Giffords’ recovery renews focus on coverage gap for veterans (Miami Herald):
- “Doctors and rehabilitation specialists have learned a great deal from the treatment of traumatic brain injuries in combat veterans of Iraq and Afghanistan. One in five veterans of those wars has suffered some form of traumatic brain injury, most commonly concussions from roadside bombs.”
- “Yet veterans’ health care doesn’t consistently cover cognitive rehabilitation therapy, the same therapy that’s helped Giffords and other well-known figures — such as Sen. Tim Johnson of South Dakota and ABC News correspondent Bob Woodruff — get their lives back to normal after major brain traumas.”
- “Cognitive rehabilitation can include speech and communication therapies, and therapies to boost memory ...

By Michael F. CannonI ask this question in today’s Richmond Times-Dispatch:
Virginia Republican Gov. Bob McDonnell…says Obamacare is unconstitutional and therefore illegitimate. Yet he has created a state commission to study whether Virginia should implement an illegitimate law. Since the answer does not appear self-evident to commonwealth officials, let’s walk through the reasons Richmond should refuse to create any new health-care bureaucracies.
Didn’t this guy take an oath to support the U.S. Constitution?

Last month, we wrote that Mylan Laboratories ceo Bob Coury has often used the corporate jet for personal matters. For instance, the drugmaker absorbed $535,590 last year so he could get where he was going, and that was up from $433,387 in 2009 and $348,988 in 2008. These are rather large sums and one corporate governance expert said all this flight activity on the shareholder dime was “inappropriate” (back story).
But where was Bob - and whoever else - going on all those trips? Well, the new Jet Tracker database, which was created by The Wall Street Journal and compiles non-commercial jet aircraft traffic between 2007 through 2010, offers some insight. For instance, the drugmaker is based in Pittsburgh, so it is not surprising that the vast majority of flights were destined for...

This is my first post for the 2nd Annual Diabetes Blog Week (thanks Karen!)
Our assignment for today is to pick a type of blogger who is different than me and talk about why they inspire me, why I admire them, and why it’s great that we are all the same but different.
There are so many great story-tellers out there, and sticking friends and family into categories, or types, feels like an extra step. A very difficult extra step.
But working my brain very hard to talk specifically about differences, I’m going to talk about two people who are very special to me. Bob and Rachel.
Bob and Rachel both live with type 2 diabetes. They are among a small minority of bloggers with type 2, and I’m so thankful they are sharing their stories. There are many fantastic bloggers wh...

One thing is clear after President Obama’s speech yesterday: He envisions a smaller national debt, but a much bigger government.
One percent is better than nothing, but it’s still pretty close to nothing.
One thing is clear about climate change: it’s causing a rising tide of red ink in Washington. See the forthcoming book Climate Coup: Global Warming’s Invasion of Our Government and Our Lives and join us for the accompanying book forum, featuring MIT meteorologist Richard Lindzen and American Meteorological Society fellow Bob Ryan, on Wednesday, May 4 at 4:00 p.m. Eastern.
I’ve long been a fan of Elisha Goldstein’s work on mindfulness because, more than any author on that topic, he seems to communicate the practice in a way that doesn’t totally overwhelm me and make me want to run the other way. With Forrest Gump.
I know this isn’t a very sophisticated image, but I keep going back to Homer Simpson in the Simpsons movie on his roof trying to hammer down the roofing, and the cameras zero in on the nail as he says to Bart, “Steady …. Steady … Steady …” and then he whacks the hell out of something: his eyeball instead of his thumb.
I can’t help but compare that image to how I do meditation. I start out right: easy … easy … but then I somehow getting really turned around. Kind of like my son David who, for Halloween this year, was ...

By Walter OlsonIn recent weeks the press has been reporting widespread alarms about shortages of many frequently used hospital drugs [L.A. Times/Chicago Tribune, Scranton Times-Tribune, KMGH (Colorado hospitals swapping drugs in short supply), The Columbian] The drugs running short include various antibiotics, anesthetics, chemotherapy drugs and others, including many generic compounds long since approved by the federal Food and Drug Administration (FDA). "The most troubling aspect is that it is critical drugs for which there are limited alternatives. Many are involved in cancer care and surgery," one hospital pharmacist told the Chicago Tribune's reporter.
While a variety of factors have played a role in the shortages, including lawsuits and economic retrenchment by some drugmakers, there...

Dr. Ari Brown, a pediatrician and author, was [recently] on The Dr. Oz Show. She was asked to join a discussion about autism. Dr. Brown is a board-certified developmental pediatrician, a mom of two, and an advocate for science. She is passionate and clear about what she believes. She is speaking all over the country about how to protect children from illness, particularly when making decisions about vaccines. She contributed ideas in my series in late 2010 entitled, “Do You Believe in Vaccines: Part I, Part II, and Part III.”
On Dr. Oz, she was asked to contribute to a discussion about autism that ultimately focused on fears about vaccines. I worry that the discussion wasn’t a representation of most American families, and even Autism Speaks refused to join the show.
Dr. Brown has s...

By Sallie JamesA bill that would have set a troubling precedent indeed was killed in the Senate last week. I've written previously about the Trade Adjustment Assistance program, and its fate has been tied up with the Generalized System of Preferences, a scheme by which certain developing countries gain duty-free access to the U.S. market for many of their goods. Congress was trying -- and failed -- to pass an extension of the programs together, along with the Andean Trade Preference Act.
Well, in an effort to extend for eighteen months the stimulus-enhanced TAA program (they were less fulsome in their enthusiasm for the other part of the bill; the barrier-reducing ATPA), Senators Bob Casey (D-PA) and Sherrod Brown (D-OH) introduced what they deemed to be a legislative "fix" to the thorny...

