Rainbow row in Charleston. Rows upon rows of homes right up against their neighbor. It's famous for it's beauty. I look up and think of Noah's rainbow, the sign that humans would never be extinguished by God again.En masse that is. Many have been killed since the rainbow, including His own son Jesus Christ, who took our place before the wrath of God.I hear about God - that He is good, that He has overcome eternal death, that He always gives a way of escape. Where is my way of escape? Where is His goodness?Round church in downtown Charleston. How about that He works ALL things for the good of those who diligently seek Him. That God's plan does not always match up with my plan. He seems so very, very far away. I heard something else - something considered a fallacy - that God is a distant cos...

I must admit that I’m not much of a book guy. Especially since there’s so much free information available on the internet about just about any subject you could want. However, I’ve been quite intrigued by the number of healthcare IT related books that I’ve seen coming out of late. Here’s a quick roundup of some of the ones I’ve seen.
Getting to Meaningful Use and Beyond: A Guide for IT Staff in Health Care by Fred Trotter and David Uhlman – I’ve been a big fan of Fred Trotter for a while. So, I’m glad he’s working on this book. Turns out the book isn’t even published, but in Fred Trotter open source style fashion, the book is available for free online right now. Of course, they’re hoping you’ll provide feedback.

My first question concerns the role of diet in ADHD and sugar in kids’ food. This is one of the most frequently asked ADHD questions. People are convinced that sweet candy and soda pop can cause or at least exacerbate ADHD. There is some evidence that sugar, additives, colourings, preservatives and so on can add to hyperactivity in children but this is not exclusive to ADHD children. Any child is going to get a high from eating a sugary snack and then will crash later on. The only sensible answer is to make sure that there are lots of fresh fruit and vegetables in the child’s diet. There should also be a concentration on more protein and fewer carbohydrates, especially at breakfast.
I know many parents who have found that by omitting gluten and dairy products from their childre...

The winds are 60 mph, and the rain like a monsoon. Safe in our house Sunday night, we all sleep right through. Without a worry. In the morning, I wake to view the damage, and see this spiderweb, intact, on the window next to my chair in my bedroom. There are four things which are little upon the earth, but they are exceeding wise: the ants are a people not strong, yet they prepare their meat in the summer; the conies are but a feeble folk, yet they make their houses in the rocks; the locusts have no king, yet they go forth, all of them in ranks; the spider taketh hold with her hands, and is in kings' palaces. Proverbs 30:24-28Saturday night, we were in our tent when the storm hit. Not one of us slept through it and there was anxiety electric in the air like lightening.
Attendees of the breast cancer awareness symposium “Bridging the Gap: Promoting Breast Cancer Prevention, Screening and Wellness” were given the chance to submit questions on breast cancer in the minority community. This is the first part of these questions answered by Dr. Preya Ananthakrishnan, Assistant Professor of Clinical Surgery and a host of the event.
Q: I am a 51 year old Black women, whose mother died 13 years ago from breast cancer & her sister was diagnosed last year. I had a mammography 2 weeks ago and got the dreaded come back letter. Should I get genetic counseling?
Dr. Ananthakrishnan: I would suggest that your sister with the breast cancer get tested first, and if her test result is positive then you should get tested. Furthermore, it is likely that even though you...

Sometimes when I’m in need of a little inspiration, I head out for a short walk. Today, as I meandered through the tree lined path of my apartment complex, I found it.
I kept ending up in the direction of a beautiful fountain. The sound of the water as it sprung up in the air like fireworks and then gently falling as it lapped softly against the rocks was soothing. I thought about what the water represented, that regardless of whether it was thrust up in the air or moved gracefully to the bottom, it was the same unchanged substance. I realized that no matter what you did to it, the water was still water flowing in a fountain.
The same could be said about you. You may have emotions that carry you from the highest mountain peaks to the valley lows, but you are at the core that unchange...

