What did Larry Page know and when did he know it? Well, the US Department of Justice says that Google ceo Larry Page knew for years that online Canadian pharmacies were allowed to place ads that targeted US consumers and, ultimately, caused medicines to be imported illegally, The Wall Street Journal writes. Google, you may recall, last week agreed to forfeit $500 million (read here).
“Larry Page knew what was going on,” Peter Neronha, the Rhode Island US Attorney who led the investigation tells the paper. “We know it from the investigation. We simply know it from the documents we reviewed, witnesses that we interviewed, that Larry Page knew what was going on…Suffice it to say that this is not two or three rogue employees at the customer service level doing this on t...

Those free medical journals - which are sent at no cost to doctors and are financed by pharma ads - are much more likely to recommend drugs that are mentioned in its pages than journals that are funded by subscription fees, according to a new study. Consequently, some physicians may not readily notice any possible bias as they go about absorbing information needed for patient care.
The researchers identified 11 German educational medical journals from 2007 that are widely read by general practitioners, and these were divided into types according to revenue sources: free journals financed completely by paid ads; mixed-revenue journals that charge readers and also take paid advertising, and subscription-based journals that are completely financed by readers.
Can You Fake Feeling Remorse?

An offender in the criminal justice system often seeks to portray themselves as feeling remorse, especially when it comes time for sentencing in front of a judge, or parole hearings and the like. It may be easier to relate to someone who feels genuinely sorry for their crime. And it may be easier to show some mercy to a person who appears to be displaying genuine remorse.
Deception is also a good part of any skilled criminal’s behavioral toolkit, because dumb, honest criminals don’t usually last long.
So how can you detect whether someone is feeling genuine remorse, versus deceptive remorse in order to gain some favor with another person?
Canadian researchers from the University of British Columbia and the Memorial University of Newfoundland set to find out.

Conclusions
I’m anxious for alternative depression treatments to become more widely available. While I think psychotherapy is a powerful treatment for depression, too many people are simply resistant to trying it, or try it and don’t have a good match with a professional, and so give up on it too soon. Antidepressants have also been shown to be effective, despite the placebo research mentioned above (some of the research was done on data sets from decades ago and since that time, we’ve had a lot of new research to support their continued use).
Wouldn’t it be nice to have more modern brain tools in our arsenal (rather than the archaic and non-FDA approved ECT)? I think so, especially for these cases of serious, chronic, unremitting depression where both drugs and psy...

The Canadian Medical Association Journal (CMAJ) has published a new primer designed to help physicians when they counsel pregnant women. They note that sex during pregnancy is normal and is generally considered safe. The authors point out that there are very few proven contraindications and risks regarding intercourse in normal pregnancy.
Pregnant women and their partners are often afraid to have sex. Men may think they are “invading” the home of the fetus and could actually harm the baby. In fact, the fetus is quite safe, ensconced in the uterus (womb) and the cervix (opening of the uterus) is closed in normal pregnancy. The penis has no contact with the fetus or the uterus during normal intercourse, no matter what the position.
When is intercourse considered risky? Only f...

A trade deal being negotiated between Canada and the European Union could cost Canadians another $2.8 billion annually in drug costs - notably, by delaying the availability of lower-cost generics in Canada by about 3.5 years - if certain proposals are cemented, according to a new report commissioned by generic drugmakers.
During the talks, the EU has sought various changes in Canadian laws and regulations governing intellectual property concerning brand-name meds. These include extending the term of patent protection by up to five years if drugs are bogged down in the regulatory approval process; lengthening the period of data exclusivity from eight years to 10 years or more; and strengthening notice of compliance regulations by adding an appeals process.
The upshot is that Canadian payers...

Homeopathy: Why Is The Canadian Government Regulating A Scam?

Regular readers of the Better Health blog are familiar with the shoddy science behind homeopathy (an outdated system of “medical” treatment that relies on water dilution and shaking to ‘”strengthen” the effects of drugs). But because homeopathic placebos have been marketed so successfully (even receiving paid endorsements from hockey teams), the Ontario government has decided to regulate homeopathic practices.
In this terrific news exposé, reporters ask if it’s appropriate for the government to regulate health scams. In doing so, are they not lending credibility to modern-day snake oil? Check out these videos and let me know what you think. Is there a roll for government in regulating homeopathy?

