            <description>This article reports on a systematic review and meta-analysis of randomised controlled clinical trials (RCTs) assessing associations of structured exercise training regimens (aerobic, resistance, or both) and physical activity advice with or without dietary cointervention on change in haemoglobin A1c (HbA1c) in type 2 diabetes patients. The article concludes that structured exercise training that consists of aerobic exercise, resistance training, or both combined is associated with HbA1c reduction in patients with type 2 diabetes. Structured exercise training of more than 150 minutes per week is associated with greater HbA1c declines than that of 150 minutes or less per week. Physical activity advice is associated with lower HbA1c, but only when combined with dietary advice.

            <description>This article aims to  evaluate the association between low health literacy and all-cause mortality and hospitalisation among outpatients with heart failure. The article concludes that among patients with heart failure in an integrated managed care organisation, low health literacy was significantly associated with higher all-cause mortality.

            <description>This article is a case-cohort study examining the association of maximum prescribed daily opioid dose and dosing schedule (“as needed,” regularly scheduled, or both) with risk of opioid overdose death among patients with cancer, chronic pain, acute pain, and substance use disorders. The study concludes that among patients receiving opioid prescriptions for pain, higher opioid doses were associated with increased risk of opioid overdose death.

Fade Skinny:Although incarceration during pregnancy is both stressful and dehumanizing, most studies paradoxically document better outcomes for pregnancies managed behind bars than for women of similar socioeconomic status whose pregnancies are managed in the community. Using the case of Ms A as a springboard for discussion, the issues, benefits, and challenges of caring for an incarcerated pregnant woman are addressed, as is the importance of family planning services to those about to be released.

            <description>This study examines the accuracy and outcomes of mammography screening in women with a personal history of breast cancer (PHBC) relative to screening of similar women without PHBC. The article concludes that mammography screening in PHBC women detects early-stage second breast cancers but has lower sensitivity and higher interval cancer rate, despite more evaluation and higher underlying cancer rate, relative to that in non-PHBC women.

            <description>This study aimed to describe the inpatient resource utilisation, clinical characteristics, and admission reasons of patients recurrently readmitted to children&amp;#8217;s hospitals in the United States. The article concludes that among a group of paediatric hospitals, 18.8% of admissions and 23.2% of inpatient charges were accounted for by the 2.9% of patients with frequent recurrent admissions. Many of these patients were rehospitalised recurrently for a problem in the same organ system.

            <description>The objective of this study is to perform a systematic review and meta-analysis of published randomised controlled trials (RCTs) to determine the overall risk of fatal adverse effects (FAEs) associated with bevacizumab, an angiogenesis inhibitor, used in conjunction with chemotherapy. The article concludes that in a meta-analysis of RCTs, bevacizumab in combination with chemotherapy or biological therapy, compared with chemotherapy alone, was associated with increased treatment-related mortality.

            <description>This article aims to evaluate the relationship between gait speed and survival in geriatric patients. Pooled analysis of 9 cohort studies (collected between 1986 and 2000) measured the survival rates and life expectancy in geriatric patients. Predicted survival           based on age, sex, and gait speed was as accurate as predicted based on age, sex, use of mobility aids, and self-reported           function or as age, sex, chronic conditions, smoking history, blood pressure, body mass index, and hospitalisation. The article concludes that in this pooled analysis of individual data from 9 selected cohorts, gait speed was associated with survival in older adults.

A prospective longitudinal study with 20 years of           follow-up was carried out 1985-1986 to 2005-2006 of average annual changes in BMI and waist circumference of 3554 men and women aged 18 to 30 years           at baseline. The article concludes that maintaining high activity levels through young adulthood may lessen weight gain as young adults transition to middle age,           particularly in women.

            <description>This article aims to examine the benefits of aerobic training alone, resistance training alone, and a combination of both on haemoglobin A1c (HbA 1c) in individuals with type 2 diabetes. The article concludes among patients with type 2 diabetes mellitus, a combination of aerobic and resistance training compared with the nonexercise control group improved HbA 1c levels. This was not achieved by aerobic or resistance training alone.

