I am smacking myself on the forehead and saying, “Why didn’t I think of this?” Dr. Richard Parker, Medical Director at Beth Israel Deaconess Medical Center, has sent out a list to his physician colleagues of 56 common medical tests and procedures. What is revolutionary is that there are prices next to each item. You non-physicians may be surprised to know that we doctors have no idea what the tests or drugs we order actually cost. Unless we get billed as a patient, we are as clueless as you are.
As I wrote before, the ostrich excuse just won’t fly any more. We all need to be aware of the cost of care and have skin in the game. Some will argue that price can’t be the only driver. I’ve heard physicians say you can’t compare one price ...

By Jim HarperLate last week, the Project on Government Oversight‘s Danielle Brian took a little umbrage at a Huffington Post piece by former U.S. Deputy Chief Technology Officer Beth Noveck, who had been implementing the Obama Administration’s Open Government Initiative until she recently returned to New York Law School.
Brian’s piece suggests a slight schism in the transparency community, between what I believe are the “insider” and “outsider” camps. Brian leaves to the end a crucial point: “[C]an’t the two camps in the open government world peacefully co-exist? There’s just too much work to be done for us to get bogged down in denigrating each others’ agendas.” They most certainly can.
Noveck was a bit dismissive of ...

After months of overseeing a series of controversial moves at the FDA press office, Beth Martino has resigned to take a job with the American Health Care Association as a public affairs advisor. Her departure caps a brief, but tumultuous reign for the 31-year-old former press aide to Kathleen Sebelius, the former Kansas governor who is now Department of Human Health & Services secretary.
As much as she attempted to manage the news, Martino succeeded in making news herself. In December, her office purged a few senior specialists who are 50 years and older. Three staffers were let go and a fourth, career employee Donna Avallone, 62, was stripped of her title and reassigned by Carl Chitwood, a 37-year-old Martino appointee, although he later denied forcing anyone out of a job (back story...

Yet another personnel problem is weighing down the FDA media office. This time, the FDA Inspector General is investigating hiring practices after Carl Chitwood, the recently appointed director of web communications, resigned amid allegations that he hired a former colleague for an FDA job, even though other individuals were better qualified for the position.
The tumult may eventually envelope Beth Martino, the embattled associate commissioner for external affairs, since she approved the hiring, according to the AssurX blog. In response to questions, Meghan Scott, the FDA deputy director for external affairs, would only write that: “I am unable to comment on these personnel matters, as the investigation into Carl Chitwood is ongoing.”
The recent hire was reportedly tapped for a ...

The workings of the media may not always be clear, but one long-standing practice - especially among medical journals - is to apply an embargo on information before publication. Lately, however, this has become controversial as a growing number of journals and institutions are adding various requirements, notably barring journalists from seeking expert comment prior to the moment an embargo is lifted.
The issue has gained considerable traction thanks, in part, to coverage provided by a relatively new blog called Embargo Watch. And the discussion picked up steam recently when the FDA adopted the same approach as new policies for approving medical devices were announced. And so the Association of Health Care Journalists has written the agency for clarification.
“The restriction imposed...

There isn’t much new in the latest iteration of the “Dietary Guidelines for Americans.” Three years in the making, the 2010 guidelines (released a tad late, on January 31, 2011) offer the usual advice about eating less of the bad stuff (salt; saturated fat, trans fats, and cholesterol; and refined grains) and more of the good stuff (fruits and vegetables; whole grains; seafood, beans, and other lean protein; and unsaturated fats). I’ve listed the 23 main recommendations below. You can also find them on the “Dietary Guidelines” website.
The guidelines do break some new ground. They state loudly and clearly that overweight and obesity are a leading nutrition problem in the United States, and that a healthy diet can help people achieve a healthy weight. They also r...

I also will be the moderator of a panel discussion on patient empowerment. As I prepare, I am interviewing the panelists and their stories are very inspiring.
One panelist is Bonnie Addario. Bonnie had been an oil company executive in the Bay Area. She began having chest pain. Was it her heart? No. Was it a nerve problem? No. Doctors were stumped. Bonnie was frustrated, but she was also a woman of action — a “powerful patient.” She went on her own for a full body scan. The news was not good.
Oh, to be young. And that appears to be a requirement to work in the FDA press office, at least according to FDA Webview, which reports that FDA associate commish for external affairs Beth Martino, who is a 31-year-old former Kansas aide to HHS secretary Kathleen Sebelius, is purging senior specialists who are 50 and older.
The moves were described as abrupt and undertaken to make room for younger people closer to her age. Three of those let go: Elaine Gansz Bobo, 52, Dick Thompson, 65, and Ira Allen, 62, who were told they could be terminated as probationary hires for “incompetence” or resign, FDA Webview writes. Bobo and Allen had to immediately clean out their desks and were escorted off the premises, while Thompson, a former Time magazine Washington bureau science editor and high-...

