            <description>Rule No. 1 of blogging: post often enough to keep your audience. I seem to have broken that rule in the past eight days.
The problem is, I&amp;#8217;ve been doing so much (paying) work for others that I have neglected this site. For example, I have a new gig as a contributor to a fairly new, blog-style news site, EHR Outlook, published by Access Intelligence of Rockville, Md. (which just happens to be my home town). I&amp;#8217;ll be writing weekly for that site, which provides fairly basic EHR-related information and advice for physician practices, a return of sorts to my roots in healthcare journalism. My first post went up last week, and a second should get posted Monday.

To be completely honest, while I have my issues with them, Medicare and Medicaid are well-run programs. They allow their money to be spent all helter-skelter, they are the most efficient programs in the entire government. The overhead of CMS is abysmal compared to your other big players like BCBS, Paid, Anthem, and Caremark.
I&amp;#8217;m not sure why the post office is always busy. I&amp;#8217;m not sure why the DMV is slow as hell &amp;#8212; these are subcontracted out anyway, so blaming the government is stupid. WebMD states that dysthymia is a &amp;#8220;less severe form of depression,&amp;#8221; but it can last for years. The Journal of the American Dental Association ran an article in 2005 that claimed 10% of surveyed dentists experienced dysthymia, but of those, only 15% sought medical treatment.             <description>Results of the 2007 British Academy of Cosmetic Dentistry audit show that our English neighbors across the pond really like their cosmetic dentistry. The BACD is a non-profit organization that promotes ethics and advancements in cosmetic dentistry. The long and short of the audit:

Female patients accounted for over half of all cosmetic dentistry cases.
Orthodontics is up 345%! This includes clear braces and traditional ortho. Removable braces make up over half of the cases.
Veneer cases held steady, but the number of male cases increased.

From most popular to least popular, cosmetic dentistry procedures fall in this order: white fillings; crowns, inlays, onlays; veneers; whitening; bridges; implants; orthodontics; gum contouring.

While cosmetic dentistry and dental spas were hot topics two and three years ago, today, many dentists are working hard to break even by providing bread-and-butter services.

An article at www.mlive.com, a Michigan news site, reports that one dentist found a solution for patients who didn&amp;#8217;t qualify for credit with traditional patient financing companies. He met with a local bank to create a payment option that would draft patients&amp;#8217; bank accounts monthly. Their program was called SmartPay, and it worked.             <description>The Maine State Board of Dental Examiners has proposed legislation that will require dentists to use a heart monitor on any patient undergoing &amp;#8220;moderate sedation.&amp;#8221; This would include nitrous oxide, IV sedation, and anxiolysis. The new law comes at the same time as other changes recommended by the ADA. However, it is not part of ADA recommendations. The Maine Academy of General Dentistry does not back the law.
Required heart monitoring will cause dental costs to rise, and it may also discourage fearful patients from visiting the dentist. Furthermore, there is talk that the regulation will discourage dentists from catering to fearful patients. The Maine Dental Board is open to hearing public opinion on the matter. A vote will take place Friday, September 12th.

            <description>At the WealthyDentist.com, a recent survey reveals that over half of the dentists who responded have completely or partially stopped accepting dental insurance. The 44% who claim that they only partially work with insurance companies may mean that they are not in network with any insurance company, but they will file claims as a courtesy. They may or may not accept the patient&amp;#8217;s copay or deductible.

For some dentists, rejecting insurance would put the kiss of death on their practice. Particularly in rural areas, general dentists must accept insurance if they are to sustain a business.             <description>I am fascinated by many things. Well to be accurate, I’m perplexed by some, awed by others, and for better or worse I’m amazed by the rest. One of the events that I find fascinating is the token celebration. You know- the event/occasion/or person we celebrate on a designated day, week, or month. I can appreciate the merit of having a time of distinction for people, places, and events that have value. Parents, civil rights, secretaries (I mean administrative professionals), and hotdogs (yes there actually is a hotdog day). Truly, I get it-“demonstrate value by remembrance and recognition.” To be honest, like everybody else, I overeat and buy random presents because I’m supposed to.

Most dentists and labs I talk to see a slowdown. At times like these, dentists tend to stop presenting treatment plans as normal, and they change their behaviors to compensate for what they believe their patients are thinking. We sense the sky is falling and create our own reality. It doesn&amp;#8217;t have to be that way, though. We can take the slow time to work on the practice from the inside out.