Parents look for ADHD answers…sometimes in the wrong places. Attention Deficit Hyperactivity Disorder or ADHD is often misdiagnosed and misunderstood, partly because so many kids are affected by it (around 3-5% of all children), and partly because adults somehow expect children to act up and misbehave at some point.
Below are the top five most common concerns of parents who have kids with ADHD. Hopefully, these ADHD answers can help guide you in your quest for effective ADHD management and treatments, as well.
1. Is ADHD just a phase?
Respected paediatric, psychiatric, and psychological associations have classified ADHD as a real disorder. It is not merely a phase that comes with childhood, in the same ways bed-wetting or lack of motivation is in fact, ADHD is widely accepted by speci...

Learning ADHD or ADHD which gets in the way of learning is common both in school and behavior. There is nothing worse for a student than to thoroughly prepared for a test or examine only to draw a blank when testing time rolls around. These students are often labeled as slackers or underachievers, when in fact they may try as hard as or harder than anyone. Another interesting fact about individuals with learning ADHD is that they often have very high IQ levels. In order to find answers about learning ADHD we must first delve into the condition itself then investigate possible solutions.
The three primary symptoms of ADHD are inattention/distractibility, impulsivity, and hyperactivity or restlessness. Symptoms begin before the age of seven, last for a minimum of 6 months, and present majo...

The good people at HITECH Answers are conducting a survey to gage EHR customer satisfaction — 10 questions; takes 10 seconds. I think the questions they are asking are interesting and could provide some interesting results even if it’s not a real scientific survey.
So, if you’re a user of an EHR software, go and fill out the survey. If the results are valuable and interesting, I’ll be sure to share the results of the survey in a future post.

Skye and Smoky cuddling - Smoky's eyes like jasper in the sun. It has been a fun weekend with my aunts and uncle. I am going to my parents Sunday night to stay, and then over to my house around 6 a.m.!The reunion is the silver lining to the separation! I miss my little stinkers and can't wait to squeeze them. I was thinking back to other reunions (this is our 4th time through this) and I know from experience I can't plan to get anything accomplished tomorrow...other than sitting still and letting myself be smothered by the pile of kids that follows me like little steel babies to the mother magnet. Click here if you want to read old posts about previous homecomings. So today, rejoice with me?Their very first independent snowman - no help from parents!Praise God, from whom all blessings ...

EMR Stimulus Question and Answer: Has Anyone Gotten Paid Yet?

Today I got a good question from Gene about whether anyone has gotten a check for the adoption of EMR yet. You can see the full question and my answer below. Also, be sure to check out the other EMR Stimulus Question and Answers I’ve done.
Has any doctor actually received any money from the government regarding the use/adoption of an EMR? OR does this not come until 2013
Short Answer: No
Longer answer: Medicaid is getting close to cutting the first checks. Medicare takes 90 days of meaningful use, so the first checks won’t be cut until after the first quarter of 2011. So, we won’t have to wait until 2013, but the EMR stimulus has taken a while to be put in place.
I guess this wasn’t one of the “shovel ready” ARRA projects that we heard all about.

Pamela sent me the following EMR stimulus question in response to my post about Doctor’s having no choice with meaningful use:
Can we show meaningful use without a certified EMR or would we still be penalized since our EMR vendor has no intention on becoming certified?
The current HITECH law requires meaningful use of a certified EHR. So, you have to use a certified EHR to get the EMR stimulus money and to avoid the Medicare penalties. They have been looking at doing site certifications so you could show that you’ve put together everything to satisfy the certified EHR portion. However, so far I’ve only seen the site certification really apply to hospitals. I haven’t seen a site certification for smaller doctors offices (yet?).
Just be aware that some are suggesting ...

A lot of “Health 2.0″ tools seek to help people change their behaviors to lead more healthy, productive lives. This is an admirable goal, and one I wholeheartedly endorse. Some of the tools are really “gee-whiz” neat!
However, many people involved with building Health 2.0 tools have little or no formal background in human behavior. How do you expect to build tools that seek to change human behavior, with no human behavior experts — you know, psychologists — consulting with you or on your staff?
That’s like trying to write a piece of software without a programmer.
In reply to a query on this topic, and how people change their behavior, I wrote the following over at the Society for Participatory Medicine’s e-patients.net blog. I think it enca...