We recently reported our interview with Dr. Henry Feldman of the Beth Israel Deaconess in Boston and his experience using the iPad as his sole computing device while attending on the wards. Overall, his experience was positive, while accessing the hospital networks, using clinical applications and questions about security. Be sure also to check out Future Docs blog and Dr. Arora’s experience using the iPad on the wards to get more real-world perspectives on using the iPad on the wards.
Among the few difficulties Dr. Feldman had, one was that typing long notes on the glass keyboard was cumbersome, requiring the use a desktop computer for admission and discharge notes. This may now turn out to be one of the easiest problems to solve, if two recently announced iPad cases are any indication...

The Heart Health Risks Of Being A Couch Potato

We’ve all made the excuses: You can’t face the drive to the gym, you’re too tired at night, getting up in the morning is a chore, or it’s too hot or cold outside. So you cozy up on the couch in front of the television. If you’re a couch potato, you’re a gambler — with your life.

Unfortunately you’ll need a big sofa because you’re not the only one whose heart isn’t in physical activity. About 60 percent of adults in the U.S. are not getting the exercise they need, according to a report from the U.S. Surgeon General.
It’s time to get up and face — or better yet, dance to — the music! Here are a few facts that may get you moving for your heart’s sake. (more…)

Why do doctors bad mouth other doctors ?

Many patients find that the doctor you go to for a second opinion will often be very critical of the first doctor. When a new doctor criticises what the old one did, patients feel frustrated, because they feel it reflects badly on their choice of a doctor. Why are doctors so happy to criticise their colleagues and peers at the drop of a hat ?Some of this is simply a power game. Doctors do their best to try to look good for their patients, and by demonstrating the errors and shortcomings of the earlier doctors, they are emphasising how much good they are ! The hidden subtext is - You made the right decision when you came to me - I am the best !Some of this is simple oneupmanship. Doctors are highly competitive and proud of their intellectual prowess and surgical skills. They take delight in ...

I recently spoke at the panel on transparency at Edelman’s New Media Academic Summit. Ben Boyd was the moderator and Ellen Miller from the Sunlight Foundation was my fellow panelist.
Reviewing some of the #nmas10 tweets from the audience, I figured I should provide some links for the anecdotes I mentioned:

Beth Israel Deaconess Medical Center CEO Paul Levy’s blog is still the starting point when talking about transparency in medicine today. I had the chance to speak with him a few years ago.
Ed Bennett has done an extraordinary job following hospital social media adoption and highlights effective new media policies as well.
Hospitals are using twitter and billboards to broadcast emergency department waiting room times. This is not without risk, as billboards may not clarif...

It’s time to ask patients whether they text and drive. An important perspective piece from the New England Journal of Medicine urges doctors to include that question during preventive health exams. The data surrounding texting and driving is grim:
Although there are many possible distractions for drivers, more than 275 million Americans own cell phones, and 81% of them talk on those phones while driving. The adverse consequences have reached epidemic proportions. Current data suggest that each year, at least 1.6 million traffic accidents (28% of all crashes) in the United States are caused by drivers talking on cell phones or texting. Talking on the phone causes many more accidents than texting, simply because millions more drivers talk than text; moreover, using a hands-free device does...

.fullpost{display:inline;}As any left-handed person that has struggled to use a pair of scissors or a fountain pen will verify, the design of a tool impacts the quality of the work done with that tool. That’s true for electronic health records, as well. EMRs support complex cognitive processes, and the way they are designed directly impacts the speed and accuracy of a clinician’s work. (Source: Healthcare IT News Blog)

Many providers have noted in various surveys and many speakers at conferences have said the federal incentives will not drive EHR adoption because there is no business case to implement EHRs.
Now is the time for health-system pioneers of EHRs to analyze their data and highlight where EHRs have contributed to increases in patient safety and quality of care and decreases in cost. Third-party entities need to validate the results. Then, the results need to be broadcast as widely as possible. (Source: Healthcare IT News Blog)

