A few pieces of interesting news...Dr. Daniel Carlat has been busy. He aptly notes that Pristiq is an Effexor copycat that apparently provides no special benefits over soon to be generic venlafaxine. Hey, didn't I just write a piece or two about Effexor? In addition, Carlat continues to hammer the corrupting, I er, continuing medical education industry. He also documents the use of deceptive "surveys" to market antipsychotics. Excellent work -- keep it up! Dr. Grohol at PsychCentral pointed out another set of potential problems with the surveys. Furious Seasons puts forth Ye Olde Pimp Slappe on antidepressant use in bipolar disorder with a side dish of I Told You So. He has indeed questioned the use of antidepressants in bipolar disorder and the latest data continue to question the utility...

I haven't written about my fun day I had with an Invega drug rep sales guy. I was in the psychiatrist's waiting room, and there he was, all shiny in his suit, his badge on and his big ass suitcase full of Pharma freebies. I walk in and stop in my tracks. Oh, a Pharma rep, right here in person I look into the counter area and see he has already handed over enormous latte'style coffee mugs that are bright orange with INVEGA on it. Loaded like candy inside were lovely Invega samples. Damn it looked good, like a party favor, or a teacher's gift resembling one's I've received, only those were filled with Hershey kisses. Not Invega, the dollar store version of Risperdal.~Me: "Oh are you here with the Invega?"Rep: "Yes."Me: "The Son of Risperdal?" [heh heh heh thanks CL Psych]Rep: "Um, yeah."Me: "...

SWEET!-CL Psych.Drugs of the Future-Paliperidone"Paliperidone is an active metabolite of risperidone (Risperdal), a widely prescribed antipsychotic agent used for the treatment of schizophrenia and bipolar disorder. Paliperidone acts as a dual antagonist of dopamine D2 receptors in the mesolimbic pathway and 5-HT2A receptors in the prefrontal cortex. Its ability to bind to these receptors corresponds with its antipsychotic effect and stabilization of some of the antisocial behaviors in patients with schizophrenia. Paliperidone has a favorable pharmacokinetic profile and is being developed using the OROS extended-release formulation technology with the aim of providing smooth 24-h exposure to the...

CL Psych-Marketing, Lying, CHeating, Geodon, PfizerLOVE the photo CL psych used on that post. That's how I see the pharma reps when they wheel into the psych offices, medical centers, and psych hospitals. It's hard not to want to claw their eyes out when I'm in the elevator with the rep, and the Chinese food or Mexican food delivery guy.GEODONI researched it in 2001 or so, before it came "out" into market, when a psych suggested my then 13 yr. old take it in replacement of Zyprexa. That of course, being she was medicated for childhood bipolar disorder than wasn't in fact that at all.Oldest daughter early 20's cut her arms up, OD'd, did a bunch of stuff, took herself to a psych and was dx bipolar. She agrees with it, she suffered the depression and mania in severe form, and Lamictal is what ...

Wyeth's attempt at an antipsychotic (bifeprunox) was rejected by the FDA today. Liz Spikol has a post on the topic that I found immensely entertaining. Daniel Carlat points out why the drug was rejected -- because it does not work very well. Yes, some doses showed a bit of improvement over placebo, yet bifeprunox did not impress when tried head-to-head against risperidone or olanzapine. Adding this drug to an already crowded antipsychotic market would seemingly do little for people with schizophrenia but would help Wyeth, which is looking a little desperate these days considering it has little to replace its flagship antidepressant Effexor. What about Pristiq, the "son of Effexor," -- well, that ain't looking so promising (1, 2, 3). Good luck, Wyeth. It appears you're going to need it! (Sou...

The drugmaker and its partner, Solvay Pharmaceuticals, just disclosed that the agency issued an action letter, and that its bifeprunox isn’t approvable at this time. Efficacy data wasn’t sufficient, and the FDA wants more ‘human metabolism’ info, along with details of one patient death. Here’s the Wyeth statement:
“The FDA stated in the letter that bifeprunox demonstrated effectiveness in the long-term maintenance study and indicated that a second positive maintenance study could be sufficient to support a maintenance claim for bifeprunox. (We) believe bifeprunox offers distinct benefits for the long-term maintenance of patients with schizophrenia. (We) will meet with the FDA to discuss study design and to assess how this additional study combined with o...

My personal research for over 2 years on this medication is why I write about it so much. At the time I found one of the first links I link to in some of my posts, my daughter was on Zyprexa. Anything at that point looked good. Being that Abilify caused severe mania, I think my hope for Biff isn't as strong as it was before. Just another psych med coming down the pipeline as others go generic and Pharma looks for another latest and greatest, newest drug to market, I can hardly wait for the DTC to come out. The day my daughter's psychiatrist offers bifeprunox is the day I'm gonna say, well what do ya know? Saw it coming X years ago. What's new? More pens and notepads coming our way.Carlat on Bifeprunox coming down the marketing pipeline (Source: soulful sepulcher)

Aren't Two Antipsychotics Better Than One? The Most Important Article on Psychiatry, Part 3"1. There may come a day where a drug is invented that works through some other mechanism, maybe glutamate, and maybe this drug gets invented by Wyeth, but as of right now, all the available antipsychotics work through D2 blockade. Everything else is irrelevant. Now, these other receptors might be relevant for other effects (reducing anxiety, antidepression, etc) but let's try to focus on the specific problem and not get all Andrew Weil here."-the last psychiatristIs it bifeprunox mesilate that will be sold by Wyeth in the US? I am a voice, and I have learned how to be one that is listened to in the world I too---hate.But to be heard, sometimes we have to be quiet and listen. I don't think we will see this new anti psychotic [which will be prescribed for bipolar as well as schizophrenia, until 2009.]The med appears to work for Parkinson's as well, which leads me to believe this will be a broad-brushed prescribed anti psychotic, that will enter the Pharma arena as a possible replacement for Zyprexa, Abilify,Risperdal,Seroquel and others, due to it's promised tolerable [and lack of severe]side effects such as weight gain, etc. One would hope with the few choices of anti psychotics out there that one will prove to be more efficacious than what we have now for choices.I also find it interesting that anti psychotics are being used for bipolar disorder, when in fact most ...
