Dori Carlson, O.D.
In a recent interview with the president of the American Optometric Association (AOA), Dr. Dori Carlson, I learned the surprising statistic that about 1 in 4 school age children have an undetected or undiagnosed vision problem. School vision screenings, while helpful, still miss more than 75% of these problems. And for those kids who are discovered to have a vision problem during a school screening, upwards of 40% receive no follow up after the diagnosis. Clearly, we need to do better at diagnosing and treating childhood visual deficits. My full conversation with Dr. Carlson can be listened to below:

An Avastin Recommendation & Conflicts Of Interest

Earlier this month, the National Comprehensive Cancer Network, a non-profit group of oncologists whose guidance is closely followed by leading treatment centers, voted overwhelmingly in favor of maintaining its recommendation that Avastin should be used to treat breast cancer. The vote came shortly after an FDA panel voted 6-to-0 to revoke the breast cancer indication for Avastin.
The endorsement is important because oncologists will likely continue to use Avastin even if FDA commish Margaret Hamburg rescinds the breast cancer indication. Roche and its Genentech unit had appealed a decision last December by the agency to pull the indication for their best-selling med after new studies showed Avastin does not prolong overall survival in breast cancer patients or provide a sufficient benefi...

Do you consider “neuromuscular dentistry” a bad word, but you’re not exactly sure why? The truth of the matter is that neuromuscular principles, e.g. a comfortable bite, can help you deliver longer-lasting results and contribute to happier, healthier patients. After all, who wants to live with headaches, bruxism, and chronic tooth wear?
The Academy of Comprehensive Esthetics understands that dentists are sometimes hesitant about all the neuromuscular bruhaha. At their upcoming conference “ACE Bite Camp,” they’ll dispel common myths about neuromuscular dentistry, explain the theories and principles, and most importantly, show how you can market TMD treatment to your patients in an easy-to-understand fashion.
Conference goals include:

Women are the fastest growing segment in the US military, already accounting for approximately 14 percent of deployed forces. According to statistics from the Department of Veterans Affairs (VA), 20 percent of new recruits and 17 percent of Reserve and National Guard Forces are women. As the number of women continues to grow in the military, so does the need for health care specifically targeted to their unique concerns.
Historically, lower rates of female veterans have used the VA system. “Research has shown that women didn’t define themselves as veterans in the past, and this is changing,” said Antonette Zeiss, PhD, a clinical psychologist and Acting Chief for Mental Health Services at the VA Central Office in Washington, DC.
Now, “Women are among the fastest growing segments of ...

Major philanthropic foundations, such as the Bill & Melinda Gates Foudation, regularly make the news with their donations and initiatives aimed at improving global health. But there is an aspect to their efforts that may be overlooked - such organizations can have links with drugmakers that could constitute a conflict of interest, according to an analysis published in PLoS Medicine.
The researchers examined the five largest US private and/or family foundations that focus considerably on global health - besides the Gates Foundation, the list included the Ford Foundation; W K Kellogg Foundation; the Rockefeller Foundation and the Robert Wood Johnson Foundation, which is a philanthropic outgrowth of a Johnson & Johnson founder. They analyzed publicly available endowment disclosures...

By Ilya ShapiroThe legal battle over SB 1070 is far from over, so neither side should cheer or despair. The upshot of the Ninth Circuit’s splintered and highly technical opinion is merely that the district court did not abuse its discretion in enjoining four provisions. The court could not and did not rule on the legislation’s ultimate constitutionality and, of course, SB 1070’s remaining provisions—the ten that weren’t challenged and the two on which Judge Bolton rejected the government’s argument—remain in effect.
But the legal machinations are only half the story. While I personally think that all or almost all of the Arizona law is constitutional, at least as written (abuses in application are always possible), it’s bad policy because it harms the state’s economy and ...

A trade deal being negotiated between Canada and the European Union could cost Canadians another $2.8 billion annually in drug costs - notably, by delaying the availability of lower-cost generics in Canada by about 3.5 years - if certain proposals are cemented, according to a new report commissioned by generic drugmakers.
During the talks, the EU has sought various changes in Canadian laws and regulations governing intellectual property concerning brand-name meds. These include extending the term of patent protection by up to five years if drugs are bogged down in the regulatory approval process; lengthening the period of data exclusivity from eight years to 10 years or more; and strengthening notice of compliance regulations by adding an appeals process.
The upshot is that Canadian payers...

