My wife and I just returned from helping to chaperone 20 middle and high-school students at a church camp populated by a total of 600 youth. We were at SummerSalt, the flagship camp of the South Carolina Baptist Convention, located near Winnsboro, SC at White Oak Conference Center. (Actually, I&amp;#8217;ve always referred to it as &amp;#8216;Hotternhades, South Carolina,&amp;#8217; but that&amp;#8217;s just me.)

            <description>My son Sam and I were driving along (well, he was driving) and discussing all of the things we&amp;#8217;d like to do in life.  His interests are expansive, just like his dear old Papa, aka me.  We both recognize that life is not long enough to try all of the careers, degrees and experiences we would so love to enjoy.  &amp;#8216;I think that&amp;#8217;s what heaven is for, Sam.  Maybe we want to do those things because we&amp;#8217;re supposed to do them in eternity!&amp;#8217;
Sam, who sometimes toys with the idea of being a physician, pondered it all then said &amp;#8216;No offense, but I don&amp;#8217;t think we&amp;#8217;ll need doctors.&amp;#8217;

            <title>A metaphor for the death and resurrection of Jesus</title>

            <description>A friend recently asked how to explain the death and resurrection of Jesus to his young son in a non-judgmental, values-neutral way.  I have to admit, while a noble thought it&amp;#8217;s very difficult.  Jesus himself said he came  &amp;#8216;not to bring peace, but a sword,&amp;#8217;  and that he would cause divisions even in households.  It&amp;#8217;s tough to explain it without embracing the story or discounting it.

Two services and Sunday school on Sunday, Wednesday Bible Study, Thursday choir practice. Women’s Mission teas, and various types of training during the week, from Kids for Christ to Bible Drill to adult discipleship classes. We&amp;#8217;re all a bit tired, because we just returned from a short trip to Myrtle Beach. After our difficult winter, Jan and I felt that the family needed some distraction. When I met my wife at a college Halloween Party, we were 19 years old. She was a black-haired, blue eyed beuaty dressed as a mime. I was dressed as a doctor. (I was a big pre-med nerd.). After the party, our group of friends spent all of our free-time together, in and around our dorm at Marshall University.
Several months later, after a day when the school was closed by snow and we went sledding, a fire began to flicker between us. Soon we went on our first date. I have long loved kitchens. The kitchen of my childhood home was cozy, with a small bar where I ate my breakfast and where my sleepy brain came around in preparation for school, to the background of the radio built into the wall. My parents passed through hurriedly, kissing and feeding me as they entered their own worlds until evening.

It&amp;#8217;s probably nothing. That&amp;#8217;s what we heard over and over. My wife, Jan, had a swollen lymph node on the left side of her neck, just under the angle of her mandible. That&amp;#8217;s what I told myself when she first complained of it a few months ago. A tender, swollen lymph node after all of us had colds, sore throats, and fever. Probably nothing.
In November, it was more painful. (Painful node; probably nothing, right?) So it was off to the ENT. A cautious, competent surgeon, he said it was probably nothing, but it needed to be evaluated. Never in my life have I understood, so personally, what it means to pray, and have others pray for, my family. You see, my wife Jan was recently diagnosed with a cancer in her throat. Christmas brings hope and joy amidst life&amp;#8217;s pain. 
(And life can really punch you in the gut!)
This is one of our favorite seasons as a family. One Sunday, we put up all our decorations in a frantic three hour effort between morning and evening church activities. As usual, my wife has made the place beautiful by directing all of us the way a conductor directs the orchestra. &amp;#8216;No, those lights here. No, there&amp;#8217;s a gap in the tree. Yes, put the couch here, and the nativity set on that table.&amp;#8217; Her tastes are exquisite.