            <description>This article investigates the survival of patients with out-of-hospital cardiac arrest using compression-only CPR (COCPR) compared with conventional CPR.  The study design was a 5-year prospective observational study of survival in patients at least 18 years old with out-of-hospital cardiac arrest. The study concludes that among patients with out-of-hospital cardiac arrest, layperson compression-only CPR was associated with increased survival compared with conventional CPR and no bystander CPR in this setting with public endorsement of chest compression–only CPR.

            <description>This article aims to determine the efficacy of brief interventions addressing violence and alcohol use among adolescents presenting to an urban Emergency Department. All patients underwent a computerized baseline assessment and were randomized to a control group that received a brochure (n = 235) or a 35-minute brief intervention delivered by either a computer (n = 237) or therapist (n = 254) in the ED, with follow-up assessments at 3 and 6 months. The article concludes that among adolescents identified in the ED with self-reported alcohol use and aggression, a brief intervention resulted in a decrease in the prevalence of self-reported aggression and alcohol consequences.

Fade Fave: Smoking cessation: the potential role of risk assessment tools as motivational triggers
Fade Skinny: Utilising tests of lung function and/or genetic susceptibility personalises the risk and have been reported to achieve 1 year quit rates of 25%. This is comparable to quit rates achieved among healthy motivated smokers using smoking cessation drug therapy. This paper reviews existing evidence and proposes that identifying those smokers at increased risk of an adverse smoking related disease may be a useful motivational tool, and enhance existing public health strategies directed at smoking cessation.

Fade Skinny: Aims to derive and validate risk algorithms so that the risks of four clinical outcomes associated with statin use can be estimated for individual patients. It acheives this in the case of acute renal failure, moderate/serious myopathy and cataract but not in the case of an algorithm to predict liver dysfunction. The algorithms developed could be used to identify patients at increased risk of these adverse effects enabling patients to be monitored more closely.

Fade Skinny: The Portraits of Care study used portraiture to investigate ideas about care and care giving at the intersection of art and medicine. The study employed mixed methods involving both qualitative and quantitative research techniques. All aspects of the study were approved by the Institutional Review Board. The study included 26 patient and 20 caregiver subjects. Patient subjects were drawn from across the lifespan and included healthy and ill patients. Fade Fave: Victorian medical politics: the fate of Dr Alfred Stephens (1821–90) 
Fade Skinny: Dr Alfred Stephens was the co-founder of the Liverpool Children&amp;#8217;s Infirmary. The institution was funded initially by Matthew Gregson (1800–76) and later by public subscription. Opened in 1851, it was the second Children&amp;#8217;s Hospital in England. He later perceived that the free treatment of children at the Infirmary affected his income and he redirected patients to his practice. This led to confrontation with his colleagues and the Board of the Infirmary, and good relations were never restored. His name has not been commemorated nor his charitable work remembered.

Fade Fave: Advances in antiplatelet treatment for acute coronary syndromes
Fade Skinny: Recent publications of TRITON-TIMI (prasugrel) as well as PLATO (ticagrelor) have introduced new and potent antiplatelet agents, but at the same time have left clinicians with multiple choices without clear directions regarding the most appropriate use of these agents. Finds both prasugrel and ticagrelor are potent antiplatelet agents with improved ischaemic benefits in comparison with clopidogrel. The improved benefit, however, comes at the price of increased bleeding. As such, careful patient selection and balancing of benefit-risk is warranted to optimise their use in clinical practice.

Fade Skinny: Aims to to determine the accuracy of the diagnosis of gastro-oesophageal reflux disease (GORD) by the Reflux Disease Questionnaire (RDQ), family practitioners, gastroenterologists and a test of esomeprazole therapy. Finds that RDQ, family practitioners and gastroenterologists have moderate and similar accuracy for diagnosis of GORD. Symptom response to a 2 week course of 40 mg of esomeprazole does not add diagnostic precision.

Fade Skinny: Aims to assess whether the localisation of the postprandial acid pocket and its interaction with a hiatal hernia (HH) play a role in the occurrence of acidic reflux during transient lower oesophageal sphincter relaxations (TLOSRs). Finds that the position of the acid pocket is largely determined by the presence of a HH. Entrapment of the pocket above the diaphragm, is a major risk factor underlying the increased occurrence of acidic reflux during a TLOSR in patients with gastro-oesophageal reflux disease (GORD).