A Florida’s judge’s ruling that the Accountable Care Act (ACA) is unconstitutional doesn’t resolve the underlying constitutional issue (which will ultimately have to be decided by the U.S. Supreme Court) but it has introduced new uncertainty for the $2.3 trillion health care industry, and emboldened the law’s critics to push even harder for repeal (not that they weren’t trying already).
The Wall Street Journal’s (WSJ) health blog reports that “states and companies that are supposed to be implementing the law trying to figure out what to do next. The WSJ reports that the 26 states that are parties to the suit are considering whether to ask the Supreme Court to take up the case now, before it has fully wended its way through the legal system.
Federal law generally prohibits physicians from referring their own patients to a diagnostic facility in which they have an ownership issue — a practice called “self-referral” — unless the facility is located in their own practice. This exemption exists to allow patients with access to a laboratory test, X-ray, or other imaging test at the same time and place as when patients are seeing their physician for an office visit. Less inconvenience and speeder diagnosis and treatment — what could be wrong with that?
Much, say the critics, if it leads to overutilization and higher costs and doesn’t really represent a convenience to patients. This is the gist of two studies by staff employed by the American College of Radiology, published in the December issue of Health Affa...

[Soon] the new GOP-controlled House of Representatives will be voting on and is expected to pass a bill to repeal the Affordable Care Act (ACA) – lock, stock, and barrel. There is virtually no chance the repeal bill will get through the Senate, though, which maintains a narrow Democratic majority, and President Obama would veto it if it did.
But let’s say that the seemingly impossible happened, and the ACA was repealed. What would the impact be on healthcare coverage, costs, and the federal deficit?
In a letter to Speaker John Boehner (R-OH), the Congressional Budget Office (CBO) released its preliminary estimates of the impact of repeal on the deficit, uninsured, and costs of care, and found that it would make the deficit worse, result in more uninsured persons, and higher premiu...

On January 1, Kathleen Casey-Kirschling became the first of the baby-boom generation to qualify for Medicare. She’s hardly alone: The baby-boom generation will cause enrollment in Medicare to soar. According to the Kaiser Family Foundation, Medicare enrollment will increase from 47 million today to 64 million in 2020 to 80 million people by 2030. At the same time, the ratio of workers paying into the program to support each Medicare enrollee will drop from 3.4 (2010) to 2.8 (2020) and then to 2.3 workers per beneficiary in 2030, denying the program the tax revenue needed to sustain it.
What happens then? Well, the President and Congress would have a dismal menu of political and policy choices. They could impose huge tax increases, inflicting great harm on working families and the economy...

Today’s blog will be my last for 2010, as I will be taking a break to spend the Christmas and New Year’s holidays with family and friends.
In keeping with a tradition I started two years ago, I again have taken the liberty of mangling a beloved holiday song, story, or rhyme to give a humorous (I hope!) perspective on current politics. In December, 2008, I adapted “Twas the Night Before Christmas” to convey President-elect Obama as being a not-so-jolly old elf besieged by lobbyists demanding stimulus gifts. Last year, I depicted the GOP as the Grinch trying to stop “ObamaCare” from coming.
Today, I’ve re-written the “Twelve Days of Christmas” carol so that it is the government bestowing “gifts” (based on actual provisions of the Affordable Care Act) that the new Co...

One of the canards slung at the Affordable Care Act is that it creates “death panels” that would allow the government to deny patients lifesaving treatments, even though two independent and non-partisan fact-checking organizations found it would do no such thing.
I don’t bring this up now to rehash the debate, but because the New York Times had a recent story on Arizona’s decision to deny certain transplants to Medicaid enrollees — “death by budget cuts” in the words of reporter Marc Lacey. His story profiles several patients who died when they were unable to raise money on their own to fund a transplant. Lacey quotes a physician expert on transplants who flatly states: “There’s no doubt that people aren’t going to make it because of this decision.”

In assessing the “best and worst” of the recommendations from the National Commission on Fiscal Responsibility, Washington Post blogger Ezra Klein accuses the Commission of “cowardice” in addressing healthcare spending:
“The plan’s healthcare savings largely consist of hoping the cost controls . . . and various demonstration projects in the new healthcare law work and expanding their power and reach. . . In the event that more savings are needed, they throw out a grab bag of liberal and conservative policies . . . but don’t really put their weight behind any. . .[their] decision to hide from the big questions here is quite disappointing . . . ”
Pretty harsh words, considering that in other respects Klein gives the Commission high marks. But I think there is a lot mo...

Calling upon more than twenty years of clinical experience, fifty years of navigating life’s ups and downs, and an array of thinkers and pop icons – from Alan Watts to Albert Einstein, Billy Graham to Bob Dylan, the Dalai Lama to the English mystic Julian of Norwich – he weaves a gentle compassion and a tart wit into this compelling look at human nature and our never-ending quest for happiness.

AA's logoAmerican history includes many social movements that aimed to help people stop drinking. There was Prohibition, of course. But there was also the Anti-Saloon League, the American Temperance Society, the Washingtonian Temperance Society, the Women’s Christian Temperance Union, and more. Only one such movement survived — Alcoholics Anonymous (AA).AA not only survived, it spread across the world. Today, AA lists its membership at over 2 million, with over 100,00 groups in Australia, Africa, Asia, and Europe as well as North and South America, even Russia. If ever there was evidence that sobriety can be mass-produced, it is in AA.AA began with the chance meeting of two people on May 12, 1935: Bill W., an alcoholic stockbroker from New York, and Dr Bob S., an alcoholic surg...

