Conclusions.  The eNOS 894T allele carriers are at greater risk for both MtS and ED, suggesting that eNOS G894T gene polymorphism might play an implication as a common genetic susceptibility factor to develop both disorders.
Conclusions.  PDE5 represent first line ED therapy only for SCI patients, though treatment results through meta‐analysis were not possible. Encouraging results are reported for Parkinson's and MS patients. PDE5 use for other CND patients is limited for various reasons, such as ED and concomitant libido impairment caused by depression and/or sexual endocrinology dysfunctions, and because PDE5 may cause a worsening of neurological illness. Medical centers staffed by health professionals able to counsel patients on the possible use of PDE5 are needed. Lombardi G, Nelli F, Celso M, Mencarini M, and Del Popolo G.
Conclusions.  While 8‐OH‐DPAT‐treated female marmosets display decreased sexual receptivity and increased aggressive interactions with their male pairmates, flibanserin‐treated female marmosets demonstrate increased affiliative behavior with their male pairmates. Such pro‐affiliation attributes may underlie flibanserin's effectiveness in treating HSDD in women.
CONCLUSIONS: State programs and policies should pay particular attention to increasing support for family planning services for minority groups. Findings also suggest that insurance coverage and receipt of Medicaid among women of reproductive age deserve further exploration as potentially important mechanisms for reducing state unintended pregnancy rates.
Dr. Ray Blanchard, of the DSM 5 sexual disorders work group, has written a misleading blog that portrays the introduction of 'Hebephilia' into DSM 5 as no more than a minor change. In his rendering, DSM IV already permits the diagnosis of pedophilia if victims are 13 or younger and DSM 5 is just raising the age threshold to 14.
Conclusions.  Rehabilitative vaginal dilator use is a complex, multifaceted, and personal phenomenon that carries deep psychological and emotional implications that make it intrusive. These findings may enhance the way in which vaginal dilators are introduced and help healthcare providers address better women's difficulties and concerns with the dilators. Ultimately, it may also lead to improved health maintenance and quality of life for women recovering from gynecological cancer. Cullen K, Fergus K, DasGupta T, Fitch M, Doyle C, and Adams L.
SummaryIn 1972, a pair of remarkably attractive Greek statues, later named Riace Bronzes, were found along the Mediterranean coast in the province of Reggio Calabria, Italy. Being located on the left testicle (as in 90% of cases) and ...

MedWorm Sponsor Message: Find the best January Sales in the UK.

In conclusion, exposure of the male rat foetus to additional androgens prior to or during the MPW does not advance or enhance any measured parameter of reproductive development.
The prognosis of patients with HIV and lymphoma has improved in the highly active anti-retroviral therapy era. The British HIV Association's clinical standards1 state that the care of patients with diagnosed or suspected lymphoma should be transferred to a HIV centre provider of specialised HIV oncology services. A novel clinic was therefore set up at the Barts and the London National Health Service Trust in September 2007. Patients were seen jointly by a HIV and a haemato-oncology consultant in a fortnightly clinic in the HIV department. This aimed to improve patient outcomes and satisfaction by facilitating communication between the specialties, decreasing outpatient attendances and ensuring access to the specialist HIV multidisciplinary team. Potential problems included insufficient tim...

Chlamydia trachomatis infection is the most common sexually transmitted bacterial infection with an estimated incidence of 50 million cases in the world.1 The infection may remain asymptomatic or, if not treated, can lead to severe clinical manifestations including acute urethral syndrome, urethritis, bartholinitis, cervicitis, upper genital tract infections and perihepatitis.2 Despite the recognised need for early diagnosis and treatment of C trachomatis and other curable sexually transmitted infections, the strategies for detection of these infections are currently limited, particularly in low-resource countries.3 Migration and population mixing have been shown to increase the risk of sexually transmitted diseases;4 however, the prevalence and determinants of C trachomatis infection, and...

The paper by Platt et al reports that recent experience of physical or sexual violence from a client was commonly reported among female sex workers in London.1 Experiencing violence from clients was associated with drug use at work and a history of arrest or imprisonment. Migrant women were as likely as UK-born sex workers to report sexual violence. This rate was felt to be an underestimate. There was a perceived reluctance among the migrant group to report to the police. We would like to highlight some successful and important work in the East London. This work supports those working in the sex industry who have been raped or sexually assaulted. In 2010, a locally commissioned Independent Sexual Violence Advisor (ISVA) was appointed for the Olympic boroughs of Tower Hamlets, Newham and Ci...

MedWorm Sponsor Message: Find the best January Sales in the UK.

In recognition of the potential of sexually transmitted infection clinics to reach parents (especially fathers) and thus positively impact on sexual and broader health, we surveyed a local sexual health (SH) clinic's clients. We aimed to identify the scale of parents' attendance and their characteristics including sexual and substance misuse behaviour to assess needs and inform mitigating action. Seven hundred consecutive clients attending a clinic in inner city London were invited to complete a doctor-administered questionnaire after their consultation. The questionnaire had items on demographics (including parental status), condom education, SH clinic attendance and number of previous sexual partners. Consent was obtained from 653 clients giving a 93% response rate. Just over one-third o...