This article (full reference below, free access) came to my attention via MDLinx. It was interesting to read. The conclusion verified my expectations rather than surprised me.
The authors conducted a retrospective review of patient demographics and implant information from three university settings: Kelowna (British Columbia, Canada), Loma Linda (California, USA), and Temple (Texas, USA). Each cohort included 100 consecutive breast augmentation cases. Characteristics analyzed included age, height, weight, BME, parity, and average implant volume.
When considering the 300 as one cohort, the average age was 34 years with a height of 163 cm (5’4”), weight of 58.1 (127.8 lb) and parity of 1.7 . The average implant size was 370 ml. (more…)

This past September, a group of medical residents at my institution began seeing primary care patients at a free clinic down the street from our tertiary academic medical center (“hospital clinic”). Far from my expectations, the care we are able to provide at our free clinic is in many ways better than our hospital clinic. Somewhat paradoxically, the experience has given me a taste of what the practice of medicine is like in single-payer healthcare systems like Canada’s.
When I volunteered to start seeing patients at a nearby free clinic, I had little idea what I was signing up for. The term “free clinic” conjured up memories as a medical student in East Baltimore tending to patients at a local homeless shelter with severe frostbite or at a student-run clinic rummaging through th...

Canada goes too far in excluding gay men from blood donationIt was fairly predictable that Canadian Blood Services would win their negligence suit against Kyle Freeman, the gay Ontario man who introduced syphilis into the blood supply after lying about his sexual history in a donor screening interview. Mr Freeman lied, he gambled on the safety of his blood, and he lost. He must now repay the $10,000 that CBS spent tracking down and destroying his blood. Public sympathy is most unlikely to be on his side. But things might have been very different if a more honest gay man had forthrightly challenged, in the courts, a policy that many consider discriminatory. Several such cases are now in the works, and here, the CBS is on much shakier ground. The current policy demands that would-be male don...

Psychology Secrets: Most Psychology Studies Are College Student Biased

Psychology, like most professions, holds many little secrets. They’re well known and usually accepted amongst the profession itself, but known to few “outsiders” or even journalists — whose job it is to not only report research findings, but put them into some sort of context.
One of those secrets is that most psychology research done in the U.S. is consistently done primarily on college students — specifically, undergraduate students taking a psychology course. It’s been this way for the better part of 50 years.
But are undergraduate college students studying at a U.S. university representative of the population in America? In the world? Can we honestly generalize from such un-representative samples and make broad claims about all human behavior (a trai...

Michael Ignatieff is making a damn solid point. It's the sort of point a conservative leader should be making; that your government is only as good as the data it has on hand... unless of course you are governing to American Conservative standards, where "post-modernism" and wishful thinking have taken the place of facts. The allergy of the modern Western Conservative to facts in evidence is akin to a virus, and it's spread, methinks, by the like of Rupert Murdoch, who's propagandist organs are waved in our faces daily.God forbid a government should actually do some hard science to find out what it's citizens, it's small business communities, it's regions and it's future require in terms of sensible and sustainable public policy. They are an iconic organization, inextricable from our collective identity as Canadians. We expect them to do far more, with far less, with more scrutiny than others and we expect them to do all this while serving as one of the great symbols of our nation. The great difference between police in other nations (and indeed, even other Canadian police forces) and the RCMP is that to some degree, people think of police in general as "them," while somehow, the RCMP are seen as "us."Astonishingly, this works. More often than not, members of the RCMP live up to this impossible standard with elan. And while everyone knows it's an impossible ideal, that could only be achieved in film and st...

Excellent article, fascinating discussion thread. Libertarians and the censusThe announced reasons for making the 2011 census long form voluntary is that it is 'intrusive' and that it is 'coercive' to make it mandatory. If this were the the position of a principled, reality-based libertarian government, then it would be a powerful argument. But it isn't.But that's a policy wonk blog. Very perceptive - indeed, perhaps too perceptive to quite grasp the Harper government's essential thinking... to be charitable about it.From the Globe and Mail:while many voters won’t notice or care about what happens to the census, “it will resonate deeply with certain swathes of voters by communicating to them that this government shares their suspicion of stats and the pointy-headed, out-of-touch academ...