CONCLUSIONS
Just as the idea of hospice care revolutionized death and dying in America, the idea of bundling many aspects of Alzheimer’s
care under one roof in a cognitive shop could change the way we approach this dire disease—one that has no cure and leaves no survivors. Certainly, the scope of the problem poses medical and economic risks for the country. These risks, and potential steps for a solution, were charted by the bipartisan Alzheimer Study Group in the spring of 2009. The report, issued by the Alzheimer Study Group co-chaired by former congressman Newt Gingrich and former senator Bob Kerrey, minces few words. It likens the failure to address the impact of Alzheimer’s to the failure to strengthen the levies of New Orleans against an overwhelming hurricane. “Alzheimer’...

This may give new meaning to the notion of playing ball. Hoping to entice doctors to prescribe more meds, Novartis assembled a star-studded line up of athletes to pitch its drugs at dinners between 2006 and 2009. Among them: baseball Hall of Famers Bob Gibson and Johnny Bench, New York Giants quarterback Eli Manning and former New York Knicks forward Walt ‘Clyde’ Frazier.
As you might imagine, the athletes gave short speeches, answered questions about their careers, signed memorabilia and posed for photos. Novartis reps later brought the photos when they called on the docs. In all, the drugmaker paid $3.6 million in fees to 150 top former and current sports figures - from $8,000 to $35,000 an appearance, according to The Washington Times.
Interestingly, the events were used to...

Will Congress ever pass a bill that limits pay-to-delay deals? The Federal Trade Commission has been trying to convince Congress for months to do so, but opposition is mounting. Five Democratic Senators are objecting to a bill recently passed by the Senate Appropriations Committee because it contains a provision that would restrict these patent settlements (see this).
In a letter to Senate Majority Leader Harry Reid, the endangered Nevada Democrat, and Appropriations Committee chairman Daniel Inouye, a Democrat from Hawaii, the Democrats say they have “substantive concerns with the content” of the provision and that the decision to include it in the appropriations bill “contradicts both the spirit and the letter of the Senate rules” (see the letter).

My beautifully drained toenail bruise, after Dr. Bruce Lashley's skillful procedure
Thanks to the mysterious bruise under my left big toenail, over the past week I’ve learned more about podiatry, acral lentiginous melanoma, and Bob Marley than I’d ever thought possible. That’s because my esteemed foot doctor, Dr. Bruce Lashley, was concerned that said bruise might actually be the nasty acral lentiginous kind that killed the Buffalo Soldier. This type of melanoma under the nails, on the soles of the feet, and palms of the hands is, along with behind the eyes, the most common place for melanoma to appear. Wait, but what about skin cancer, you ask? Turns out, that’s much more often basal cell or squamous cell cancer, not melanoma. (I asked Dr. Lashley the same ques...

Last month the U.S. Census Bureau released its annual survey on health insurance coverage. The results were startling, yet few politicians seemed to take notice:
– The number of people with health insurance declined for the first time ever in almost two decades. In fact, as reported by CNN this is the first time since the Census Bureau started collecting data on health insurance coverage in 1987 that fewer people reported that they had health insurance: “There were 253.6 million people with health insurance in 2009, the latest data available, down from 255.1 million a year earlier.” The percentage of the population without coverage increased from 15.4 percent to 16.7 percent.

From its inception, Medicare has been agnostic about the effectiveness of different treatments when it sets payment rates. Once a treatment is found to be “reasonable and necessary,” Medicare establishes a payment rate that takes into account complexity and other “inputs” that go into delivering the service. But it is prohibited by law from varying payments based on how well an intervention works.
This would change under a “dynamic pricing” approach proposed by two experts in this month’s issue of Health Affairs. The article itself is available only to Health Affairs subscribers, but the Wall Street Journal health blog has a good summary.
The researchers propose that Medicare pay more for therapies with “superior” results and the same f...

Like most kids who grew up in the 1960s, I spent many a night watching the adventures of Fred, Wilma, Barney and Betty, the coolest cavemen ever (sorry, GEICO). It is hard to explain the appeal of the Flintstones, which [recently] celebrated the 50th anniversary of its first broadcast. Its animation was primitive, the stories campy and cliché, and it was horribly sexist — but the characters were lovable, the dialogue funny, and who couldn’t love the way it depicted “modern conveniences” (like washing machines) using only stone-age technologies (bones, stones and dino-power?)
What does Fred Flintstone have to do with healthcare? Not much, really, although Fred was the victim of a medical error. According to Answers.com: “A 1966 episode had Fred can’t st...

The technique is too cute by half. I could just as easily come up with a series of quotes by people who believe that the costs of the war in Afghanistan far exceed the benefits. (e.g. The widow of a soldier killed in Afghanistan, upon reading the Woodward excerpts, bursts into tears. “Why have we chosen to fight a war that Gen. Petraeus admits we will likely never win, and which our children and grandchildren will be fighting?”)

Do you end your day with a longer to-do list than you started? Are you stressed over how you spend your time? Here are eight steps you can take right now to help regain control of your clock.
Eliminate time wasters. At the top of the list is television. The average American watches 5 hours of TV a day. That equals an incredible 6 full days in front of the tube each month. What could you do with an extra 6 days this month? Turn off the television and find out.
Cut back on possessions. Bigger living spaces and more stuff come with a double price: the cost to purchase them and the time to maintain them. A simpler lifestyle frees up time previously spent on vacuuming, fixing, ...

Several people I admire and respect—and who are far more versed in the legal aspects of the “war on terror”—have already weighed in on whether the U.S. Government is authorized to kill U.S. terror suspects abroad, so I defer to those experts.
But what’s interesting is that the U.S. Government has killed “many Westerners, including some U.S. passport holders” in Pakistan’s tribal areas dating all the way back to the Bush administration, according to Bob Woodward’s new book.