All women attending for TOP between 1 April 2008 and 31 March 2011 were offered HIV serology and vulval swabs for Neisseria gonorrhoeae and Chlamydia trachomatis. Four thousand three hundred twenty-six women attended and 2308 (53%) accepted HIV testing. A further 291 (7%) had an HIV test at the time... (Source: Sexually Transmitted Infections)

Conclusions
In high-prevalence countries, a large fraction of stable partnerships were affected by HIV and half were discordant, whereas in low-prevalence countries, fewer stable partnerships were affected by HIV but a higher proportion of them were discordant. The findings provide a global view of HIV infection among stable partnerships in SSA but imply complex considerations for rolling out prevention interventions targeting discordant partnerships. Oral shedding of HIV-2 RNA is less common than HIV-1 RNA, a likely consequence of lower overall viral burden. Both systemic and local factors may contribute to shedding of HIV in the oral cavity. (Source: Sexually Transmitted Infections)

Conclusion
Many younger women in an urban community health centre believed that cervical cancer screening also screens for chlamydia and were confused about chlamydia screening schedules. As there is limited knowledge of the revised ACOG cervical cancer screening guidelines, there is a risk that currently low chlamydia screening rates may decrease further after these new guidelines are better known. Obstetrician gynaecologists and primary care providers should educate younger women about the differences between chlamydia and cervical cancer screening and encourage sexually active younger women to have annual chlamydia screening. Addressing young people at the time of military enlistment may be a good time to consider new strategies for accessing and counselling this population, thus allowing the implementation of a more appropriate healthcare policy.
Conclusions
The AC2 with AGC confirmation performs well at genital and extra-genital sites for detecting GC. Culture for GC using transport swabs performs poorly in asymptomatic men, symptomatic and asymptomatic women and at extra-genital sites. With the improved performance of nucleic acid amplification tests and the increase in GC antimicrobial resistance, research is needed into how best to optimise GC culture in settings where direct plating is not feasible.
Can we improve partner notification rates through expedited partner therapy in the UK? Findings from an exploratory trial of Accelerated Partner Therapy (APT)

Conclusions
Both methods of APT were feasible and acceptable to many patients and led to higher rates of partner treatment than routine PN. Preferences and recruitment rates varied greatly between settings, suggesting that organisational and cultural factors may have an important impact on the feasibility of an RCT and on outcomes. Mindful of these factors, it is proposed that APT should now be evaluated in a cluster RCT. (Source: Sexually Transmitted Infections)

Is Accelerated Partner Therapy (APT) a cost-effective alternative to routine patient referral partner notification in the UK? Preliminary cost-consequence analysis of an exploratory trial

Conclusion
The hotline strategy costs more than both the alternative PN strategies. If we accept that strategies which identify and treat partners the fastest are likely to be the most effective in reducing reinfection and onward transmission, then APT Hotline appears an effective PN strategy by treating the highest number of partners in the shortest duration. Whether the additional benefit is worth the additional cost cannot be determined in this preliminary analysis. These data will be useful for informing development of future randomised controlled trials of APT. (Source: Sexually Transmitted Infections)

Conclusions
Patient routes to GUM shortened between 2004/2005 and 2009. While GUM patients in 2009 were less likely overall to have STIs diagnosed, perhaps reflecting lower risk behaviour, there remains a substantial proportion of high-risk individuals requiring comprehensive care. Behavioural surveillance across all STI services is therefore essential to monitor and maximise their public health impact. (Source: Sexually Transmitted Infections)

The pattern of antimicrobial resistance in Neisseria gonorrhoeae is depressingly predictable. An antibiotic is chosen and used for a few years but resistance develops, so the dose is increased to maintain efficacy before the drug finally fails irrevocably. For penicillin, this process took a number of decades but with the subsequent use of tetracyclines, macrolides and fluoroquinolones, the cycle has shortened, and the utility of oral cephalosporins is now threatened within a few short years of their introduction as preferred therapy in many countries.1 Following the emergence of N gonorrhoeae strains with decreased susceptibility to oral cephalosporins, clinical treatment failures soon appeared in Japan and Hong Kong;2–4 more recently, similar outcomes have reported from other regio...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Can criminal justice make a positive contribution to the way states respond to sexually transmitted infections (STIs)? Sections 18 and 20 of the Offences Against the Person Act 1861 make it an offence punishable by imprisonment to inflict recklessly or cause intentionally any sort of grievous bodily harm. If particular STIs (such as herpes or HIV, the two infections to have attracted the attention of English criminal prosecutors to date) are harms to be taken at least as seriously as a broken leg then it makes sense to a criminal lawyer to treat both its intentional and ‘reckless’ transmission as a criminal offence.1 2 It comes, therefore, as something of a surprise to some lawyers that so much of the reaction amongst healthcare practitioners and public health policy-makers to ...