Canada's Conservatives have been trying to turn the Conservative Party into The Northern Tea Party, but unfortunately for them, Canadians do not tend to polorize along idiologial fracture lines as easily as do people in the US. Therefore, "Red Meat Treats," such as the Harper Government's decision to reward the Social Conservatives of the Prairie Provinces has rather backfired. But on the very eve of Canada day, during the Queen's Visit, and immediately subsequent to the 100th' birthday of the Canadian Navy.1 this is the top story.Yes, there is the odd murder. Yes, we are still spilling blood in Afghanistan, our own and those of Afghanis. And yes, being Canadian, part of our contribution is a rather large contingent of medical professionals. Once someone HAS been shot and is unable to shoot back - if they were ever so inclined in the first place - we think that it is utterly unremarkable for us to be concerned about their well-being.
Can omega 3 help treat depression? According to new research, the answer is yes.
In one of the largest studies on omega-3 supplements done to date, Canadian researchers found that for people who don’t also have an anxiety disorder with their depression, the popular omega-3 fish supplements helped improve depression symptoms.
The improvements the researchers found in this study were similar to improvements found in studies of antidepressants, suggesting that for some people, omega-3 may be an inexpensive antidepressant alternative.

From October 2005 to January 2009, 432 male and female participants with major unipolar depression were recruited to take part in this randomized, double-blind study (neither patients nor researchers knew which capsules patients received).

I've been gradually dragging myself out of a serious writer's block, arising out of a sense of utter futility. On any number of issues, I feel as if what I write is of no value and has influenced nobody who would not already agree with me.Well, that's not true, of course. And not in the sense of bragging. It's literally impossible for anyone who communicates with other people to avoid some degree of influence. And as I climb slowly out of what now seems to have been a rather profound depression that eventually forced me to take some time off, I look back at what I've written. And some of it doesn't suck at all. Some of what I've written is pretty good. Some of it continues to bring traffic. If it does have any influence at all, it looks like it will have the sort of influence I would like. ...

It’s not been a good news week for SSRIs, a popular and common class of antidepressants that are widely prescribed by family physicians, interns and psychiatrists alike.
A study released in the journal Ophthalmology showed that in seniors, SSRIs can increase the risk of developing cataracts in your eye by 15 percent in general. But researchers found it was even worse for some specific kinds of antidepressants — Fluvoxamine (Luvox) increases the risk by 39 percent, venlafaxine (Effexor) increases the risk by 33 percent and paroxetine (Paxil) increases the risk by 23 percent.
A separate study also published this week found that taking SSRI antidepressant medications (and the SNRI Effexor) significantly increased — by 68 percent — the risk of miscarriage.

This post is more from USCAP 2010. I attended the "Advanced Molecular BIology" Special Course at USCAP on March 23. One of the highlights was Dr. Rob West's presentation, "The Role of Stroma in Neoplasia." I also had the pleasure of sharing a van ride with him to Dulles where we had a chance (because of miserable traffic) to talk for a long while about the cancer microenvironment, cancer stem cells, and, of course, our children. What follows is my summary of his talk. The role of the stroma in cancer can be summarized by two broad concepts:
Wound response: "normal wound-healing gone awry"
Cancer as an organ: altered communication between the cancer cells and the surrounding stroma

What are the components of the cancer microenvir...

Final recap from Dr. Adam Bagg's lecture at USCAP 2010 on molecular testing in lymphoma. The highpoint of the lecture for me was his comments on diffuse large B-cell lymphoma (DLBCL) since this is the most common lymphoma type we see in our practice. It so happens that there have been some recent papers that I've read that complement Dr. Bagg's lecture notes and I cite those below. I think if you consider his comments and have a look at these papers, you should be well-prepared for the next DLBCL you see.DLCBL is the most common lymphoma in adults but it is a heterogeneous clinical, morphological, immunophenotypic, molecular, and genetic "category." Some differences in clinical behavior can be predicted by the recognition of certain between...