The agency is doing so after analyzing clinical studies that found Avastin does not prolong overall survival or provide a sufficient benefit in slowing disease progression to outweigh serious side effects (back story).
Among the insurers are UnitedHealth and WellPoint, which have cited a recommendation by the National Comprehensive Cancer Network, a non-profit group of oncologists whose guidance is closely followed.
The day Elizabeth Edwards announced that she had breast cancer, my heart sank. Finding a lump in the breast only heightens the suspicious that the prognosis may not be good and in Elizabeth’s case, it wasn’t.
We all admired Elizabeth for different reasons. In my case, it was her love for healthcare reform that quickly grabbed my attention. Elizabeth advocated universal healthcare and comprehensive insurance for all Americans, not a “compromised” version based on partisanship and politics. As the years wore on, she discussed her diagnosis of incurable breast cancer with passion stating that she knew that she had access to the best possible care, but empathized with women who were not as fortunate.
It is said that behind every successful man lies the power behind the throne, and we k...

By Daniel GriswoldA Pew Hispanic Center report released today confirms what has been widely known, that the number of illegal immigrants in the United States has dropped sharply since 2007. The real argument is over what’s behind the decline.
According to Pew’s Jeffrey Passel and D’Vera Cohn, the annual inflow of unauthorized immigrants dropped by two-thirds during 2007-09 compared to 2000-05. That plunge has contributed to an overall decline in the total number of illegal immigrants in the United States from a peak of 12 million in March 2007 to 11.1 million in March 2009. Pew calls this “the first significant reversal in the growth of this population over the past two decades.”

By Daniel GriswoldPresident Obama spoke this morning at American University on the need for comprehensive immigration reform. The president deserves credit for turning his attention to a thorny problem that desperately needs action from Congress, but the speech failed to hit at least one important note.
While the president called for comprehensive reform, he neglected to advocate the expansion of legal immigration in the future through a temporary or guest worker program for low-skilled immigrants. Even his own Secretary of Homeland Security, Janet Napolitano, has said such a program is the necessary “third leg” of immigration reform, the other two being legalization of undocumented workers already here and vigorous enforcement against those still operating outside the system.

Will The Medicare Review Unravel Provenge?

Last night, the latest chapter in the Provenge saga was unexpectedly written - the news that the Centers for Medicare & Medicaid Services was reviewing coverage for the Provenge prostate cancer vaccine sent Dendreon shares plunging as much as 23 percent in after-hours trading. Coupled with manufacturing constraints that are causing rationing, investors were caught off guard.
That’s because a National Coverage Determination usually isn’t issued for cancer treatments. In fact, an NCD is uncommon for new drugs, in general. What likely prompted this move were requests from local Medicare providers, or what RW Baird analyst Chris Raymond calls “local fiscal intermediaries.” Why?
By Archelle Georgiou. On Wednesday, the FDA suddenly decided to impose their regulatory authority on personalized genetic test kits after Walgreens and Pathway Genomics announced they’d be selling them in local pharmacies. But, what triggered this response from the FDA?
Are they new? No. These kits have been available to consumers via the Web for 3 years.
Have they been off the regulatory radar screen? No. As far back as 2008, the rapid emergence of genetic testing fueled the passage of GINA, a federal law prohibiting health insurers and employers from discriminating on the basis of genetic information.
Have these companies been quietly launching their strategy and staying invisible? No. They have made major investments in marketing with an abundant amount of media coverage in women&...

By Daniel GriswoldIn a gutsy display for a newspaper, the Arizona Republic in a front-page editorial yesterday castigated the state’s top politicians for a failure of leadership on immigration.
Prompting the editorial was the passage of Arizona’s tough new law making it a crime to be an illegal immigrant in the state. Under the banner headline, “STOP FAILING ARIZONA; START FIXING IMMIGRATION,” the state’s major newspaper fired with both barrels:
We need leaders.
The federal government is abdicating its duty on the border.
Arizona politicians are pandering to public fear.
The result is a state law that intimidates Latinos while doing nothing to curb illegal immigration.
This represents years of failure. Years of politicians taking the easy way and allowing the debate to descend in...