By Benjamin H. FriedmanOn the National Interest‘s Skeptics blog, I have a new post about my lack of outrage over the revelations in Bob Woodward’s new book about Obama and Afghanistan.
Unlike John Bolton and Heritage, I don’t think that the President’s comment that we can withstand another terrorist attack like 9-11 is offensive. After all, we can, and saying so doesn’t mean you want to try it.
As I put it there:
What’s truly outrageous is the notion that the only valid response to terrorism is cowering fear at home and endless warfare abroad. Somehow, for much the right, crediting our enemies with the ability to wreck our society is required, and it is verboten to say that we are something other than a pathetic, brittle nation that cannot manage adver...

By Malou InnocentThe New York Times reports that the book, Obama’s Wars, by longtime Washington Post reporter Bob Woodward that is scheduled for publication next week, depicts an administration completely at odds over the war in Afghanistan.
According to Woodward, the president concluded from the start that “I have two years with the public on this.” He implored his advisers at one meeting, “I want an exit strategy,” and he set a withdrawal timetable because, “I can’t lose the whole Democratic Party.”
It’s unfortunate that the policy debate over Afghanistan will be further spun into a left-vs.-right issue. After all, there are growing, if nascent, signs that some on the political right have reservations about our continued military involvem...

There is a 12 noon (Eastern Standard Time) deadline for Johnson & Johnson to hand over some documents to the House Oversight and Government Reform Committee today. As a result, the Congressional stage is, once again, set for drama. The last time this committee demanded documents from J&J, the health care giant was accused of whizzing past the deadline, although the committee never followed through with its implied threat of issuing a subpoena (look here).
What is it the committee wants this time? A copy of an alleged agreement between J&J’s McNeil Consumer Healthcare, which has recalled tens of millions of bottles of over-the-counter meds, and the FDA that supposedly gave McNeil a green light to pursue a “soft market withdrawal” instead of a formal recall. ...

By Roger PilonToday POLITICO Arena asks:
Does the Republican Party have a sore-loser problem?
My response:
Lisa Murkowski is Exhibit A of the GOP sore-loser syndrome. Poor little thing: She thought she was entitled to the seat. After all, Daddy gave it to her.
But she’s not alone: Charlie Crist, Bill McCollum, Bob Bennett, Bob Inglis, Mike Castle, Dede Scozzafava — all sitting on the sidelines, running against the primary opponents who beat them, or even endorsing the Democrat in the race. They confirm the Tea Party contention: They have no clue about the changes taking place beneath their feet. Lisa Murkowski talks about the bacon she’s brought back to Alaska. But unlike the people marching in Paris to protest moving the retirement age from 60 to 62, the growing Tea Par...

Citing a new study by the Dartmouth Atlas, the Wall Street Journal’s health blog provocatively asks: “Has the notion of ‘access’ to primary care been oversold?”
The Dartmouth researchers found “that there is no simple relationship between the supply of physicians and access to primary care.” That is, they found that having a greater supply of primary care physicians in a community doesn’t mean that the community necessarily has better access to primary care. Some areas of the country with fewer primary care physicians per population do better on access than other areas with more primary care physicians.
The researchers also report that the numbers of family physicians is more positively associated with better access than the numbers of internists...

One of the more surprising twists and turns in the continuing debate over healthcare reform is that many physicians who now object to the Affordable Care Act (ACA) were just a few years back advocates for more federal regulation. In fact, in the early 2000s, more than 200 “provider” and consumer groups — including many state medical and national medical specialty societies that now oppose the ACA because of concerns about “excessive regulation” — were among the fiercest champions of federal legislation to mandate that health insurers comply with a Patient Bill of Rights.
A bipartisan bill introduced by Senator John McCain (R-AZ) and the late Senator Ted Kennedy (D-MA) would have ensured that patients have the “right” to appeal insurance compa...

We like him even more for his modesty: “It’s embarrassing,” he said backstage. “You don’t want to be awarded for doing what you’re supposed to be doing.” Class act, that Clooney.

The other day I was driving when I heard a familiar song playing on the radio. It was Bob Marley’s Buffalo Soldier. In fact, as I type this I can hear it playing in my head.
The funny thing is that the sound automatically took me back to my childhood. My cousins and I were sitting in someone’s living room. The radio was playing. That song was on. And my older cousin was sitting on this huge comfy chair while the rest of us kids were sitting on the ground.
Why do I remember this seemingly mundane event?
My cousin spontaneously began belting out the song, dancing to the beat and being as silly as a kid can be. We rolled on the floor and laughed until our sides hurt. It was a memorable moment. We were young, spontaneous and free.
How does this relate to this week’s top post...

We spend a fair amount of time on this site tracking the controversy over continuing medical education and the extent to which industry may unduly influence the physician mindset. For the second year running, though, a drugmaker - it happens to be Pfizer - is underwriting an extensive seminar on cancer issues for journalists that is organized by the National Press Foundation.
Here are the basics: the NPF is awarding 15 fellowships to journalists who will attend a four-day session in October on cancer issues that will be presented by several experts (you can read more here). The idea, says Bob Meyers, the NPF prez and a former editor and reporter for The Washington Post, is to give journalists a chance to learn needed info about complex subjects, especially at a time when media resources ar...

By Christopher PrebleDefense Secretary Robert Gates has again made headlines with a proposal to slow the growth of the Pentagon’s budget — already higher than at any point since World War II — by cutting overhead, waste and a top-heavy command structure.
The proposed shuttering of Joint Forces Command (Jif-Com) has elicited most of the press attention today, and prompted an impassioned plea from Virginia politicians, including Gov. Bob McDonnell, that the command remain open. Unhelpfully for Gov. McDonnell, outgoing Jif-Com head James Mattis (who will assume the title of CENTCOM), reportedly supports Gates’s decision.