Almost 10 per cent of Canada's entire population of seven million people served in the armed forces during the First World War, and nearly 60,000 died. The great Allied victories of 1918 were spearheaded by Canadian troops, perhaps the most capable soldiers in the entire British order of battle.Canada was repaid for its enormous sacrifice by downright neglect, its unique contribution to victory being absorbed into the popular memory as somehow or other the work of the "British".
Ole Ole Olson, writing at News Junkie Post concludes (with graphs!) that what folks like me have been saying all along is true; that the bullshit to the right of us is so deep that you need a diving suit and a support vessel to find any useful nugget of truth. And to the left - well, it's still a challenge, but you don't need life-support. Now, I'm probably too stubborn for my own good, still shouting out from time to time that I am, indeed a Conservative. An anti-authoritarian, to be sure. A minarchist to a degree. Occasionally I add that I'm a "Progressive Conservative," just to add a Canadian tinge of irony to my quixotic insistence on upholding the values of Burke, moderated by a good span of practice that has given us many ideas that seem well worth preserving...

So far in the new years, the only thing that has happened is that I’m freezing my balls off. Today is better than the last couple of days. I live on the top floor corner unit and the building’s roof is being redone. With the insulation pulled off, the first day of real winter arrives and my place turns into a refrigeration unit, thus forcing me to engage in a Canadian cultural experience by having to head down to Canadian Tire.
Even with the heater, when the temperature drops, I’ve resorted to turning on the oven with the door cranked open, and letting my dryer run a bit. Hopefully it won’t take long for them to get this work done, and Mother Nature will give us a break with some nice weather.
The big question for me for this new year is: Will I actually let someone...

The Canadian Medical Association is getting plenty of flak about their $780,000 CME deal with Pfizer, and anyone with half a brain can see why. Pfizer is not in the business of altruism. When they spend lots of money, they do so in order to make their shareholders happy. For example, when the company cut a $12.3 million CME deal with a collaborative including the University of Wisconsin and the California Academy of Family Physicians, they did so in order to encourage doctors to talk to their patients about smoking cessation. Presumably, this increased sales of Pfizer's anti-smoking drug Chantix. While this CME program, called CS2 Day (Cease Smoking Today), does not have the blatant infomercial feel of most commercial CME, it treats Chantix with kid gloves. As covered in the Milwaukee Journ...

The Canadian Medical Association is being criticized for agreeing to accept $78,000 in funding from Pfizer’s Canadian subsidiary to create continuing medical education programs (here is the CMA statement). Several prominent physicians argue the CMA may cross ethical lines in ways that could negatively influence treatment decisions.
“Partnering with a pharmaceutical company sends the wrong message to doctors – that we don’t have to pay for our continuing education, that it’s the responsibility of others. This is unlike how any other profession deals with continuing education,” Joel Lexchin, a health-policy professor at York University in Toronto, tells us.
“The sponsorship provided by Pfizer sets up a ‘gift relationship’ between Pfizer and the docto...

Last month, Canada’s Health Committee appointed Bernard Prigent, Pfizer Canada’s vp and medical director, to the Governing Council of the Canadian Institutes of Health Research, which sets policy, directions, and strategies for publicly funded health research. At the time, CIHR president Alain Beaudet said he hopes to create closer ties with industry to ensure involvement and investment.
But the move has not gone over well with some Canadians, who fear the appointment creates ethical quandaries. And so a petition is being circulated in hopes of convincing the committee, which meets on Monday, Nov. 30, to change its collective mind and rescind the appointment. Two reasons are cited for drawing up the the petition:
1 - Pfizer has a “well-documented history of transgressions...

Canada’s Health Committee plans to review the controversial appointment of a Pfizer exec to the board of the Canadian Institutes of Health Research, the government agency that oversees health research in Canada, The Tyee reports.
Bernard Prigent, Pfizer Canada’s medical director, was appointed last month to the CIHR’s governing council (see here). Last month, CIHR president Alain Beaudet said that he hopes to create closer ties with industry to ensure involvemetn and investment, but the move has stirred concerns since the organization is responsible for allocating research funding across the country, the paper writes.
“There’s no place in our scientific organizations like CIHR for a drug company official,” NDP MP Judy Wasylycia-Leis, tells The Tyee. “It’s shocking ...

