            <description>&amp;lt;p&amp;gt;So my oldest DD was admitted Tuesday after her PFT&amp;apos;s took a serious nose dive.&amp;nbsp; She went from mid 70% down to 45% in a month.&amp;nbsp; This was after she had been on an oral antibiotic.&amp;nbsp; After about 3 days of Tobramycin and Fortaz her PFT&amp;apos;s went back up to mid 70&amp;apos;s.&amp;nbsp; She is home now on IV&amp;apos;s and goes back in week after next.&amp;nbsp; For the first time she managed to cough up not one but TWO green chunks for the clinic.&amp;nbsp; Both were tested and apparently the only thing that has grown was staph and they said it was moderate.&amp;nbsp;&amp;lt;/p&amp;gt;
&amp;lt;p&amp;gt;So my question is what do we do now?&amp;nbsp; Do we treat the staph more aggressively than they have in the past? She was also diagnosed a few months ago with Immune Complex Ar...</description>

            <description>Job itself is not bad. I would not mind being a pharmacist for a couple of months but doing this for YEARS? 

I work over the counter so my experience is limited but I see the pharmacists doing almost the exact same thing every single day and they can easily be confused with the technicians. 

Of course, I expected this but it was not until I experienced it that I began to realize how unfulfilling this must be. Pharmacy is more of a job than a career. The 9-5 mentality is strong and it shows with most of the pharmacists I worked with; none of them actually want to be there.

I used to only think about how I would be helping people. Turns out that I do not like being around the old, sick, fragile and frail. These people require a special level of sensitivity that I simply lack.

            <description>&amp;lt;p&amp;gt;My pulmo ordered some blood work a few weeks ago and I just got the results in the mail. There was a category for an anti-neutrophil test and it had a few things under it, one being RA factor. Most everything was normal but the RA was 4 which apparantly is positive. When I researched what this was testing for all the information was about rheumatoid arthritis and I dont understand how this links to lung disease. It seems that this is a really low RA value but is this significant in any way to my health?             <description>Does anyone include rheumatology in their practice, specifically injections. It seems to take quite a bit of skill. I did a rotation and very much enjoyed it (was able to ease a lot of pain) but I would blow my brains out if this is all I did every day, please take no offense just my feelings. (this is why I picked family med over Internal med).  Guess my question is can I keep this skill set on the back burner and feel competent to pull it out when I need it, or could I even have a subset patient base? Do you all just refer it off? Oh, and did you just do electives in Rheum or would a sports medicine fellowship be something I should be gunning for (I did not get to do any rotation time in this)?

            <description>I know some of you have switched over from Pancrease MT whatever to the new Pancreaze in what ever dosage of Lipase you need. Some have commented on the package change from the nice lightweight plastic bottle to the heavier glass bottle that comes in yet another package, a box. But has anyone had any difficulties with the stupid child-proof cap on these new bottles? I am neither a child, a person with arthritis, or a senior citizen and I have had quite a few of these bottles difficult to open.             <description>So I'm going to retry for SGUL 2012 entry. My work experience so far is:

- 10 months care home 4 hours a week
- 5 months befriending individuals with mental health problems 4 hours/week
- 3 days hospital shadowing - rheumatoloy/haemotology/pathology
- 3 days GP shadowing
- 2 days pathologist shadowing
- 3 days psychiatrist shadowing - doing in 2 weeks

I applied ages ago to do some work with THT in HIV/AIDS because it was something I was interested in but they are doing less work with befriending/support and more work in managing long term. 

Does anyone have any advice as to what else I can/should do over the next year or so? Also I remember the form for SGUL only asked for the past five, so is it pointless to have more?

            <description>I am plagued with daily arthritis. Some days I can barely move-- joints are stiff and swollen. I do moist heating... heating pad. Tylenol helps take the edge off enough on bad days so that I can get good sleep. I have been tempted to see a rheumatologist. I know for a fact that I have osteoarthritis in my hip joints and feet (Dr told me when I was 15 years old). My wrists swell and my finger joints swell off and on. My ankle joints freeze up on me on occasional as well-- makes the stairs in my home lots of fun.I don't like prednisone so I don't use it to treat my arthritis. I just stick to the moist heat, some massage and warm showers. Now if I could just get used to the cracking sound that my joints make while moving around.             <description>Hello,
X-ray will show arthritis in the shoulder joint. If present, a cortisone shot may help. 
Otherwise, I would evaluate the shoulder for adhesive capsulitis (aka frozen shoulder) and rotator cuff impingement/bursitis/tear. Both can be assessed clnically and both conditions can occur simultaneously. Therefore if physio therapy has not helped, consider an MRI to evaluated the soft tissue structures in the shoulder.

            <description>Hello,
From an x-ray point of view, the findings in your spine are minimal and I agree with the general diagnosis of arthritis. Mild scoliosis is also noted.
You may consider adding physical therapy to your treatment for pain relief and to improve function including sleep. 
The meds prescribed are relatively safe with low side effect profile. None are classified as narcotics which should be avoided in the treatment of long term/chronic back pain.
All in all, it seems your treatment plan is on the right track - just add some rehabilitation for a month or so then continue with home exercise program.

The findings on x-ray suggest long term development of spurring, disc space narrowing and in general &amp;quot;degenerative changes&amp;quot;. The slip and bump on the back of the head did not cause the bony neck issues. The bump may have set off a spell of muscle spasm and arthritis in the neck. 
Surgery should be an option of last resort unless the nerves exited the cervical vertebrae are being damaged severely by the spurs. In this case, surgery may be required. 
Check with a neurosurgeon or physiatrist regarding proper nerve testing.

                   try motrin 800mg first. motrin is an nsaid (non-steroidal anti imflammatory) tylenol is not. you can use tylenol with the motrin and see if that helps first. the inflammation is probably the cause of most of your pain. motrin 800 gives me more relief from my swelling and joint pain than even prescription meds. dont take more tylenol than bottle says and no more than 800mgs of motrin. try that before filling your rx. this is my opinion based on what works for me for my rhematoid arthritis. good luck (Source: Suboxone Forum)</description>

                   ok, so i'm kinda heated. last night's intervention was a follow-up show about 2 former addicts and how they're doing now. 

brooke has rheumatoid arthritis and abused the hell outta some pain pills. goes to a nice treatment program for like 6 months. then while she's visiting with her parents, she tells them that she is now on low doses of suboxone and norco. 

i'm like, what did she just say?!? suboxone plus norco?!? so now i'm confused. does she not know what she just said? what would be the point of taking an opiate on top of suboxone?