By Daniel GriswoldThe latest catch phrase in the immigration debate is that we must “get control of our borders” before we consider actually changing the current immigration law that has made enforcement so difficult in the first place.
In his Washington Post column yesterday, George Will wrote that “the government’s refusal to control [the U.S.-Mexican] border is why there are an estimated 460,000 illegal immigrants in Arizona and why the nation, sensibly insisting on first things first, resists ‘comprehensive’ immigration reform.”
On the other side of the political spectrum, Democrats in Congress this week unveiled the outlines of an immigration bill that would postpone any broader reforms, such as a new worker visa program or legalization of workers already here, until...

By Daniel GriswoldA silver lining of the Arizona immigration law is that is has turned up the heat on Washington to re-examine federal policy. As I’ve made the rounds of talk radio shows today, one of the questions that keeps coming up is just what changes should be made in federal law to tackle illegal immigration. Glad you asked.
In brief, the single most effective change would be to expand opportunities for legal immigration, including for low-skilled workers who make up the large majority of the illegal population.
I make the case for comprehensive immigration reform in an op-ed in today’s Philadelphia Inquirer.
For a more comprehensive case for comprehensive reform, see the lead article I wrote for the current issue of the Albany Government Law Review, titled “Comprehensive Immi...

By Daniel GriswoldLawmakers in Arizona must believe the state’s law enforcement officers have too much time on their hands.
A bill passed by the legislature yesterday will make it a misdemeanor to be in Arizona without proper immigration paperwork. It also directs Arizona police to question anyone about their immigration status if they have reason to suspect the person is in the country illegally. Failure to produce the proper documents could result in arrest, a $2,500 fine, and up to six months in jail.
Making and enforcing immigration law is a federal responsibility. State and local police should focus their resources on preventing crime and apprehending real criminals who pose a danger to public safety.
Police in Arizona seem to agree. According to an Associated Press report,

This Briefing explains how Comprehensive Area Assessment will work, including the implications for health bodies. Key points:

Local outcomes will for the first time be assessed collectively, with six inspectorates working together.
Sector-specific assessments will continue, but will be developed alongside and feed into CAA. The Care Quality Commission is responsible for more detailed assessment of health and social

In response to criticism, the National Comprehensive Cancer Network, which is an alliance of 21 leading cancer centers, has decided to expand its disclosure policy and make public all potential conflicts of interest of all individual expert panel members by the end of 2008.
The move, which was made last week, followed complaints that the NCCN’s Drugs and Biologics Compendium for failing to disclose corporate ties of the 20 to 24 experts who sit on 44 guideline-writing panels. Instead, the NCCN listed all companies that gave money or research support to any committee member without listing any specific member or the amount given. The compendium may be used by Medicare to justify reimbursement for off-label use of cancer drugs.
Historically, the NCCN has disclosed the names of companie...

Medicare is nearing approval of three additional drug compendia that can be used to justify reimbursement for off-label use of cancer drugs, including one that violates agency guidelines on conflict-of-interest disclosure, according to Integrity in Science Watch.
The National Comprehensive Cancer Network’s Drugs and Biologics Compendium, which is drawn from its Clinical Practice Guidelines, fails to disclose corporate ties of the 20 to 24 experts who sit on its 44 guideline-writing panels. Instead, it lists all the companies which gave money or research support to any committee member without listing any specific member or the amount given, ISW writes.
In 2006, the Medicare Evidence Development and Coverage Advisory Committee recommended that any new compendia approved by the Center for ...

In the wake of the Virginia Tech murders earlier this year, Virginia’s legislators are looking at whether to make it easier to commit people who are a danger to themselves or others. This is a complete and utter deflection of the problem that states like Virginia actually face — a decline in support and funding of comprehensive, public mental health services. Virginia lawmakers are basically tinkering with the wording of their laws while Rome burns around them. The Washington Post has the story in yesterday’s edition entitled, Commitment Rule Is Key To Changing The System.
It should be noted that Cho, the person who committed the Virginia Tech murders was, in fact, detained for overnight observation and was examined and found not to be a danger to himself or others back ...

Filed under: Thought for the Day, SurgeryWhat do you think is the best way to choose an experienced and quality surgeon? Some researchers think it's by reputation. That's how I got mine. A friend told me who he'd use if his wife or mother developed breast cancer and then more and more people began recommending the same surgeon. It seemed logical I'd use him too. And I'm glad I did.A study published in the January 20 Journal of Clinical Oncology reveals that women who actively choose their surgeons by reputation are more likely to be treated by experienced surgeons in hospitals with comprehensive cancer programs. Reputation even beat out accepting referrals from other doctors or health plans.Read | Permalink | Email this | Linking Blogs | Comment...