Too many women are still finding breast cancer too late only because they don’t have insurance and can’t afford regular check-ups. Even with the best hospitals, treatment facilities, medicine, doctors and follow up care in the world, it is not helping those who don’t have access to it. Several of the comments that were posted to my blog last week on genetic testing were from people desperate for the test, needing it, but not able to afford it. It may appear that I have been silent about health care but the truth is that I have been discussing it on another forum. I have had the opportunity to participate in a panel to discuss health care reform on The Washington Post’s Web site. As a member of the panel I get to give my views on a weekly question concerning health c...

For years we have been told--and I sure used to believe--that the Canadians had the best approach to health care. Single payer funding, privately employed doctors--seemingly, a good mix. Except it's not. From a column by Nadeem Esmail in yesterday's Wall Street Journal:Canadians often wait months or even years for necessary care. For some, the status quo has become so dire that they have turned to the courts for recourse. Several cases currently before provincial courts provide studies in what Americans could expect from government-run health insurance. In Ontario, Lindsay McCreith was suffering from headaches and seizures yet faced a four and a half month wait for an MRI scan in January of 2006. Deciding that the wait was untenable, Mr. McCreith did what a lot of Canadians do: He went sout...

In July, the Canadian supplier, Total Care Pharmacy of Winnipeg, notified city officials it was terminating its deal because there were few participants. When the last Canadian drugs are shipped to Boston retirees in December, it will mark the quiet end to an initiative that generated headlines, the paper writes. City Hall blamed its demise on a lack of interest from city retirees eligible to participate.
Most city and state programs have died down, after Springfield, Massachusetts began the trend in...

Regular readers of SHS will (I hope) recall the comment I made the other day about an article in the Journal of Pediatric Nursing that was so neutral on infanticide, it seemed to me to be greasing the skids toward moving policy toward professional permissability. In that entry, I said in part:Terminal Non Judgmentalism Alert: An important professional journal aimed at pediatric nurses has discussed killing sick and profoundly disabled patients with studied neutrality. This is precisely how the Culture of Death permeates our society. A bioethical practice once almost universally condemned is promoted at the fringes. The initial response is resistance. But soon, the non judgmentalism arrives, usually in professional journals and among "progressive" pundits, asserting that these issues are "co...

We hear much about the deficiencies of the American health care system. But things aren't well north of the border, either--according to Claude Castonguay, one of the architects of the Canadian system. From the Investors Business Daily editorial:
Four decades later, as the chairman of a government committee reviewing Quebec health care this year, Castonguay concluded that the system is in "crisis."

"We thought we could resolve the system's problems by rationing services or injecting massive amounts of new money into it," says Castonguay. But now he prescribes a radical overhaul: "We are proposing to give a greater role to the private sector so that people can exercise freedom of choice."...

What would drive a man like Castonguay to reconsider his long-held beliefs? Try a health care syste...

I am incredibly excited at the prospect of research in to the use of the Wii to assist stroke survivors in re-learning movement. What a great example of our need as therapists to move with the times and exploit modern technology to engage clients in meaningful, therapeutic activities. I can just visualise Mrs Jones extending her shoulder back, flexing her hip and knee and going in for that killer ‘virtual’ strike!!!!!
Examples of it’s potential rehabilitative qualities include ‘77-year old Jerry Pope, a former semi-pro tennis player. Following his stroke in June, he’s been using the Wii Tennis activity of Wii Sports and swings of the Wii Remote to help regain movement in his hands and feet, along with his balance’
He claims the Wii “is extremely motivational and gives you the...