Many conservatives are up-in-arms about President Obama’s decision to appoint Don Berwick, a pediatrician and renowned expert in quality improvement and patient safety, to lead the Center for Medicare and Medicaid Services (CMS). They object to Dr. Berwick’s views on a range of issues, and to Obama’s decision to use his office’s authority to appoint Dr. Berwick while the Senate was out on a short Independence Day holiday recess. In the past, he had fought the urge by talking to other alcoholics, who truly understood his struggle. Through a church group, he found local surgeon Robert Holbrook Smith.
Dr. Bob and Bill W., as Alcoholics Anonymous members know them, promised to keep each other sober, following Bill W.’s strategy: a simple set of principles—later refined into 12 steps—that would become the foundation of America’s self-...

That’s what two professors have concluded after a preliminary study of web sites for the 100 best-selling prescription meds in 2009. Their preliminary analysis, which was presented at a recent conference in Boston, finds the sites are a “bewildering and patchy mess of information” that often make safety data hard to find and makes it hard to distinguish between promotion and education.
“Our analysis suggests there is a blurring of the lines which is not in anyone’s interest,” Lewis Glinert, a Dartmouth University linguistics professor and one of the co-authors, tells us. ““There should be a lot more regulation of these websites. The FDA should be more hands-on.”
For instance, they found that about three-quarters of the sites provide as much as...

Medical schools are traditionally ranked on criteria like research funding and technological innovation. These rankings are highly significant. A place on the U.S. News‘ annual “Best Medical School” list is a coveted spot indeed.
So that’s why there was some media attention paid to a recent study from the Annals of Internal Medicine, which ranked medical schools according to their “social mission” — a phrase that defines a school’s commitment to primary care, underserved populations and workforce diversity. Using this new criterion, some of the traditionally high ranking schools fell significantly. (more…)

After months of dithering, delaying, denying, and defaulting on a decision, Congress ended up…doing as little as possible to address the Medicare physician pay cut problem.
Thursday night the House of Representatives acceded to the Senate’s bill to provide physicians with a 2.2 percent update retroactive to June 1. This respite, though, lasts only through the end of November, when physicians and patients will again face another double-digit cut. And if the past is prologue, a lame-duck Congress then will wait until the very last minute to enact another short-term patch, or worse yet, allow the cut to go into effect on December 1 and then pass some kind of retroactive adjustment.
You know that the situation has gotten ridiculously bad when the President says this about the bill he ...

Friday, the Senate — in a rare stroke of bipartisanship — voted by unanimous consent to reverse the 21 percent SGR cut and provide positive updates of 2.2 percent through November 2010. The legislation is fully paid for by offsets in other spending programs.
Unfortunately, though, the cut remains in effect and claims are being processed at reduced rates, because the House of Representatives has recessed for the weekend and won’t be back until Tuesday. At that time, I expect that the House will pass the Senate’s six-month reprieve and Medicare will make doctors “whole” for the period of time that the cut was in effect.
Not that any of this is a cause for celebration. In the meantime, claims still are being paid at reduced rates, creating havoc for physicians...

Intellectual Enrichment Helped Preserve Memory and Learning in Multiple Sclerosis Patients, Study Says (WebMD)

“A small study of multiple sclerosis (MS) patients shows that maintaining an intellectually active lifestyle can help preserve learning and memory, even among patients with a high degree of brain damage.”
“Although there’s no indication that being mentally engaged protects against brain damage itself, the findings do suggest that an active mind may be better equipped to retain its functions even in the event of brain damage.”
“The findings suggest that enriching activities may build a person’s ‘cognitive reserve,’ which can be thought of as a buffer against disease-related memory impairment,” says study author James Sumowski, Ph...

I’m sorry about the mess I’ve made of my life. I want to turn away from all the wrong things I’ve ever done and all the wrong things I’ve ever been.

Thank You, God for getting my attention long enough to interest me in trying it Your way.
God, please take over the management of my life and everything about me. I am making this conscious decision to turn my will and my life over to Your care and am asking You to please take over all parts of my life.

Each year, thousands travel to Akron to recognize Dr. Bob Smith for co-founding Alcoholics Anonymous.
This year, Akron will thank Dr. Bob in a special way — by naming part of a street after him.
Akron City Council on Monday voted to designate the section of Olive Street from North Main Street to North Howard Street ”Dr. Bob’s Way.” This section of Olive is on the north end of St. Thomas Hospital, which featured the first hospital specialty unit to treat alcoholism as a medical condition.

What if some physicians actually like the way primary care is currently practiced? It’s hard to believe, considering the majority of studies suggest marked dissatisfaction among primary care doctors, and an increasing prevalence of physician burnout.
The ACP’s Bob Doherty recently summarized an epic Health Affairs article devoted to fixing primary care. The bottom line was that paying primary care doctors better isn’t enough. The whole field needs to be re-invented. (more…)

Now that we are preparing our 2010 market report we are analyzing in depth a number of important recent developments. A major one, whose implications haven’t yet been properly digested, was the publication in the UK of a fantastic series of policy, scientific and technology reports by the Foresight Project on Mental Capital and Wellbeing. If you want to have a stimulating and substantial read, you can download the Executive Summary (and most other reports) for free.
I was thinking about their main recommendation (the need to focus more attention, as a society and as individuals, on building mental capital and wellbeing trajectories along the lifecourse), as I came across these apparently completely separate news. Doesn’t the lifelong mental capital framework add new light on t...