Our focus has been on ambulatory practices, and EHR systems geared toward them. (Source: Healthcare IT News Blog)

The health care industry has been a bit “late to the game” when it comes to social media. However, this week at the Mayo Clinic in Scottsdale, approximately 100 health care communications professionals came together to explore strategies for catching up.
At an event hosted by Ragan Communications, speakers from Mayo Clinic , Beth Israel Deaconess Medical Center, Operation Smile , Kaiser Permanente and others all came to share their experience jumping into the world. Each of us are exploring the opportunities that social media presents to us as communicators, while at the same time balancing the regulations within a fairly conservative industry. Concerns about protecting patient privacy and overcoming cultures that too often fear transparency were significant challenges for all.

Our home is our castle. A place to feel secure and safe.
But after reading Beth Greer’s book Super Natural Home, I have to say, I’m starting to wonder exactly how safe my house really is. According to Beth Greer, probably not all that safe, especially if you start taking into account all the dangerous chemicals that we invite in and have around our house at all times.
Beth used to be just like you and me, living what she considered a healthy but busy lifestyle. But when a medical crisis (a 5-centimeter tumour in her chest) caused her to re-evaluate her lifestyle, she discovered that maybe her lifestyle wasn’t as healthy as she thought. In fact, she discovered instead of being healthy, it was downright toxic.
So she spent six months cleaning up ‘her act’, eating an all-organic die...

We recently posted about executives at two different not-for-profit health care insurance companies/ managed care organizations whose pay seemed to keep levitating, despite organizational financial losses, and commented on how the compensation of top executives of health care organizations seems always to go up, regardless of the financial fortunes, or quality of the products or services provided by their organizations. (Posts here and here.)Today's Boston Globe, however, provided a contrast. The background is that the Beth Israel Deaconess Medical Center (BIDMC), a renowned Harvard teaching institution, is facing a budget shortfall.Paul Levy, the guy who runs Beth Israel Deaconess Medical Center, was standing in Sherman Auditorium the other day, before some of the very people to whom he m...

We just discussed how the incredible scrutiny being given to the case of Bernard Madoff, the alleged author of the financial scandal of the century, has opened many windows on the mismanagement and misgovernance of health care organizations. For example, consider the attention given to a Madoff crony, one Robert Jaffe. The Boston Globe reported,The scene was a society party at Donald Trump's famed Mar-a-Lago Club in Palm Beach, Fla. One guest was Robert Jaffe, 64, who had recruited investors for Bernard L. Madoff, the money manager who last week admitted to a Ponzi scheme in which he lost $50 billion of his clients' money. Another was 78-year-old Jerome Fisher, founder of the upscale shoe store chain 9 West, who reportedly lost millions with Madoff and was upset by Jaffe's presence at the ...

It seems that news about concentration and abuse of power in health care, about ill-informed, ill-advised, conflicted, self-interested, even corrupt management of health care organizations, has almost been swamped by stories of even worse concentration and abuse of power elsewhere, from mysterious hedge funds, to US state government, to countries on multiple continents. And yet, health care and health care organizations seem to have been swept up into these larger fiascos. We commented briefly earlier on one health care connection to the allegations that the Governor of Illinois tried to auction off an appointment to a US Senate seat. Now it turns out that the spectacular collapse of a financial organization that really was a giant Ponzi scheme also has ensnared many health care organizatio...

Barack Obama Pro- Gay political brochure
In a policy reversal Barack Obama has come out against The Limit on Marriage Constitutional Amendment, Proposition 8 which will be on California’s November Presidential ballot.
Democratic presidential candidate Barack Obama, who previously said the issue of gay marriage should be left up to each state, has announced his opposition to a California ballot measure that would ban same-sex marriages.
In a letter to the Alice B. Toklas LGBT Democratic Club read Sunday at the group’s annual Pride Breakfast in San Francisco, the Illinois senator said he supports extending “fully equal rights and benefits to same-sex couples under both state and federal law.”

Timm Herdt of the Ventura County Star has some interesting observations on the Tony Strickland vs. Hannah-Beth Jackson California Senate, District 19 race. Observations are derived from June primary election data.
The total numbers were just about exactly what anyone should have expected. Strickland got 52 percent of all the votes cast districtwide. Given that Republicans have a 2 percentage-point advantage in voter registration and that a higher proportion of GOP voters typically turn out in primaries, a 4 percent margin sounds just about right. No anomalies in that margin to suggest that voters of either party are a...