However, I’ll offer what I know and hopefully the readers of the site can also chime in with their thoughts in the comments of this post. This question was posted in the comments of my previous EMR Stimulus question and answer post.
Can you help out another reader with a pressing question? Where can I find a good listing of EHRs for dentists? I’m looking specifically for ONC certified products that cater to dental practices and I’m coming up short so far. I know that DDS/DMDs can qualify for incentives under ARRA and they’ll be dinged on the Medicare side if they don’t use EHRs, but how are they supposed to comply if the software’s not out there for them? She's more subdued than usual, pale and under the weather as her cough worsens every day. But she has not been screaming in pain!
You know how I love when readers ask me questions. Especially when I can use them to create a blog post. Why you ask? Because then at least I know I’m making one reader happy. Although, if one reader had the question, it’s also quite likely that MANY more had the same question. On that note, the following is a question sent to me by Radhika:
To qualify for EMR stimulus money we need $24,000 in allowable medicare charges per physician. I am not sure whether we have to show the $24,000 over a period of 1 year or 90 days. Can you please clarify?
This is a great question. The short answer is you have the entire year to accrue the $24,000 in allowable Medicare charges that’s required to get the full EMR stimulus money in year 1 (you get paid 75% of your Medicare allowable char...

Was looking at the Spiegel website the other day; it had various links to continuing issues from WWII. It had a more spiffy than usual picture of a young Adolf in a brown shirt and brown tie. We usually look at WWII as the Germans had too high a debt load and were crushed too much etc. This might have been true in some emotional sense. The war aim of Germany during what became WWI was to gain some strategic depth from Russia. The German generals were afraid that Russian railroads could deliver troops to the eastern frontier of Germany too quickly. With the defeat of Russia and the creation of Poland that war aim seems to have been achieved however. (Source: a psychiatrist who learned from veterans)

Pauline Murphy asked the following EMR stimulus question:
If a chiropractor does not write perscriptions, does he still have the same e-prescribing requirements for “meaningful use”? How does that work, does anyone know?
Amanda Doss from Computer Link offered this answer:
No, the chiropractor would not have the same e-prescribing requirements for Meaningful Use. Page 61 of Meaningful Use discusses the “inapplicability” of measures to various provider types.
Basically, your chiropractor will have a denominator of “zero” for that measure. Also, on page 99 of Meaningful Use, you find (talking about e-prescribing) “this objective and associated measure do not apply to any Eligible Provider who writes fewer than one hundred prescriptions during the EHR reporting period.”...

Corey sent me the following question:
Are imaging costs recoupable under the HITECH act?
Corey,
There are 2 major sections to receive stimulus money from the HITECH act. One is through Medicare and the other is through Medicaid. The Medicare related stimulus money doesn’t have any provisions on how you use the money you receive. It is paid out as a bonus on your Medicare reimbursement. So, you could use the EMR stimulus money that’s paid out through Medicare for anything you want, including imaging costs.
Medicaid on the other hand has to be for qualified expenses (or some similar term like that). I personally haven’t seen any details on what HHS will consider an expense that can qualify for the stimulus money. Plus, I won’t be surprised if each state has different ...

Question: My EMR claims to have all of the capabilities they will need to demonstrate meaningful use and qualify for ARRA incentives. Does that mean I will get the EMR stimulus money?
First of all, you might want to consider another EMR vendor. If they are saying this, then they are trying to mislead you and make more sales. No EMR vendor can claim that they have “all of the capabilities” for you to show meaningful use and qualify for the EMR stimulus money.
Why can I say this? Because the details for Meaningful Use are still not final. We don’t know what you’re going to have to do to show meaningful use and so how can an EMR vendor claim to have all of the capabilities? Short answer is that they can’t.
EMR and HIPAA Answer to EMR Stimulus Questions
Today’s question comes from an EMR and HIPAA reader:
If I may back up, I am not having much luck in finding how we, as a clinic, get ’signed up’ if you will for this program. We already have EHR implemented. We are a rural health clinic that accepts Medicare and caid. We have been told by our EHR provider that they are CCHIT Certified. From what I have read, we are likely eligible however not sure how to get started. Could someone please advise. Thank you
Unfortunately, the answer to this question is that we still don’t know how Eligible Professionals (EP) or Eligible Hospital(EH) are going to be able to “apply” or “sign up” for the EMR stimulus money.
Be careful for nothing; but in every thing by prayer and supplication with thanksgiving let your requests be made known unto God. ~ Philippians 4:6This verse is an oldie, but goody (my brother Daniel used to say that as a small boy, and it still tickles me deep within to say it). Tonight I am up as the first pink glaze of sun appears in the pines out my bedroom window, heralding the end of night and beginning of another morn...up all night trying hard to put actions to this verse. It has been a night of prayer, a night of reading, a night of closing my eyes in near delirium and still the prayers flow and the sleep does not come. How many times had patients sat there waiting for her to announce her decision after a similar moment of respite? Invariably the decision was based on science and s...