For years, the Garden State has bragged about being the nation’s medicine chest. Global headquarters dotted the map. Fledgling biotechs toiled in their shadow. Tens of thousands of employees commuted from manicured suburbs to manicured corporate campuses. Crucial meetings were held at golf courses. And there was that close relationship with offiicals in Trenton, the state capital.
But New Jersey’s dominance in the pharmaceutical world is lessening. Consider the tale of Steve Weissman. Like thousands of other highly skilled professionals, he became a victim early last year of the cutbacks and mergers that are sweeping the industry. After working 18 years as a chemist at Merck, he found himself looking for a job and spent months were spent crisscrossing the state, networking and ...

Some legal scholars, including some who normally lean to the left, believe the states have identified the law’s weak spot and devised a credible theory for eviscerating it…
Jonathan Turley, who teaches at George Washington University Law School, said that if forced to bet, he would predict that the courts would uphold the health care law. But Mr. Turley said that the federal government’s case was far from open-and-shut, and that he found the arguments against the mandate compelling.

By Michael F. CannonUtah Republicans will not be re-nominating U.S. Senator Bob Bennett (R-UT) for another term. A principal reason appears to be their displeasure over a health care bill that Bennett teamed with Sen. Ron Wyden (D-OR) to sponsor.
Some commentators decry how Bennett’s ouster demonstrates that the Republican party has lurched to the right. That’s a reasonable interpretation if you believe, as conservative Washington Post columnist Kathleen Parker does, that Wyden-Bennett was a moderate, “market-driven” bill.
Rather than a moderate, market-driven bill, Wyden-Bennett would have created “Medicare Advantage for All.” It would have made health insurance compulsory for all Americans, and imposed stringent government controls on what type ...

By Justin LoganWashington, DC is a company town, and the company is the federal government. Between the executive branch, the Congress, the Supreme Court and all the rent-seekers and hangers-on that come to court power, the city is a veritable petri dish that breeds and nurtures the worst human impulses. Some Cato people live here, too.
Local politics in DC is so dominated by Democrats that the Republican Party in DC is a perennial butt of jokes. Between its role as the seat of the federal government and the Democratic Party’s Turkmenbashi-level control of local government, it is a city that conservatives love to hate.
Across the river in Virginia, by contrast, there are lots of Republicans, and they are still able to compete against the Democrats statewide. Lots of Republica...

Who is speaking when a corporation talks? Can a corporation represent all of its shareholders and workers in political speech? How will corporations decide who to represent? In “Corporate Governance Redux in the Light of Citizens United,” Robert A.G. Monks will detail the history of corporate personhood and how this case relates to corporate governance.

We talk a big game about freedom, liberty, and democracy, but the most honest quote about our country came from one of its worst presidents, Mr. Calvin Coolidge, when he noted that “the business of America is business.” It took us almost a century to go to war over the idea that black people maybe weren’t property, and that was long after...

In Memory Of Bob Franks, Pharma Guy

Over the many years covering the pharmaceutical industry, we have met all types of supporters and critics - some are thoughtful, some less so. But seeking out insights, not just news, is always a valuable exercise and one of our favorites sources was Bob Franks, a former New Jersey Congressman who, most recently, spent the last few years heading the HealthCare Institute of New Jersey, a trade group for drug and device makers.
At his heart, Franks was a gentleman. At a time when politics seems increasingly defined by divisiveness and idealogues, he was as thoughtful and gracious as he was clever and quick. And he brought those qualities to his role at HINJ, where he was a tireless advocate for the pharmaceutical industry during a stretch when supporters always seemed to be on the defensive ...

Recently, there have been a few Blisstree posts about baby names (What Not to Name Your Kid, Crowdsourcing, Bad Baby Names), and, given my own sordid baby-naming history, I have to join the conversation. Seven years ago, we gave our very sweet little girl a very wrong name. Okay, so it wasn’t Tu Morrow (born of Rob Morrow) or Fifi Trixibell (born of Bob Geldof). In fact, there was nothing inherently wrong with the name itself. It was pretty actually. (Just please don’t make me say it). But it was wrong. Wrong. Wrong. And so we changed it.
Truth is, we had been pretty smug about our baby-naming skills. While some insist that whatever name you choose, a child will grow into it, as a kid I couldn’t even name a kitten or a puppy ...

By Ilya ShapiroEarlier this week, I reacted to reports that the University of Washington couldn’t find any legal scholars willing to question Obamacare’s constitutionality by issuing a challenge: I will debate the constitutional merits of Obamacare against anyone, anytime, anywhere (as long as the sponsoring group/individual covers my travel expenses). Here’s a video version of my challenge:

Cato adjunct scholars Dave Kopel and Ilya Somin, blogging at the Volokh Conspiracy, go into much greater depth on the myth of an expert consensus.
And if you’re tired of hearing the debate about the debate and want to hear the latest on why the individual mandate is unconstitutional, listen to my colleague (and Cato’s chairman) Bob Levy’s podcast. (Source: Ca...

Mommies-to-be spend countless hours browsing baby books and websites to figure out how they should address their kid for the rest of their life. After all, some research suggests that a child’s name can really impact their life growing up. Sure, it was weird for Gwyneth Paltrow to dub her daughter Apple, or Anne Heche to anoint her son Atlas, but they don’t even come close to making our list of the five worst baby names.

Sister Ignatia – Second Edition
Mary Ignatia Gavin (1889–1966) of the Sisters of Charity of St. Augustine was known as The Drunk’s “Angel of Hope”
Sister Ignatia epitomized the spirit of love, service, and honesty that today are the hallmarks of Alcoholics Anonymous.
As a hospital admissions officer in the 1930s in Akron, Ohio, Sr. Ignatia befriended Dr. Bob Smith, co-founder of AA, and courageously arranged for the hospitalization of alcoholics at a time when alcoholism was viewed as a character weakness rather than a disease.
Between 1935 and 1965 she successfully treated thousands of alcoholics. Sister Ignatia pioneered the recognition of alcoholism among priests and nuns. She was remembered for her kindness, honesty and nonjudgmental love.

