced by the metatarsal pad results in an increase in space between the metatarsal heads. This extra space could play a role in pain reduction produced by a metatarsal pad.

Conclusions: This study of asymptomatic individuals did not confirm that static ND can be used to individually predict dynamic ND during treadmill walking. It was demonstrated that the start position of Brody's test is not well correlated with navicular height at heel strike, with this being the main reason for the weak relationship between static and dynamic ND measures.

Conclusions: Short-term medical missions could play a significant role in the transformative learning experience in podiatric medical education. This could affect the empathy and multicultural awareness of podiatric medical students. Further and more extensive evaluations of the potential impact of short-term medical missions in podiatric medical education should be explored because it could influence curriculum and global health in the field of podiatric medicine.

    Abstract
Molecular genetics is changing our understanding of the developmental translation of genotype to phenotype between and within different phylogenetic groups. Together with a growing understanding of our own evolutionary relationships to common ancestors, the epigenetic processes involved enforce a reexamination of what is regarded as a normal foot structure. A revised populationist approach is proposed and supported by paleoanthropologic evidence that reflects a picture of emerging suitability for bipedalism that is driven by natural genetic divergence.

Authors: Carlson RM, Smith NC, Stuck RM, Sage RA
    Abstract
This case report presents a rare postoperative dislocation of the fifth metatarsal base following a healed open partial fourth and fifth ray amputation of a 62-year-old male veteran with poorly controlled diabetes mellitus. The dislocated fifth metatarsal base subsequently created a chronic ulceration and an inhibition of normal gait. The patient was taken to the operating room where the fifth metatarsal base was resected with transfer of the peroneus brevis tendon to the cuboid to maintain biomechanical stability.

    Abstract
Sural nerve impingement is frequently reported and often arises from localized trauma but much less understood are its mechanical etiologies. This case report describes the effects of local traction on the lateral calcaneal branch of the sural nerve. The association is confirmed anatomically and symptoms are alleviated with a heel lift.

We report the case of a 42-year-old woman who developed a pseudarthrosis of the first metatarsal after percutaneous retrocapital distal osteotomy of the first metatarsal for a mild hallux valgus deformity. The operative treatment consisted of debridement of fibrous nonunion with plating followed by application of pulsed electromagnetic fields (PEMF) with an external device.

Lisfranc fracture-dislocations are complex injuries that require a high skill set from foot and ankle surgeons to diagnose and treat. Conservative treatment is seldom an option for treatment of Lisfranc injuries. The authors believe that even subtle injuries require surgical intervention. The comparison between open reduction internal fixation, partial arthrodesis, and complete arthrodesis is discussed, as well as various fixation techniques to accomplish these procedures when approaching a Lisfranc injury.
Arthrodesis of the first metatarsophalangeal joint (MTPJ) is used primarily for end-stage hallux rigidus whereby pain, crepitus, and limitation of motion is noted at the joint. Arthrodesis at the first MTPJ also has it uses as a primary procedure for rheumatoid arthritis when severe deformity is present, as well as for salvage procedures for failed joint arthroplasties with or without implant, fractures with intra-articular extension, avascular necrosis, and infection management. A first MTPJ arthrodesis should provide stable fixation, attain suitable positioning for a reasonable gait, maintain adequate length, and create a stable platform for a plantigrade foot type.
My colleagues and I appreciate the honor and opportunity to participate in writing various articles for Clinics in Podiatric Medicine and Surgery, undertaking the extensive and broad-range topic of arthrodesis of the foot and ankle. Serving as a guest editor, I recognize the ultimate responsibility to provide a thorough and comprehensive review and inclusion of various topics on arthrodesis that would be relevant to foot and ankle surgeons. Arthrodesis can be a technically complex procedure in inexperienced hands. The ultimate decision to perform arthrodesis procedures is contingent on the need to perform salvage procedures due to a patient’s continued pain and disability after alternative treatments have failed.
The powerful outcome of arthrodesis for the management of various foot and ankle pathologies is being studied continuously through biomechanical and clinical research. The gratifying result of a successful arthrodesis and alignment along with the unforgiving possible sequela of a nonunion and/or malunion still exists in every surgeon’s mind each time the procedure is performed. Abstract
In diabetic foot syndrome, a series of complications of late-stage diabetes affect the foot. These complications, which culminate in foot amputation, include peripheral vascular disease and neuropathy, Charcot arthropathy, plantar ulceration, and osteomyelitis. In recent years, the medical community has paid greater attention to diabetic foot syndrome, and our understanding of its pathophysiology and management has advanced. Although the podiatrist is charged with caring for the diabetic foot, as dermatologists we occasionally act as consultants. This review therefore offers dermatologists an update on the causes and management of skin lesions in the diabetic foot.