Time again for everyone’s favorite topic (or so it seems), EHR stimulus questions and answers. Today’s EMR stimulus question comes from Jamie who asked the following question on a previous post:
Does anyone know whether or not the government is implementing some kind of incentive benefit for non-Medicare/Medicaid users? Or are these practices totally out of luck?
The short answer (at least for now) is not really. The ARRA EHR stimulus money is provided through Medicare and Medicaid programs as “bonuses” for those who show “meaningful use” of a “certified EHR.”

Her lab tests so far have been good news: no new infection, so what we are observing is still part of the same illness. However, her neurologist is considering changing her diagnosis from Acute Disseminated Encephalomyelitis (ADEM) to Multiphasic Disseminated Encephalomyelitis (MDEM). MDEM is slightly less dangerous and damaging than multiple sclerosis. However, it is a more severe illness than ADEM, with multiple episodes of demyelination that can be triggered by viral or bacterial illnesses, stress, or unknown environmental factors. Each episode of demyelination carries the risk of permanent damage, all the way up to paralysis, mental disability, or ...

I don’t think the book of Job is about suffering as a theoretical problem — why do the righteous suffer? I think it’s about suffering as a practical problem — when (not if) the righteous suffer, what does God expect of them? And what he expects is trust. When the righteous cannot connect the realities of their experience with the truths of God, then God is calling them to trust him that there is more to it than they can see. As with Job, there is a battle being fought in the heavenlies.Trust in God, not explanations from God, is the pathway through suffering.~ Ray Ortlund on the Book of JobI cannot connect the reality of this experience to truths of God that I hoped for in this circumstance - mercy, hope, love, joy, peace.
EMR and HIPAA Answer to EMR Stimulus Quesitons
When will they send the EMR stimulus checks?
I already posted about when the first EMR stimulus checks will go out. This question is a little different since it is about when during the year will you get paid the EMR stimulus once you’ve shown “meaningful use” of a “certified EHR.”
I honestly didn’t know the answer to this question until a few days ago. I’m still not completely sure on the process. Plus, I’m not sure if all the details have really been answered for this question. With that said, Justin Barnes from Greenway said during a webinar that you’ll receive your EMR stimulus check once you reach the max for that year. So, according to Justin, you’ll receive the check once you r...

Covered in mold, pock-marked with bruises and a hideous gray-green color, I had my doubts that anything lovely would be found within. Yet what that ugly shell held, protected and cloistered deep inside, was the most beautiful, soft orange fruit and glistening black seeds. I have felt a bit hideous myself over the past three weeks. Rent open by the ax that was the marriage conference, battered and bruised by day after day on my feet, poring my energy into food for 100 and trying to ignore the pain in myself, my husband, my children, I peered over the edge of my soul into the fissure there with some trepidation...

EMR and HIPAA Answer to EMR Stimulus Quesitons
When’s the earliest we can get the ARRA EMR Stimulus Money?
Medicare Related EMR Stimulus Money
The earliest payments for those wanting the $44k per provider of stimulus money from Medicare will be Spring of 2011. Providers will need to show “meaningful use” of a “certified EHR” for 90 days in 2011 to be able to qualify for the first year payment of $18k. So, the very first checks won’t be sent out until at least Spring of 2011.
Medicaid Related EMR Stimulus Money
Unfortunately, we don’t really know the answer to this question. It’s also made even more difficult since each state will be able to decide when and how they are going to pay out the $63,750 in EMR stimulus money for Medicaid providers....