Over the last decade, a growing body of literature has described and evaluated interventions designed to improve partner notification and treatment for curable sexually transmitted infections (STIs). For the most part, these studies have described expedited partner therapy (EPT), interventions meant to increase partner treatment by making it easier for partners to receive medication.1 The need for new approaches to partner treatment grew out of widespread recognition of the mismatch between the size of the STI problem and the paltry resources available for traditional partner services (PS). To date, EPT has most often involved patient delivered partner therapy (PDPT), the practice of giving persons diagnosed with an STI (index patients) medication for their sex partners. Although interest ...

New approaches to partner notification have emerged over the past decade. This month we publish the clinical and economic results of a trial of ‘Accelerated Partner Therapy’, a form of expedited partner therapy adapted to the legislative framework of the UK.1 An editorial by Dombrowski and Golden2 provides a perspective from ‘across the pond’ on the potential of Accelerated Partner Therapy. Policymakers and practitioners will always have to adapt partner notification practice to local professional and cultural contexts, so it is good to see new models of partner notification emerging that may serve the needs of countries where straightforward patient delivered therapy is not approved. We are pleased to include an editorial this month by David Gurnham, an academic la...

Conclusions.  Not only could the penile waveform analyzing system assess penile endothelial function in young adults, but the results also showed significant associations with their serum testosterone levels and metabolic parameters. The findings suggest that PAWA ratio may serve as an indicator for early identification and treatment of young adults at risk of erectile dysfunction. Wu H‐T, Lee C‐H, Chen C‐J, and Sun C‐K.
Conclusions.  Based on a brief structured interview, diagnosis according to DSM‐IV‐TR criteria for HSDD identified a population of men comparable with men without HSDD in age, serum testosterone, concomitant illness and medication use but distinct in their experience of sexual desire and distress associated with low desire. These controlled data characterize HSDD in men as a distinct sexual dysfunction. DeRogatis L, Rosen RC, Goldstein I, Werneburg B, Kempthorne‐Rawson J, and Sand M.
Conclusions.  Data collected using the FGSIS were found to be valid and reliable in this sample, further supporting the utility of the scale and aiding in the understanding of college women's genital image using an underlying two‐factor approach. This has important implications for the development of sexual health and women's health programs and discussions. Specifically, a two‐factor FGSIS can aid medical and health professionals in better understanding relationships between genital image and gynecological exam behavior.
Conclusions.  From a financial standpoint, an elective orchiectomy could have cost the health care system significantly less than a hospital admission with its associated additional costs. From a patient safety standpoint, elective orchiectomy is preferable to self‐castration which carries significant risks such as hemorrhage, disfigurement, infection, urinary fistulae, and nerve damage. Healthcare providers of transsexual women should carefully explore patient attitudes toward self‐castration and work toward improving access to elective orchiectomy to reduce the number of self‐castrations and costs to the overall health care system. Further research on the financial implications of self‐castration from different health care systems and from a series of patients is needed. St. Pe...

Conclusions.  Overall, the rate of EF postoperative recovery in patients left untreated after surgery is modest (35.8%). Although younger patients with a good preoperative EF may experience good EF recovery rates even without any treatment, use of PDE5 after surgery further improved their functional outcomes. Therefore, a therapy for ED should be offered to all patients treated with BNSRP.
Conclusion.  To our knowledge, this is the fourth case of proven postcoital penile drug eruption in a patient following vaginal use of triple sulfa vaginal cream in his partner. Our case illustrates the importance of history taking. In clinical practice of urology, it is not rare to see patients who present with strange penile lesions following coitus. To reach a correct diagnosis, one should obtain a drug history of the sexual partner and allergic history of the patient in such cases.
Conclusion.  Albuminuria is an important independent risk factor of ED in men with diabetes after adjustment of age and diabetes mellitus duration. Identification and control of albuminuria and other associated risk factors might play a role in the prevention or reversal of ED.
Sex therapists agree that the most satisfying, most deeply erotic lovemaking involves slow sex--leisurely, playful, massage-inspired, whole-body sensual touch all over. But sometimes there's no time for it, and sometimes lovers want fast, rip-your-clothes-off quickies. Sex doesn't always have to be elaborate.read more (Source: Psychology Today Sex Center)

MedWorm Sponsor Message: Find the best January Sales in the UK.

ABSTRACTThe sexual consequences of breast cancer and its treatments are well known and previously reviewed. Alterations in body image, with or without breast reconstruction, changes in sexual self‐esteem and self‐efficacy, vulvovaginal atrophy as a result of chemotherapy and/or adjuvant hormone therapy, and loss of libido secondary to dyspareunia and body image issues are common in survivors of breast cancer. Medications that are prescribed for long‐term use including those in the class of aromatase inhibitors can have far‐reaching implications on quality of life by contributing to vulvar and vaginal atrophic changes.