Salvage Arthrodesis for Charcot Arthropathy

The principles of fusion of a Charcot joint arise from the assertion that successful fusion requires removal of all cartilage, debris, and sclerotic bone. The authors believe that reconstruction can prevent amputation in patients who have unbraceable or unstable deformities, or recurrent ulcerations. The goal with any Charcot reconstruction procedure is to achieve a plantigrade foot free of ulceration, and to prevent any future collapse, deformity, or ulcerations. The authors strongly believe arthrodesis of unstable joints of the Charcot neuropathic foot can lead to limb salvage and better quality of life.
Patient expectations of podiatric surgery in the United Kingdom

Conclusions:
This paper demonstrates the expectations of a large cohort of podiatric surgery patients. For the most part patients expect pain relief, improved mobility and improved shoe fitting, while a small number of patients also expect a cosmetic improvement. Further research is required to determine the relationship between patient expectation and health related quality of life, and to determine whether podiatric surgery is successful in addressing the expectations of patients. We discussed the alterations of tendon signal intensity and result of conservative therapy after partial rupture with big hematoma in the long term. A 29 year-old man, applied with pain and swelling in the retrocalcaneal region of left ankle. He did not have chronic metabolic disease. He was not active in physical activities. X-ray radiograms were normal. At magnetic resonance images (MRI), there was an intratendinous big hematoma, subcutanous fat planes were edematous around tendon. The diagnosis was partial rupture and giant hematoma. Hematoma was drained. The conservative treatment was applied and...

We report a case of osteosarcoma in the proximal phalanx of the hallux in a 45-year-old man. In patients with foot-related symptoms, a high index of suspicion for pedal osteosarcoma is required. Delayed or inappropriate diagnosis may compromise limb-sparing surgery and survivorship.
We report a case of giant cell tumor involving the distal phalanx of great toe in a 27 year old female who presented with swelling of great toe of right foot. Radiography showed an expansile lesion in distal phalanx of great toe. En bloc resection of phalanx was done. Biopsy showed giant cell tumor and regular follow up of this patient for two years showed no recurrence of tumor. Giant cell tumor at such a location is unusually aggressive and needs regular follow up to detect local recurrence. (Source: The Foot)

Abstract: The Achilles tendon is believed to have first developed two million years ago enabling humans to run twice as fast. However if the Achilles tendon is so important in terms of evolution, then why is this tendon so prone to injury – especially for those more active like athletes. The Achilles tendon had an integral role in evolving apes from a herbivorous diet to early humans who started hunting for food over longer distances, resulting in bipedal locomotion. Evolutionary advantages of the Achilles tendon includes it being the strongest tendon in the body, having an energy-saving mechanism for fast locomotion, allows humans to jump and run, and additionally is a spring and shock absorber during gait. Considering these benefits it is therefore not surprising that studies have show...

MedWorm Sponsor Message: Find the best January Sales in the UK.

This study investigated the role of age and type of foot wear as predictors of flatfoot in school age children in urban and rural areas in south-western Nigeria. Methods: 560 children between 6 and 12 years, divided into two groups; rural and urban were studied. Each subject's static footprint was taken on a white duplicating paper after which the instep was measured for classification into high arch, normal, flat or severe flat. The BMI of each subject was calculated from the anthropometric data. The type of footwear with which the subjects were shod was also recorded. Data analysis was by Epi Info statistical package version 3.5.1 (2008).Results: Chi-square showed a significant (p (Source: The Foot)

Conclusion: There is no evidence that a sock that is “aerodynamically designed” will help an athlete go faster. This may be more product rhetoric than reality, and further work is justified if such claims are being made. (Source: The Foot)

Abstract: Background: The second metatarsal head is commonly involved in cases of metatarsalgia. As part of the conservative treatment, metatarsal bars and metatarsal pads are often prescribed. Objective: To compare the effectiveness of metatarsal bars and metatarsal pads in reducing impulse on the second metatarsal head.Method: Thirty-five healthy subjects were monitored with an insole scanning system during walking in four different conditions: (a) wearing shoes only, (b) shoes plus metatarsal pads and shoes plus metatarsal bars, placed either (c) perpendicular to the foot axis or (d) oblique to the foot axis. The impulse under the second metatarsal head was measured using the first condition as a control. Both feet were examined in each subject resulting in a total of 840 measurements.Re...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

A prospective audit of referral patterns to a dedicated Foot and Ankle surgical service

Conclusion: Only 65% of GP referrals to this surgical service were deemed appropriate as they progressed directly to surgery. However, it is important to establish what constitutes ‘appropriate’ since within the remaining 35% of referrals only 66 (5.8%) required no intervention at all. Of the residual group who required ‘treatment’ approximately 17% required steroid injection or orthoses and 143 (13%) received advice and went away to contemplate the consultation. Referral pathways could be improved by greater communication between GP and foot and ankle service with the construction and implementation of referral guidelines which would enhance the referral and treatment pathway to the service.
