To ensure rational and responsible dissemination of this new
technology (transcatheter aortic valve replacement [TAVR]), government,
industry and medicine will need to work in harmony.”
- David R. Holmes, Jr., MD, FACC
President, American College of Cardiology
Today, Edwards Lifesciences’ will request pre-market approval of its SAPIEN Transcatheter Heart Valve from the FDA’s Circulatory Systems Devices Panel of the Medical Devices Advisory Committee. And for the first time, the groundwork for our complicated new era of health care rationing will be exposed.
To win an expensive technology on behalf of patients these days, there will have to be “harmony” between doctors and their professional organizations and government regulators. If not, patients lose.

Voiceover speaks in the background..."It has worked for years, tirelessly opening and closing. Now, saddled with the challenges of age, Barbara Walters spends a few comfortable moments interviewing her critically narrowed aortic valve, for what comes next...""So, A.V., this must be very hard for you, being replaced and all."Tears welling..."Yes.... yes, it is. I mean all these years opening and closing, opening and closing and just because I get a little stiff, this is the thanks I get. The prospect of some cow or pig thing doing what I've been doing for years.... It's devastating....""What will you do next?" "I mean, that's just it! Don't you see? I'll just sent packing to some cold, silver kidney basin to the basement, only to be palpated and sampled, frozen and sectioned, while they get...

DISCLAIMER: I am no Barbara Streisand. I’ve never taken a singing lesson in my life. Not one. (Ed. - Really??)
The inspiration for this song came two weeks before Thanksgiving when I received a call from my doctor: we had better get together a plan for lowering my cholesterol.
Yeah. Sure. Getting to it… right after I fix my pituitary tumor, abnormal aortic valve, bipolar brain, and facial fungus.
My high cholesterol is not a new issue. Somewhere around the time David was conceived I found out that my cholesterol scores were higher than my verbal SATs. But for the last six years I didn’t do anything about it because (honest confession coming up here) many of those days I was so depressed that I really wanted to be with God in his mansion upstairs. Dropping dead of a heart...

Further, if you want to go ahead and calculate your own STS (Society of Thoracic Surgery) Risk Calculator for placement of a heart valve, heck, you can do that also. Do not, however, look for objective criticism of the various valve types since most device literature there fails to include warnings, containdicati...

He's an exerpt from a candid video where Robin Williams discusses his rationale for choosing the type of valve replacement he received: "... here are your choices: pig valve, which is great, you can find truffles, mechanical valve, which clicks and you know, weird, (eds note: that also requires chronic anticoagulation)and bovine - great, 'I'll go bovine, Bob, for 20!'"The video provides a very real (and, yes, humorous) glimpse into the emotional and physical impact of aortic valve surgery as only Robin Williams can portray.-WesMusings of a cardiologist and cardiac electrophysiologist. Source: NICE
The Skinny: Endovascular stent-grafts are recommended as a possible treatment for people with abdominal aortic aneurysms if:

the aneurysm is below their kidney and has not burst,
surgery is appropriate, and
the person and their specialist have discussed the risks and benefits of this and other procedures and decided that endovascular stent–grafts are appropriate.

Aneurysm repair using endovascular stent–grafts should only be carried out in specialist centres by staff experienced in treating people with abdominal aortic aneurysms.
Endovascular stent–grafts are not recommended for people whose abdominal aortic aneurysm has burst unless they are taking part in research with the data collect...

I’ve been trying to master mindfulness in the last few weeks like it’s a cute step sequence in a line dance. I have unofficially hired Dr. Elisha Goldstein, author of Psych Central’s blog, “Mindfulness and Psychotherapy” as my mindfulness personal trainer because he knows this stuff inside and out, and because I don’t have the time or money to hang out with the Buddhist monks in Tibet.
I’ve always aspired to better live in the moment–it was one of the gems I picked up in support group meetings back in college–but now I honestly feel like it could save my life–or at least keep my pituitary tumor from growing any wider and shield my heart from any more damage to the aortic valve.
How do you practice presence, or mindfulness?

Heart surgery for Robin Williams: Success

Robin Williams underwent heart surgery on March 13th, which included replacing the aortic valve and repairing the mitral valve. The 57-year-old comedian is reportedly doing very well.
The aortic valve is a tricuspid valve - it has three leafs that open and close. The aorta, which is the largest artery in our body pumps cleaned, oxygenated blood from the heart to the rest of the body. The aortic valve allows the oxygenated blood out and then closes so that the blood can’t back up. The valve can either malfunction or become sclerotic , which means that it starts to thicken, narrowing the passageway, making it more difficult for the valve to work properly.

Aortic valve surgery isn’t unusual, even at 57. There are many reasons why the valve may malfunction. Som...

Heart valve replacement surgery is preformed more than 250,000 times per year.
Even with this large a number of surgeries, there was little information on the subject, as Adam Pick and his family would find.
Adam found at even the young suffer any number of problems with their hearts. He was diagnosed with severe stenosis and regurgitation in his aortic valve, and that both the aortic and pulmonary valves would have to be replaced through surgery.
After receiving this information Adams family and friends started a search for information on open heart surgery. They searched for books, literature, and on line articles on open heart surgery to no avail. The material that was found did not give them any peace of mind.
As we all know stress is the biggest enemy that there is for people with hea...

A 92 year old Australian women is the first to undergo a ground breaking procedure. In the radical procedure, a new aortic valve is attached to a long rod and fed through a small incision in the groin up to the the heart, where it defrosts and expands. Wow!
This will take the place of open heart surgery for those that are too old and frail to undergo surgery. This will also help with cost and recovery time.

Here's a fungal infection you don't hear much about. One of the fungi we work on, a model for mushroom development as it can be fruited in the lab is Coprinopsis cinerea (previously named Coprinus cinereus). C. cinerea is a saprobric coprophillic fungus so it is usually found on dung. Although rare in human infections there are a few reports in immunocopromised patients. Below is an abstract describing isolation of C. cinerea from an implanted heart valve from a pig. This definitely not its typical habitat and Coprinus growing in yeast form I'm sure I've really heard of either. Would be great to see if the clinical strains are still sexually competent and/or are significantly different in other ways (growth rate, resistance to drugs and oxidative stress) from the wild or laborat...