            <description>So, Jan&amp;#8217;s tonsil fragment was not malignant, so we don&amp;#8217;t know where the primary site is.  It isn&amp;#8217;t showing up on any of her biopsies, except in some lymph nodes on the left (all of which appear intact on the pathology report&amp;#8230;that is, they haven&amp;#8217;t spilled out locally).
Therefore, it will be a combination of chemotherapy and radiation, then frequent follow-up for a primary site to manifest itself.  Then, possibly more surgery.
I knew most of it already.  I was happy the nodes were intact, and had prayed hard for good news today.  I feel as if I received some, and I&amp;#8217;m glad we now have a plan.
Still terrified, but confident that God will provide.  We are hopeful and prayerful and believe in healing miracles.  In Psalm 103, it says it is God &amp;#8216;who ...</description>

            <title>Update on Jan’s surgery</title>

            <description>For those of you unaware, Jan had surgery Monday for a swollen lymph node in her neck.  It turned out to be a metastatic squamous cell cancer.  Usually only found in elderly men who smoke and/or drink excessively.   The primary appears to have been some leftover tonsil tissue at the base of her tongue.  (Tonsillectomy at age seven.) Her surgeon, the excellent Dr. Charles Theivagt, feels that he removed all of the primary, as well as the affected node and everything else in her left neck.  (She had a modified radical neck procedure.  Her comment?  &amp;#8216;I look like Frankenstein!&amp;#8217;  She said it with a smirk.  I think she looks just beautiful.)

            <title>Lost people act lost</title>

            <description>Recently I drove the church bus for a short trip with our youth group.  It&amp;#8217;s the first time I&amp;#8217;ve been trusted with the bus since I tried to drive it under the church rain-shelter (which was several feet shorter) a few summers ago.  Trusted with the bus once more, I was elated!  I had outlived my unfortunate nickname:  &amp;#8216;Crash.&amp;#8217;
However, despite the excitement I was later deflated.  A bus full of teens is always overflowing with energy, laughter and gentle teasing.  But the bus on the way home was filled with the sounds of anger and taunting.  There were shouts and accusations, threats and counter-threats.  I maintained control (of the bus and my temper).  We made it back to the church without blood loss or law-enforcement involvement.

            <title>The Clock’s Tick-Tock And Our “Tickers”</title>

            <description>With the daylight savings fall-back date for 2010 rapidly approaching (remember: &amp;#8220;Spring forward, fall back&amp;#8221; &amp;#8212; which is this Sunday, November 7th, 2010), I&amp;#8217;m reminded of some research I read a few years back suggesting a link between daylight savings and heart attack risk. The research suggested the Monday effect of increased heart attacks was not related to stress, but rather the sleep cycle.
When looked at from the daylight savings fall-back perspective, the research suggests the extra hour of sleep we gain from the November 7th, 2010 daylight savings fall-back date will be protective against heart attack risk. Good to know, especially if you&amp;#8217;re the cardiologist on call the week following either date.

            <title>Tangible comforts, like pancakes</title>

            <description>Many mornings, Jan and I make breakfast for the children.  Because they are home-schooled, we have the time to eat with a bit of leisure.  Today, as so often, we had chocolate chip pancakes, bacon and (for the Mama) a BLT.  Cups of hot tea followed.
It is a small thing, breakfast, and food in general; but not so small.  One of the things I was taught in my training was to offer food or drink to agitated or mentally unstable patients.  I&amp;#8217;ve done it many times, and have had remarkable success.

            <description>Every once in awhile I have the distinct &amp;#8220;pleasure&amp;#8221; of being a patient. This week I was reminded about how awful it is. I didn&amp;#8217;t mind the blood draws, poking and prodding, injections, or interaction with my physician, but it was the rudeness of the ancillary and administrative staff that really got under my skin. I had forgotten how unfriendly people can be, and how especially hard it is to deal with when you&amp;#8217;re not feeling well. Context is everything when it comes to rolling your eyes and sighing heavily. Let me explain.

It is my honor to speak a little humor into this very sad day.  Considering the situation, it’s a tough crowd!  You are all broken-hearted.  You are all so hurt by our loss.  Howard would want you to do one thing for him right now:

            <description>Emergency rooms are overwhelmed, in South Carolina and around the nation, with mental health emergencies.  It seems that every day I&amp;#8217;m asking someone about their suicidal thoughts, exploring their mental health history, trying to discern if their behavior is due to drugs, alcohol, underlying mental illness or simple acting out.
I can&amp;#8217;t remember the last shift when our emergency department wasn&amp;#8217;t 1) holding a psychiatric commitment or 2) watching the ambulance roll up with  yet another person thinking of suicide or acting on their thoughts.