A brief look at Kengi’s blog reminded me to take stock of how grateful I am to be living where I do, as I do, with rent geared-to-income.
This spring marks the beginning of my nineteenth year in this apartment – I’ve had to do a couple rounds of finger counting just to come to believe [...] (Source: My journey with AIDS)

About once a year I discover a workbook that allows me to put all the steps that I learn in therapy into practice. I’ve mentioned in past blog posts David Burns’s 10 Days to Self-Esteem, and how the exercises in that workbook allowed me to recognize distorted thought patterns and practice ways of untwisting them. Two years or so ago, when I didn’t know whether or not I should have my son treated for anxiety, my therapist recommended I read Understanding Your Child’s Puzzling Behavior, which was very, very helpful. And now fellow blogger and mindfulness expert Elisha Goldstein has published, with co-author Bob Stahl, a comprehensive workbook — A Mindfulness-Based Stress Reduction Workbook — that teaches the art of mindfulness in relieving and reducing str...

The Truth About Selling? It’s Really Giving – Guest article from Bob Burg
As I took my seat in the crowded airplane, the woman in the next seat over smiled. “Headed home?” she asked.
I nodded. “Jupiter, Florida.”
“You’re a long way from home!” she exclaimed. We were on the tarmac in Regina, Saskatchewan. “What brought you to Regina?”
I told her I had been conducting a seminar for a sales organization. She wrinkled her nose. “Oh,” she pronounced, “I could never sell.”
This wasn’t the first time I’d heard this. In fact, most people not actually in sales seem to feel this way. I asked her, if she didn’t mind, how would she define “selling”? I was curious as to what it was she felt she could neve...

By Christopher PrebleLast week, Iraq’s independent electoral commission disqualified 511 candidates — most of them Sunnis — from running in the parliamentary elections scheduled for March. Today’s Washington Post reports that Vice President Joe Biden is hurrying off to Baghdad to try to convince the Iraqis to change their minds. U.S. troop withdrawals were supposed to accelerate after the elections were held and a new government seated. But the elections have already been postponed at least once, and the administration is worried that the obvious bias against Sunnis could stoke sectarian tensions.
“U.S. officials are in a precarious position,” the Post story explains:

The agency plans to hold an advisory committee meeting because studies have found that high dosages of Amgen’s Aranesp and Epogen and Johnson & Johnson’s Procrit boost the risk of heart attacks, strokes and blood clots in anemia patients with kidney disease. The purpose of the meeting, which hasn’t been scheduled yet, is to determine the appropriate dosages.
Known as erythropoiesis-stimulating agents, or ESAs, the drugs boost hemoglobin levels in anemics, but the drugmakers have been plagued by cardiovascular risks that caused sales to decline. As it turns out, using large doses to cause hemoglobin to return normal levels may also increase the risk of heart attacks and strokes, instead of lowering the risk.
“Randomized trials have endeavored to show that using ESA...

The Christmas Tree Caper and the Fifth Warning Sign.....
This post isn't about me, its about Bob Blackwell and his blog on the USA Today newspaper website.

I want to personally thank Bob and Carol (especially Carol) for including the link to the article I wrote recently -- Communicating in Alzheimer's World -- in their blog post.

The link was included in their article -- The Christmas Tree Caper and the Fifth Warning Sign.

The article is pretty funny (if it didn't happen to you).

If you have the time, go over to the blog and take a look. You can also click through to Bob's home page by clicking the link at the bottom of his profile.

Alzheimer's Reading Room

Bob DeMarco recently posted the story of Eldon Foster's death after wandering away from an assisted living facility.

Besides the sadness I felt for Eldon, I was highly angered by the stupidity of comments left on the article describing Eldon's death.

I have taken a couple days to process this so I don’t unleash my thoughts and anger, but a couple days after the fact continues to leave me angry and sad.

The stupidity and ignorance expressed in the comments reinforces that there is a lack of knowledge and understanding when it comes to Alzheimer’s disease.

Persons suffering from this devastating disease are victimized ...

After months of serious problems, Genzyme added an independent director to its board under pressure from Relational Investors, an activist hedge fund, The Wall Street Journal reports. The biotech added Bob Bertolini, who was chief financial officer at Schering-Plough before it was acquired by Merck (see statement).
Genzyme has endured nothing but trouble this year as quality control problems forced the biotech to suspend shipments of key products for such ailments as Gaucher’s and Fabry diseases, and the subsequent shortages prompted the FDA to allow rivals to jumpstart plans to produce their own meds to fill the void (see coverage here and here).

The difficulties have shaken confidence in Genzyme management, leading to speculation the board would be pressured to make changes. Addi...

This is Thanksgiving week … the week some of us are very thankful that we don’t live in the same city as our relatives. So I’m calling in the experts.
My friend, Elisha Goldstein, who writes the “Mindfulness and Psychotherapy” blog on Psych Central, offers readers like myself, who are having difficulty with a formal meditation practice, several quick tips for mindful living that can be implemented throughout the day. He writes in his post, “Hectic Life? Quick Tips for Mindful Living” that “even without the time and place in life to set up some formal practice, from the minute you get up in the day to the moment you lay your head on the pillow there is opportunity to engage mindfulness as a way of life, opening you up t...