EMR and HIPAA Reader Submitted Question:
Is there a way for a provider to get stimulus money for EMR if not currently a Medicare provider?
Besides Medicare, there is also stimulus money available through Medicaid. In fact, Medicaid has more money per provider available than Medicare, but also has different requirements to obtain the EMR stimulus money. Outside of that, ONC has a number of grants that might be available for organizations that qualify and want to go through the grant writing process. Otherwise, NO there is no stimulus money if you don’t take Medicare, Medicaid or qualify for some special grants.

EMR and HIPAA Reader Submitted Question:
Can you get ARRA stimulus money for systems already installed? Thank you.
The simple answer is YES!
The longer answer is that ARRA makes no differentiation between those that have previously installed an EMR, are currently installing an EMR or that will install an EMR in the future. the HITECH Act (ARRA) just requires that a provider shows “meaningful use” of a “certified EHR.” So, as long as the EMR which you previously installed is a “certified EHR” (still yet to be defined) and can support you showing “meaningful use” (almost defined), then you’d be entitled to the EMR stimulus money.
If you’d like to have a question for me to answer, submit it to the EMR and HIPAA contact us page.

If not, to God be the glory as well. These photos are from a few days ago. What do you do when cancer lurks once again in the wings? Pull out a tea set! John Piper (12/27/09) preached on suffering and Romans 8:15-25 at the Village Church who recently learned that their young senior pastor, Matt Chandler, has a malignant brain tumor. (See these posts on suffering).Piper introduced his sermon by stating that the first thing believers should do do with those who are suffering is hug them. No words. Just silent hugs. But, Piper also noted that those who are hugging must have a place to stand. That is, if we are going to make it through the suffering in this life apart from anger and bitterness, then we must know sound doctrine and how...

I asked God if Theodore was the price for my sins. My many sins. I have sneered at him, in my soul - this God who says He is all-loving, all-powerful, ever-present, merciful, provident, healing, providing; Savior, Deliverer, Shield, Redeemer. Taking my child, the child I never even asked for, planting that child in me, in a place He knew it would die. What is the purpose of that, God?
There is so much that is raw right now. It stops up my words, all this pain that threatens to bubble over the top and consume my days. I find comfort in the darkest books of the Bible...Lamentations, Daniel, Joel, Habakkuk, Isaiah, some of the Psalms. In a season that is all about praise, I am more on my knees in wordless agony of emptiness and concomitant desperation of waiting for resolution. Waiting for heaven. I had two miscarriages before this one, and I haven't ever suffered so. There is something so tangible about this baby...my sister-in-law down the street, one of my dearest friends. Calling me to tell me her baby is moving. Her baby that she found out about two days before I found out about mine. It just seems so cruel that I have to suffer this now, that I will suffer this for t...

In his book “Eastern Wisdom for Western Minds,” Victor M. Parachin tells a Japanese tale about how powerful our emotions can be, and how we must manage them, not vice versa. He writes:
A Japanese samurai warrior visited a Zen master, seeking answers to questions that had plagued him for some time.
“What is it you want to know?” asked the Zen master.
“Tell me, sir, do heaven and hell exist?”
“Ha!” laughed the Zen master in a contemptuous tone. “What makes you think you could understand such things? You are only an educated, brutish soldier. Don’t waste my time with your ridiculous questions.”
The samurai warrior froze in shock. No one spoke to a samurai that way. It meant instant death. Increasing the tension, the Zen master ...

As I watched them walk out the door ahead of me, waiting for the perfect shaft of light to illuminate girlhood and infancy lilting through the sunset and out of the cavernous old barn, a million old emotions flooded back. That moment...watching someone I love dearly walk off with my son, facing forward toward the beauty and possibility that is life outside the cavern...I remember the moments I spent watching my kids dance in a lamp lit living room in November. Isolated, shivering, standing in the dark, surrounded by the eerie loneliness of the sounds of city nightlife. Feeling my life slip like so many grains of sand through an open palm. The glass shut me out, just like the length of that barn floor did. Being an outsider is never pleasant, especially when your heart is heavy with worry.T...

So, at long last, here it is your interview with me. I hope you enjoy. –Trevis!