Fade Skinny: Assesses whether the benefits of combining spirin, β-blockers, statins, renin angiotensin system (RAS) blockers and thienopyridines (termed optimal medical therapy, OMT), will result in a reduction of mortality in clinical practice. Finds that OMT over 1 year was associated with a significantly lower mortality of patients with acute myocardial infarction in clinical practice. However OMT is provided to less than half of eligible patients leaving room for substantial improvement.

Fade Skinny: Aims to estimate the long-term true change variation (‘signal’) and short-term within-person variation (‘noise’) of the different lipid measures and evaluate the best measure and the optimal interval for lipid re-screening.  Finds that the signal-to-noise ratios of standard single lipid measures (TC, LDL and HDL) are weak over 3 years and decisions based on these measures are potentially misleading. The ratios, TC/HDL and LDL/HDL, seem to be better measures for monitoring assessments. The lipid re-screening interval should be &amp;gt;3 years for those not taking cholesterol-lowering drugs.

Fade Skinny: One of the United Kingdom’s best selling authors has called for an assisted suicide tribunal to which people could apply for permission to end their lives at a time of their own choosing. The call by Terry Pratchett, who has early onset Alzheimer’s disease, came as the BBC released the findings of a poll showing that nearly three in four people believe that a friend or relative should be able to assist a terminally ill loved one to commit suicide without fear of prosecution.

Fade Skinny: Evaluates the impact of motivational interviewing-based health coaching on a chronically ill group of participants compared with non-participants. Specifically, measures that could be directly attributed to a health coaching intervention on chronic illness were assessed. It finds hypertension predicts progression to dementia in older subjects with executive dysfunction but not memory dysfunction. Control of hypertension could prevent progression to dementia in one-third of the subjects with cognitive impairment, no dementia.

Fade Fave: Patient use and compliance with medical advice delivered by a web-based triage system in primary care
Fade Skinny: We studied a web-based triage system which was accessible to the general public in the Netherlands. In a retrospective analysis we investigated the type of complaints that were submitted and the kind of advice provided. Over a period of 15 months, 13,133 different people began using the web-based triage system and 3812 patients went right through the triage process to the end. The most frequent complaints were common cold symptoms, such as cough and a sore throat (22%), itch problems (13%), urinary complaints (12%), diarrhoea (10%), headache (8%) and lower back pain (8%).

Older techniques involve removal of the outer skin layers using mechanical (dermabrasion and salabrasion), chemical, or thermal (cryosurgery and cauterization) methods. Fade Skinny: Otitis media with effusion, a collection of fluid behind the ear drum without inflammatory signs, is often called &amp;#8220;glue ear&amp;#8221; when present for six weeks. It is an increasingly common presentation in primary care and is probably the most common reason for surgery in children. Otitis media with effusion can lead to significant hearing loss, especially when both ears are affected, and has an important impact on children’s lives and development.

Fade Fave: Infectious Burden and Risk of Stroke: The Northern Manhattan Study
Fade Skinny: Considers the association between a composite measure of serological test results for common infections (Chlamydia pneumoniae, Helicobacter pylori, cytomegalovirus, and herpes simplex virus 1 and 2) and stroke risk in a prospective cohort study. It shows a quantitative weighted index of infectious burden was associated with risk of first stroke in this cohort but this requires further research to confirm these findings.

Fade Skinny: The challenges to providers to develop high-quality services for their customers have never been greater. In the UK, the Darzi Review has focused the attention of clinicians and managers on the need to sustain and improve quality. This paper examines these challenges and considers how the Southampton University Hospitals NHS Trust anticipated and responded to them by creating an innovative patient improvement framework linking patient experience, outcomes and safety with its corporate strategies. The paper then reflects on the shared learning from and results of this approach for clinicians and managers.

Fade Skinny: A normal view of the world has been simulated for millennia as a clear image over an entire scene: on the sides of caves, in art, and currently in photography and electronic media. Indeed, this is a valid representation insofar as an individual with normal vision can scan the world to examine each detail with clarity, wherever it may be. In this scheme of representation, macular disease is typically simulated as a hazy or gray spot in the center of a picture, and many variations of this approach have been used for physician and patient education.