Long considered to be overseer of important regulatory matters, Temple has been promoted to Deputy Director for Clinical Science within the agency’s Center for Drug Evaluation & Research, according to The RPM Report, which notes the shift gives Temps broader scope on technical scientific issues
Although Temple will remain Acting Director of the Office for Drug Evaluation I, which includes the divisions for cardiovascular and renal, neurology and psychology drug products, he’ll have a reduced role on drug promotion and marketing issues. And this happens just as the FDA tries to figure out how to regulate promotion of medical products on the Internet and in social media, RPM points out.

The Washington Post devotes most of a page to summarizing the views of Virginia gubernatorial candidates Bob McDonnell and Creigh Deeds on the major issues of the election. (The article seems to have no real headline online, and isn’t linked from anywhere obvious, but in the actual paper, it dominates page C4 under the headline “Where do they stand on the issues? A Rundown of Competing 4-Year Agendas for Virginia.”)
And what are the issues the Post thinks are important? Education, transportation, energy and environment, abortion, gun control, health care, and labor. All fine issues to debate.
But what about taxes? Or government spending? Or the size and scope of government? McDonnell’s television ads focus heavily on Deeds’s apparent willingness to raise taxes...

Almost two months ago I quoted a Washington Post op-ed that said that this fall’s gubernatorial race in Virginia would depend on
the all-important independent voters — the disproportionately moderate, young, prosperous, suburban and libertarian-leaning people who typically decide Virginia contests.
It looks like Frank B. Atkinson, a high-powered Richmond lawyer who served in the Ronald Reagan and George Allen administrations and has written two books on Virginia politics, knew what he was talking about. At least on my television here in the Virginia suburbs of Washington, D.C., the race has been dominated by two kinds of ads: Democratic nominee Creigh Deeds tells us over and over again that his Republican opponent Bob McDonnell is a reactionary social conservative. McDonnell counters ...

One of the perennial tropes of the network neutrality debate has been the tendency of the pro-regulation side to paint it as a David-and-Goliath struggle between big, evil corporations and the little guy. Way back in 2006, James Gattuso pointed out how silly this is: in fact, the push for network neutrality is backed by some of the largest companies in Silicon Valley. Julian points out a particularly lazy example of this kind of ad hominum that happens to target Cato: It seems that we’re one of the “15 greatest enemies of net neutrality.” And that along with CEI, Cato “seems to draw its funding from a smattering of every major corporation ever to fund lobbyists.”
As Julian points out, if “VoIP News” had done its homework, it might have discovered that Cato...

Veteran political columnist Robert D. Novak, 78, died today of a brain tumor at his home in Washington DC.
Odd that the self-described Prince of Darkness was one of the nicest persons I’ve ever known. Unlike many here in Babylon-by-the-Potomac, Bob became less enamored of the Establishment the longer he was here. He believed in liberty and was a great friend of the Cato Institute.
He will be sorely missed. (Source: Cato-at-liberty)

President Obama’s new drug czar, Gil Kerlikowske, says he wants to banish the idea of a “war on drugs” because the federal government should not be “at war with the people of this country.”
At a Cato policy briefing on Capitol Hill on July 7, former Libertarian Party presidential candidate Bob Barr explained why carrying out an end to the “war on drugs” will require a bipartisan solution. (Source: Cato-at-liberty)

A YouTube video that catches Bob Chanin, retiring general counsel of the National Education Association, calling right-wing groups ”bastards” for attacking his soon-to-be-former employer has recently been making the rounds. Not surprisingly, some right-wingers haven’t been too happy about Chanin’s retirement speech, not caring for the “bastard” label. I, however, want to thank Mr. Chanin for his salty valedictory.
Why? First off, because his pugnacious presentation has a certain Teamsters feel to it, furnishing almost visceral confirmation that the National Education Association is a labor union pure-and-simple — not the high-brow “professional employee organization” it bills itself as — ready to slash tires or do whatever else it thinks necessary ...

In yesterday’s Washington Post, veteran newsman Bob Woodward recounts a recent meeting between National Security Advisor James Jones and a few dozen Marine officers in Afghanistan’s Helmand province under the command of Marine Brig. Gen. Lawrence D. Nicholson.
The subject on everyone’s mind: force levels. Saying that he was “a little light,” Nicholson hinted that he could use more forces, probably thousands more. “We don’t have enough force to go everywhere,” Nicholson said.
Of course he doesn’t. One senior military commander confided, in Woodward’s telling, ”that there would need to be more than 100,000 troops to execute the counterinsurgency strategy of holding areas and towns after clearing out the Taliban insurgents....

Pediatrician Dr. Bob Sears makes it his business to stay on top of the latest developments and news regarding vaccinations. He is the author of The Vaccine Book: Making the Right Decision for Your Child. He spoke on the topic of vaccines at the La Leche League conference I went to last weekend.
Posing with Dr. Bob Sears after his lecture
La Leche League conferences are great because children are welcome at the lectures. As I chased my 10-month-old around the back of the room (until she finally fell asleep in the sling as you can see above), I managed to take in a lot of valuable information from the lecture. Dr. Sears discussed the occurrence and severity of various childhood illnesses, how vaccinations have affected the prevalence of those illnesses, toxins in vaccines, delayed and alter...

Medical ethicists have made amazing contributions to health care.
They’re heroic in their fearless pursuit of better care for beings – especially the human ones. They seem never to forget that is what they do. They work on the edges of what we know and are usually ahead of what we believe. They poke and prod us to do better. God bless ‘em.
Bob Veatch – one of my very favorites – has written a brilliant new book1 on the subject and I recommend it highly. It is a product of decades of deep thinking; he’s produced yet another gem.
Unfortunately, the title does not reflect the depth of his thinking; it’s far shallower than that.
The text pushes the edges of what we know and is ahead of what we can produce in health care today, but the title pushes it over the edge. I suspect some...