Thanks to each and everyone of you that took the time to comment on this website, on my Facebook Wall, and who sent me personal emails regarding the untimely death of our wonderful personal care physicians--Doctor Carlos Chiriboga.

Your kind words give me comfort, and importantly help empower me to move forward.

I already started the search for a new personal care physician. Unlike, last time, I actually know what I am doing this time.. Many of you know it took me 14 months, and 4 physicians to get it right last time around. I am now realizing how little I knew at the time.

I now realize, I was asking the wrong questions and looking in the wrong places.

Here are the dumbest questions I asked last time around.

Do you have patients in your practi...

Why do banks fail?
In the US, banks fail when the FDIC decides they are under-capitalized and force them to sell to another company. This happens when a company makes a bunch of loans that go bad. Much of the current financial crisis was brought on by banks investing in complex investments in mortgages that turned out to be much more risky than originally thought.

It is not only those newly diagnosed (or awaiting confirmation of diagnosis) who wonder if the things happening to their bodies are really caused by multiple sclerosis. I’ll admit to a time, now and again, when I wonder, aloud sometimes, “Do I really have MS?”
My medical team assures me.
I’m not saying that I/we don’t battle our symptoms daily. I’m not trying to minimize the effects this disease can/does have on us. I’m just saying…

Why We Believe Medical Myths

Why do we keep clinging to myths, even when research or other facts tell us the myths aren’t true? That’s the question posed by Newsweek’s Sarah Kliff, discussing a new book put out by Vreeman and Carroll, who blow away 66 new medical myths in their new book, Don’t Swallow Your Gum!
The research offers only a few answers as to why we keep believing things like we must drink 8 glasses of water a day (myth) and the belief that vitamin C helps cure the common cold (myth):

The body of research on belief formation is relatively sparse. One expert in the field, York University psychologist James Alcock, admits that it’s difficult to trace where beliefs start.
“Even as individuals we usually can’t explain where beliefs come from,” says Alcock, who...

Trip Answers is a new site from the people that are behind the wonderful Trip Database (see posts Use TRIP to Navigate to Best Evidence, …and I though QI involved Stephen Fry,
Was that Trip to Bethlehem Critical? for details). It aims to be a resource where clinicians can easily find answers to their question by building a repository of clinical questions and answers drawn from a wide number of sources around the world.
Well laid out, simple to navigate with great use of tag clouds (love the Tag Cloud of Uncertainty as a concept) and a clear rating system for the answers given it’s everything we expect from a Trip product.

Sue’s palliative care questions
Sue left this comment at do you have a liver cancer story?
God Bless You for what you are trying to do.
I am taking care of my 81-year-old mother who up until last year looked and behaved like a 50-year-old. It is so hard to see her in the state that she is in now. She is so weak and does not want to eat anything. I beg her to eat because of all the meds she is taking but she does not want anything. She does drink a lot of water though. She does not want to go to palliative care and so I am doing the best that I can at home. It is so scary and sad.
How do I know when the end is approaching? I would appreciate any help you can provide.

Heath Ledger’s autopsy inconclusive so far

When death strikes someone at a young age, it is always tragic - and there are always questions. For most, the questions are limited to the family, friends and perhaps to some in the local community. For celebrities, the questions become national, even international. Sometimes the questions are about the cause of death, but they always involve the whys. Why them, why now, why our child? These latter questions are existential in nature and are rarely, if ever, fully answered. The former, however, are the purview of the pathologist, a physician with specialized training in solving the physical causes of death. Sometimes the answers are obvious, sometimes not. On rare occasion, they are never determined. In the case of Heath Ledger, the questions remained unanswered as of this writing.

From Panic to Peak Performance - Possible?

If you find yourself overwhelmed with workloads, venting about demanding colleagues, or panicked through financial pressures … you’ll be interested in new research that links panic to heart trouble down the road. Your own brain actually holds answers that could turn panic into peak performances.Determination is not enough to eliminate panic ... nor is venting, though. For some time, researchers warned us that persistent panic can lead you into places of depression… and now heart attacks and stokes have been added to panic dangers. One study of 3000 participants just published in Archives of General Psychiatry reported that rapid pulse and shortness of breath that comes with panic attacks – may even signal heart trouble at a later date.