Surprisingly, very little large-scale research has been conducted on the effectiveness of suicide hotlines, whether they actually save people&amp;#8217;s lives, and what kind of followup they provide for individuals in crisis. 
In one recent research study, Mishara et al. (2007) found that suicide hotline call center quality and the nature of their interventions varied considerably.             <description>Today I had a university student shadowing in the emergency department.  AF is a bright student, a hard worker who will make a wonderful physician.  She is always curious and insightful when I ask her questions, or show her new things.  Today,  she saw something that was new for her, but perhaps too common for me.
I walked into the room of an infirm, frail old gentleman who was gracious and polite, as were his family.  It turns out he came to us with a terminal illness.  I did not know it, but his physician was meeting him.  So, as AF and I walked into the room, the patient&amp;#8217;s physician walked in after us, and continued a conversation about hospice that he had apparently begun earlier in the day.

            <description>Some of my favorite rants and tirades in emergency medicine have to do with things my patients do that are just stupid.  Years ago, I saw someone who was playing with a rattlesnake and suffered a life-altering bite.  Recently, I saw someone whose friend offered to help him with the pain of an injured hand&amp;#8230;by injecting cocaine into it.  Once I saw a young man with part of his forearm missing&amp;#8230;because he was &amp;#8216;making firecrackers,&amp;#8217; aka pipe-bombs.  And of course, there was the fellow feeding his dog bacon from his own lips.
I was thinking about this the other day, and feeling superior.  I mean, look at me!  I&amp;#8217;m a doctor who uses good judgment!  I think ahead, I plan, I never, ever do anything stupid&amp;#8230;or do I?

            <description>I have to give my dentist credit. He and his staff know when I am due for a cleaning and call me to schedule an appointment without fail. They also call to remind me the day before an appointment. Many dentists, I understand, do similar kinds of things for their patients.
As a patient, I like being reminded &amp;#8212; it&amp;#8217;s a great service. I also like the fact that someone’s looking out for me. From a business perspective it makes a lot of sense as well. Fewer “no shows,” more cleanings, more billings, and so on.
It’s too bad that more physicians don’t routinely follow up with their patients, particularly when it really counts. (more&amp;#8230;)

            <description>It&amp;#8217;s not like they don&amp;#8217;t have a contact number or email - they do located somewhere in the rather busy MOH Webpage
The problem is when you call the MOH, you will likely get an operator and then get passed around from Department to Department until you reach the right place only to find out the officer in charge is away at a meeting! Call again Sir? Sure, only an hour later to play the musical department game again.
As for email, you&amp;#8217;ll be lucky to get any reply.
Such is the sad state of affairs that a comment in my Facebook wall got my attention. Is the MOH contemplating having a call centre?

            <description>Recently I asked if people were still putting ICE (In Case of Emergency) information in their cellphones.&amp;#160; (The surprising answer was yes, showing that it’s an idea that resonates, at least with my audience).
I lamented that I have that information in my iPhone, but since it’s passcoded no rescuer would be able to access my dutifully entered ICE contact information.&amp;#160; (I then had delusions of retiring on just such an app).
There is a solution.&amp;#160; It’s brilliantly simple, and works. Polka have written an iPhone app called “Close Call” that fits the bill perfectly.&amp;#160; And it’s free!

Anyone with flu-like symptoms and with high fever that persists for more than 48 hours should now be treated with anti-viral drugs, Datuk Seri Liow Tiong Lai said.
The Health Minister said this was a change from the previous policy of prescribing the drugs only when patients showed symptoms and were suspected to have contracted the A (H1N1) virus.
He said the ministry had directed all public and private hospitals to administer immediate treatment to these patients.
Liow said there were three groups involved — those with influenza-like symptoms and suffering from underlying medical conditions; with symptoms and high fever that persist for more than two days; and those who tested positive for In-fluenza A (H1N1).