ADHD Support Group of the Month Idea

I thought about adding a new feature to this blog and my ADD resource website.
I used to run the Vancouver Adult ADD support group for 3 years and I’m on the board of another one, CHADD Vancouver. I have a whole section on my website on how to find Canadian, US and International ADHD support groups and how to start, run and promote a support group. I even did a teleclass on how to start one, The Imperfect Guide to Starting an ADD Support Group.
So I guess it’s obvious that I think ADHD support groups can be very useful for Adults with ADHD and parents of children with ADHD. It’s nice to talk with a group of people who you don’t have to “explain” what Attention Deficit Disorder is, they just get it. You get to listen to ADHD expert speakers, meet other AD...

Canada now allows dental hygienists to set up independent practice. The same issue has been presented in the US in the past. I believe that hygienists play an imperative role in oral health, and they do not need a dentist hanging over their shoulder to tell them how to clean teeth. However, in my understanding, the purpose of separating hygiene from the dental practice is to lower the consumer cost. Problem is, when a hygienist goes solo, overhead increases – which in turn is transferred to the patient. This negates the original intent. Furthermore, the hygienist is an essential member of the dental practice team, as are the assistants, administrative staff, and the dentist. Together, this team can comprehensively help patients and attend to all of their oral health concerns. Another pro...

The results of psychological research surround us every day, but few of us are aware of them. Psychology is interested in the study, observation and explanations for individual human behavior. It’s not about studying mice in labs anymore (although that’s still done, mostly in undergraduate psychology classes) so much as it is about studying real people in pseudo-real situations to better understand how and why people act, think or feel in the ways that they do.
Sometimes that research results in unintended offspring, such as the U.S. television show, Deal or No Deal.
Hosted by Canadian comedian and actor Howie Mandel, the show revolves around a single contestant who has to make a simple risk aversion choice — choose to keep an unknown amount of money the contestant hold...

It addresses the ethics (and practical issues) related to buying medications from online Canadian pharmacies. The answer to the question if it is “a good idea to buy drugs from Canada,” Wired Magazine says you are “technically violating US law.” But, they go on to explain you are unlikely to be prosecuted as “The Man is tacitly cool with your ordering prescriptions drugs from north of the border.” (Remember, this is Wired Magazine that is answering the question.)Wired goes on to say that you may actually have an ethical obligation to get cheap drugs from Canada “if those meds are staving off serious health problems and you really can’t manage the c...

We know much more about how certain foods enable the human brain to focus more, stay motivated, improve memory, and maybe even slow down brain decline that comes with age.Get back into gear after a holiday, Leslie Beck, Canadian based dietician at Globe Life Health reminded readers today - with a diet that starts when your feet first hit the floor.Research suggests breakfast, for instance, can improve mental performance and concentration. How so? Cereals, toast and fruit raise blood glucose levels, which in turn fuels the brain after a night of fasting. Glucose kick starts mental energy, as it makes acetylcholine, a memory neurotransmitter. It's thought these breakfast foods provides a slower and more sustained release of glucose … and can boost memory and attention. To tak...

Now here comes another news story. Same device, but this time it's about a Canadian teen. Fifteen-year-old Melissa Mills spent 146 days connected to her artificial heart. During that time she and her family were waiting for a transplant opportunity. Like the UK baby, however, her heart healed itself and she's now doing just fine - no surgery required! "For sure it's a miracle," one heart specialist was quoted as saying.Even a few years ago, it would have been considered just about impossible for someone like Melissa to survive without a heart transplant.
I was interviewed in the current edition of Canadian Living magazine (it’s the April 07 version on the newstands now, even though it’s March) on the topic of adult ADHD in women and ADHD coaching.
Unfortunately the article isn’t available online but is currently available in magazine stores across Canada. It’s a 6 page article written by Mark Witten,
who has won a number of Gold National Magazine Awards, and national awards from the Canadian Science Writers’ Association and the Canadian Nurses Association for health writing.
I’m in the sidebar section on p.101 “Coping with ADHD: Coaching and Support Groups can help” It’s quite a well written and thorough article; it discusses diagnosis co-morbid conditions, coaching therapy, medication support ...

Just in case there might be an election this spring, the NDP is fund-raising for, and testing, three new television ads. I am happy to join fellow Blogging Dippers by showing them here.

The ads are positive, under the banner “leadership and fairness”, and that last “Don’t let them tell you it [...] (Source: My journey with AIDS)
